3 people e25521ecf3 fix(server): add a stranded in-review feedback recovery backstop ...

Periodic reconciliation skipped in-review issues without a typed execution participant. Review feedback could therefore stay silently stranded when its comment run failed before launch.

Add a narrow backstop for that shape. It queues one bounded normal-model replay for the original assignee. It opens one explicit recovery action when replay is exhausted or the assignee cannot run.

Both recovery paths now share one fingerprint, replay key, recovery-action kind, cause, and activity key. This prevents a parallel delivery lane when terminal cleanup and periodic reconciliation race.

Legitimate waits remain unchanged. The backstop skips Board confirmations, approvals, monitors, blockers, human-owned reviews, typed review participants, pause holds, system wakes, and terminal issues.
