fix(server): schedule governed review response recovery (#17702)
## Thinking Path > - Paperclip manages agents, tasks and their external chat responses. > - A completed chat turn can owe a response while an earlier review gate remains pending. > - Periodic recovery revisits that response history even when no work can advance. > - This change uses the existing delivery coordinator and saved retry deadlines. > - Approval, interaction and authority changes wake the affected issue or run after commit. > - The existing presentation owner verifies the durable result and current authority. > - Settled and dormant history becomes quiet without losing future eligible responses. ## Linked Issues or Issue Description Refs #17696 (merged) and #15625. This is slice 49, now based on master. It owns governed-review response presentation. Accepted responses and pending decision effects keep their existing owners. **What happened?** Native finalization recovery repeatedly inspects committed review responses. Removing the scan without covering approval, interaction, task-policy and authority changes would strand responses that become eligible later. **Expected behavior** Recover an owed response once from existing durable facts. Enforce authority at publication, wake after relevant outer transactions commit, retain real retry deadlines and restart/drain recovery, and stop recurring SQL when no response can advance. **Steps to reproduce** Complete an external chat turn while an earlier completion review remains pending. Block its response behind an approval or interaction, then resolve that blocker. The response must appear once without a periodic scan or resolving the original review gate. ## What Changed - Add a scoped governed-review worker using the existing coordinator, finalizations, decisions and comment receipts. No queue table or transaction wrapper is added. - Keep recovery and publication policy in the native finalizer. Preserve binding, source/session authority, ownership, deadline and deleted-comment fences; match coordinator-before-issue lock ordering. - Notify through actual approval, interaction, OAuth, task-policy and lifecycle producers before their owned writes. Serialize approval resolution with task linking so a concurrent link retains its wake. Nested transactions still settle at outer commit. - Scope ordinary milestones, finalization and coalesced input to their issue/run. Use the explicit company authority boundary for genuine global restoration, including membership changes. Both response workers share this boundary. - Register under maintenance and drain gates, disable the two periodic repair entrances, and retain fresh finalization and explicit recovery. Add the concurrent 0413 comment-receipt index and migration registration; retain five snapshots. - Preserve the producer inventory, source qualifications and earlier failures in the owner audit. Retain a real Photon regression proving routine ownership assertions stay quiet and concurrent authority restoration wakes each worker once. ## Verification - Scoped repair source `2341714720b5c273f0ce011b48345af0ef39227f` passes **2053 distinct cases**: 1703 in 15 complete producer/authority/recovery files, including all 273 governed-response cases and 95 wake-queue adapter cases, plus 350 selected chat cases. Another 1463 chat cases are outside that selection. Thirteen fresh PostgreSQL 17 fixtures migrate through unchanged 0413; the one temporary queue-fixture import restores exactly. All sixteen captured hashes match. - Two actual repeated coarse-hint SQL regressions fail before repair and pass afterward. Actual membership restoration passes before and after. The same assertions and time limits prove dormant work stays quiet and real authority restoration still publishes once. Real coalesced input covers both queues after commit. - Combined source `586f5674cd9798b33cd22437255e0480b5726d20`, including independent master #18129, passes **1297 distinct cases**: 1093 in five complete server files, 185 in five runner files, 14 Node protocol cases and five UI registry cases. This includes the complete 22-case Photon file for both response workers. The external Photon transport is mocked; Paperclip callbacks, transaction fences and observed SQL are real. The earlier and combined case counts are separate, with no cumulative double-counting. - A normal merge retains both complete owned and independent master binary patches. All twelve new-context hashes and sixteen earlier repair hashes match. Three dedicated fresh 0413 fixtures plus the live-event helper's own fresh database use PostgreSQL 17 without source adaptations. Database build, server types/build, boundaries, full-SHA migration order and generated TS/Rust profiles pass. The new inherited Runner vendor inputs rebuild successfully. - Database bytes remain identical to the previously qualified 0413 source. The recorded populated 0412-to0413 upgrade retains all 411 prior receipts and 38 companies, appends one receipt and none on replay; the index is valid and ready. The earlier 2828-case acceptance, CLI journey, setup failures and their repairs retain their recorded source scope in `doc/plans/2026-10-26-governed-review-response-sleep-audit.md`. - Parent #17696 merged with 58 clean checks, Greptile 5/5 and Ponytail 5/5. Its Photon warning was disproved by real callback/SQL evidence and the security check became successful. This PR still requires its own fresh exact-head CI and review gates. No deployed sleep, external alarm or fleet savings claim is made. ## Risks - Notifications use the existing process-local writer contract. Independent SQL writers and uncoordinated replicas are outside its coverage. - Genuine startup and company authority reconstruction remain proportional to the owed backlog. Queries page 100 rows, but discovery accumulates the candidates; this is not a constant-cost global operation. - Current publication authorization, durable binding, ownership and lock ordering remain mandatory. The worker does not resolve the original review gate. - The concurrent index uses existing migration replay handling. Other recovery duties and Cloud sleep eligibility remain separate. ## Model Used OpenAI Codex, GPT-6 family, with reasoning, repository tools, shell execution and PostgreSQL tests. The runtime does not expose a more specific serving snapshot or context-window size. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> Co-authored-by: Codex <noreply@openai.com> |

feat(cases): add persistent workflows with same-task agent handoffs (#16959)
## Thinking Path > - Paperclip manages AI agents and their work. > - An agent workflow needs persistent facts and a safe way to transfer work. > - Creating a task for each phase splits the objective, discussion, workspace, and review policy. > - A case can retain the facts while one primary task moves between agents. > - Transfers must survive process failure and preserve private access, budgets, and completion gates. > - This pull request adds durable case handoffs, native case tools, and read-only case pages. > - Prompts describe the workflow. Existing task controls decide when work can run and finish. ## Linked Issues or Issue Description **Problem or motivation** Prompt-driven agent teams need a persistent record that can move through triage, implementation, verification, and correction. Ordinary reassignment does not provide a durable instruction, safe release barrier, or recoverable delivery receipt for an agent handing off its own running task. **Proposed solution** Extend standalone cases with one primary task, a descriptive phase, retained access sources, and a handoff ledger. Apply assignments after source execution and workspace settlement. Deliver each transfer through the existing durable wake queue. Provide `create_case`, `read_case`, `update_case`, and `handoff_case` in the native runner. Creation requires kickoff and explicit workflow text. An explicit handoff to `done` closes the case when normal task completion is accepted. **Alternatives considered** Do not add a workflow engine, filesystem queue, configured state graph, or one task per phase. The workflow document combines transition prose with an optional Mermaid `stateDiagram-v2` map. The server does not execute the diagram. The Pipelines experiment removed on master stays removed. **Roadmap alignment** This extends Agent Reviews and Approvals, Enforced Outcomes, Bounded Run Recovery, and Activity Log attribution. Existing approval, review, budget, privacy, workspace, and responsible-user rules remain authoritative. Related work: [ordinary handoff wake recovery](https://github.com/paperclipai/paperclip/pull/15019). This feature adds a separate persisted case-handoff receipt and release barrier. ## What Changed - Add case routing fields, retained access sources, durable handoff history, delivery leases, and replay-safe migrations. - Keep the same primary task across agent phases. Preserve each returning agent's own session and the task's workspace. - Fence outgoing mutations, wait for provider and workspace release, and recover delivery without duplicate task comments or runs. Recheck current assignment permissions as the original caller before applying a delayed transfer. Scope a run Stop to that run’s own handoff intents, so repeated old Stop requests preserve newer work. Cancelling terminal intent without a source run also requires fenced current task ownership, including native retries and preparation with an old checkout. For Cloud callers, revalidate current stack membership with an uncached Cloud RPC and derive owner elevation from the current managed setting. - Keep case reads, documents, assets, Audit, and live events inside retained task/project permissions. - Add native case creation/read/update/handoff tools. Store required workflow text and initial documents on the first task. Page oversized case facts and referenced document bodies through the dedicated tool with bounded UTF-8 JSON, current routing versions, snapshot conflicts, and current source authorization on every page. - Add read-only case lists and detail pages, task-style list controls, compact dotted field tables, linked handoff tasks, and case filters in Audit. - Add native/legacy integration tests, migration/restart tests, browser coverage, and bounded live receipt fixtures. Extend the trusted sandbox condition to cover the new subscription fixture profile. - Hide obsolete paused-agent warnings on completed or cancelled task threads. - Synchronize the company-switch settings test with React effects after a CI failure. Preserve its security assertions and production remount behavior. ## Verification The branch directly includes master `9ef434fe79`. The required `ci / verify` and `ci / e2e` gates pass on head `765fccd67c`. [Current-head CI](https://github.com/paperclipai/paperclip/actions/runs/39464348614) passes typecheck, build, tests, and all eight browser shards. Security checks pass. [Greptile gives this exact head 5/5](https://github.com/paperclipai/paperclip/pull/16959#issuecomment-6291156549), with no unresolved review threads. The optional Paperclip Review check remains queued because its assigned agent is paused. It is not a required branch check. No check result was overridden. Local workspace typecheck and build pass after the master merge, along with database migration checks, token gates, map checks, Storybook catalog checks, and the Copilot profile identity check. Standard builds refresh Runner vendor output. Focused checks pass 2,008 API/paging/context tests and 7 strict case protocol tests. The final commit changes one test fixture only; its ten tests, UI typecheck, and token gates pass. No production source changed in that commit. On current head `765fccd67c`, all **194 critical workflow tests** executed and passed with no skipped tests in those files: - 38 Stop/ownership tests, including board completion intent, native retries, stale Stop, and preparation with an old checkout. - 2 actual legacy process/HTTP handoff tests, including scratch failure, a closed-stdio child that continues writing, and recovery through a fresh worker after the process group stops. - 5 migration tests, including SQL column metadata, preserved null authority context, and all six prior-numbering receipts. - 3 process-settlement and 5 scratch tests. - 88 handoff-service, 18 heartbeat handoff, and 21 native case-tool integration tests. - 7 bounded case/document-read projection tests and 7 strict protocol-contract tests. The current CI checkout was `fed65d49`, merging `765fccd67c` into master `9ef434fe79`. Its new document regressions reconstruct exact Unicode/escaped bodies and revisions through real native authority and authenticated HTTP with generic API tools disabled. They cover case-owned, primary-task, and linked-task documents; routing on every page; stale document snapshots; access loss and unlinking; unrelated/foreign/private sources; system documents; and quarantined content. Active-task document tools retain their existing behavior. These are current-head test results from the first CI attempt; the browser-only retry did not rerun these suites. The full `64fdd3403f` CI failed one separate settings test: after a company switch, it observed an unsettled API-key draft. The production component and test were unchanged from prior green source. Production switches remount both keyed settings layers; the test directly reused an unkeyed view and used a timer tick instead of waiting for React effects. The final commit replaces that fixture synchronization with React `act`. The exact secret-clearing assertions and all timeouts remain unchanged. The original assertion failure remains recorded; no unchanged retry was used. The narrow unchanged baseline and fixed fixture each pass 10 tests. The upstream result-paging merge preserves case capture namespaces and current asset privacy. Its API integration suite executed 39 of 41 tests successfully, with two skipped; the optional assigned-result PRP integration test was skipped. Neither skipped suite is counted in the 194 critical tests or as live provider proof. All 361 upstream migration entries and SQL bodies remain unchanged. Six case migrations follow master as 0363–0368 with identical SQL hashes. Official Drizzle generators produced the combined metadata and five retained snapshots. Generation reports no schema changes, and the snapshot drift test passes. The earlier `ce361ae5d0` CI passed on attempt 1, including its then-current 179 critical tests. Fresh review later identified unusable document references with generic API tools disabled; the bounded document reads and ten additional authenticated HTTP tests address that finding. The full local `pnpm test:run` attempt was cancelled after this Mac exhausted PostgreSQL semaphore capacity; it is **not** counted as a pass. Later database attempts also stopped before assertions or skipped, including one sandbox socket denial. Two original stale-Stop regressions did reproduce the bug against the old source. A REST serialization assertion passed after two preserved database-startup failures. No test timeout was increased. All 194 assertions listed above executed and passed in current-head CI. The complete 385-file, 3,963-test UI shard also passed after the fixture repair. A configured-provider receipt workflow passed 13 backend checks across triage → implementation → verification → correction → done, using one task and four work runs. The saved receipt is revision 2 with total 6 USD; case and task both closed. The original canonical attempt remains failed because its browser assertion confused the terminal destination with completed-task tooltips. The corrected full browser verifier passed on the same retained database and original runs, with no additional provider calls. The last clean-build browser readback is on `6a86c54a53`; later changes affect recovery, authorization, bounded reads, and settlement. Accounted work cost is a $1.010574 rate-card estimate; provider billing and setup-probe usage are unavailable. Copilot profile 49 passed the unchanged seven-fixture private protocol pack (46/46 checks, no retries) and the product file-edit canary after master's completion-replay change. All 60 bound implementation hashes remain unchanged on the current source. Historical profile-47/48 identities and results remain intact. See the [profile-49 compatibility record](https://github.com/paperclipai/paperclip/blob/codex/persistent-case-handoffs/doc/plans/2026-10-20-copilot-completion-recovery-compatibility-v49.json). This is bounded compatibility evidence, not a fresh full matrix, installed-package product test, or Daytona rollout. Preserved CI failures and retries: - On `20662930ac`, Slack managed setup failed a five-second success assertion after reload. Its test/application path was unchanged from a prior green head. One unchanged-source retry passed all 25 tests in that shard; no source or timeout changed. - On `9402b4f17a`, two CI workers received shutdown signals at 15:24:19 UTC. Neither log had an assertion failure before interruption. One unchanged-source retry of each passed all Runner static checks and all 32 tests in browser shard 6; both aggregates passed. The next head, `7b4374edaf`, passed all CI without retries. - On `295f289b5a`, serialized shard 9 timed out during its first device-login test, then the next test found an active login lease. Direct login/test paths were unchanged from a prior green head. Cold import plus a timed-out async body using the next test's mutable actor/store is the supported fixture-race explanation; no request trace proves that sequence. The original failure remains recorded. One unchanged-source retry passed all 294 tests in 19 files, including all 41 device-login tests. No login source or timeout changed. The subsequent `ce361ae5d0` CI passed without retries. - On `64fdd3403f`, one company-switch settings assertion observed an unsettled draft. The final test-only commit uses React `act` without changing assertions, timeouts, or production code. On `765fccd67c`, all ten settings tests and the complete 385-file, 3,963-test UI shard passed. - On `765fccd67c`, browser shard 3 completed the split-handoff assertions, then cleanup received HTTP 500 from run cancellation after a one-second PostgreSQL row-lock timeout. The extra chat run was reporting completed work while teardown began. Cleanup stopped before restoring a global experiment flag, causing a later assertion to fail. Relevant production and test paths are unchanged from an earlier green commit. Stop's lock order is consistent, and no nested self-lock was found; the exact PostgreSQL blocker was not recorded. One unchanged-shard retry passed. The original failure, trace, and downstream flag failure remain recorded. No Stop implementation, cleanup assertion, or timeout changed. The retry supports intermittent cleanup contention; it does not identify the original blocking transaction. ## Risks - Local process transfers require host-owned PID/group absence. Missing or uncertain termination evidence keeps delivery pending, independently of scratch setup. - Handoff admission and task cancellation share execution ownership. New regression tests cover temporary holds, late source writes, and uncertain provider termination. - Retained private sources use an intersection of existing permissions. This intentionally fails closed when a source is missing or an existing task cannot preserve that boundary. - Migrations add tables and indexes. They preserve master history and can replay over prior feature installs. Migration 0368 adds nullable internal authorization provenance with no default or authority backfill. Historical transfers still awaiting release without this context fail closed; an operator must cancel them and request a new handoff. - Delayed Cloud handoffs depend on the current Cloud membership RPC. An unavailable, ambiguous, expired, or mismatched response holds the handoff for retry; it does not reuse cached authority or grant access. - Bounded reads reload and reauthorize the selected projection per page. Underlying work grows with the data size and number of pages. Changed data or routing requires restarting from offset zero without the old snapshot. - Phase names and workflow diagrams remain advisory. They do not satisfy review or approval gates. - This is a large, cross-cutting PR. The maintainer requested one PR for the whole feature; the commits separate data, runtime, UI, and qualification work. ## Model Used OpenAI Codex, GPT-6 family, with tool use, code execution, and delegated code review. The exact deployment model ID and context-window size were not exposed in the session. Live fixture agents used the explicitly configured provider model recorded in their test evidence. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

Connect and configure GitHub Copilot through Apps (#15705)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Apps and AI Connections store credentials and govern access to agent runtimes. > - Users need a supported saved-token connection and config values that survive save and reopen. > - The release must preserve saved-token access and task lifecycle guarantees. > - This pull request will connect Copilot through the normal Apps and AI Connections path. > - The benefit is a reviewable part of the Copilot production release. ## Linked Issues or Issue Description Refs #15701 and #15703. Supersedes setup work in #15562. Base: #15703. Next: #15706. ## What Changed - Require configuration update authority before Copilot credential-bearing model discovery; default to the saved environment and preserve explicit selection for editors. - Keep Copilot discovery passive during reconnect reconciliation; refresh on explicit opening, account/environment changes and reconnect. - Add GitHub AI token connections with encrypted storage, ownership, grants and audit logging. - Keep repository access separate from Copilot runtime authentication. - Probe the selected environment without sending a model prompt and preserve authenticated model selection through save/reopen. - Keep fresh execution admission consistent with normal Copilot setup; persist task model choices without changing provider or permission policy. - Refresh authenticated models after reconnecting the same saved Copilot account, including recovery from a previous authentication error. - Apply the additive provider-default constraint migration through the normal generated migration workflow. ## Verification Runtime #15701 is merged at `d855e550ad8d2a267312983f26457aabf5393c3a`. History #15703 is merged at `df80616d98119cbf5abd40549794d9980a5fabf6`. Remaining source [`42bcbd61299655f7e37aabe6bb5f19bba770ad1d`](https://github.com/paperclipai/paperclip/commit/42bcbd61299655f7e37aabe6bb5f19bba770ad1d) contains the four-file authorization repair. The remaining PR trees preserve six nonoverlapping files from the newer public mainline managed-settings fix and the CI-owned lockfile. No unrelated provider is enabled. The security finding is repaired: existing-agent discovery requires `agent_config:update` before connection selection or token resolution. The saved environment is the default. Authorized editors can select a prospective environment. Both denied board-viewer and agent-reader regressions fail on the original source. All49 route tests and server typecheck pass with the fix. Denied requests perform no connection selection, credential resolution or probe. Current-head security scans pass on both remaining PRs; the repaired thread is resolved. Copilot CLI1.0.88 and profile47 retain digest `sha256:55044aa235e88b3fe36ec8a556ee1e3382590870aab907e4462ef7d64af49cea`. The [qualification record](https://github.com/paperclipai/paperclip/blob/42bcbd61299655f7e37aabe6bb5f19bba770ad1d/doc/plans/2026-10-20-copilot-qualification-v47.json) reports20/20 Product cells,154 assertions and7/7 protocol cases,46 assertions against frozen normal installed source `c2616370800f5f6df45246b0890ba35a7cd2faab`. The exact qualification model is `gpt-5.6-luna`. Every passing Product case confirms cleanup. Original failures remain recorded; automatic behavior retries are disabled. The route-only authorization repair changes no bound runtime/profile bytes and does not relabel the frozen campaign. The ordinary installed remote journey passed saved-token selection, model discovery, normal UI save/reopen, metadata-only Test, completed task, accepted receipt and exact19-byte browser download. An open model catalog caused no additional discovery requests during117seconds. Actual installed runtime smokes passed on macOS ARM64, macOS x64 under Rosetta and native Linux x64 on Daytona. Raw token browser entry was not exercised; encrypted connection creation used the normal API. The first Daytona file attempt timed out during delayed semantic dispatch; its precise stalled layer remains unresolved. Independent ordinary remote delivery and the other19 Product cells passed before one instrumented recheck passed8/8 with unchanged source, prompt, assertions and deadlines. No causal latency repair is claimed. Before the authorization repair, all four final PR heads passed required verify/E2E and fresh Greptile5/5 reviews with zero unresolved threads. Current-head CI/review is running for the two repaired heads. Local focused checks retain129 connection,59 runner,72 shared-eval and88 recovery/workspace passes, plus migration safety and token gates. The additive Copilot migration is361; released master history is preserved. [Cloud normal-package run39414239704](https://github.com/paperclipai/paperclip/actions/runs/39414239704) builds the pre-security source4f3f. All four native producers and Linux package assembly pass; the final clean installed consumers are finishing. All18 retained archives match their hashes;37,863 regular files contain zero selected credentials. That package does not contain the later route repair. The actual final published package must include it and pass the local/Daytona live canaries. The frozen Daytona image remains published and independently verified. No separate full-image scan is claimed. Docker builds run in the cloud. **Remaining:** current-head CI/review, setup and qualification merges, final publication, and ordinary local/Daytona canaries against the actual shipped package. The user explicitly authorizes the maintainer review override; CI remains a merge gate. No production deployment is claimed. Provider USD remains unknown and GitHub-attributed. ## Risks Bad credentials, missing entitlement, runtime installation and unavailable models must remain distinct failures. A connection must not adopt ambient credentials. The constraint migration changes accepted provider values, with no credential-storage migration. Current-master NewAgent fixture extraction is retained. Copilot cases use that shared support and test both passive discovery and explicit reconnect under the same setup lifecycle. The onboarding retry regression shares the component suite’s saved-connection, authenticated-model, draft, agent-list, hire and render lifecycle fixtures. Keeping it beside the existing onboarding failures makes adoption versus duplicate hire observable under the same lifecycle; this is the concrete cohesion reason for the existing large test file. ## Model Used OpenAI Codex, based on GPT-6. The exact serving model ID and context window are not exposed in this task. Used for code changes, review and tool-assisted verification. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(decisions): add first-class propose mode (#10010)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Agents can currently perform many mutations directly, while humans often need a durable review point before cross-issue or destructive actions occur > - Existing approvals and issue-thread interactions do not provide a standalone, reusable object for presenting options, collecting typed inputs, detecting stale targets, and auditing effect execution > - The control plane therefore needs a first-class propose mode that separates an agent's recommendation from the governed mutation it may cause > - This pull request adds Decisions v1 across the database, shared contracts, server execution and telemetry, agent skill guidance, and operator UI > - The benefit is that agents can propose multi-option actions safely while operators get explicit provenance, fail-closed execution, per-effect results, and a focused attention workflow ## Linked Issues or Issue Description ### Subsystem affected Cross-cutting: `packages/db`, `packages/shared`, `server`, and `ui`. ### Problem or motivation Agents need a governed way to propose consequential work without immediately mutating issues, especially when one choice can affect several issue trees. Existing approvals and issue-thread interactions do not provide a standalone object with typed options, target snapshots, effect-level authorization, expiration, execution outcomes, and reusable attention-feed presentation. ### Proposed solution Add first-class Decisions that store options and typed inputs, surface open proposals in the operator attention feed, validate target freshness and the origin-agent/operator authorization intersection at decision time, execute a bounded set of auditable effects, and retain terminal outcomes. Decisions v1 supports comments, status and assignee changes, follow-up issue creation, blocker resolution, and issue-tree cancellation, plus bundle grouping, expiration/dismissal, rule-key telemetry, and agent-facing API guidance. ### Alternatives considered - Extend approvals with arbitrary effects: rejected because approvals represent governed yes/no actions and would become an unsafe generic mutation envelope. - Model every proposal as an issue-thread interaction: rejected because decisions can span several targets and need independent lifecycle, telemetry, idempotency, and effect results. - Let agents perform the mutation and ask for retrospective review: rejected because it removes the pre-execution governance boundary this feature is meant to provide. ### Roadmap alignment Aligns with `ROADMAP.md` sections **Agent Reviews and Approvals**, **Enforced Outcomes**, **MCP Tool Gateway & Apps (governed tool access)**, and **Activity History** by making explicit decisions, authorization gates, auditable execution, and terminal outcomes first-class control-plane objects. ### Additional context This does not replace existing approvals or issue-thread interactions, and it does not add an unrestricted generic mutation effect. ## What Changed - Added company-scoped decision, option, target, and effect-execution schema plus migration and shared TypeScript/Zod contracts. - Added decision routes and services for propose, list/get, decide, dismiss, cancel, target freshness checks, authorization intersection, idempotency, activity logging, and execution auditing. - Added rule-key decision telemetry and attention-feed metadata so open decisions are visible and measurable. - Added agent skill documentation for proposing and resolving decisions through the Paperclip API. - Added the Decisions UI: API client, query keys, inline attention resolver, bundle grouping, target-issue strip, terminal history, destructive confirmation, and per-effect result rendering. - Added server service coverage, DecisionCard state tests, and Storybook stories for the supported visual states. ## Verification - `pnpm -r typecheck` — passed. - `pnpm test:run` — 2,876 passed, 1 skipped, with one unrelated cross-suite cleanup-order failure in `heartbeat-responsible-user-invariant.test.ts`; the failing file passes in isolation (`6/6`). - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/heartbeat-responsible-user-invariant.test.ts` — passed. - `pnpm --filter @paperclipai/ui exec vitest run src/components/DecisionCard.test.tsx` — passed (`9/9`). - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/authz-existence-oracle-guard.test.ts src/__tests__/openapi-routes.test.ts` — passed (`5/5`). - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/decisions-service.test.ts` — passed (`16/16`). - `pnpm --filter paperclipai exec vitest run src/__tests__/company-import-export-e2e.test.ts` — passed (`1/1`). - `pnpm --filter @paperclipai/server typecheck` and `pnpm --filter paperclipai typecheck` — passed. - `pnpm build` — passed. - Rebased-head focused suite — passed (`6` files, `88` tests): shared decision contracts, Decisions service, OpenAPI routes, startup feedback export, DecisionCard states, and attention helpers. The follow-up stale-secondary-target regression passes in the DecisionCard suite (`10/10`). - Rebased-head scoped typechecks — passed for `@paperclipai/shared`, `@paperclipai/db`, `@paperclipai/server`, and `@paperclipai/ui`. - Rebased-head migration numbering and safety checks — passed after renumbering the additive migration to `0193` and making it replay-safe for environments that applied the earlier feature-branch number. - `pnpm check:token-gates` — passed with all gates clean. - GitHub PR workflow and Greptile review for `1f9f7645882d05dfdd9c99377c03a1f53f20e8be` — running after the stale-secondary-target fix and PR metadata refresh on July 27, 2026. - `pnpm --filter @paperclipai/ui build-storybook` exposes an existing Storybook version mismatch (`storybook` 10.4.6 vs `@storybook/addon-docs` 10.5.0); Decisions stories were validated with the docs addon temporarily disabled and the tracked config remains unchanged. ## Risks - **Migration:** Adds replay-safe migration `0193`; migration numbering and safety checks pass. The new tables and indexes are additive. - **Authorization:** Effect execution intersects the proposing agent's permissions with the responsible user context and fails closed; mistakes could reject a valid proposal rather than silently over-authorize it. - **Concurrency:** Target snapshots and idempotency keys protect against stale or duplicate execution, but reviewers should focus on mixed-effect partial outcomes and retry behavior. - **UI:** Decisions are integrated into the existing attention feed rather than a separate navigation surface, reducing routing risk but increasing the importance of attention-item metadata compatibility. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. [codex] Add built-in agents and Reflection Coach bundle (#9206)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Operators need first-party agent capabilities for repeatable company work, not just manually created one-off agents. > - Built-in agents need to behave like normal company-scoped agents while preserving approval gates, permissions, budgets, and audit trails. > - Reflection and coaching work also needs bundled instructions, skill content, and a routine so the feature can be installed and reset predictably. > - The API, database, UI, portability, and tests all need to agree on the built-in lifecycle from not provisioned through setup, approval, ready, paused, and reset. > - This pull request adds built-in agent provisioning and the Reflection Coach bundle end-to-end. > - The benefit is a safer first-party path for Paperclip-managed agents without bypassing the same governance model used for operator-created agents. ## Linked Issues or Issue Description No public GitHub issue was found for this exact built-in agent and Reflection Coach bundle work. Problem/motivation: - Paperclip did not have a first-party built-in agent lifecycle for product-owned agents. - Bundled agent resources such as default instructions, skills, and routines needed managed ownership and reset semantics. - Approval-gated companies needed built-in setup to preserve requested adapter, budget, manager, and permission state through board approval. - The board UI needed clear built-in badges, setup affordances, readiness state, and bundle status without exposing secrets. Proposed solution: - Add a company-scoped built-in agent registry, provisioning/reset/reconcile/status APIs, and Reflection Coach bundled resources. - Track bundled managed resources in the database with idempotent migration behavior. - Reuse existing agent approval, authorization, budget, and activity-log paths instead of creating a bypass. - Add UI setup, badges, gates, bundle panels, and route coverage for built-in agents. Duplicate search: - Searched GitHub PRs for `built-in agents Reflection Coach repo:paperclipai/paperclip`; only this PR was returned. - Searched GitHub issues for the same query; no public issues were returned. ## What Changed - Added built-in agent definitions, lifecycle state derivation, provisioning, reset, reconcile, status, and routine-control routes. - Added the `built_in_managed_resources` migration and schema exports for bundled instructions, skill, and routine ownership. - Added the Reflection Coach built-in bundle with default instructions, skill catalog content, routine template, default permissions, and managed-resource drift handling. - Added approval-aware provisioning behavior that preserves requested adapter config, budgets, manager assignment, and built-in permissions through hire approval. - Added authorization and mutation gates for built-in agent and skill changes, including consented Reflection Coach change paths. - Added UI surfaces for built-in agent setup, roster/detail badges, readiness gates, bundle status, routine controls, and route filtering. - Added company import/export and validator coverage for built-in managed resources and low-trust/red-team presets. - Addressed Greptile follow-ups for pending approval reconciliation, consent-gate error propagation, config-read authorization fallback, approval-path manager preservation, and non-model adapter provisioning. ## Verification Local verification: - `git diff --check public/master..HEAD` passed. - `pnpm check:token-gates` passed with all gates clean. - `pnpm exec vitest run ui/src/components/ConfigureBuiltInAgentModal.test.tsx` passed: 1 file, 4 tests. - `pnpm exec vitest run ui/src/components/EntityRow.test.tsx ui/src/pages/Agents.test.tsx ui/src/components/BuiltInAgentGate.test.tsx ui/src/components/ConfigureBuiltInAgentModal.test.tsx ui/src/components/BuiltInBundlePanel.test.tsx ui/src/pages/InstanceExperimentalSettings.test.tsx ui/src/pages/Routines.test.tsx` passed: 7 files, 64 tests. - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/built-in-agents.test.ts src/__tests__/authorization-service.test.ts src/__tests__/company-skills-routes.test.ts` passed: 3 files, 91 tests. - `pnpm --filter @paperclipai/db check:migrations` passed. - `pnpm -r typecheck` passed after the rebase; `pnpm --filter ui typecheck` passed after the final UI review fix. Remote verification on latest head `1c61f693a4ec881d739022b0e75a8ca8bf8c2cd8`: - Merge state: `CLEAN`. - Greptile: `5/5`, zero unresolved Greptile threads. - PR check rollup: all checks successful, neutral, or skipped as expected. - Passing gates include Build, Typecheck + Release Registry, all server shards, all workspace shards, all serialized server suites, e2e, Canary Dry Run, policy, review, verify, Socket, Superagent, and Snyk. ## Risks - This adds a new managed-resource table and migration; the migration uses idempotent create/add/index guards and passed migration safety checks. - Built-in agent provisioning touches approval and authorization paths; tests cover pending approval preservation, stale retry rejection, consent gates, and config-read fallback behavior. - Reflection Coach creates managed instructions, skill, and routine resources; drift/reset behavior is covered by service tests and redacted API responses. - Non-model adapter setup now provisions a `needs_setup` built-in row before command/endpoint fields are complete; this matches the server lifecycle and is covered by the setup modal regression test. ## Model Used OpenAI Codex coding agent based on GPT-5. Exact hosted model ID, context-window size, and reasoning-mode labels are not exposed in this runtime; tool use, shell execution, GitHub CLI/API access, and local code editing were enabled.

[codex] Split backend control-plane QoL slice (#4700)
## Thinking Path > - Paperclip is the control plane for autonomous AI companies, so backend task ownership, recovery, review visibility, and company-scoped limits need to stay enforceable without UI-only coupling. > - Closed PR #4692 bundled those backend changes with UI workflow, docs, skills, workflow, and lockfile churn. > - PAP-2694 asks for a clean backend/control-plane slice from that closed branch. > - This branch starts from current `master` and mines only the `cli`, `packages/db`, `packages/shared`, and `server` contracts/tests needed for the backend behavior. > - It explicitly excludes UI workflow/performance work, `.github/workflows/pr.yml`, `pnpm-lock.yaml`, docs, skills, package-script, adapter UI build-config, and perf fixture script changes; the only UI files are fixture/test updates required by the tightened shared `Company` contract. > - The benefit is a smaller reviewable PR that preserves the control-plane fixes while staying under Greptile s 100-file review limit. ## What Changed - Added company-scoped attachment-size limits through DB schema/migrations, shared company portability contracts, CLI import/export coverage, and server attachment upload enforcement. - Added productivity review service/API behavior for no-comment streak, long-active, and high-churn review issues, including request-depth clamping and issue summary exposure. - Hardened issue ownership and recovery/control-plane paths: peer-agent mutation denial, issue tree pause/resume behavior, stranded recovery origins, and related activity/test coverage. - Preserved related backend contract updates for routine timestamp variables and managed agent instruction bundles because they live in shared/server contracts from the source branch. - Addressed Greptile feedback by making `Company.attachmentMaxBytes` non-optional, simplifying review request-depth clamping, fixing the migration final newline, and enforcing the process-level attachment cap as the final ceiling for uploads. - Added minimal company fixtures needed for repo-wide typecheck/build and kept the PR to 66 changed files with forbidden/non-slice paths excluded. ## Verification - `pnpm install --frozen-lockfile` - `git diff --check origin/master..HEAD` - `git diff --name-only origin/master..HEAD | wc -l` -> 66 files - `git diff --name-only origin/master..HEAD -- .github/workflows/pr.yml pnpm-lock.yaml package.json doc skills .agents scripts packages/adapters` -> no output - `pnpm exec vitest run --config vitest.config.ts packages/shared/src/validators/issue.test.ts packages/shared/src/routine-variables.test.ts packages/shared/src/adapter-types.test.ts cli/src/__tests__/company-import-export-e2e.test.ts cli/src/__tests__/company.test.ts server/src/__tests__/productivity-review-service.test.ts server/src/__tests__/issue-tree-control-service.test.ts server/src/__tests__/issue-tree-control-routes.test.ts server/src/__tests__/issue-agent-mutation-ownership-routes.test.ts server/src/__tests__/issue-attachment-routes.test.ts server/src/__tests__/heartbeat-process-recovery.test.ts server/src/__tests__/issues-service.test.ts` -> 12 files, 147 tests passed - `pnpm exec vitest run --config vitest.config.ts cli/src/__tests__/company-delete.test.ts cli/src/__tests__/company-import-export-e2e.test.ts server/src/__tests__/productivity-review-service.test.ts` -> 3 files, 18 tests passed - `pnpm exec vitest run --config vitest.config.ts server/src/__tests__/issue-attachment-routes.test.ts` -> 1 file, 6 tests passed - `pnpm --filter @paperclipai/db typecheck && pnpm --filter @paperclipai/shared typecheck && pnpm --filter @paperclipai/server typecheck && pnpm --filter paperclipai typecheck` - `pnpm --filter @paperclipai/server typecheck` - `pnpm --filter @paperclipai/ui typecheck && pnpm --filter @paperclipai/ui build` ## Risks - Includes migrations `0073_shiny_salo.sql` and `0074_striped_genesis.sql`; merge ordering matters if another PR adds migrations first. - This is intentionally backend-only apart from fixture/test updates forced by shared type correctness; UI affordances from PR #4692 are not present here and should land in separate UI slices. - The worktree install emitted plugin SDK bin-link warnings for unbuilt plugin packages, but the targeted tests and package typechecks completed successfully. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected; check the roadmap first. test: isolate CLI company import e2e state (#4560)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies, and its CLI import/export path is part of how operators move company state safely between environments. > - The `paperclipai company import/export` e2e test is supposed to validate that portability flow inside a hermetic harness, not against a developer's live Paperclip home. > - This regression showed nested CLI subprocesses could silently fall back to ambient `PAPERCLIP_*` state and mutate a real local instance by creating extra companies such as `CLI-1-Roundtrip-Test`. > - The first job was to pin the test subprocesses to isolated config, home, instance, auth, and context paths, and to add a regression assertion that proves the nested CLI writes stay inside the test-owned state. > - Once the PR was up, CI and Greptile exposed two follow-on issues that were blocking merge: plugin SDK typecheck bootstrap was racing across packages in fresh CI, and the new lock helper needed one more fix to release its lock on failure. > - This pull request therefore ends up doing two tightly related things: fixing the original CLI isolation leak, and hardening the supporting typecheck/bootstrap path enough for the fix to verify cleanly in CI. > - The benefit is that the portability e2e test is now actually isolated, and the PR verification path is stable enough to catch regressions instead of introducing its own nondeterministic failures. ## What Changed - Hardened `cli/src/__tests__/company-import-export-e2e.test.ts` so nested CLI subprocesses re-seed isolated `PAPERCLIP_CONFIG`, `PAPERCLIP_HOME`, `PAPERCLIP_INSTANCE_ID`, `PAPERCLIP_CONTEXT`, `PAPERCLIP_AUTH_STORE`, and throwaway `HOME` values instead of falling back to ambient machine state. - Added a regression assertion around `paperclipai context set --json`, then cleared the temporary `context.json` so the isolation check and the later export/import flow stay independent. - Passed the same isolated `HOME` into the server subprocess so both sides of the e2e harness are symmetric. - Introduced locking in `scripts/ensure-plugin-build-deps.mjs` and switched the server/plugin example `typecheck` scripts to use that helper instead of launching concurrent raw `@paperclipai/plugin-sdk` builds. - Fixed the helper failure path so it releases the lock before exiting non-zero, which prevents stale-lock timeouts during parallel typecheck runs. ## Verification - `pnpm vitest run cli/src/__tests__/company-import-export-e2e.test.ts --project paperclipai` - `pnpm --filter paperclipai typecheck` - `pnpm -r typecheck` - PR checks now pass on the current head, including `policy`, `verify`, `e2e`, `security/snyk`, and `Greptile Review`. ## Risks - Low risk. The product-facing behavior change is scoped to test harness code in the CLI e2e suite. - The CI stabilization changes only affect bootstrap/typecheck helper paths for the server and plugin/example packages, but they do touch shared verification plumbing; the main risk is changing how fresh build artifacts are prepared in local/CI typecheck runs. ## Model Used - Anthropic Claude via Paperclip `claude_local`, model `claude-opus-4-7`, high-effort local coding agent, used for the initial implementation and first peer-reviewed verification. - OpenAI Codex via Paperclip `codex_local`, model `gpt-5.4`, high reasoning-effort local coding agent with tool use, used for CI triage, Greptile follow-up fixes, verification, and PR maintenance. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge |

[codex] Stabilize tests and local maintenance assets (#4423)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies > - A fast-moving control plane needs stable local tests and repeatable local maintenance tools so contributors can safely split and review work > - Several route suites needed stronger isolation, Codex manual model selection needed a faster-mode option, and local browser cleanup missed Playwright's headless shell binary > - Storybook static output also needed to be preserved as a generated review artifact from the working branch > - This pull request groups the test/local-dev maintenance pieces so they can be reviewed separately from product runtime changes > - The benefit is more predictable contributor verification and cleaner local maintenance without mixing these changes into feature PRs ## What Changed - Added stable Vitest runner support and serialized route/authz test isolation. - Fixed workspace runtime authz route mocks and stabilized Claude/company-import related assertions. - Allowed Codex fast mode for manually selected models. - Broadened the agent browser cleanup script to detect `chrome-headless-shell` as well as Chrome for Testing. - Preserved generated Storybook static output from the source branch. ## Verification - `pnpm exec vitest run src/__tests__/workspace-runtime-routes-authz.test.ts src/__tests__/claude-local-execute.test.ts --config vitest.config.ts` from `server/` passed: 2 files, 19 tests. - `pnpm exec vitest run src/server/codex-args.test.ts --config vitest.config.ts` from `packages/adapters/codex-local/` passed: 1 file, 3 tests. - `bash -n scripts/kill-agent-browsers.sh && scripts/kill-agent-browsers.sh --dry` passed; dry-run detected `chrome-headless-shell` processes without killing them. - `test -f ui/storybook-static/index.html && test -f ui/storybook-static/assets/forms-editors.stories-Dry7qwx2.js` passed. - `git diff --check public-gh/master..pap-2228-test-local-maintenance -- . ':(exclude)ui/storybook-static'` passed. - `pnpm exec vitest run cli/src/__tests__/company-import-export-e2e.test.ts --config cli/vitest.config.ts` did not complete in the isolated split worktree because `paperclipai run` exited during build prep with `TS2688: Cannot find type definition file for 'react'`; this appears to be caused by the worktree dependency symlink setup, not the code under test. - Confirmed this PR does not include `pnpm-lock.yaml`. ## Risks - Medium risk: the stable Vitest runner changes how route/authz tests are scheduled. - Generated `ui/storybook-static` files are large and contain minified third-party output; `git diff --check` reports whitespace inside those generated assets, so reviewers may choose to drop or regenerate that artifact before merge. - No database migrations. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI Codex coding agent based on GPT-5, with shell, git, Paperclip API, and GitHub CLI tool use in the local Paperclip workspace. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge Note: screenshot checklist item is not applicable to source UI behavior; the included Storybook static output is generated artifact preservation from the source branch. --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

[codex] Add structured issue-thread interactions (#4244)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies. > - Operators supervise that work through issues, comments, approvals, and the board UI. > - Some agent proposals need structured board/user decisions, not hidden markdown conventions or heavyweight governed approvals. > - Issue-thread interactions already provide a natural thread-native surface for proposed tasks and questions. > - This pull request extends that surface with request confirmations, richer interaction cards, and agent/plugin/MCP helpers. > - The benefit is that plan approvals and yes/no decisions become explicit, auditable, and resumable without losing the single-issue workflow. ## What Changed - Added persisted issue-thread interactions for suggested tasks, structured questions, and request confirmations. - Added board UI cards for interaction review, selection, question answers, and accept/reject confirmation flows. - Added MCP and plugin SDK helpers for creating interaction cards from agents/plugins. - Updated agent wake instructions, onboarding assets, Paperclip skill docs, and public docs to prefer structured confirmations for issue-scoped decisions. - Rebased the branch onto `public-gh/master` and renumbered branch migrations to `0063` and `0064`; the idempotency migration uses `ADD COLUMN IF NOT EXISTS` for old branch users. ## Verification - `git diff --check public-gh/master..HEAD` - `pnpm exec vitest run packages/adapter-utils/src/server-utils.test.ts packages/mcp-server/src/tools.test.ts packages/shared/src/issue-thread-interactions.test.ts ui/src/lib/issue-thread-interactions.test.ts ui/src/lib/issue-chat-messages.test.ts ui/src/components/IssueThreadInteractionCard.test.tsx ui/src/components/IssueChatThread.test.tsx server/src/__tests__/issue-thread-interaction-routes.test.ts server/src/__tests__/issue-thread-interactions-service.test.ts server/src/services/issue-thread-interactions.test.ts` -> 9 files / 79 tests passed - `pnpm -r typecheck` -> passed, including `packages/db` migration numbering check ## Risks - Medium: this adds a new issue-thread interaction model across db/shared/server/ui/plugin surfaces. - Migration risk is reduced by placing this branch after current master migrations (`0063`, `0064`) and making the idempotency column add idempotent for users who applied the old branch numbering. - UI interaction behavior is covered by component tests, but this PR does not include browser screenshots. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. ## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies. > - The REST API is the control-plane boundary for companies, agents, plugins, adapters, costs, invites, and issue mutations. > - Several routes still relied on broad board or company access checks without consistently enforcing the narrower actor, company, and active-checkout boundaries those operations require. > - That can allow agents or non-admin users to mutate sensitive resources outside the intended governance path. > - This pull request hardens the route authorization layer and adds regression coverage for the audited API surfaces. > - The benefit is tighter multi-company isolation, safer plugin and adapter administration, and stronger enforcement of active issue ownership. ## What Changed - Added route-level authorization checks for budgets, plugin administration/scoped routes, adapter management, company import/export, direct agent creation, invite test resolution, and issue mutation/write surfaces. - Enforced active checkout ownership for agent-authenticated issue mutations, while preserving explicit management overrides for permitted managers. - Restricted sensitive adapter and plugin management operations to instance-admin or properly scoped actors. - Tightened company portability and invite probing routes so agents cannot cross company boundaries. - Updated access constants and the Company Access UI copy for the new active-checkout management grant. - Added focused regression tests covering cross-company denial, agent self-mutation denial, admin-only operations, and active checkout ownership. - Rebased the branch onto `public-gh/master` and fixed validation fallout from the rebase: heartbeat-context route ordering and a company import/export e2e fixture that now opts out of direct-hire approval before using direct agent creation. - Updated onboarding and signoff e2e setup to create seed agents through `/agent-hires` plus board approval, so they remain compatible with the approval-gated new-agent default. - Addressed Greptile feedback by removing a duplicate company export API alias, avoiding N+1 reporting-chain lookups in active-checkout override checks, allowing agent mutations on unassigned `in_progress` issues, and blocking NAT64 invite-probe targets. ## Verification - `pnpm exec vitest run server/src/__tests__/issues-goal-context-routes.test.ts cli/src/__tests__/company-import-export-e2e.test.ts` - `pnpm exec vitest run server/src/__tests__/plugin-routes-authz.test.ts server/src/__tests__/adapter-routes-authz.test.ts server/src/__tests__/agent-permissions-routes.test.ts server/src/__tests__/company-portability-routes.test.ts server/src/__tests__/costs-service.test.ts server/src/__tests__/invite-test-resolution-route.test.ts server/src/__tests__/issue-agent-mutation-ownership-routes.test.ts server/src/__tests__/agent-adapter-validation-routes.test.ts` - `pnpm exec vitest run server/src/__tests__/issue-agent-mutation-ownership-routes.test.ts` - `pnpm exec vitest run server/src/__tests__/invite-test-resolution-route.test.ts` - `pnpm -r typecheck` - `pnpm --filter server typecheck` - `pnpm --filter ui typecheck` - `pnpm build` - `pnpm test:e2e -- tests/e2e/onboarding.spec.ts tests/e2e/signoff-policy.spec.ts` - `pnpm test:e2e -- tests/e2e/signoff-policy.spec.ts` - `pnpm test:run` was also run. It failed under default full-suite parallelism with two order-dependent failures in `plugin-routes-authz.test.ts` and `routines-e2e.test.ts`; both files passed when rerun directly together with `pnpm exec vitest run server/src/__tests__/plugin-routes-authz.test.ts server/src/__tests__/routines-e2e.test.ts`. ## Risks - Medium risk: this changes authorization behavior across multiple sensitive API surfaces, so callers that depended on broad board/company access may now receive `403` or `409` until they use the correct governance path. - Direct agent creation now respects the company-level board-approval requirement; integrations that need pending hires should use `/api/companies/:companyId/agent-hires`. - Active in-progress issue mutations now require checkout ownership or an explicit management override, which may reveal workflow assumptions in older automation. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used OpenAI Codex, GPT-5 coding agent, tool-using workflow with local shell, Git, GitHub CLI, and repository tests. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [ ] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

Fix embedded Postgres initdb failure in Docker slim containers
The embedded-postgres library hardcodes --lc-messages=en_US.UTF-8 and strips the parent process environment when spawning initdb/postgres. In slim Docker images (e.g. node:20-bookworm-slim), the en_US.UTF-8 locale isn't installed, causing initdb to exit with code 1. Two fixes applied: 1. Add --lc-messages=C to all initdbFlags arrays (overrides the library's hardcoded locale since our flags come after in the spread) 2. pnpm patch on embedded-postgres to preserve process.env in spawn calls, preventing loss of PATH, LD_LIBRARY_PATH, and other vars Co-Authored-By: Paperclip <noreply@paperclip.ing> |
