Phase 1 of PAP-13031: markdown-backed shipped defaults for the built-in
Reflection Coach — agent AGENTS.md, paused-by-default recent-agent-reflection
routine template, and the bundled reflection-coach operations skill. Grounded
in the live coach but cleaned up: adds the interaction-gated apply model
(displayed diff + accepted interaction + separate follow-up run) now that the
built-in ships with skill/instruction update permissions.
