1477d1ecea test: remove the no-op sequential describe modifier (#15286)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Paperclip runs the server and runner test suites with Vitest.
> - Vitest 5 removes the deprecated `describe.sequential` property, so
the pending Vitest 5 upgrade fails the type-check and test jobs.
> - `describe.sequential` only changes behaviour inside a
`describe.concurrent` suite, or when `sequence.concurrent` is on.
> - This repository has neither, so the modifier changed nothing at run
time.
> - The benefit is that the Vitest 5 upgrade can land, and the test
files lose a modifier that did no work.

## Linked Issues or Issue Description

Refs: #12969

## What Changed

- Replace every `describe.sequential` use with a plain `describe` call.
- Drop the `{ concurrent: false }` suite option from the two runner test
files.
- Add a comment to `server/vitest.config.ts` that records why these
suites must run one test at a time.
- Leave the package manifests and the lockfile unchanged.

## Why the modifier did nothing

The Vitest documentation states that `describe.sequential` is useful to
run tests in sequence inside a `describe.concurrent` suite, or with the
`--sequence.concurrent` option. `sequence.concurrent` defaults to
`false`.

This repository satisfies neither condition:

- No test file uses `describe.concurrent`, `it.concurrent`, or
`test.concurrent`.
- `server/vitest.config.ts` sets `sequence.concurrent: false`, with
`maxWorkers: 1`, `maxConcurrency: 1`, and `isolate: true`.
- `packages/paperclip-runner/vitest.config.ts` sets no `sequence` block,
so the `false` default applies.

`packages/db` and `cli` already run the same embedded-Postgres suites
with a plain `describe`, and those jobs are green. The server package
was the only outlier.

The modifier did carry one real piece of knowledge: these suites need
their tests to run one at a time. The new comment in
`server/vitest.config.ts` records that reason next to the setting that
enforces it.

## Verification

- `git grep` for `describe.sequential` returns nothing outside
`node_modules`.
- The author ran the changed server test files under the installed
Vitest 4, and the results match the results without this change.
- Two very large embedded-Postgres test files exceeded the author's
local memory limit, so the CI test jobs cover those two.
- The two changed runner test files have pre-existing local failures
caused by a missing Rust toolchain and a missing global `pnpm` binary.
The failures are identical with and without this change.
- The author type-checked the changed files and found no new error.
- CI must pass the typecheck, build, server test, and runner verify
jobs.

## Risks

- Low risk. Suite execution stays serial, because the Vitest config
enforces it.
- The change adds no dependency and changes no package manifest or
lockfile.
- A future change that turns `sequence.concurrent` on would break these
suites. The new config comment warns against it.

## Model Used

- Claude Sonnet 5 — code edits and local verification.
- OpenAI Codex, GPT-5 — the earlier revision of this branch.

## Test plan

- [x] Every CI check reaches a terminal green state. A pending or queued
check is not a pass.
- [x] The `Typecheck + Release Registry` job passes. This change must
not introduce a type error.
- [x] The `Build` job passes.
- [x] The server test jobs and the runner verify jobs pass.
- [x] Greptile re-reviews this commit set and posts a passing verdict.
The dependabot waiver does not apply to this pull request.
- [x] `mergeable` reads `MERGEABLE` as a terminal value.

Devin FoleyandPaperclip f858207161 Guard routine UUID lookups without narrowing valid inputs (#15313)
## Thinking Path

> - Paperclip manages work for AI agents and their companies.
> - Routines expose details, triggers, and management actions through
resource IDs.
> - These resource IDs are UUIDs. The UI and CLI do not resolve short
prefixes.
> - A malformed ID reaches a PostgreSQL UUID comparison and returns a
server error.
> - A lookup guard can return the existing not-found response before
that query.
> - This PR preserves valid PostgreSQL UUID input forms and the existing
access checks.

## Linked Issues or Issue Description

Refs #11471. This is a credited continuation of the lookup-guard
approach from @mv2woods. That older PR remains open and unchanged. Its
review requested the required PR description sections. This continuation
uses current master and adds UUID compatibility and authorization
coverage.

The earlier `isUuidLike` guard restricts UUID versions to 1–5 and trims
input. The database currently accepts other UUID values and input forms.
This guard preserves the existing [PostgreSQL UUID input
contract](https://www.postgresql.org/docs/current/datatype-uuid.html),
including uppercase, paired braces, omitted hyphens, and hyphens after
groups of four digits. It rejects whitespace and short prefixes without
rewriting the value sent to the database.

## What Changed

- Guard the shared routine and private-trigger UUID lookups. Malformed
resource IDs return null, so existing routes return their normal 404
response.
- Keep company access, assignee permissions, body validation order, and
database error propagation unchanged.
- Add real PostgreSQL tests for stored UUIDv4, UUIDv7, nil, and max
values in six input forms. Add no-query checks for malformed input and
HTTP coverage across all 15 root-resource route handlers.
- Document complete resource IDs and distinguish them from opaque public
webhook IDs.

## Verification

- `pnpm exec vitest run server/src/__tests__/routines-service.test.ts
server/src/__tests__/routines-e2e.test.ts`: 91 passed, including the new
PostgreSQL and HTTP regressions.
- Independent review found no blockers and passed all 9 focused
PostgreSQL and HTTP regression cases on this commit.
- Full local `pnpm -r typecheck` and `pnpm build` passed on Node 24.21.0
with pnpm 9.15.4.
- Full Linux CI passed on `0a2990eca2f904335821027a322e4db868151eb3`: 53
successful checks and 2 inapplicable Storybook skips. No retries were
needed. This provides aggregate suite coverage; a duplicate full local
aggregate was not run.
- [Greptile reviewed this exact
commit](https://github.com/paperclipai/paperclip/pull/15313#issuecomment-6010222312)
at 5/5 with no findings or unresolved threads. The PR is ready for
review and has no merge conflicts.
- `git diff --check` and the added-diff secret and PII scan passed.

## Risks

Malformed routine and private-trigger IDs now return 404 instead of a
database error. Full UUIDs retain the existing company and assignee
checks. This change does not add prefix lookup, alter public webhook
IDs, or validate unrelated nested revision/thread IDs or query
parameters. No migration is required.

## Model Used

OpenAI GPT-6 through Codex, with reasoning, repository editing, code
execution, and independent agent review. The exact deployment model ID
and context window are not exposed in this session.

DottaandPaperclip f589660ec0 feat(routines): add safe webhook setup and in-routine run management (#13637)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Routines turn scheduled work and external events into tasks for an
assigned agent.
> - Webhook setup was disabled, and actor authentication rejected valid
webhook bearer keys.
> - Operators need to connect and test a sending app before events can
start work.
> - This pull request adds a guided setup with durable connection tests
that cannot dispatch a task.
> - It keeps trigger management, execution tasks, and activity within
the routine.
> - The benefit is a webhook that can be configured, verified, and
operated from one place.

## Linked Issues or Issue Description

Fixes #11937.

Related: #13216 adds provider-specific Sentry support. This PR addresses
general routine setup and ingress. #6841 addresses legacy secret
bindings; this PR retains the existing secret service.

**Current behavior**

Webhook creation is disabled. Bearer deliveries can fail in agent
authentication before the routine checks its key. Setup has no safe
connection test. Runs and Activity send the operator away from the
routine.

**Proposed behavior**

Choose a schedule or a webhook. Follow the setup steps, copy credentials
or complete agent instructions, and test delivery without creating work.
Finish setup to allow future events to start tasks. Edit or remove
compact trigger cards, undo removal, and inspect tasks and activity
inside the routine.

**Reason and benefit**

An operator can verify credentials and delivery before enabling
automatic work. Durable setup state survives refreshes and restarts.
Retry receipts prevent an old test event from starting work after
activation.

## What Changed

- Add a production trigger wizard using reusable Slack setup navigation
and footer components.
- Add schedule and webhook choices, one-time credentials, agent
instructions, and live connection feedback.
- Persist pending setup, test delivery receipts, connection status, and
reversible trigger removal.
- Keep setup checks free of routine runs, tasks, and agent wakeups.
Preserve delivery idempotency after activation.
- Add compact trigger cards, inline editing, key rotation, pause
controls, removal, and Undo.
- Keep Runs and Activity in the routine. Use the shared task list and
compact activity rows.
- Permit only exact public delivery POSTs through actor authentication.
Retain webhook authentication, JSON-object validation, and log
redaction.
- Add production-backed Storybook states and focused server, database,
and UI coverage.
- Document signing modes, setup checks, retries, rotation, HTTPS
ingress, and navigation.

## Verification

- Full workspace typecheck, build, and token gates passed on the rebased
branch. Storybook also builds.
- Focused routine, middleware, logging, shared wizard, and UI coverage
passes on the rebased branch: 195 tests across 14 files. The migration
passed on a fresh PostgreSQL database and on two repeated applications.
- Browser testing used the real app, database, and a deterministic
process worker through Tailscale HTTPS and the current Cloud proxy code.
- Verified rejected keys, safe setup deliveries, persisted state after
restart, activation, retry deduplication, key rotation, schedule
editing, removal, and Undo.
- Fresh bearer and GitHub-signed deliveries created tasks that the
worker checked out and completed. Runs and Activity stayed within the
routine.
- Current Cloud ingress tests passed. Public delivery POSTs passed
through without a browser session; management routes remained gated.
- All 54 current-head PR checks pass, including general and serialized
tests, all eight browser E2E shards, typecheck, build, runner checks,
security checks, and the canary dry run. Two optional Storybook jobs are
skipped by workflow conditions.
- Greptile is 5/5 on commit `7ea63a61e`, with no unresolved review
threads. The stale connection-status finding is fixed and covered by a
regression test.
- No production deployment was performed.

## Risks

- Migration 0281 adds three trigger columns and a test-receipt table. It
is additive and safe to reapply. Apply it before running the new server.
Existing triggers remain live by default.
- Requests without delivery IDs are new events after activation. Senders
must reuse an event's delivery ID for retries.
- Completed webhooks keep normal dispatch behavior. Their management
connection check can start work; the UI states this.
- Removing a trigger archives it. Undo restores the URL and credentials.
Permanent deletion remains available through the existing API.
- Public ingress must remain restricted to the delivery POST route. The
tenant verifies credentials. Cloud sleeping-stack behavior is unchanged.
- Shared setup components also serve Slack. Existing setup contracts and
navigation tests cover that integration.
- Senders must use application/json with an object. Other media types
receive 415.

## Model Used

OpenAI Codex, based on GPT-6, with reasoning, repository tools, shell
execution, and browser testing. The exact deployment model ID and
context-window size are not exposed in this session.

DottaandPaperclip 30ff3d7c58 feat(routines): expose activity gate API (#9438)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work
> - Scheduled routines provide recurring control-plane work without
manual intervention
> - The new activity gate can suppress scheduled runs when no external
work occurred
> - The core scheduler and database support landed without a public
create/update contract
> - Agents, operators, and managed plugins need validated fields plus
discoverable semantics to opt in safely
> - This pull request exposes the activity gate through routine APIs,
revisions, plugin contracts, tests, and skill documentation
> - The benefit is backward-compatible control over idle scheduled work
without losing activity-triggered follow-up

## Linked Issues or Issue Description

- Refs #8534

## What Changed

- Added shared activity-gate policy and scope enums with create/PATCH
validation.
- Persisted activity-gate fields through routine creation, updates,
revision snapshots, pipeline snapshots, and revision restores.
- Defaulted legacy revision snapshots during restore and added
regression coverage for pre-field snapshots.
- Extended managed-plugin routine declarations, production
reconciliation, and the SDK test harness to preserve non-default gate
settings.
- Added end-to-end API coverage for create/PATCH/list/detail
round-trips, defaults, and invalid enum rejection.
- Documented schedule-only semantics, activity windows,
own-run/read-action exclusions, scopes, and an hourly quiet-night
watcher example.

## Verification

- `pnpm exec vitest run packages/shared/src/validators/routine.test.ts
server/src/__tests__/routines-service.test.ts
server/src/__tests__/routines-e2e.test.ts`
- `pnpm exec vitest run packages/shared/src/validators/plugin.test.ts

- `pnpm --filter @paperclipai/server typecheck`
- `pnpm --filter @paperclipai/ui typecheck`
- `pnpm check:token-gates`
- GitHub CI: all final-head checks green; Storybook visual regression
skipped by path rules.
- Greptile: 5/5 with no unresolved review threads.

## Risks

- Low risk: defaults remain `always` and `company`, preserving existing
routine behavior and old revision snapshots.
- Managed plugin manifests can now declare the same validated gate
settings as the public routine API; omitted values retain core defaults.
- Revision snapshots now include the new fields so policy changes are
not lost or treated as no-ops during restore.

> For core feature work, checked `ROADMAP.md`: this extends the existing
Scheduled Routines roadmap item and does not duplicate a separate
planned capability.

## Model Used

- OpenAI GPT-5.5 via Codex CLI, with repository tool use and code
execution; context-window size was not exposed by the runtime.

DottaandPaperclip 746e287e33 feat(control-plane): add annotation and workspace controls (#8229)
## Thinking Path

> - Paperclip is the open source app people use to manage AI-agent
companies.
> - The control plane coordinates issues, workspaces, documents,
routines, and board review flows across company-scoped data.
> - The local source branch contained related schema, service, and UI
changes for workspace issue scoping and document/routine annotations.
> - These changes need to move together because db schema, shared types,
server services, and UI consumers form one contract.
> - This pull request extracts the migration-bearing control-plane work
from the source branch onto `origin/master`.
> - The benefit is a standalone branch with deterministic migration
order and focused review for the highest-risk part of the split.

## Linked Issues or Issue Description

No GitHub issue exists for this branch split. Internal source task:
[PAP-11234](/PAP/issues/PAP-11234).

Problem/motivation:
- Workspace operations need explicit issue scoping so readiness and
blocker handling can be derived correctly.
- Document annotations need reliable live updates, save failure
surfacing, normalized activity keys, and better comment panel behavior.
- Routine descriptions need the same annotation contract as issue
documents so operators can discuss and edit routine text without
special-case infrastructure.

Proposed solution:
- Add the workspace-operation `issueId` migration and readiness scoping.
- Add routine document/annotation schema, shared types, services,
routes, and UI editing support.
- Keep the related migrations in one PR so the renumbered `0106` and
`0107` migrations land in a deterministic order after current `master`.

Alternatives considered:
- Split migrations into separate PRs, rejected because that would create
migration-numbering conflicts and make each branch less standalone.
- Merge this with UI polish, rejected because this branch needs deeper
server/db review.

Roadmap alignment:
- Checked `ROADMAP.md`; the roadmap mentions future recurring routine
capabilities generally, but no duplicate implementation PR for these
annotation/workspace changes was found.

## What Changed

- Added `0106_workspace_operations_issue_id.sql` and
`0107_routine_description_annotations.sql`, plus schema exports.
- Scoped workspace readiness to blocker issues and attached workspace
operation issue ids.
- Scoped issue-thread interaction accept finalization to the source run.
- Added routine document annotation contracts across
db/shared/server/UI.
- Improved document annotation live updates, activity-key normalization,
save failure surfacing, and comment panel behavior.
- Added issue workspace property controls and compact
blocked-by/quick-control UI updates.
- Added focused server and UI regression tests for the new contracts.

## Verification

- `CI=true NODE_ENV=development pnpm install --frozen-lockfile
--prefer-offline`
- `NODE_ENV=test pnpm exec vitest
server/src/__tests__/document-annotation-routes.test.ts
server/src/__tests__/issue-thread-interactions-service.test.ts

server/src/__tests__/routine-document-annotation-routes.test.ts

ui/src/components/IssueDocumentAnnotations.test.tsx
ui/src/components/IssueProperties.test.tsx
ui/src/components/WorkspaceRuntimeControls.test.tsx
ui/src/context/LiveUpdatesProvider.test.ts --run` — 10 files, 273 tests
passed.
- `NODE_ENV=test pnpm -r --filter @paperclipai/db --filter
@paperclipai/shared --filter @paperclipai/server --filter
@paperclipai/ui typecheck` — passed, including db migration numbering
check.

## Risks

- Migration-bearing PR; merge this branch before any later PR that adds
migrations with higher numbers.
- Cross-layer contract risk across db/shared/server/ui, mitigated with
targeted tests and affected-package typecheck.
- Review should pay special attention to company scoping in new
routine/document annotation paths.

DottaandPaperclip 9a8d219949 [codex] Stabilize tests and local maintenance assets (#4423)
## Thinking Path

> - Paperclip orchestrates AI agents for zero-human companies
> - A fast-moving control plane needs stable local tests and repeatable
local maintenance tools so contributors can safely split and review work
> - Several route suites needed stronger isolation, Codex manual model
selection needed a faster-mode option, and local browser cleanup missed
Playwright's headless shell binary
> - Storybook static output also needed to be preserved as a generated
review artifact from the working branch
> - This pull request groups the test/local-dev maintenance pieces so
they can be reviewed separately from product runtime changes
> - The benefit is more predictable contributor verification and cleaner
local maintenance without mixing these changes into feature PRs

## What Changed

- Added stable Vitest runner support and serialized route/authz test
isolation.
- Fixed workspace runtime authz route mocks and stabilized
Claude/company-import related assertions.
- Allowed Codex fast mode for manually selected models.
- Broadened the agent browser cleanup script to detect
`chrome-headless-shell` as well as Chrome for Testing.
- Preserved generated Storybook static output from the source branch.

## Verification

- `pnpm exec vitest run
src/__tests__/workspace-runtime-routes-authz.test.ts
src/__tests__/claude-local-execute.test.ts --config vitest.config.ts`
from `server/` passed: 2 files, 19 tests.
- `pnpm exec vitest run src/server/codex-args.test.ts --config
vitest.config.ts` from `packages/adapters/codex-local/` passed: 1 file,
3 tests.
- `bash -n scripts/kill-agent-browsers.sh &&
scripts/kill-agent-browsers.sh --dry` passed; dry-run detected
`chrome-headless-shell` processes without killing them.
- `test -f ui/storybook-static/index.html && test -f
ui/storybook-static/assets/forms-editors.stories-Dry7qwx2.js` passed.
- `git diff --check public-gh/master..pap-2228-test-local-maintenance --
. ':(exclude)ui/storybook-static'` passed.
- `pnpm exec vitest run
cli/src/__tests__/company-import-export-e2e.test.ts --config
cli/vitest.config.ts` did not complete in the isolated split worktree
because `paperclipai run` exited during build prep with `TS2688: Cannot
find type definition file for 'react'`; this appears to be caused by the
worktree dependency symlink setup, not the code under test.
- Confirmed this PR does not include `pnpm-lock.yaml`.

## Risks

- Medium risk: the stable Vitest runner changes how route/authz tests
are scheduled.
- Generated `ui/storybook-static` files are large and contain minified
third-party output; `git diff --check` reports whitespace inside those
generated assets, so reviewers may choose to drop or regenerate that
artifact before merge.
- No database migrations.

discuss it in `#dev` before opening the PR. Feature PRs that overlap

first. See `CONTRIBUTING.md`.

DottaandPaperclip a957394420 [codex] Add structured issue-thread interactions (#4244)
## Thinking Path

> - Paperclip orchestrates AI agents for zero-human companies.
> - Operators supervise that work through issues, comments, approvals,
and the board UI.
> - Some agent proposals need structured board/user decisions, not
hidden markdown conventions or heavyweight governed approvals.
> - Issue-thread interactions already provide a natural thread-native
surface for proposed tasks and questions.
> - This pull request extends that surface with request confirmations,
richer interaction cards, and agent/plugin/MCP helpers.
> - The benefit is that plan approvals and yes/no decisions become
explicit, auditable, and resumable without losing the single-issue
workflow.

## What Changed

- Added persisted issue-thread interactions for suggested tasks,
structured questions, and request confirmations.
- Added board UI cards for interaction review, selection, question
answers, and accept/reject confirmation flows.
- Added MCP and plugin SDK helpers for creating interaction cards from
agents/plugins.
- Updated agent wake instructions, onboarding assets, Paperclip skill
docs, and public docs to prefer structured confirmations for
issue-scoped decisions.
- Rebased the branch onto `public-gh/master` and renumbered branch
migrations to `0063` and `0064`; the idempotency migration uses `ADD
COLUMN IF NOT EXISTS` for old branch users.

## Verification

- `git diff --check public-gh/master..HEAD`
- `pnpm exec vitest run packages/adapter-utils/src/server-utils.test.ts
packages/mcp-server/src/tools.test.ts
packages/shared/src/issue-thread-interactions.test.ts
ui/src/lib/issue-thread-interactions.test.ts
ui/src/lib/issue-chat-messages.test.ts
ui/src/components/IssueThreadInteractionCard.test.tsx
ui/src/components/IssueChatThread.test.tsx
server/src/__tests__/issue-thread-interaction-routes.test.ts
server/src/__tests__/issue-thread-interactions-service.test.ts
server/src/services/issue-thread-interactions.test.ts` -> 9 files / 79
tests passed
- `pnpm -r typecheck` -> passed, including `packages/db` migration
numbering check

## Risks

- Medium: this adds a new issue-thread interaction model across
db/shared/server/ui/plugin surfaces.
- Migration risk is reduced by placing this branch after current master
migrations (`0063`, `0064`) and making the idempotency column add
idempotent for users who applied the old branch numbering.
- UI interaction behavior is covered by component tests, but this PR
does not include browser screenshots.

> For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and
discuss it in `#dev` before opening the PR. Feature PRs that overlap
with planned core work may need to be redirected — check the roadmap
first. See `CONTRIBUTING.md`.

## Model Used

- OpenAI Codex, GPT-5-class coding agent runtime. Exact model ID and
context window are not exposed in this Paperclip run; tool use and local
shell/code execution were enabled.

Lempkey d0a8d4e08a fix(routines): include cronExpression and timezone in list trigger response (#3209)
## Thinking Path

> - Paperclip orchestrates AI agents for zero-human companies
> - Routines are recurring tasks that trigger agents on a schedule or
via webhook
> - Routine triggers store their schedule as a `cronExpression` +
`timezone` in the database
> - The `GET /companies/:companyId/routines` list endpoint is the
primary way API consumers (and the UI) discover all routines and their
triggers
> - But the list endpoint was silently dropping `cronExpression` and
`timezone` from each trigger object — the DB query fetched them, but the
explicit object-construction mapping only forwarded seven other fields
> - This PR fixes the mapping to include `cronExpression` and
`timezone`, and extends the `RoutineListItem.triggers` type to match
> - The benefit is that API consumers can now see the actual schedule
from the list endpoint, and future UI components reading from the list
cache will get accurate schedule data

## What Changed

- **`server/src/services/routines.ts`** — Added `cronExpression` and
`timezone` to the explicit trigger object mapping inside
`routinesService.list()`. The DB query (`listTriggersForRoutineIds`)
already fetched all columns via `SELECT *`; the values were being
discarded during object construction.
- **`packages/shared/src/types/routine.ts`** — Extended
`RoutineListItem.triggers` `Pick<RoutineTrigger, ...>` to include
`cronExpression` and `timezone` so the TypeScript type contract matches
the actual runtime shape.
- **`server/src/__tests__/routines-e2e.test.ts`** — Added assertions to
the existing schedule-trigger E2E test that verify both `cronExpression`
and `timezone` are present in the `GET /companies/:companyId/routines`
list response.

## Verification

```bash
# Run the route + service unit tests
npx vitest run server/src/__tests__/routines-routes.test.ts server/src/__tests__/routines-service.test.ts
# → 21 tests pass
# Confirm cronExpression appears in list response
curl /api/companies/{id}/routines | jq '.[].triggers[].cronExpression'
# → now returns the actual cron string instead of undefined
```

Manual reproduction per the issue:
1. Create a routine with a schedule trigger (`cronExpression: "47 14 * *
*"`, `timezone: "America/Mexico_City"`)
2. `GET /api/companies/{id}/routines` — trigger object now includes
`cronExpression` and `timezone`

## Risks

Low risk. The change only adds two fields to an existing response shape
— no fields removed, no behavior changed. The `cronExpression` is `null`
for non-schedule trigger kinds (webhook, etc.), consistent with
`RoutineTrigger.cronExpression: string | null`. No migration required.

## Model Used

- **Provider:** Anthropic
- **Model:** Claude Sonnet 4.6 (`claude-sonnet-4-6`)
- **Context window:** 200k tokens
- **Mode:** Extended thinking + tool use (agentic)
- Secondary adversarial review: OpenAI Codex (via codex-companion
plugin)

DottaandPaperclip 7f893ac4ec [codex] Harden execution reliability and heartbeat tooling (#3679)
## Thinking Path

> - Paperclip orchestrates AI agents for zero-human companies
> - Reliable execution depends on heartbeat routing, issue lifecycle
semantics, telemetry, and a fast enough local verification loop to keep
regressions visible
> - The remaining commits on this branch were mostly server/runtime
correctness fixes plus test and documentation follow-ups in that area
> - Those changes are logically separate from the UI-focused
issue-detail and workspace/navigation branches even when they touch
overlapping issue APIs
> - This pull request groups the execution reliability, heartbeat,
telemetry, and tooling changes into one standalone branch
> - The benefit is a focused review of the control-plane correctness
work, including the follow-up fix that restored the implicit
comment-reopen helpers after branch splitting

## What Changed

- Hardened issue/heartbeat execution behavior, including self-review
stage skipping, deferred mention wakes during active execution, stranded
execution recovery, active-run scoping, assignee resolution, and
blocked-to-todo wake resumption
- Reduced noisy polling/logging overhead by trimming issue run payloads,
compacting persisted run logs, silencing high-volume request logs, and
capping heartbeat-run queries in dashboard/inbox surfaces
- Expanded telemetry and status semantics with adapter/model fields on
task completion plus clearer status guidance in docs/onboarding material
- Updated test infrastructure and verification defaults with faster
route-test module isolation, cheaper default `pnpm test`, e2e isolation
from local state, and repo verification follow-ups
- Included docs/release housekeeping from the branch and added a small
follow-up commit restoring the implicit comment-reopen helpers that were
dropped during branch reconstruction

## Verification

- `pnpm vitest run
server/src/__tests__/issue-comment-reopen-routes.test.ts
server/src/__tests__/issue-telemetry-routes.test.ts`
- `pnpm vitest run server/src/__tests__/http-log-policy.test.ts
server/src/__tests__/heartbeat-run-log.test.ts
server/src/__tests__/health.test.ts`
- `server/src/__tests__/activity-service.test.ts`,
`server/src/__tests__/heartbeat-comment-wake-batching.test.ts`, and
`server/src/__tests__/heartbeat-process-recovery.test.ts` were attempted
on this host but the embedded Postgres harness reported
init-script/data-dir problems and skipped or failed to start, so they
are noted as environment-limited

## Risks

- Medium: this branch changes core issue/heartbeat routing and
reopen/wakeup behavior, so regressions would affect agent execution flow
rather than isolated UI polish
- Because it also updates verification infrastructure, reviewers should
pay attention to whether the new tests are asserting the right failure
modes and not just reshaping harness behavior

## Model Used

- OpenAI Codex coding agent (GPT-5-class runtime in Codex CLI; exact
deployed model ID is not exposed in this environment), reasoning
enabled, tool use and local code execution enabled

dottaandPaperclip 5602576ae1 Fix embedded Postgres initdb failure in Docker slim containers
The embedded-postgres library hardcodes --lc-messages=en_US.UTF-8 and
strips the parent process environment when spawning initdb/postgres.
In slim Docker images (e.g. node:20-bookworm-slim), the en_US.UTF-8
locale isn't installed, causing initdb to exit with code 1.

Two fixes applied:
1. Add --lc-messages=C to all initdbFlags arrays (overrides the
library's hardcoded locale since our flags come after in the spread)
2. pnpm patch on embedded-postgres to preserve process.env in spawn
calls, preventing loss of PATH, LD_LIBRARY_PATH, and other vars
