DottaandPaperclip 56ead750d3 feat(server): add scoped chat milestone recovery worker 
Reconstruct issue-scoped projection work with the shared coordinator. Signal existing run-status, presentation, and question-receipt transactions; verify real cancellation and continuation producers. Keep production polling until remaining producer coverage is complete.

DottaandPaperclip 362bf75b8f refactor(server): separate chat lifecycle and native progress projection 
Preserve combined reconciliation and its shared budget. Retain PostgreSQL timestamp precision so rejected lifecycle candidates cannot pin pagination.
