Devin FoleyandPaperclip a12329b8a9 fix(db): complete bounded response attribution (#17537) ...

Raise bounded database-response attribution to 2048 groups and add numbered 128-entry log pages. Preserve the overflow bucket and exact completed-call and byte totals. Document complete page retrieval and validation.

Verified with 30 focused tests, the full build, and typecheck. All CI checks pass. Exact-head Greptile is 5/5 at Medium impact with no unresolved comments.
