Follow-up to PR #7575. Adopts items 1-6 from PR #4189:

bug_report.yml
  - PII privacy notice + mandatory privacy checklist
  - Pre-submission preflight checkboxes
  - Additional optional env fields: install method, adapter(s) checkbox
    list, database mode, access context
  - PII warnings inline on logs and config paste fields

feature_request.yml
  - Required subsystem dropdown (server/ui/db/shared/adapters/plugins/
    cross-cutting/unsure)

enhancement.yml (new)
  - For improvements to existing behavior with current/proposed/breaking-
    changes fields

docs_issue.yml (new)
  - Lightweight doc-issue form (type/location/description/suggestion)
