DottaandPaperclip ecdb43b64f Fix heartbeat concurrency normalization ...

Honor maxConcurrentRuns values below the default so a configured value of 1 keeps same-agent assignment wakes queued while a run is active. Add a regression test covering the queued second assignment case.
