Devin FoleyandPaperclip 24758b9da1 Avoid loading tool schemas for effective assignments (#16969)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Tool profiles define which tools an agent can use.
> - Run preparation and connection checks resolve these assignments from
the database.
> - These callers need tool IDs and names, but the existing lookup also
reads complete schemas and provider metadata.
> - Repeated lookups transfer data that these callers discard.
> - This pull request adds a small assignment projection through the
same permission resolver.
> - The benefit is less database transfer and JSON work with current
permission checks and the same public response.

## Linked Issues or Issue Description

**What existing behavior does this improve?**

Effective tool assignment reads during run preparation, native runtime
setup, and connection policy checks.

**Subsystem affected**

server/ — tool access and orchestration services.

**Current behavior**

Internal callers use the public effective-profile lookup. Its catalog
query reads complete tool rows, including input schemas, output schemas,
annotations, and provider metadata. These callers only use catalog IDs
and names.

**Proposed behavior**

Internal callers use an assignment lookup that selects catalog identity
and selector fields. Both lookups share one resolver. The public
effective-profile endpoint keeps its complete definitions.

**Reason and benefit**

Large catalogs increase database transfer and JSON work for every
assignment lookup. The smaller query avoids that payload without caching
permissions or changing query frequency.

**Breaking changes**

None. No route, schema, migration, permission rule, or public response
changes.

Related: Refs #16761, Refs #16723. The former narrows profile summary
reads. The latter changes how the gateway presents tool definitions.
Neither narrows this internal effective-assignment lookup.

## What Changed

- Share effective-profile resolution between full definitions and
assignment records.
- Select the existing catalog identity and selector projection for
internal assignments.
- Use the assignment lookup in run preparation, native runtime setup,
and connection policy checks.
- Add real PostgreSQL tests for selectors, exclusions, current profile
and binding state, company boundaries, and returned payload size.
- Document the database read contract.

## Verification

- Five focused test files passed: 521 tests. They cover tool access,
connection intents, runtime preparation, native runtime context, and the
new assignment lookup.
- `pnpm -r typecheck` passed.
- `pnpm check:module-boundaries` passed.
- The PostgreSQL fixture compares the full authorization result with the
assignment result. It checks all selector types, excludes removed tools,
and observes catalog, profile, and binding changes on later calls.
- The payload test measures rows returned by PostgreSQL before
application mapping. With large schema and annotation fields, the
assignment result is less than 1% of the full catalog payload. This is a
synthetic fixture result, not a fleet savings estimate.
- `pnpm build` passed.
- The full local suite is in progress. It found two Git fixture failures
that also reproduce on unchanged master with `init.defaultBranch=main`.
Both pass with a process-only `init.defaultBranch=master` setting. The
global Git configuration was not changed. This earlier failure remains
recorded; it is not counted as a clean first pass.
- Apex review: 5/5, no actionable comments. All 54 CI checks passed; two
conditional jobs were skipped.
- The broader local run also reached two UI test timeouts and one
adapter test timeout. All three passed when rerun in isolation with
unchanged source. The exact cause of these timeouts is not established.
No UI or adapter implementation is changed by this PR. The CLI
import/export journey passes its assertions but its local teardown fails
with `ENOTEMPTY` while a runtime context asset is written. Its CI check
passed. The complete local suite is not reported as a clean pass.

## Risks

- Both lookups share permission resolution. Existing tool access tests
and result parity checks cover that extraction.
- Future assignment callers must use the fields in the smaller result.
Typechecking rejects attempts to read full schemas from it.
- The lookup still scans the active company catalog and resolves current
connections on every call. This change reduces returned bytes; it does
not reduce query count or introduce a cache.
- No database migration or operational configuration change is required.

## Model Used

- OpenAI GPT-6 through Codex, with tool-assisted source inspection,
implementation, and test execution. The exact model variant and context
window are not exposed in this session.

DottaandPaperclip 2cbca4ceb0 fix: retain channel tools across task continuations (#16806)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Chat channels give an agent tools for its assigned task.
> - Each later run must resolve the task and its accepted requester
again.
> - Some native runs store the task in a database column without the
older JSON fields.
> - Tool discovery read only the JSON fields. GitHub calls also required
a new inbound comment.
> - This pull request uses the durable task binding and accepted request
history.
> - Later runs retain their channel tools while current access checks
still apply.

## Linked Issues or Issue Description

**What happened?**

A GitHub task can find `begin_review` on its first run, then receive an
empty tool list on a later native run. If discovery is repaired alone, a
call can still fail because the later run has no new inbound comment.
Slack and voice tool access also read the older JSON task fields.

**Expected behavior**

A later run for the same task can find and call its assigned channel
tools. The current requester, task, company, agent, endpoint, and access
rules must still allow the call. Revoked access must fail.

**Steps to reproduce**

1. Start a GitHub task and accept its linked requester.
2. Confirm that the runtime can find `begin_review`.
3. Start a later native run with `nativeIssueId`, accepted parent
identity, and no JSON task ID or new wake comment.
4. Search for `begin_review` again. Before this change, the search
returns an empty list.
5. Repair discovery alone and call `read_pull_request`. Before the
request-history fix, the call fails with no active conversation
capability.

**Paperclip version or commit**

Reproduced in source tests before this patch. The branch includes master
at `fa50cab5685b9cdbb26ad9c610cc5b6c373d3ff4`.

**Additional context**

This is a core run-binding bug. The tests use PostgreSQL and synthetic
provider responses. They do not send live reviews, Slack messages, or
phone calls.

## What Changed

- Resolve run task IDs from durable columns, with legacy JSON fields as
a fallback.
- Use the same binding in the gateway, access policy, identity service,
and channel runtime preparation.
- Recover accepted channel request history across later runs. Keep
company, task, agent, and requester boundaries.
- Recover older wake sources through verified parent runs, including
after same-user board follow-ups. Clear the board message’s own legacy
wake fallback. Keep cycle checks without a fixed limit on valid task
history.
- Retain the GitHub origin after a board comment from the same accepted
user. Skip GitHub origin lookup results from other providers.
- Keep ordinary board tools available when the requester changes. Accept
legacy user comments without an author-type field. Still reject explicit
malformed task IDs.
- Prevent a later failure reply from replacing an earlier final GitHub
answer. Recheck this rule under the publication lease.
- Rotate the native tool contract to replace retained sessions with an
older tool list.
- Add continuation, publication, revocation, and boundary tests. Update
the access and feature maps.

## Verification

- Red: the later GitHub assigned-tool search returned an empty list
before the binding fix.
- Red: the later GitHub read failed with no active conversation
capability before the request-history fix.
- Green: runtime MCP discovery, GitHub read/begin/submit review, and
response publication pass for monitor and same-user board continuations.
- Green: Slack history, retained access controls, revocation, and voice
gateway continuation pass.
- Green: 35 channel, failure-publication, legacy-origin, long-history,
and provenance tests pass. Both review findings had failing tests before
their fixes.
- Green: 107 gateway, operation-credential, identity, and provenance
tests pass. These cover both first-run CI failures without changing
their tests.
- Green: 122 runtime, discovery-policy, and native-session tests pass; 2
unrelated tests are skipped.
- Map checks and whitespace checks pass.
- Updated to master in `cfcaba22628772d7fe1fb32b57ff7031c7072781`. The
sole conflict was two tests added at the same location. Both the
continuation test and the publication 403/401 test are retained.
- Current local verification: all 179 gateway, credential,
requester-identity, access-policy, runtime-context, and session-resume
tests passed. Repository typecheck and build passed. Map and whitespace
checks passed.
- The new review regression first returned no source for a same-user
board follow-up over legacy wake metadata. After the repair, all 27
provenance and channel cases passed. The combined GitHub
discovery/read/begin/submit path and revoked access are covered.
- The broader local channel run passed 176 cases and failed four
timing/setup cases: a 15-second Slack authorization timeout,
webhook-verification polling, a Slack acknowledgement deadline, and
Discord setup polling. All four passed in isolation without source,
assertion, or deadline changes. The earlier failure record is retained;
the exact load/order cause is not established. Full CI must pass on the
latest commit.
- Current head: `7b9a08dcbc7b8140b589742f136944270b0496bb`. Repository
typecheck and build pass on this commit. Greptile completed on this
exact commit with 5/5; all four review threads are resolved. The
final-head [CI
run](https://github.com/paperclipai/paperclip/actions/runs/39278921653)
passed: 55 checks passed and 2 intentionally skipped, with no failures
or pending checks. This includes the full sharded test matrix, browser
tests, typecheck, build, and Runner verification. Results were delayed
during the [GitHub Actions
incident](https://www.githubstatus.com/incidents/8tzbpcnl75jp). An
additional unsharded local full test run has not completed; the
completed full-suite evidence is CI.
- Earlier verification: the first PR commit failed two server suites.
Those failures were fixed and verified before this update. The original
4 GiB sandbox could not complete full local typechecking or tests; the
current checkout completed typechecking and build. The full test suite
runs through the CI shards. No full local test-suite pass or live
provider delivery is claimed.

## Risks

- Request history can restore the wrong authority if its scope is too
broad. The resolver checks each accepted parent against the task,
company, agent, and requester. Tests cover invalid and cyclic history.
- The native contract change starts a new provider session for older
contracts.
- No database migration or public API schema changes are required. Each
tool call still checks live access.

## Model Used

- OpenAI Codex, GPT-6 family. The runtime does not expose the exact
deployment model ID or context-window size. The agent used reasoning,
repository tools, code edits, and local test execution.

Nicky LeachandPaperclip fa9f3f8a08 feat(runner): support preconfigured user homes for Codex and ACPX (#16486)
## Thinking Path

> - Paperclip manages AI agents and their work.
> - Runner launches the selected provider and keeps durable session
state.
> - Runner currently prepares private provider homes for each session.
> - Operators can already have working login, skills, and MCP
configuration on the execution machine.
> - This PR adds an explicit mode that uses that existing setup.
> - Runner keeps its task bridge and session ownership while leaving the
user's configuration in place.

## Linked Issues or Issue Description

**Problem or motivation:** Rebuilding a provider home is unnecessary
when the execution account is already configured.

**Proposed solution:** Add `homeMode: "preconfigured"` to Runner adapter
config and runnerd transport options. Keep `"isolated"` as the default.
Initially support native Codex and ACPX Claude.

**Alternatives considered:** Continue copying user configuration into a
private home, or add an approval workflow. Both add work outside this
feature's scope.

**Roadmap alignment:** This extends the existing cross-provider Runner.
It does not add another runtime.

Related work: #14056 covers managed Codex credentials. #14474 covers
command environment delivery. This mode uses user-owned credentials and
retains the existing launch boundaries.

## What Changed

- Add a closed v7 launch contract for the selected home mode. Existing
contracts retain their behavior.
- Keep homeMode under operator control. Reject task overrides and
agent-authenticated changes, including removal through replacements,
adapter changes, or rollback. Ignore stored task values and exclude it
from automatic hire inheritance.
- Skip managed AI login, assigned-skill materialization, and external
MCP setup in preconfigured mode. Preserve the Runner task/completion
bridge.
- Keep protocol state and provider lifetime fences scoped to the Runner
session. Different sessions can share a home.
- Fail preconfigured ACPX planning/read-only launches before provider
admission until a read-only user-home launch is qualified. Use isolated
mode or native Codex for planning.
- Reject ACPX approval requests. Keep native Codex on `never`. Local
permission rules and hooks remain effective. Initially require the
default Runner ACPX `approve-all` setting; other Runner policies are
unqualified in this mode and fail before launch.
- Keep the internal ACPX bridge in a stable session namespace,
preserving an existing user MCP server named `paperclip`.
- Reject preconfigured ACPX Codex before launch until a session
permission profile can enforce Runner-state denial. Remove the
unprotected full-access mode switch; use native Codex or isolated mode
for that model family.
- Bump Pi to profile 23 because its attestation covers shared Runner
source changed here. Preserve profile 22 for historical recovery, and
update Docker/package verification inputs.
- Fence recovery on mode and home bindings. Fail missing
provider-history recovery without starting a replacement session.
Exclude the external Codex home from backups and credential cleanup.
- Discover remote HOME on the execution machine. Do not project
controller HOME or XDG paths onto it.
- Separate task instructions from Codex ACP isolation and update the
generated provider digest.
- Keep remote configured-environment manifests consistent when dropping
controller paths. Preserve exact Claude model IDs through per-launch
environment settings, without editing the user home or patching the
provider.

## Verification

- Targeted local checks cover native contracts and launch allowlists,
user-config preservation, provider admission, permissions, session
recovery, remote environment projection, heartbeat preparation, and
operator-only configuration. Runner, shared, and server TypeScript
checks and the Rust binary build pass; generated ACPX profiles are
current.
- Following the latest master integration, all 757 server
configuration/executor tests and 115 Runner admission/recovery tests
passed. Additional review-fix checks passed: 103 planning contract/host
tests, 78 provider-backend tests, 95 route/preparation/inheritance
tests, 43 task-validator tests, and 43 workspace-policy tests. Counts
describe separate runs and can overlap.
- The real pinned Claude ACP/SDK regression reproduces exact-model
aliasing without the fix (`claude-sonnet-5-5` becomes `sonnet`) and
passes with the fix. All 89 adapter tests pass. User settings remain
byte-for-byte unchanged.
- Localhost-only Claude SDK probes with synthetic credentials verify the
private bridge runs, local `ask` reaches the existing reject callback,
local `deny` blocks without a callback, and a user MCP server named
`paperclip` remains available. No user model allowlist is required.
- Review regressions cover fail-closed provider admission, saved-mode
recovery before routing, controller-home path removal without
configured-environment manifest drift, pre-launch rejection of
unqualified read-only ACPX launches, and denial of task/agent changes to
the operator-owned home mode.
- Before rebase, `d4d4ee2aebe4cb88c4f4d9ef8e22fed4b36b87be` received
standard Greptile 5/5 with no unresolved threads. Prior Apex findings
are fixed and covered by regressions. No manual Greptile retrigger is
requested for this rebase, per reviewer direction.
- The prior PR head and master hit the same GitHub runner encoding crash
in workspace-b on three PR attempts, including diagnostic logging
([prior PR
CI](https://github.com/paperclipai/paperclip/actions/runs/38818474563/job/116714538405),
[prior master
CI](https://github.com/paperclipai/paperclip/actions/runs/38819528748/job/116712423193)).
Rebased onto `origin/master` at `c3ffa3f0dd` to pull in its bridge-test
index correction. The rebased head
`edd153db410bea063b645e0000aa68649f287b00` passes all CI checks ([CI
run](https://github.com/paperclipai/paperclip/actions/runs/38824676914)),
including workspace-b. Greptile's automatic standard review is 5/5 on
this exact head, with no unresolved threads; no manual review was
requested.
- Local workspace-b investigation passed 911 shared tests and 3,868
tests across the remaining 15 workspace projects. The catalog run
separately hit an existing ignored
`.agents/skills/design-guide/SKILL.md` description-budget violation in
this checkout; that file is untracked, outside the PR, and left
untouched. Repeating shared tests with CI reporting enabled also passed.
- Rebase verification: the final Git tree exactly matches the previously
reviewed branch combined with current master. All 192 Runner
admission/contract tests, 61 server preparation/provider tests, and 68
bridge tests pass, as do Runner/server TypeScript and generated-profile
checks. The real Claude adapter fixture hit an `ENOTEMPTY` cleanup race
on its initial run; all 89 adapter tests passed on retry.
- Apex on rebased head `edd153db` returned 2/5: ACPX Codex selected full
access without enforcing Runner-state denial, and agent configuration
replacement could remove the operator mode. Both are fixed in
`26e14dbbf`. Preconfigured ACPX Codex is temporarily unqualified;
re-enabling it requires session permission enforcement on thread/turn
operations and a real filesystem-denial probe. The config guard covers
effective replacements, adapter changes, and rollback while allowing
ordinary agent merges and operator removal. All 190 focused Runner
tests, 103 server tests, both typechecks, and generated-profile checks
pass. Standard review and CI passed before the next Apex request.
- CI on `26e14dbbf` passed 1,967 Runner-shard tests but hit the
previously observed `ENOTEMPTY` fixture cleanup race after successful
Claude model admission. `eb83284fb` adds bounded filesystem retries to
that fixture's teardown; all 87 adapter tests pass. Assertions and
production cleanup behavior are unchanged. Head
`eb83284fb42ee19a15d9a93b106c1bbb07e1695e` passes all 59 CI checks ([CI
run](https://github.com/paperclipai/paperclip/actions/runs/38829500857))
and standard Greptile at 5/5 with no unresolved threads. Apex also
completed at **5/5** on that exact head ([Apex
result](https://github.com/paperclipai/paperclip/pull/16486#issuecomment-6201559527)).
All review threads are resolved, the branch has no merge conflicts, and
the PR remains unmerged.
- No live paid-provider or remote-machine smoke was performed. The
lockfile remains unchanged under the repository's refresh-bot policy;
local dependency installation resolved the changed patch hash.

## Risks

This explicit opt-in grants Codex tools access to the execution home
(read-only in planning), including credentials. Native Codex separately
denies Runner-owned protocol state. ACPX Claude retains its existing
provider trust boundary, and the isolated default is unchanged.
Providers may refresh credentials and update their own metadata.

User configuration can block tools or the completion reporter. Existing
blocked results and failure diagnostics remain the recovery path. Edit
the local settings and retry; no new approval workflow is added.

External provider history remains on the execution machine and is not
backed up by Runner. Remote HOME discovery requires `getent` and fails
explicitly when unavailable. Other harnesses need separate
qualification.

The Codex ACP bundle digest changes. Existing sessions tied to the old
bundle may need a fresh session. No database migration is required.

## Model Used

OpenAI Codex, GPT-6, with reasoning and code execution. The exact
runtime model ID was not exposed.

DottaandPaperclip cccde7122b Trace execution preparation and durable response persistence (#16419)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - The server prepares an execution environment before it starts an
agent.
> - Operators need to distinguish preparation, provider wait, and
response persistence.
> - Existing preparation phases start separate traces, and the native
session trace ends before the final response is saved.
> - This pull request adds an execution-attempt trace and detailed spans
at the existing work boundaries.
> - The benefit is a measurable startup and settlement path without
changing execution policy.

## Linked Issues or Issue Description

**What existing behavior does this improve?**

Operator-configured OpenTelemetry tracing for agent preparation,
startup, and durable response storage.

**Subsystem affected**

Server orchestration, native Runner startup, and the Honeycomb run link.

**Current behavior**

Preparation phases start independent roots. Workspace lock waits,
authorization groups, runtime asset work, provider-pack fallback, and
the gap after provider completion are hard to attribute.

**Proposed behavior**

Use a server-owned async context for each execution attempt. Parent
nested work to that attempt. Link the separately sampled native session
because its ingress and queue history can begin earlier. Record
transaction entry, lock waits, grouped authorization, asset preparation,
artifact fallback, copyback, and response commit.

**Reason and benefit**

Operators can identify expensive work from recorded boundaries instead
of inferring the cause from elapsed time. Nested and concurrent
durations remain visible and must not be added together.

**Breaking changes**

No API or execution-policy change. New Observability span names and
closed count attributes are documented. The existing local
run-performance schema also records the additional provider-pack and
harness measurements.

Refs: #16178, #16205, #16330.

## What Changed

- Add `task.execution_attempt` with same-run async ownership,
independent root sampling, and native-session links.
- Measure workspace transaction entry and settlement, advisory and row
waits, source scans, reader groups, notifications, and writes.
- Measure runtime selection and each concurrent instructions, skills,
and MCP branch. Separate manifest, cache, materialization, and
verification work.
- Measure provider-pack discovery, verified reuse or upload, final
verification, and remote harness state checks.
- Measure workspace settlement, accounting, liveness, response
authorization, comment commit, and publication enqueue.
- Preserve the silent endpoint-off path, including when another library
has registered a tracer. Bound repeated detail spans to 16 per name per
attempt.
- Extend the Honeycomb run link and document the actual coverage and
remaining gaps.

## Verification

- After resolving the conflict with retained-reader batching on
`b98cdaea531c8447637cd4dcc19c61b3f5f41844`, all 61 targeted query,
authorization, workspace-preparation and trace-boundary tests passed.
The server typecheck passed. Reader tests still bound query count, avoid
loading retained run payloads and enforce fresh project grants on every
publication attempt.
- Before that base integration, the tracing/exporter group passed 855
tests with 2 optional-SDK skips across 10 files and no inherited OTLP
endpoint. It covers real PostgreSQL waits/rollback, durable response
commit, assembled heartbeat preparation, concurrent parentage, sampling
and tracer failure isolation. Native workspace/history tests passed 23
cases. These results remain supporting evidence for the unchanged
tracing paths.
- Full workspace typecheck and build passed during implementation. Full
sharded CI, release validation, E2E and Apex 5/5 passed on prior head
`8cebad80f3856dbcff6f3a6d5d34880ff77a20f2`. Fresh CI, security and Apex
review are required on the current head before merge.
- `pnpm check:token-gates` passed. No styles changed.
- Earlier CI runs exposed exact-source-expression assertions changed by
wrappers, missing run hashes on transaction-boundary spans, and sampling
fixtures without an enabled endpoint. The fixes preserve the calls, add
the declared hash and span cap, and enable tracing explicitly in the
sampling fixtures. Focused repairs passed; the full prior-head CI passed
after those repairs. Earlier failed run:
https://github.com/paperclipai/paperclip/actions/runs/38729131366.
- The full local `pnpm test:run` was stopped after the base advanced
during that process. Its mixed-version results are not final-head
evidence. A full local-suite pass is not claimed.
- The old-head Superagent scan stalled at 35%. The current head has a
fresh scan; no security pass is inferred from the old pending result.
- No deployed Honeycomb acceptance is claimed by this code change.

## Risks

- Enabled tracing creates more spans. Repeated detail spans are bounded,
and inventories use group spans.
- The attempt and native session remain linked traces. Historical queue
time can precede the attempt, and nested intervals overlap.
- A committed comment proves durable server storage. Publication enqueue
and realtime dispatch do not prove external delivery or browser paint.
- New custom attributes use only the existing run hash and validated
file/byte counts. No SQL, paths, permissions, content, or exception text
is added.

## Model Used

OpenAI GPT-6 through Codex. The serving model ID and context window size
are not exposed in this session. The agent used reasoning, tools, code
editing, and code execution.

DottaandPaperclip 3ec7309d91 feat(voice): add experimental Speko task connection runtime (#15727)
First PR: runtime. UI: #15728. Qualification: #15729.

## Thinking Path

> - Paperclip manages agent work through tasks and governed connections.
> - Chat connections already bind external conversations to those tasks.
> - Speko handles voice and telephony, while the selected Paperclip
agent does the work.
> - A call needs durable session authority, ordered follow-ups, and safe
result delivery.
> - This PR adds an explicit voice transport to the existing connection
runtime.
> - The UI and qualification suites follow in linked PRs. Speko stays
experimental.

## Linked Issues or Issue Description

**Problem or motivation**

Phone and browser voice conversations cannot use the chat connection
task lifecycle. A separate voice runtime is needed to preserve task
authority, retries, and reply history.

**Proposed solution**

Add company-scoped voice sessions, signed callbacks, durable replies,
inbound intake, and an agent-facing outbound-call tool. Reuse existing
tasks, queues, secrets, and publications.

**Roadmap alignment**

This extends the experimental chat connector entry points under Agent
Chat and Connected Apps. The project owner requested this feature.

Refs #11418 for the original provider catalog proposal. This
implementation uses the newer chat connection infrastructure rather than
an MCP catalog-only integration. Refs #15721 for remote OpenCode
startup.

## What Changed

- Add additive voice schema, shared contracts, session APIs, and
provider callbacks.
- Bind browser, inbound, and outbound sessions to an endpoint, task, and
selected agent.
- Add low-trust guest intake in an explicitly selected sandbox and keep
ordinary approval rules.
- Submit task work through session-scoped tools and push approved
replies through Speko POST /v1/calls/{id}/messages.
- Distinguish provider acceptance from confirmed spoken playback.
Reconcile reports without downloading recordings.
- Add protocol, migration, persistence, provider, and
delivery-diagnostic tests.

## Verification

- Master 3389bea708 is integrated. Database/shared builds, server
typecheck, migration upgrade/replay checks, and 46
communication-guidance/provider/protocol tests passed.
- The final persistence suite passed 85 real-database tests. Five
focused native continuation/authorization regressions passed. Five
callback ownership/rotation tests passed after the last runtime repair.
- Review repairs cover reconnect ownership, current caller authority,
duplicate delivery, multipart answers, task-page answers, guest question
handling, and saved sandbox recovery.
- A real OpenCode/OpenRouter DeepSeek task ran in Daytona and saved a
correct disk-space answer. This did not establish live-call playback.
- Full local typecheck/build passed before the final repairs; focused
typechecks passed after them. Latest-head CI runs the repository gates.
- The separate integrated browser qualification passed two journeys and
failed browser answer retrieval. The feature remains experimental. The
report in #15729 records this failure and outstanding real-provider
acceptance.
- No lockfiles are included. Speko migrations follow master as
0324–0330.

## Risks

- Voice sessions add migrations and provider-dependent lifecycle
behavior. Migrations preserve existing chat and AI transports.
- Callback origins must be reachable HTTPS. Provider keys and signing
secrets remain vaulted.
- Unverified callers receive task-scoped low-trust authority. Caller ID
does not grant private task access.
- Outbound telephony incurs provider charges. Provider acceptance is not
proof that the caller heard an answer.
- Keep the feature behind the existing experimental chat setting until
the real browser, inbound, outbound, and delayed-result acceptance
matrix passes.

## Model Used

OpenAI Codex, GPT-6, with tool use and code execution. The exact serving
model ID and context window are not exposed in this session.

DottaandFry 3ca196b0a6 feat(agents): persist agent files across tasks without revision history (#14420)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - An agent needs personal files across tasks and sessions.
> - AGENTS.md is one file in that directory. Supporting files need the
same persistence.
> - The Instructions Editor and agent runs must share one current
directory.
> - Concurrent runs should apply only the files they change. The last
sync of the same file wins.
> - This pull request uses existing file transport and removes temporary
copies after sync.
> - Old instruction-only sessions keep their restore contract. New saves
do not create revision history.

## Linked Issues or Issue Description

Refs #14325. This replaces its revision-oriented design with persistent
agent files. Keep #14325 unmerged.

Transport prerequisite #14416 merged first at
`d172197117a14b80a1eb2d2835a0e7cce2679656`. This PR now targets master
and remains below 100 changed files.

Related work: #4513 and #8798 cover instruction tooling. This change
handles run synchronization, cross-task personal files, browser editing,
and old-session restoration.

## What Changed

- Keep one current directory per company and agent. Point AGENT_HOME at
a temporary working copy for each active run. Keep task files and
provider HOME separate.
- Restore text, binary files, and nested folders through workspace
transport. Exclude remote agent files from task Git snapshots with a
self-ignoring file inside the reserved runtime directory; never write
through repository-controlled Git metadata.
- Collect after the provider and child processes have stopped. Keep
resumable conversation state.
- Apply changed and deleted files under the agent lock. The last sync
wins for the same file. Unrelated concurrent changes survive.
- Remove temporary copies after successful sync, rejected sync, and
staging failure. Register ownership before copying so restart recovery
can remove interrupted preparation. Retry transient synchronization up
to three times. Preserve the original remote lease reference until
deletion succeeds; restart cleanup never acquires a replacement sandbox.
Do not create captured directories or a conflict-review queue for new
runs.
- Keep browser editing, stale-draft protection, and streaming binary
downloads. Keep the instruction entry and text editor limited to 1 MiB.
- Keep historical agent-folder sync failures on their affected runs
instead of repeating them above current saved instructions. Preserve
legacy candidate review and current browser-save errors. Avoid duplicate
quota warnings while retaining separate sync failures when they describe
a different problem.
- Require target-scoped caller grants for peer instruction access, while
preserving self edits, responsible-user checks, and protected-change
consent.
- Treat full storage as a nonblocking run warning, never an agent pause
or run-admission failure. Restore already-over-quota saved folders so
ordinary agent cleanup can recover; warn on each run until cleanup. The
run detail view shows the warning.
- Allow 256 MiB per file, 2 GiB per directory, and 100,000 entries. Hash
large files as streams. Check editor-save quotas with metadata instead
of hashing unrelated files.
- Preserve old native inputs, instruction-only copies, paths, digests,
and pending legacy candidates. Adopt old revision heads once. New writes
do not append history rows.
- Add idempotent migration 0287 and verify upgrades from the preview
tables and receipts.
- Add nine interactive stories under **Agents / Persistent files**,
including automatic incoming edits, stale browser drafts, and
storage-limit diagnostics.

## Verification

- Merge candidate: `4f5390107ec6ffd80a76d1d2e85530e66f21d079`, after
merging current master and the landed transport prerequisite.
Integration required no manual conflict resolution; the feature remains
99 changed files. Full workspace typecheck, production build, token
gates, and 715 focused tests passed on this merge candidate. Fresh
Greptile review is 5/5 with no unresolved findings. All 55 checks
passed, with four conditional skips, including the build, typecheck,
browser E2E, and canary dry run. A single retry recovered four jobs
interrupted by runner shutdowns; no source changes were required.
- Historical-warning UI fix: all 6,834 UI tests across 640 files passed,
including regression coverage for three old failures, legacy preserved
edits, and warnings scoped to the affected run. Full workspace
typecheck, production build, Storybook build, and token gates passed.
Browser-verified Storybook playtests passed for Historical Failures
After Successful Save, Storage Limit, and Full Storage Run Warning.
- Review follow-ups at `4e20c9fb2`: all 18 focused tests passed,
including external Git directories, linked worktrees, symlinks,
hardlinks, and distinct I/O failures alongside storage warnings. Server
and UI typechecks, token gates, and the production build passed.
- Storage warning regressions at `0724f3012`: all 33 directory tests and
all five heartbeat-list tests passed, with no skips in their successful
runs. They cover repeated runs while full, an already-over-quota saved
folder, cleanup, warnings retained after unrelated save failures, and
bounded warnings in large result JSON. Server typecheck passed after the
final warning fixes.
- Full workspace typecheck, production build, and token gates passed
during this follow-up. Product E2E harness: 631 tests passed across 52
files; harness typecheck passed. Earlier native session/context and
directory/legacy collection suites passed 537 tests; Runner
unit/transport suites passed 329 tests.
- **Real E2E at `0724f3012` (before this follow-up):** legacy local
Codex and native Daytona Codex each passed six tasks, one server
restart, seven independent assertions, and cleanup verification. Both
prove browser-to-agent edits, agent-to-browser edits, nested/binary
restoration, per-file last-sync-wins, a successful run after an
oversized save rejection, and cleanup clearing the warning.
- Native local Codex also passed the six-task quota flow before the
final warning-retention fixes. That pass began at `918d1ed02` while the
bounded-result warning fix was being edited, so it is not claimed as
exact-final-head evidence. Its final-head rerun failed during embedded
PostgreSQL bootstrap before any provider run: the macOS host had 87,365
of 87,381 SysV semaphores occupied. No unrelated services or kernel
limits were changed.
- The final-source report intentionally records **2/3 cells passed**,
preserving the blocked native-local attempt:
`tests/runner-e2e/results/agent-files-quota-final-20260928-report/`.
Earlier failed attempts and provenance notes remain under
`tests/runner-e2e/results/agent-files-quota-final-20260928-input/` and
the original campaign directories.
- Daytona used immutable image
`ghcr.io/paperclipai/paperclip-daytona-runner@sha256:5643f0d801417cae3581833a1a3bc6715b325e028602738d2652c44cac5dc6bf`
and its exact Linux runner binary. Controller source is `0724f3012`;
- Legacy-session compatibility and all three ACP Stop/resume browser
regressions passed on the prior validated feature head
`169fab46d5af21caa2269b4c1b29b69c933a6951`. They assert the same
provider session is retained and interrupted writes are not replayed.
Migration upgrade tests also passed earlier.
- Nine interactive stories are under **Agents / Persistent files**,
including **Full Storage Run Warning**. Its playtest and visual browser
inspection passed; the warning states that runs continue and the editor
remains available.
- Prior-head checks on `4e20c9fb2`: 55 passed, two conditional jobs
skipped, no failures or pending checks. All eight browser E2E shards and
their aggregate passed. Fresh Greptile review is 5/5 with no findings;
all review threads are resolved, the security scan passed, and GitHub
reports no merge conflicts.
- The broad local follow-up test run was interrupted after host
semaphore exhaustion affected isolated PostgreSQL instances. It also
encountered the existing macOS long-path fixture failure and two timeout
failures. This is not a claim that the full local suite passed. Logs are
retained; focused storage/warning tests passed.

## Risks

- A later sync can overwrite an earlier edit to the same file, including
a saved browser edit. There is no text merge or retained version. This
is the intended last-sync-wins policy.
- A save that exceeds a storage limit is rejected and its temporary copy
is discarded. The run itself continues normally, and later runs restore
the last saved files with a warning until cleanup. Transient sync
failures get bounded retries. An I/O failure partway through a sync can
leave some files updated; a failed receipt does not claim whole-folder
success.
- Larger folders increase copy time, network traffic, and temporary disk
usage. Active runs still need working copies. Terminal runs do not
accumulate archives. Operators must provision disk for agents and
configured concurrency; these limits are not company-wide quotas.
- A restored old native session remains instruction-only until a fresh
session starts. Its original conflict fence and existing pending
candidates remain compatible.
- Provider processes close at the collection boundary. Conversation
resume remains available, but warm process reuse is lost.
- Backups must include the instance filesystem and database. External
bundles keep their existing behavior until explicitly moved to managed
storage.

## Model Used

OpenAI Codex, GPT-6 family. The session does not expose a more specific
model ID or context-window size. Reasoning, code execution, and browser
tools assisted this change. Real provider E2E uses `gpt-5.6-sol`.

DottaandPaperclip 8813a50105 feat: run GitHub review bots through Paperclip agents (#13717)
## Thinking Path

> - Paperclip manages agent work as tasks and runs.
> - GitHub chat brings repository conversations into those tasks.
> - A review bot needs the assigned agent, its authority, and governed
provider tools.
> - The existing channel connection did not supply that review workflow
or a complete setup journey.
> - This pull request adds GitHub App setup, account access, event
prompts, task-bound review tools, and exact-commit checks.
> - Operators can inspect each review through the same task, run, and
activity systems.

## Linked Issues or Issue Description

**Subsystem affected**

GitHub chat, governed connection tools, task execution, shared/database
contracts, and connector setup UI.

**Problem or motivation**

Operators need a GitHub review bot that runs their assigned Paperclip
agent. Mentions and PR events must preserve task ownership and requester
authority. Provider publication must use the bot App identity and
enforce the configured permissions.

**Proposed solution**

Extend the existing GitHub chat connector with resumable App onboarding,
linked-member and sponsored-guest access, editable event prompts, and
governed review operations. Validate structured assessments on the
server and compute a stable Paperclip Review check for the exact head
commit.

**Alternatives considered**

A separate review scheduler would duplicate Paperclip execution and
permissions. Reusing personal GitHub credentials would change the bot
identity and credential boundary.

**Roadmap alignment**

This extends the existing Connected Apps and governed-tool
infrastructure. The project owner requested and approved this design.
Related PR #8645 imports external Codex review feedback; this change
runs an assigned Paperclip agent and publishes its results through the
existing chat connector.

## What Changed

- Include the current Paperclip instance origin in the copied setup
prompt. Storybook uses its configured Paperclip origin; callback
parameters and URL credentials are excluded.

- Add a Claude/Codex copy button in the real setup and Storybook opening
step. Its detailed prompt asks four setup questions and guides
embedded-browser setup, verification, and optional required checks.
Clipboard failure exposes selectable instructions.
- Add a tutorial that explains why App installation, review scheduling,
and required checks are separate choices.

- Add manifest registration, an existing-App path, separate installation
and repository selection, repository refresh, and explicit account
confirmation.
- Add low-trust agent guidance, effective capability verification,
member selection, and explicit restricted guests with a sponsor.
- Add configurable PR events, prompts, repository overrides, rating
thresholds, and separate formal-review permissions.
- Give the assigned agent governed App tools to read PRs, comment, begin
an assessment, submit findings, and optionally submit a formal review.
- Bind review history, root PR events, and inline replies to ordinary
tasks. Deduplicate deliveries/findings and reject stale publication.
- Link check Details to the underlying task on the current trusted
hostname, or to Reviews before task creation.
- Add schema migration 0283, API contracts, production UI, and 49
interactive Storybook states.
- Repair local lease recovery. Keep the Cloud Dockerfile identical to
master; no provider-pack layer or runtime-default environment variable
is added.
- Retry only rolled-back wake-admission transactions after transient
endpoint-lock contention. A deterministic held-lock regression proves
one accepted wake.

## Verification

- Current head: `7ba761fe007bb798400d3e62346fa964f607f0f8`, rebased on
master `d9b3a5653e41f2ee5a1345b97c86a238f7a5c8e9`. Dockerfile has zero
diff against master. Final workspace typecheck and build passed. The new
PostgreSQL migration regression passed and preserves existing relation
and constraint identities after replay.
- Greptile reviewed this exact head at 5/5. There are zero unresolved
review threads and no merge conflicts.
- All current-head checks are green: 54 passed and two conditional
Storybook jobs skipped. This includes complete server/workspace test
suites, build, typechecks, policy checks, Runner suites, browser suites,
and security status. One timing-sensitive callback-ordering test passed
in isolation and its CI shard passed one retry. The duplicate local
full-suite run was stopped after CI completed; it is not counted as a
local full-suite pass.
- Before the final Slack rebase and migration renumbering, 186 focused
GitHub tests, 14 native bootstrap cases, token gates, and Storybook
build passed. The final rebase retained the new Slack communication
guidance.
- The embedded-browser setup test copied the full detailed prompt,
including the configured Paperclip instance URL. Desktop and narrow
layouts were checked. Component tests cover successful copying and
clipboard failure with selectable text and retry.
- Live local and hosted GitHub acceptance evidence refers to application
revision `cb703ac959876a07ebf3d7a295847f9f351eb6fc`. Real agent tasks
exercised issue mentions, automatic PR reviews, inline findings,
repeated mentions, task continuation, and failing-to-passing checks
after a push. The Storybook agent generated, built, and browser-rendered
pages; missing acceptance text failed, matching text passed, and broken
JSX produced an incomplete result.
- Live cases also covered independently disabled push events, prompt
injection, duplicate signed deliveries, rapid pushes, stale-result
rejection, finding deduplication, and restart recovery. Formal reviews
were denied while disabled and published only after explicit enablement.
Check Details links pointed to the underlying task on the trusted
hostname.
- Those hosted native Claude runs used the provider-pack layer now
removed from this PR. They do not prove native Claude works on the
standard Cloud image. A replacement hosted native Codex run is not yet
verified: the disposable QA tenant has only an Anthropic AI connection.
No new staging or production deployment was made for the packaging
removal.
- Required-check merge enforcement could not be tested because the
private disposable repository's GitHub plan rejected the rules
configuration. Published success/failure/incomplete check states were
verified directly.

## Risks

- Latest master allocated migration 0282 to Slack. The GitHub migration
is regenerated as 0283 with replay-safe table/index/constraint creation;
a PostgreSQL regression verifies existing relations and constraints are
preserved. Existing preview tenants remain subject to the fleet
migration-history compatibility preflight; no bypass is introduced.

- Migration 0283 adds company-scoped configuration, registration,
review, and publication records. Existing connections retain their
behavior until reviews/tools are enabled.
- Signed webhooks and expiring registration state remain required.
Hosted installations also need the companion narrow Cloud gateway
exemptions.
- Agent assessments can be incomplete or wrong. The server enforces
coverage/result structure, current-head publication, rating policy, and
separate formal-review permission; it does not replace code-review
judgment.
- No Cloud image packaging changes are included. Remote native
ACPX/Claude and OpenCode retain their existing operator-supplied
provider-pack prerequisite. Native Codex and Codex with managed MCP
tools do not require that pack. Earlier staging deployment evidence
refers to its stated revision, not this packaging-removal head.
Production rollout and merging remain outside this change.

## Model Used

OpenAI GPT-6 through Codex, with repository, code execution, API, and
embedded-browser tools. The exact serving model ID and context-window
size were not exposed by the environment.

DottaandPaperclip c723bb4dfc fix(skills): reuse validated runtime revisions during preparation (#13042)
## Thinking Path

> - Paperclip manages AI agents and prepares their runtime inputs before
each turn.
> - Shared company skills are part of those inputs for native and legacy
adapters.
> - Runtime materialization refreshed the full inventory again for every
declared file.
> - Remote skill directories were also downloaded and rebuilt on every
turn.
> - Measured preparation took 42–73 seconds while runner execution took
7–9 seconds.
> - This change reads the inventory once and reuses validated installed
revisions.
> - Agents retain their selected skills while repeated preparation
avoids upstream work.

## Linked Issues or Issue Description

**What happened?**

One 114-skill preparation performed 407 inventory refreshes, 48
directory rebuilds, and 388 GitHub file fetches. Reusing existing local
copies took 151 ms.

**Expected behavior**

Each listing refreshes inventory once. Unchanged installed remote
revisions reuse complete, validated local copies. Local edits remain
visible. Explicit updates select new revisions.

**Steps to reproduce**

1. Import GitHub skills with supporting files.
2. Run an agent turn, then run another with the same installed
revisions.
3. Observe repeated inventory scans, downloads, and runtime directory
replacement before execution.

Related prior attempts: #2330 and #9268 (still open; #9268 last updated
July 9). Those use a marker compared with `updatedAt`. This patch
follows the required content validation, immutable revision, company
isolation, atomic publication, and read-only semantics, and removes
refresh-per-file multiplication.

## What Changed

- Split public file reading from reading an already loaded skill.
Runtime listing refreshes inventory once.
- Add a company-scoped revision cache with file manifests outside the
delivered skill directory. Fingerprints omit cosmetic metadata.
- Validate exact file inventory, sizes, and hashes before warm reuse.
Reject traversal and symlinks. Stage complete builds and serialize
atomic publication across processes.
- Preserve local/catalog direct sources, stored Markdown fallback,
explicit version snapshots, and legacy mutable-ref compatibility. Report
missing supporting files and keep older valid revisions readable.
- Clean both runtime layouts on rename/removal and record
`skills.prepare` under preparation timing.
- Add service/cache regressions and an isolated 114-skill benchmark,
including a new-process warm run.

## Verification

- Final targeted skill-service/cache/trace validation: 86 tests pass (61
embedded-PostgreSQL service tests, 19 cache tests, 6 trace tests).
Database tests executed rather than skipped. Focused skill routes,
adapter selection, and native runtime context also pass.
- `pnpm -r typecheck` and `pnpm build` pass locally at `22caa1fe4`.
- The full `pnpm test:run` matrix passes on supported Linux CI at the
final head: [CI
run](https://github.com/paperclipai/paperclip/actions/runs/34236097762).
Local full-suite execution encountered PostgreSQL startup contention, a
random allocated-port boundary, and a socket hang-up; every affected
suite passed on an isolated rerun. The interrupted local serialized run
is not claimed as a complete local pass.
- Repeatable benchmark: `pnpm --filter @paperclipai/server exec tsx
../scripts/benchmark-skill-preparation.ts`. Mixed 114-skill inventory
with 429 remote files on Linux: cold 286 ms, warm median 96 ms / maximum
153 ms including a new process. Every warm sample performs one refresh,
zero upstream fetches/rebuilds, and reports no missing entries; content
assertions pass.
- Controlled deployment against the previously deployed revision
completed with zero lost runs. Real inventory: 114 skills, 670 declared
files; 402 cached files match the prior installed copies byte-for-byte.
Ten post-deployment warm preparations: median 129 ms / maximum 208 ms;
new-process warm 194 ms, zero downloads/rebuilds/missing entries.
- Five sequential real browser questions persisted in 10.6–20.7 s
(median 12.2 s), versus 50–83 s before. Skill preparation median 240 ms,
with one 2.37 s outlier. Total preparation median 3.337 s / maximum
8.728 s **does not fully meet** the <3 s / <5 s target. The excluded
historical-run redaction query takes about 1.36 s per scan at two
preparation call sites; wider application latency coincided with the
outlier, without a cache rebuild. These residuals are reported rather
than discarded.
- Disposable skill reimport verified through actual selected-skill runs:
the next run read the changed code. Fixture removed and agent
configuration verified unchanged.
- Greptile 5/5, zero unresolved review threads, all final-head CI checks
green.

## Risks

- Cold preparation still requires upstream availability for supporting
files. An unavailable revision is reported missing and never falls back
to an older revision.
- Valid older revisions and quarantined invalid entries consume additive
disk space until skill cleanup. An abruptly killed publisher can leave a
lock that requires operator cleanup after confirming its PID is dead.
- Warm validation reads all cached file bytes. Very large inventories
still have proportional local I/O cost.
- No HTTP API, schema, agent configuration, or first-party Telemetry
changes. OpenTelemetry retains its operator endpoint gate.

## Model Used

OpenAI GPT-6 in Codex, with reasoning, repository inspection, code
editing, and test execution. The exact serving snapshot and
context-window size are not exposed in this session.

DottaandPaperclip 1cc45086d3 feat: use the responsible person's GitHub for shared agent operations (#13005)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Several people can send instructions to the same agent and task.
> - A fixed GitHub token in the provider process can keep the first
person's access after another person's message is accepted.
> - Task ownership cannot select credentials for each accepted
instruction or preserve the identity of an operation already in
progress.
> - This pull request records ordered execution identity contexts and
resolves credentials when managed Git, gh, or GitHub tools start.
> - The benefit is automatic personal GitHub access for shared agents,
with durable continuation rules and no teammate credential fallback.

## Linked Issues or Issue Description

**Subsystem affected**

Cross-cutting: orchestration, connection grants, database, runtime
adapters, native runners, and run details.

**Problem or motivation**

A shared agent must use the person whose instructions it has accepted. A
queued message must retain its author. A retry or approval without new
instructions must retain the originating identity. GitHub must remain
optional for ordinary work.

**Proposed solution**

Persist execution identity separately from task ownership. Give new
processes a run-scoped broker capability and token-free managed
launchers. Capture identity at operation start. Keep an explicit
dedicated-agent grant as an override. Show redacted diagnostics in run
details.

**Alternatives considered**

Per-task ownership, fixed provider tokens, and mutable repository author
configuration do not handle accepted steering or concurrent operations.
A manual account-selection action would add unnecessary setup to each
turn.

**Roadmap alignment**

This completes the existing Multiple Human Users, MCP Tool Gateway &
Apps, Secrets Manager, and Self-healing Runs capabilities. The
implementation follows the maintainer-approved plan.

Related work: Refs #12843, Refs #12907. Existing proposals #4618 and
#8945 cover per-agent or per-worktree author configuration. This change
instead follows the accepted human instruction across runtime types.
Refs #11831 for governed personal connection delegation; this change
preserves connection audience checks and does not use standing
delegation as a personal credential fallback.

## What Changed

- Add durable, ordered identity contexts and active run references.
Preserve message authors through consolidation, steering, retries,
delegation, approvals, routines, and restart.
- Add an authenticated operation-time GitHub credential broker and
local/remote managed git and gh launchers. Keep personal tokens out of
the long-lived provider process.
- Resolve GitHub gateway and server-side Git operations through the same
responsible-person or dedicated-grant selection rules.
- Make absent and unavailable GitHub credentials non-blocking at generic
startup. Clear host and prior-person credentials. Keep anonymous Git
access where supported.
- Add run-detail identity history and the dedicated-account warning.
Keep task ownership and queue-versus-steer decisions unchanged.
- Preserve personal OAuth declarations through connection edits. Retain
exact selected grants in the gateway.
- Fix continuation races found during real acceptance: verify a warm
owner before credential rotation, and wait for bounded durable runner
suspension before the next run starts.
- Make migrations replay-safe. Retain identity through agent/run
deletion, remove it with its company, and clean terminal launcher
directories before releasing execution environments. Document
coordinated release and rollback.

## Verification

- Full workspace typecheck, build, and token gates passed. The complete
local suite passed in its normal test groups: 17,120 passing tests,
including all 143 serialized server suites. After integrating the newly
merged runner API work, full local typecheck and build passed again,
along with 890 focused integration tests. All 31 checks on the
integrated revision passed, including build, browser E2E, release
registry, canary dry run, typecheck, security and all test suites.
Greptile is 5/5 with all review threads resolved.
- Current focused checks passed: 142 native executor tests, 67 runtime
lifecycle tests, 9 durable identity tests, 75 credential/routine tests,
19 low-trust/resumption tests, and the executable migration replay test.
- Authenticated browser acceptance with two Paperclip users and two
GitHub accounts on one shared native agent passed. Real commits and
pushes followed A → B accepted steering → queued A continuation in the
same saved conversation. GitHub commit author and committer identities
matched all three operations. Both runs succeeded and task ownership
stayed unchanged.
- Real GitHub MCP calls switched from A to B after accepted steering. A
delegated subtask retained its originating identity across a server
restart.
- Disabling B's GitHub connection left ordinary work successful. Managed
gh was unauthenticated and the provider had no inherited GH_TOKEN or
GITHUB_TOKEN.
- The browser displayed run-detail diagnostics and the exact
dedicated-account warning. A final controller-restart check followed by
another-person continuation retained the conversation, selected the
correct GitHub login and Git author, and removed each terminal launcher
directory.
- Company-lifetime migration and all five previously failing CI suites
passed locally (167 tests). Same-token gateway A → B → A and six
broker/launcher boundary tests passed.
- Remote callback, launcher, sandbox, and runtime contract tests passed.
Both native and legacy Codex completed actual Daytona executions on the
integrated revision ([campaign
results](https://github.com/paperclipai/paperclip/actions/runs/34155056509)).
The remote package-manager shim staging regression also passed locally.

## Risks

- Deploy the migrations, server broker, launchers, and runner artifacts
together. Existing processes finish with their original contract. New
managed processes need the broker endpoint for GitHub operations.
- Finish or stop new managed executions before rolling application code
back. Keep the additive schema and identity history during rollback.
- Scripts that require a persistent raw GH_TOKEN must use managed git,
gh, or GitHub gateway tools. Run capabilities authorize code executing
within that run to acquire its current identity; this is not
hostile-code isolation within one execution principal. Managed commands
prevent automatic credential carryover; arbitrary code deliberately
copying a credential is outside that boundary.
- Uncertain steering acknowledgement deliberately holds new credential
acquisition until reconciliation. Already-started operations retain
their captured identity.
- GitHub private access and provider outages can still fail the specific
operation that needs them. Dedicated grant failure does not fall back to
personal access.

## Model Used

OpenAI GPT-6 through Codex assisted implementation, review, shell
execution, and browser acceptance. The exact model variant and
context-window size are not exposed in this session. Tool use included
TypeScript and Rust tests, database integration tests, GitHub CLI, and
authenticated browser control.

DottaandPaperclip 86ebdf842e fix(runner): keep agents running when app connections expire (#12670)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Agents can receive governed access to connected apps through the
runtime MCP gateway.
> - A connected app can become unavailable when its sign-in expires or
its health state needs attention.
> - The native runner treated that optional app state as a fatal runtime
setup error.
> - One unavailable app could therefore stop all unrelated agent work.
> - This pull request removes the fatal dependency and keeps the
available app assignment immutable.
> - The benefit is that an agent can continue its work while the stream
tells the user which app needs reconnection.

## Linked Issues or Issue Description

**What happened?**

An agent could not start a native run when one assigned app connection
was disabled, degraded, failed, or missing its secret. Runtime context
creation or MCP delivery threw an error before the agent could do
unrelated work.

**Expected behavior**

The run must continue without the unavailable app. Healthy assigned apps
must remain available. The stream must explain which app needs
reconnection. A changed assignment must not give a native run new access
after its immutable context is captured.

**Steps to reproduce**

1. Assign an MCP app connection to a Paperclip Runner agent.
2. Set the connection to a state that needs attention, such as
`degraded`.
3. Start a task run for that agent.
4. Observe that native runtime setup fails before the agent starts.

**Paperclip version or commit**

Reproduced from `ee2a19062`. The branch is rebased on `dda4dff64`.

**Deployment mode**

Local development from source with embedded Postgres.

No matching public issue or open pull request was found in the GitHub
search.

## What Changed

- Filter unavailable assigned app connections from the immutable native
runtime MCP snapshot.
- Keep healthy assigned connections and their tools in the snapshot.
- Replace the fatal native MCP availability check with an optional
stream warning callback.
- Withhold MCP delivery when the current assignment digest does not
match the captured native context.
- Prevent a warning delivery failure from stopping the agent run.
- Add regression tests for disabled, degraded, mixed healthy and
unavailable, and assignment-drift cases.

## Verification

- `pnpm exec vitest run
server/src/services/native-runtime/runtime-context.test.ts
server/src/__tests__/heartbeat-runtime-mcp-servers.test.ts` passes with
8 tests.
- `pnpm -r typecheck` passes.
- `pnpm check:token-gates` passes.
- `pnpm build` passes.
- `pnpm test:run` was attempted. Unrelated workspace runtime and
port-exposure tests failed on this macOS host. The same files also
failed when run without the changed MCP tests. The changed MCP tests
remained green. Clean GitHub CI is the final full-suite check.

## Risks

- Low migration risk. This change has no schema or API contract
migration.
- An unavailable app is absent from the run MCP surface until it is
reconnected and a later run captures it again.
- Assignment drift fails closed. The agent keeps running, but the
changed gateway is not delivered.
- This pull request does not auto-block the issue before the agent
decides that the app is required. It emits reconnect guidance in the
stream. The existing connection-request interaction remains the path for
a required app.

## Model Used

- OpenAI Codex, `gpt-5.6-sol`, with high reasoning, repository tools,
code execution, and browser automation. The runtime did not expose the
context-window size.

Dotta 131f5c4065 feat(runner): add administration and observability (#12641)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Administrators need bounded controls for experimental native
execution.
> - The lower stack adds remote Codex execution and the task workspace.
> - Operators need to configure Codex safely and inspect provider
traces.
> - Unsupported providers must not appear as runnable choices.
> - This pull request adds Codex-only administration and observability.
> - The benefit is a default-off operational surface for production
diagnosis.

## Linked Issues or Issue Description

Refs #12640.
Refs #12616.
Refs #12352.

**Subsystem affected**

Agent configuration, instance experimental settings, run ledger,
provider trace inspector, and administrator actions.

**Problem or motivation**

The native runner lacks one safe operator surface for Codex permissions,
lifecycle, raw trace capture, and run inspection. The integration branch
also contains provider choices that the production backend cannot
execute yet.

**Proposed solution**

Expose only the qualified Codex controls. Keep Paperclip Developer Mode
and runner preview ingress off by default. Gate raw trace actions by
administrator access and existing trace authorization.

**Alternatives considered**

Exposing unfinished providers would create configurations that fail at
runtime. Always-on tracing would increase sensitive data and storage
risk.

**Roadmap alignment**

This work supports governed Cloud and Sandbox agents and production
diagnostics.

## Stack

- Base PR: #12640.
- Lower PRs: #12639 and #12638.
- This PR contains only its 54-file administration and observability
delta.
- This is the final feature PR in the Codex production stack.

## What Changed

- Added Codex-only Paperclip Runner permission and lifecycle controls.
- Added bounded warm idle configuration.
- Kept the provider field fixed to Codex.
- Added administrator-only one-run raw trace requests.
- Added a persistent future-run raw trace toggle.
- Added trace status, metadata, ledger, and canonical runner inspection.
- Added JSON-RPC request-origin grouping and finalization lineage.
- Restored the stateful PRP transcript parser and focused projection
tests required by trace inspection.
- Added default-off Paperclip Developer Mode.
- Added Honeycomb run links for authorized developer mode.
- Disabled the legacy operational skill for `paperclip_runner`.
- Did not expose OpenCode, ACPX, Pi, Claude Managed, or AWS runner
choices.
- Did not change migrations, workflows, dependencies, or
`pnpm-lock.yaml`.

## Verification

- GitHub Actions will run UI tests, server tests, repository typecheck,
build, browser tests, security, and policy gates.
- Tests cover Codex configuration defaults and bounds, administrator
trace actions, persistent settings, ledger inspection, trace lineage,
and Honeycomb links.
- Existing server trace authorization and retention tests remain the
backend authority.
- Local tests were not run. The requested verification policy uses
GitHub Actions for this series.
- `git diff --check runner/task-workspace-experience...HEAD` passes.
- The delta contains 54 files.

## Risks

- Raw provider traces can contain sensitive provider data.
- Existing server authorization controls access, reveal, download,
retention, and deletion.
- The UI gates trace actions by administrator access and developer mode.
- All new instance settings remain off by default.
- Fresh Paperclip Runner configuration remains Codex-only.
- Direct adapters and legacy task behavior do not change in this PR.

first. See `CONTRIBUTING.md`.

## Model Used

OpenAI Codex with GPT-5.6. The work used high-reasoning agent mode,
repository tools, GitHub tools, and parallel code-audit agents.

DottaandDev Agent 25cf079ec5 feat(runner): add Codex-native application integration (#12591)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - The runner package is useful only when the application can start,
observe, and recover a native Codex run safely.
> - Existing direct adapters must keep their current execution and
finalization paths.
> - The application boundary therefore needs additive persistence,
authorization, coordination, and recovery behind an explicit
experimental adapter.
> - This pull request adds that Codex-only boundary without activating
generalized providers, remote environments, or the later task/SDK
surfaces.

## Linked Issues or Issue Description

**Subsystem affected**

Shared contracts, database persistence, adapter utilities, server
native-runtime services, and the experimental Paperclip Runner adapter.

**Problem or motivation**

The already-landed runner package has a qualified Codex path, but the
application needs durable native-run state, guarded runtime selection,
authenticated coordination, tool security, finalization, and recovery
before the experimental adapter can be exercised safely.

**Proposed solution**

Add a Codex-only `paperclip_runner` application path behind the existing
default-off native-runner setting. Bind native state and coordination to
company/run identity, preserve persisted-run recovery, and leave every
direct adapter on its existing legacy execution path.

**Alternatives considered**

The earlier stack boundary introduced a generalized executor and
remote-environment lifecycle here. That made this PR depend on
implementations in higher PRs and changed reusable sandbox behavior
globally. Those pieces are now deferred together to #12592.

**Roadmap alignment**

ROADMAP.md does not list a conflicting native-runner integration
project. This change adds the application boundary for the existing
Runner architecture.

## What Changed

- Added native run/result/finalization/provider-trace persistence,
shared validators, and idempotent migration/replay coverage.
- Added guarded Codex-only runtime selection, authenticated PRP
coordination, recovery, finalization, and interaction services.
- Added run/company-bound tool-gateway authorization, credential
redaction, SSRF protections, and replay-safe behavior.
- Added the explicit `paperclip_runner` adapter behind the default-off
rollout setting.
- Preserved legacy answered-question wake projection and direct-adapter
execution/finalization paths.
- Hardened cancellation so only owned in-memory child processes are
signaled; persisted recycled PIDs/process groups are never trusted.
- Retained the narrow Claude ACPX isolated-context security follow-up
discovered after #12590.
- Deferred the generalized executor, provider ingress, remote lifecycle,
SDK/lab/eval work, release-process changes, and lockfile.

## Verification

- Changed-file delta against `master`: 133 files.
- GitHub Actions is the authoritative verification environment for this
PR.
- Full CI, security, and Greptile review will run on this lowest
unmerged stack PR.
- Local tests/build/typecheck were not run because this checkout is
resource constrained.
- Static diff/reference checks pass, and `pnpm-lock.yaml` is unchanged.

## Risks

- This touches central heartbeat and agent-route code, so legacy
compatibility is the primary risk.
- Runtime selection remains Codex-only and explicit; direct Codex,
Claude, OpenCode, process, HTTP, and plugin adapters remain on their
existing paths.
- Fresh native starts fail closed while the rollout flag is off;
persisted native records remain readable and recoverable.
- Cancellation, company/run binding, tool calls, status decisions, and
completion writes are guarded or replay-safe.

> For core feature work, check [ROADMAP.md](ROADMAP.md) first and
discuss it in #dev before opening the PR. Feature PRs that overlap with
planned core work may need to be redirected.

OpenAI Codex, GPT-5.6, with repository tools, code execution, and
parallel agent review.
