Count accepted database sockets as pending through initial ReadyForQuery and bound virtual clock advancement on startup readiness. Qualify the held actual driver write, disconnect and cancellation boundaries. Retain fail-closed header diagnostics; the earlier malformed-frame cause remains unknown.
