Classify Codex refresh-token auth failures across CLI, ACP, and quota probe paths without adding the removed credential telemetry emitter scope.

Stabilize verification blockers by avoiding a brittle host Codex CLI version pin, using CASCADE cleanup in heartbeat retry tests, and hiding looked-up tool access resources across company boundaries.
