Pass the channel command as an argument list, so the live launch wrapper builds
the raw-mode wrapper instead of failing on a string. The live test now runs when
the provider credential is present.

Add two live cases against the real provider. The first sends repeated channel
round trips over one open channel, measures the round-trip latency, and asserts
the provider session count never rises: zero idle polls and zero per-request
session growth. The second forces a mid-flight disconnect and asserts no leaked
session and a still-usable sandbox.
