fix(server): schedule governed review response recovery (#17702)
## Thinking Path > - Paperclip manages agents, tasks and their external chat responses. > - A completed chat turn can owe a response while an earlier review gate remains pending. > - Periodic recovery revisits that response history even when no work can advance. > - This change uses the existing delivery coordinator and saved retry deadlines. > - Approval, interaction and authority changes wake the affected issue or run after commit. > - The existing presentation owner verifies the durable result and current authority. > - Settled and dormant history becomes quiet without losing future eligible responses. ## Linked Issues or Issue Description Refs #17696 (merged) and #15625. This is slice 49, now based on master. It owns governed-review response presentation. Accepted responses and pending decision effects keep their existing owners. **What happened?** Native finalization recovery repeatedly inspects committed review responses. Removing the scan without covering approval, interaction, task-policy and authority changes would strand responses that become eligible later. **Expected behavior** Recover an owed response once from existing durable facts. Enforce authority at publication, wake after relevant outer transactions commit, retain real retry deadlines and restart/drain recovery, and stop recurring SQL when no response can advance. **Steps to reproduce** Complete an external chat turn while an earlier completion review remains pending. Block its response behind an approval or interaction, then resolve that blocker. The response must appear once without a periodic scan or resolving the original review gate. ## What Changed - Add a scoped governed-review worker using the existing coordinator, finalizations, decisions and comment receipts. No queue table or transaction wrapper is added. - Keep recovery and publication policy in the native finalizer. Preserve binding, source/session authority, ownership, deadline and deleted-comment fences; match coordinator-before-issue lock ordering. - Notify through actual approval, interaction, OAuth, task-policy and lifecycle producers before their owned writes. Serialize approval resolution with task linking so a concurrent link retains its wake. Nested transactions still settle at outer commit. - Scope ordinary milestones, finalization and coalesced input to their issue/run. Use the explicit company authority boundary for genuine global restoration, including membership changes. Both response workers share this boundary. - Register under maintenance and drain gates, disable the two periodic repair entrances, and retain fresh finalization and explicit recovery. Add the concurrent 0413 comment-receipt index and migration registration; retain five snapshots. - Preserve the producer inventory, source qualifications and earlier failures in the owner audit. Retain a real Photon regression proving routine ownership assertions stay quiet and concurrent authority restoration wakes each worker once. ## Verification - Scoped repair source `2341714720b5c273f0ce011b48345af0ef39227f` passes **2053 distinct cases**: 1703 in 15 complete producer/authority/recovery files, including all 273 governed-response cases and 95 wake-queue adapter cases, plus 350 selected chat cases. Another 1463 chat cases are outside that selection. Thirteen fresh PostgreSQL 17 fixtures migrate through unchanged 0413; the one temporary queue-fixture import restores exactly. All sixteen captured hashes match. - Two actual repeated coarse-hint SQL regressions fail before repair and pass afterward. Actual membership restoration passes before and after. The same assertions and time limits prove dormant work stays quiet and real authority restoration still publishes once. Real coalesced input covers both queues after commit. - Combined source `586f5674cd9798b33cd22437255e0480b5726d20`, including independent master #18129, passes **1297 distinct cases**: 1093 in five complete server files, 185 in five runner files, 14 Node protocol cases and five UI registry cases. This includes the complete 22-case Photon file for both response workers. The external Photon transport is mocked; Paperclip callbacks, transaction fences and observed SQL are real. The earlier and combined case counts are separate, with no cumulative double-counting. - A normal merge retains both complete owned and independent master binary patches. All twelve new-context hashes and sixteen earlier repair hashes match. Three dedicated fresh 0413 fixtures plus the live-event helper's own fresh database use PostgreSQL 17 without source adaptations. Database build, server types/build, boundaries, full-SHA migration order and generated TS/Rust profiles pass. The new inherited Runner vendor inputs rebuild successfully. - Database bytes remain identical to the previously qualified 0413 source. The recorded populated 0412-to0413 upgrade retains all 411 prior receipts and 38 companies, appends one receipt and none on replay; the index is valid and ready. The earlier 2828-case acceptance, CLI journey, setup failures and their repairs retain their recorded source scope in `doc/plans/2026-10-26-governed-review-response-sleep-audit.md`. - Parent #17696 merged with 58 clean checks, Greptile 5/5 and Ponytail 5/5. Its Photon warning was disproved by real callback/SQL evidence and the security check became successful. This PR still requires its own fresh exact-head CI and review gates. No deployed sleep, external alarm or fleet savings claim is made. ## Risks - Notifications use the existing process-local writer contract. Independent SQL writers and uncoordinated replicas are outside its coverage. - Genuine startup and company authority reconstruction remain proportional to the owed backlog. Queries page 100 rows, but discovery accumulates the candidates; this is not a constant-cost global operation. - Current publication authorization, durable binding, ownership and lock ordering remain mandatory. The worker does not resolve the original review gate. - The concurrent index uses existing migration replay handling. Other recovery duties and Cloud sleep eligibility remain separate. ## Model Used OpenAI Codex, GPT-6 family, with reasoning, repository tools, shell execution and PostgreSQL tests. The runtime does not expose a more specific serving snapshot or context-window size. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> Co-authored-by: Codex <noreply@openai.com> |

fix(server): wake approval queue seeding from committed changes (#17510)
## Thinking Path > - Paperclip manages work and human decisions for AI-agent companies. > - Automatic decision queues store membership for pending attention sources. > - The old attention maintenance scan created that membership as a side effect. > - Moving approval retention out of the scan also removed approval queue discovery. > - Queue API consumers do not build the attention feed first, so they can miss pending PR approvals. > - This pull request gives approval queue discovery its own worker and commit signals. > - The worker becomes quiet after it saves every missing membership. ## Linked Issues or Issue Description Refs #17497. Stacked on #17508, following #17505. This is a correctness dependency for the approval retention migration in #17497. Do not treat that migration as complete without this queue-population path. A real PostgreSQL regression found no PR queue when a pending approval had a linked PR work product and the maintenance feed excluded approvals. Related: #13364 proposes pruning stale seeded memberships. This PR preserves existing membership retention and does not implement pruning. #15149 documents the decision-queue APIs. ## What Changed - Add an approval queue worker that selects missing PR-queue memberships from existing durable records. Bound each pass to 100 rows. Continue past failed rows and revisit them on retry. - Reuse the queue service's seed writer through a normalized source contract. Keep queue rules, uniqueness, and audit writes in that service. Keep feed readers compatible. - Wake from committed approval and link changes, issue/company visibility, PR work-product changes and imports, and queue-rule changes. Reuse the existing transaction boundaries. - Recheck approval status under a row lock. Serialize queue-rule changes with the seed writer. Preserve required recovery during drain and shutdown. - Register startup reconstruction under the existing maintenance switch. Leave other attention-source seeding with its current owner until its own migration. - Document the complete producer audit and stack dependency. Add a validated external PostgreSQL option to the existing queue route suite so it runs on hosts without embedded database support. ## Verification Current tested revision: `7948136ef8`. This PR targets master after #17508 merged as `8406047b4e`. Normal ancestry reconciliation preserved the complete tested integration tree, before the review repairs below. Only the original 25 seeding paths differ from master, and database schema/migration metadata is identical. - **56 cases pass in the three complete affected suites:** 28 approval seeding, 6 queue routes and 22 task-watchdog cases. Server typecheck and production build pass. - Three real PostgreSQL retry regressions failed then passed: new matching arrivals cannot postpone a recovered failure indefinitely; successful partial progress resets old failure penalties; an empty cursor rollover does not reset outage backoff. Exact one- and two-second retry assertions, eventual complete recovery and existing capped-backoff/company isolation tests pass. - Three genuinely empty external databases failed before setup repair because tables were missing. Each suite now applies migrations itself. All 55 cases passed on a second set of fresh databases; adding the final rollover guard then passed all 56 cases using those now-migrated fixtures. No production migration or table is added. - The initial expanded fresh run passed 54 cases but failed a cleanup expectation that omitted a required cursor rollover pass. The fixture now permits that explicit continuation while preserving exact deadline and complete-membership assertions. All failure records remain in `doc/idle-approval-queue-seeding.md`. - The prior integrated revision `4e725e2055` passed **522 cases in fifteen complete producer/consumer suites**, plus types/build. Its unchanged consumer code and historical deadlock, cross-company amplification, backoff, gateway integration and setup evidence remain documented; those counts are not relabeled as current acceptance. Commands: server Vitest for approval queue seeding, queue routes and task watchdogs; server typecheck/build. Fresh exact-head Greptile/Ponytail reviews and all CI checks remain required. No full local repository-suite, deployed suspension or cost claim is made. ## Risks - Signals cover the supported in-process service writers. Independent SQL writers and multiple server replicas need a separate notification contract. - The source predicate must stay aligned with unscoped attention maintenance. The first linked issue, company scope, pending status, and execution visibility are tested. A pending approval itself keeps its linked external conversation active. - Existing enabled seed rules can re-add manually removed matching memberships. This preserves the old scan's behavior. Completed or archived membership is not pruned. - Queue seeding and retention are separate owners. Other attention sources still need their own event-driven discovery before the aggregate scan can be retired. - This PR does not extend Cloud wake coverage, relax sleep admission, or claim measured host/database suspension. ## Model Used OpenAI Codex, GPT-6 family, with reasoning and code execution. The exact serving model identifier and context window were not exposed. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix(server): schedule approval retention from durable deadlines (#17497)
## Thinking Path > - Paperclip manages agent work and should become idle when it owes no background work. > - The attention feed archives old pending approvals under the existing 90-day policy. > - The server rebuilds the feed on each maintenance tick, even when approval state has not changed. > - Some approvals have never been read, so retention rows alone cannot define the work. > - Approval records and committed eligibility changes define the real deadline and wake triggers. > - This PR gives approval retention its own deadline worker and reuses the existing retention and archive-delivery contracts. > - The approval lane stops idle scans while other attention sources remain separate migration work. ## Linked Issues or Issue Description Stacked on #17490. This PR owns approval retention only. Archive notification delivery is already separated in #17475. Other attention-source retention and remaining recurring server duties remain in the stack inventory. The related-PR search found no overlapping approval-retention worker migration. **What existing behavior does this improve?** Automatic archival of old pending approvals from attention. **Current behavior** The shared maintenance tick builds the attention feed for each active company. That query discovers pending approvals and materializes retention records before checking their age. An unchanged or empty approval source is read repeatedly. **Proposed behavior** Reconstruct eligible approvals at startup. Schedule the earliest approval activity timestamp plus 90 days. Wake after committed changes to approvals, links, issue visibility, company activity, or retention controls. Stop scheduling when no eligible approval remains. Archive records through the existing version checks, audit writes, and notification outbox. **Reason and benefit** Idle approval retention should generate no recurring SQL. Unseen approvals still need eventual archival. A stale candidate must not archive an approval that was resubmitted while the worker waited for its source lock. **Breaking changes** None intended. The archive age, eligibility, keep controls, audit, and notification policy remain. Normal attention reads still include approvals. No Cloud sleep blocker is relaxed. ## What Changed - Restrict tool-review signals to their existing terminal transition owners (approve/reject/cancel). Creation/link/revision/resubmit signal retention only. - Coalesce retention notifications under one thirty-second aggregate read budget. Known deadlines still preempt it; no notification or durable deadline means no scan. - Add a bounded approval-retention worker. It uses the shared deadline coordinator and reads pending approvals directly. - Share first-linked-issue ordering and execution visibility through `approval-visibility.ts`, used by both the feed and worker. Preserve actor-specific read filtering and locked archival rechecks. - Materialize unseen approvals on demand inside manual keep/archive/revive transactions. Use the saved activity timestamp and existing versioned state; retain company scoping and rollback on failure. - Add awaited pre-write signals to approval, budget, tool, link, visibility, company, and retention mutation owners. Preserve outer transaction ownership; keep provider calls outside SQL wrappers. - Narrow retention inputs to the source fields the existing service uses. Signal materialization only for inserted or changed state so unchanged reads converge. - Exclude approval collection from the maintenance feed before its query. Other source kinds remain in that sweep. - Add the pending-only approval deadline index with concurrent, idempotent creation. Keep the five newest migration snapshots. - Add real-Postgres lifecycle tests and startup wiring tests. Enable validated external test databases for existing attention and budget suites. - Document producer coverage, quiet conditions, recovery and remaining scope in `doc/idle-approval-retention.md` and the stack ledger. ## Verification Current test revision: `dd844f4e8cbbf1db681a774dc31cd552301bef48`, followed by docs-only `3efda33812`, which received both 5/5 reviews and passing CI. This candidate normally integrates landed Grok `2adcde162e`. All runtime and acceptance-test sources are byte-identical to that tested/reviewed tree; only four independent Runner E2E accounting files and the ledger differ. Migration 0392 and the parent journal prefix are unchanged. Local checks are reused for their unchanged source; fresh final-head CI and reviews are required against master. This remains the approval-retention duty. - **315 cases pass** across eight complete suites: approval retention/service, attention, decision retention, budgets, tool gateway, server startup and scoped deadlines. All 30 real-PostgreSQL lifecycle cases pass. Server production typecheck and build pass. - **Four regressions failed before repair**: unseen young keep/archive/revive returned 404; the manual keep rollback case could not reach its update. They now pass. A failed keep update leaves no retention row or audit; retry succeeds with one audit. - Both linked-issue orders are checked against the feed: hidden first/visible later is excluded; visible first/hidden later remains eligible. The feed and worker use the same owner query, with actor read filtering preserved. - Generated concurrent/idempotent migration `0392_small_frightful_four.sql` adds only the pending approval index. Every historical parent SQL migration and all 390 parent journal entries are unchanged. Five generated snapshots remain (0388–0392). Schema drift, migration safety/numbering and actual fresh apply/replay on five isolated PostgreSQL databases pass. - Existing precision, transaction, drain/resume, failure fairness and shutdown tests remain. Earlier transaction-stub, budget-import, diagnostic-client and CI failures are recorded in the owning audit and stack ledger; later passes do not erase them. - Three additional real regressions failed before the wake repair: ordinary mutations emitted tool-review signals in both resolution cases, and sequential tenant writes caused repeated global reads. Tests now prove terminal approve/reject/cancel signals and one aggregate reconciliation per thirty-second window with a different company's pending approval retained. The aggregate budget is stricter than a separate per-company budget and matches the old default cadence; known deadlines preempt it. - The initial coalescing fixture expected the concrete pending-work hold to clear before reconciliation; it now preserves that hold and measures SQL. The first broad repair passed 313 cases but failed the existing parked-review cancel case. The original cancel producer is restored; all 315 now pass without weakening assertions. At old head `5991f257bb`, security was action-required and Greptile returned service error `8fb1835c-e564-476b-b802-d5be164ea915`. Fresh repaired-head results are required, not assumed. Commands: server Vitest for `approval-retention-work`, `approvals-service`, `attention-service`, `decision-retention-service`, `budgets-service`, `tool-gateway-service`, `server-startup-feedback-export`, `scoped-deadline-work`; server typecheck/build; DB snapshot drift, migration checks and fresh apply/replay. Fresh exact-head Greptile/Ponytail and CI remain merge gates. No full local repository suite or deployed host suspension/cost claim is made. Earlier stack verification below is historical evidence at the explicitly named revisions, not this head's acceptance: Approved delegated Test-tab calls now evaluate caller and target policies in the existing claim transaction. Real HTTP regressions first showed a missing caller charge and shared-limit self-rejection. All 78 connection-management cases and 92 gateway cases now pass on the owning branch, along with server production typechecking. All 78 connection-management cases also pass at combined tip `d25f830db89c1623bff3547bfc68015c1c66e0cf`. The three new cases cover shared charges, separate charges, rollback on denial, duplicate approval, and rejection of a later authorized call. Intermediate counter/identity fixture failures and a reused-database Slack collision are recorded in [the owning audit](https://github.com/paperclipai/paperclip/blob/20557d2c3dd0949c734eaa8095fd78b9b07d7fd5/doc/idle-approved-tool-recovery.md#delegated-test-tab-admission-after-policy-changes). The earlier combined checks below remain evidence for their stated revisions. Current-head CI and review remain required. Earlier published head: `20557d2c3dd0949c734eaa8095fd78b9b07d7fd5`. This remains a single-duty PR stacked on #17490. - Integrated pinned master `8548173fc9efdc0fa07644fb92dcc0b6851a95e5` with normal merges. Every branch passed generated-schema checks and its target-branch migration-order gate. Existing SQL is unchanged; the 25 stack migrations follow master’s new migration. - Duty-specific producer coverage, lifecycle tests, and earlier failure records: [2026-10-24-recurring-server-sleep-stack.md](https://github.com/paperclipai/paperclip/blob/76a49810c7ff403ae7720bef0d453747bd844731/doc/plans/2026-10-24-recurring-server-sleep-stack.md). - Combined tip `51e6cbe67979b55b340ea9f0f434c5b5c682ccd6`: **355 startup/cancellation/recovery/dispatch/preservation tests**, **75 connection-management tests**, and production server typechecking pass. These combined checks do not mean every intermediate PR independently ran the full repository suite. - Real master-to-stack database upgrade passes: **379 unchanged prior migration receipts plus 25 new migrations**, with the gateway responsibility fixture preserved. - Owning-branch checks: 84 runner installation/settlement cases plus runner typechecking and generated TypeScript/Rust profile agreement; 253 GitHub review workflow cases. Stop-attribution and held-worker readiness regressions failed before their fixes. - Failure accounting: the initial combined run passed 349 cases and failed six idle assertions in a reused preservation database containing old cancelled runs. Repeating the combined command with a fresh preservation database passed. Earlier invalid project selection, merge import/type errors, and the interrupted full local suite are not counted as passing checks. - **Latest-head CI and review remain required.** Prior CI findings include GitHub idle observation, route teardown, attention cleanup, touch-picker E2E, workspace health, and voice-cleanup timing; local passes do not erase those failures. No full local repository-suite, deployed sleep, fleet cost, or live Copilot qualification claim is made. ## Risks - Signals cover the application database owner and its child pools. Independent SQL writers and other active processes need an explicit wake integration. Startup reconstructs committed durable state. - The source row lock prevents stale approval-activity archival. It does not add cross-table serializability for concurrent link, visibility, or company-policy changes. - Newly changed eligibility can wait up to the thirty-second notification window, matching the former default maintenance cadence. Saved deadlines still preempt; no new idle polling is added. - Failed records retain recovery debt. Bounded cursor progress lets later rows proceed without dropping failures. - The shared retention transaction now groups materialization and activity-version updates. Existing attention and retention consumers are tested against PostgreSQL. - Kept, archived, resolved or inactive-company approvals can become quiet, but their supported mutations must retain their wake notifications. - The local scheduler cannot wake stopped compute. This PR neither adds approval deadlines to Cloud's external alarm contract nor claims measured host or database suspension. ## Model Used OpenAI Codex, GPT-6 family, with reasoning and code execution. fix(server): recover approved tool actions from committed changes (#17454)
## Thinking Path > - Paperclip manages agent work and its governed tool calls. > - A human approval can persist before the approving process dispatches the tool. > - Recovery currently scans approved requests on every maintenance tick. > - Completed and deliberately inert approvals do not owe another execution. > - Test-tab approval also needs a shared execution claim before recovery can safely race the foreground path. > - This PR wakes recovery from committed changes and retries only unresolved approvals. > - Idle history becomes quiet while existing approval and execution checks remain with the gateway. ## Linked Issues or Issue Description Refs #17453. This is the fifth slice of the recurring-server sleep migration, stacked on `codex/sleep-04-tool-review-expiry`. **Problem** Approved tool requests are scanned periodically even when no executable work remains. The Test-tab foreground path lacks the request claim needed to admit concurrent recovery safely. **Proposed Solution** Use the existing transaction-aware review notification boundary and shared deadline worker. Keep signed-payload interpretation, current policy and target checks, and provider dispatch in the gateway. Share a claim between Test-tab foreground and recovery, then persist both authoritative outcome rows atomically. **Alternatives Considered** A longer interval still scans idle history. A new durable queue would duplicate existing request/invocation state. Dispatching Test-tab approvals without a claim risks duplicate external calls. ## What Changed - Recheck both principals for delegated Test-tab approvals. Save their rate receipts in the same claim transaction, share only the same bucket, and roll back new charges on denial. - Preserve delegated gateway authorization and both issuer/target rate receipts inside the existing claimed-invocation transaction; retain the durable actor during approved replay. - Expire stale managed connector payloads before consuming approved-tool admission. - Persist current rate-policy charges and their invocation receipts atomically before dispatch, preserving exact approval claim fencing and read-only preflight. - Recover approved requests at startup and after committed review changes; retry only unresolved approvals. - Leave legacy agent approvals without execute-on-approve intent inert. Repair completed Test-tab history without another provider call. - Fence Test-tab execution and outcome settlement with an atomic claim shared by both callers. Revalidate the approved target and current policy before dispatch. - Keep provider calls outside database transactions. Retain quarantine for interrupted execution and unknown outcomes. - Add a concurrent, idempotent partial index for approved-request pagination. Reuse the parent's company-scoped review signals and shared scoped deadline worker. - Remove approved scanning from the aggregate tick. Preserve the public combined sweep and scheduler-disabled startup behavior. - Document producer ownership, quiet states, lifecycle behavior, and remaining recovery duties. ## Verification Latest published head: `23868c1751c100a27f58087f7ce16b93c52d4eba`, targeting master. The review-found delegated gateway pool deadlock is repaired by passing the admission transaction through token and issuer-authority reads. Foreground and recovery each reproduced the deadlock with a one-connection pool; all three constrained-pool cases, including revoked authority, now pass. All 81 HTTP integration cases pass. The historical foreground rate-slot regression also reproduced before its repair; verified preview hashes are accepted only by read-only preflight, while consuming admission still requires the complete identity under the exact claim. Five full suites passed 329 cases; the gateway service then passed all 141 cases after two additional mixed historical hash controls. Production types/build passed before the final normal integration of master `35820cf398` (including independent Grok/Runner changes); integration validation is running. Initial fixture/assertion errors and the prior E2E restoration/cancellation deadlock remain recorded in [the owning audit](doc/idle-approved-tool-recovery.md#gateway-admission-uses-its-transaction-connection). Fresh-head CI, Greptile 5/5, and passing Ponytail are still required before merge. - Master landing reconciliation at `8dfe3958db6e4e8a0b7499f3f5be0a396ca77361` after #17453 merged: production and server test files are byte-identical to the accepted 320-case repair. The landed migration journal prefix is unchanged, and #17928's independent eval reporting changes are preserved. Fresh CI and reviews on this exact head are required before merge. - Historical-identity repair published at `096b7c7198c3ca864aca9377e261cbaa0a8aad6f` (production/test source identical to integrated acceptance `81231824d773a79fc50c835eccf7d9b3977e79fc`): all 320 accepted gateway, policy, startup, preparation-timing and authenticated HTTP management cases pass. Server types and the production server build pass. Four real historical sensitive-argument regressions failed before repair; invalid-signature, foreign-hash and stale-claim controls remain denied without dispatch or identity changes. Both agent and Test-tab calls deliver the original signed arguments once, and new rate policies still apply. - The first integrated combined run passed 319 cases and hit a Slack bot ownership collision left in a reused HTTP test database. The guard correctly returned 409. The full 78-case HTTP suite passed on a fresh dedicated fixture without product or assertion changes. Earlier fixture-construction errors are distinguished from the four real regressions in the audit document. Fresh remote CI and reviews remain required. - Parent reconciliation at `736e7f56097055e21ce13b4e45d9033391483af8`: 299 cases pass (221 full gateway/policy/startup cases and 78 assembled-server connection management cases), with no skipped cases. Server types pass. This includes real delegated Test-tab approval admission for newly added shared and caller rate rules. Fresh remote CI and Greptile review remain required. Approved delegated Test-tab calls now evaluate caller and target policies in the existing claim transaction. Intermediate counter/identity fixture failures and a reused-database Slack collision are recorded in [the owning audit](https://github.com/paperclipai/paperclip/blob/bcca5b006976a471f4afd1f70c4031aa40a2b3f7/doc/idle-approved-tool-recovery.md#delegated-test-tab-admission-after-policy-changes). The earlier combined checks below remain evidence for their stated revisions. Current-head CI and review remain required. Current head: `bcca5b006976a471f4afd1f70c4031aa40a2b3f7`. This remains a single-duty PR stacked on #17453. - Integrated pinned master `8548173fc9efdc0fa07644fb92dcc0b6851a95e5` with normal merges. Every branch passed generated-schema checks and its target-branch migration-order gate. Existing SQL is unchanged; the 25 stack migrations follow master’s new migration. - Duty-specific producer coverage, lifecycle tests, and earlier failure records: [2026-10-24-recurring-server-sleep-stack.md](https://github.com/paperclipai/paperclip/blob/a021467f04d22be42ee5b0c87b6583b2300a62a9/doc/plans/2026-10-24-recurring-server-sleep-stack.md). - Combined tip `51e6cbe67979b55b340ea9f0f434c5b5c682ccd6`: **355 startup/cancellation/recovery/dispatch/preservation tests**, **75 connection-management tests**, and production server typechecking pass. These combined checks do not mean every intermediate PR independently ran the full repository suite. - Real master-to-stack database upgrade passes: **379 unchanged prior migration receipts plus 25 new migrations**, with the gateway responsibility fixture preserved. - Owning-branch checks: 84 runner installation/settlement cases plus runner typechecking and generated TypeScript/Rust profile agreement; 253 GitHub review workflow cases. Stop-attribution and held-worker readiness regressions failed before their fixes. - Failure accounting: the initial combined run passed 349 cases and failed six idle assertions in a reused preservation database containing old cancelled runs. Repeating the combined command with a fresh preservation database passed. Earlier invalid project selection, merge import/type errors, and the interrupted full local suite are not counted as passing checks. - **Latest-head CI and review remain required.** Prior CI findings include GitHub idle observation, route teardown, attention cleanup, touch-picker E2E, workspace health, and voice-cleanup timing; local passes do not erase those failures. No full local repository-suite, deployed sleep, fleet cost, or live Copilot qualification claim is made. ## Risks - Notifications cover the existing database-owner, single-active-writer contract. Independent SQL writers and other replicas need an explicit wake path. - Unknown provider outcomes cannot be replayed safely. Failed result persistence retains an executing claim and the existing stale-execution quarantine. - Interrupted execution and terminal interaction projection repair remain periodic until their separate slices. This PR changes only approved recovery. - A pending-only review write can trigger one empty approved-recovery pass. Settled idle work schedules no further scan. - The partial index uses the existing top-level concurrent migration executor. Snapshot pruning follows repository policy. - Cloud sleep eligibility and external alarm coverage remain unchanged. Worker quiet is not proof of deployed instance suspension. ## Model Used OpenAI Codex, GPT-6 family, with reasoning and code execution. The exact serving model identifier and context window were not exposed. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

perf(budgets): scope accounting and deliver enforcement asynchronously (#17046)
## Thinking Path > - Paperclip manages AI agents and their paid work. > - Budgets must prevent new work and stop reported overruns. > - Cost settlement also ran serial lifecycle reconciliation for every eligible company agent. > - That made request latency grow with unrelated idle agents and delayed results on stop delivery. > - This change separates atomic admission and accounting from durable asynchronous enforcement. > - Scoped live usage detects overruns without writing provisional ledger charges. > - The result is constant query scope for ordinary settlement and recoverable stop delivery. ## Linked Issues or Issue Description **What happened?** Settlement and budget checks entered company-wide lifecycle reconciliation. A company with 1,000 unrelated idle agents performed 1,001 accounting transactions for one ordinary cost evaluation. User requests could also wait for provider termination. Running usage checkpoints did not enforce reported monetary exposure. **Expected behavior** Keep the final reservation gate authoritative. Commit charges, scope blocks, and enforcement intent before returning. Deliver stop requests after commit. Read only applicable policies and execution scopes for ordinary charges. Stop long-running work when supported reported or priced usage establishes an overrun. **Steps to reproduce** 1. Create a disposable company with one company budget and idle agents. 2. Evaluate a charge below the budget with 0, 17, and 1,000 unrelated idle agents. 3. Compare query counts, company accounting transactions, and duration. 4. Use the controlled partial-usage and delayed-stop fixtures to check enforcement and request responsiveness. **Paperclip version or commit** The measured baseline is `6c9b87d0922f1ebfc43e8b0c36889ac5ba7b9837` from `master`. Related public work: Refs #16976, #2006, #3539, #13763, #14296, #16853, #13075. These cover the affected service charge, cancellation, and accounting paths. This change removes their shared reconciliation overhead. ## What Changed - Batch applicable policy exposure under the existing company accounting lock. Preserve exact fractional money and zero-reservation dispatch fences. - Commit a durable final budget dispatch claim before provider I/O. Retain a separate final issue/run ownership transaction through synchronous handoff, without holding the company accounting lock or awaiting provider work. Preserve native recovery lease checks and duplicate handoff fencing. Browser handoff rechecks current policy; it reuses a consumed rate slot only for the identical policy generation. - Separate ledger spend, live observed spend, and effective held capacity. Use the maximum of reservation and live observation for each unfinished operation. - Deliver budget cancellation through the existing coordinator. Revalidate generations and persist queued/running stop fences before external termination. Bound stop concurrency to four. - Apply agent holds only to the affected agent. Project budgets leave other projects running. Project real company transitions in bounded lifecycle batches. - Replace periodic broad reconciliation with durable pending work, startup repair, receipt replay, and concrete retry/window deadlines. Retain drain and idle-safety checks. - Enforce accepted cumulative usage checkpoints without provisional charges. Final accounting atomically replaces live exposure and settles reservations. - Add operator-gated budget spans, regression coverage, map updates, and a real-PostgreSQL comparison harness. No schema or first-party telemetry change. ## Verification - Full workspace `pnpm -r typecheck` passes on the final ownership fix, rebased onto `bbcf5dfe98`. Production `pnpm build` passes on final head `4c5f4de8b1`. Map, module-boundary, token, and Storybook catalog checks pass. - Focused real-PostgreSQL coverage includes the 292-case rebased accounting/dispatch run, 63 browser cases, 52 run-control/cancellation cases, 42 policy-generation cases, 77 lifecycle/commentary cases, and the exact Muse confirmation regression. Native executor unit coverage passes all 768 cases. Final ownership coverage passes: 74 adapter/use-case cases, nine public regression cases, and the additional deleted-project race. These exercise competing ownership, budget changes and charges between phases, zero/nonzero duplicate claims, rollback, recovery contention, read-only capacity rechecks, and controlled HTTP startup after commit. Another 74 pure/startup/tracing cases pass. The complete CI matrix and both 5/5 reviews passed on `1ac9b58`; its external security scan identified a stale-owner handoff race. The final fix restores synchronous issue/run ownership fencing after the accounting transaction commits. Final head `8e84f55ce8` has fresh Greptile 5/5 with zero new comments, Paperclip review 5/5, and a passing security scan. The complete latest-head [CI matrix](https://github.com/paperclipai/paperclip/actions/runs/39535189808) passes, including budget, browser, Runner, build, and canary jobs. The unchanged Runner close fixture passed on its sole same-head job retry. - Broad local `pnpm test:run` invocations were stopped before required source updates (rebase and review fixes). They are not claimed as full passes. Final full-suite coverage comes from the complete latest-head CI matrix; local focused coverage and the incomplete broad invocations are reported separately. - Reproduce performance with `pnpm --filter @paperclipai/server exec tsx ../scripts/benchmark-budget-enforcement.ts --baseline-ref=6c9b87d0922f1ebfc43e8b0c36889ac5ba7b9837`; repeat with `--reverse`. The harness uses disposable real PostgreSQL, 20 warm samples per variant/count, and records raw timings, SQL/lock counts, agent reads, unchanged agent state, and query plans. Final production-source measurement revision: `eff713a4f72c7f4b7a4002a701cb481d4bfccd95`; the extended harness is committed in `4c5f4de8b1`. | Unrelated idle agents | Baseline median / p95 | New median / p95 | Baseline SQL / locks | New SQL / locks | New agent reads | | --- | ---: | ---: | ---: | ---: | ---: | | 0 | 1.80 / 2.33 ms | 2.99 / 3.88 ms | 5 / 1 | 7 / 1 | 0 | | 17 | 15.53 / 19.55 ms | 2.24 / 2.60 ms | 90 / 18 | 7 / 1 | 0 | | 1,000 | 978.35 / 1,790.43 ms | 2.16 / 2.52 ms | 5,005 / 1,001 | 7 / 1 | 0 | - These measurements use local PostgreSQL 18.1 and Node 26.4.0 arm64. They isolate the cost-evaluation core and complete admission path, without hosted-region or provider latency. Prior-revision reversed-order controls also preserved the constant query counts; the final measurements above use the final production source. - Complete reserve → final ownership gate → immediate offline provider result is measured separately, with one responsible agent and one active operation at each unrelated-agent count. Admission p95 baseline/new: 0 agents **11.54 / 29.77 ms**; 17 agents **9.31 / 27.62 ms**; 1,000 agents **24.51 / 74.05 ms**. Baseline admission was already scoped; the stronger final fence adds bounded overhead. New counts stay constant at 53 Drizzle statements, three transactions, two company-accounting locks, and five agent-reading statements bound to the responsible agent. Zero unrelated-agent reads, exactly one provider callback per sample, and no leftover runs or holds. All admission scenarios stay below 250 ms. No external provider calls are included. - The partial-usage/delayed-termination fixture passed 20/20: receipt persistence p95 13 ms, receipt return to observed durable Stop fence p95 28 ms, complete receipt-start-to-fence p95 50 ms, unrelated database request p95 1 ms while termination was blocked. After releasing the controlled termination fixture, acknowledgement p95 was 14 ms. These are separately measured phases; fixture acknowledgement is not live-provider termination latency. - Models and provider deadlines are unchanged. No deployment, live inference, or staging acceptance is claimed. A quiet 30-minute follow-up waits for the merged revision on staging, then repeats classifier and controlled budget-stop acceptance in staging, recording hosted bookkeeping, queue delay, and actual provider acknowledgement separately. - Development failures retained: finalizer notification regressions (fixed; 27/27 affected cases passed), invalid browser admission fixture assumptions (fixed; browser coverage now passes 63/63), and first-head lifecycle/dispatch CI regressions (reproduced and repaired). A prior-head Cases E2E test hit its unchanged announcement-dismiss handler race while opening a filter; the same journey passed on `1ac9b58`. The external security review then found a stale-owner race between accounting commit and provider entry; final verification covers a competing run in that gap while retaining provider I/O outside the accounting lock. Early PostgreSQL startup attempts hit local semaphore pressure; serial reruns succeeded. The final-head Runner lane had one failure among 1,823 tests: the unchanged `timeout-attachment` fixture failed at its first process close after 6.28 seconds. The sole same-head job retry passed all 1,819 active tests (four skipped), including that fixture in 1.85 seconds. Three isolated local probes could not run correctly because the Rust fake-provider fixture binary was absent; cleanup masked the original startup failure. CI builds that fixture through its normal lane. These probes are inconclusive and are not counted as passes. ## Risks - This changes accounting, lifecycle, and cancellation ordering. Durable generations, source/sequence checks, ownership leases, and company locking remain required for correctness. - Provider stop acknowledgement can be slow or fail. Stop intent remains pending until existing termination proof succeeds; cancelled row status alone does not acknowledge delivery. - Mid-run limits depend on provider usage reports or supported price calculations. Providers that report only at completion cannot provide exact mid-call detection. - Signals are process-local wake hints for the supported single-active-writer model. Durable state and startup recovery remain authoritative. - There are no migrations, new queue tables, or cached balances. Repeat the classifier and controlled budget-stop acceptance after normal rollout. ## Model Used OpenAI Codex, based on GPT-6, with reasoning, tool use, code execution, and parallel coding agents. The host does not expose the exact backend model revision or context-window size, so those values are not asserted. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

Own agent hiring and termination in a lifecycle module (#15631)
## Thinking Path > - Paperclip manages AI agents and their work. > - Agent creation, approval, pause, and termination change whether an agent can work. > - These changes currently use separate service and database paths. > - Some agents need setup after their record exists and before their harness can run. > - This PR adds one module that owns those state changes and their transactions. > - The module records progress and retries incomplete work after a server restart. ## Linked Issues or Issue Description **Problem or motivation** A hire can become active before its execution environment is ready. Direct agent state writes also bypass one common lifecycle boundary. Termination does not have a durable step for resource cleanup. **Proposed solution** Use agent lifecycle commands for hiring, approval, pause, resume, and termination. Keep proposed hires pending until approval. Prepare each approved hire, test its saved harness configuration, and then permit work. Wait for required plugin work before completing a step. **Alternatives considered** A general database commit hook would expose transaction details to callers. A timer alone would delay each new hire. This module owns each command transaction and starts its worker after commit. A periodic scan recovers work after a process stops. **Roadmap alignment** This change adds the lifecycle boundary for the existing agent and plugin systems. It does not add a user interface or a provider implementation. It replaces the lifecycle delivery approach in #15391. Related proposals include #11475 and #13099; this change does not add their external executor protocols. ## What Changed - Add durable approval, preparation, verification, pause, resume, stop, and cleanup states. - Keep agent creation, lifecycle state, configuration writes, and agent record deletion behind `modules/agent-lifecycle`. Keep validation, credential rules, reads, onboarding, invitations, general approvals, and company data deletion in their services. - Commit a hire before the remaining onboarding or invitation work. Serialize requests with the existing dedicated database connection helper. Reuse the hire after a failed attempt. - Keep accepted work in the run queue during setup. Require execution and cleanup evidence before completing pause or termination. - Retain the original credential owner when approving migrated hires. - Start lifecycle work after each command commits. Use database leases and revision checks for retries and late results. - Add a generic required plugin method and capability. Store the required plugin IDs for each agent. Track host and plugin completion separately. - Keep harness verification and credential rules in services. Supply their results to lifecycle through explicit ports. Reject failed logins and incomplete probes. - Remove reverse imports from lifecycle adapters into services. Share record queries and validation below both layers. - Use one hire decision path for agent IDs and approval IDs. Resolve the approval and agent change in one transaction. Expose policy reconciliation on the command object. - Save configuration and verification invalidation through one module operation. Ordinary commands own their transaction. Restrict the shared transaction integration to its listed callers. Reject identity, company, accounting, timestamp, and unknown fields. - Require completed termination before deleting agent records. The company deletion service calls the module through a separate deletion interface. - Save an approved secret before applying its agent binding. A failed binding leaves the secret approved. A retry reuses it. - Remove old permission and metadata export files. Move hire configuration rules out of lifecycle persistence. - Report failed worker steps and scans with operation context and known error codes. Omit provider messages, stacks, and configuration values. Document ordinary application use and extension rules. - Recover expired native owners only after execution stops. Retry cleanup of failed retained leases. Wait for setup in the live smoke script. - Keep manual, budget, and company pause reasons separate. Guard execution status writes with the ready state. - Add a migration, boundary checks, regression tests, and a lifecycle guide. Keep the current UI. - Limit periodic policy checks to 100 eligible agents per minute. Continue from the last agent ID. Exclude pending hires and agents in termination or a final state. Keep explicit company and budget updates immediate. ## Verification - Latest head `ccc3c86b0` aligns the shared status update schema with the PATCH route. All 15 shared tests, 104 server route/OpenAPI tests, 2 CLI tests, and shared/server/CLI TypeScript checks pass. The Greptile comment is fixed. CI is running; no new Apex review was requested for this small fix. - Rebase `75708ac94` rebases onto master at `2eb5187e4`. The backup test uses the shared 90-second timeout from master; the older timeout commit was removed. The affected backup test passes. - Agent budget edits now pass the target agent ID through enforcement. All 50 affected tests, server TypeScript, and 11 boundary tests pass. Both configuration update paths have regression coverage. CI is running on the latest head. No new Apex review was requested for these small fixes. - Rebase `9b568666a` preserves the run-control extraction and delivery-work startup changes from master. All 559 affected tests, server TypeScript, and 11 boundary tests pass. CI and Apex review are running on the final head. - The policy scan fix passes 56 lifecycle and diagnostic tests, 24 policy and caller tests, and 11 boundary tests. A regression test checks the page limit, interval, excluded states, and cursor wraparound. - Rebase `fd4698750` passes all 611 tests in eight affected lifecycle, route, and heartbeat suites. Server TypeScript and all 11 module boundary tests pass. The company deletion rollback and concurrent hire tests are included. - Follow-up `00aac7646` fixes the failed decision deletion test and the public update schema. All 126 affected server tests, 8 shared tests, 2 CLI tests, and 11 boundary tests pass. Shared, server, CLI, and UI TypeScript checks pass. - Server TypeScript passes: `pnpm --filter @paperclipai/server exec tsc --noEmit`. - All 11 boundary tests pass: `node --test scripts/check-module-boundaries.test.mjs`. The repository boundary scan also passes. - All 68 lifecycle, failure diagnostic, and harness verification tests pass. Tests reject forbidden fields through the root command, combined transition command, and restricted transaction integration. They also check invalid fields returned by configuration preparation. - Failure tests cover host work, plugin work, claims, renewals, deferrals, and scans. Log tests check known error codes and exclusion of private error contents. - All 355 tests in 10 existing caller suites pass. These tests cover secret approval, pairing, pending hires, credentials, company import, built-in agents, managed agents, instructions, and skills. - The branch is rebased onto `origin/master` at `2eb5187e4`. The queue guards and cancellation lock fix use the extracted heartbeat queue. Agent cancellation uses the extracted run-control file. The source diff scan found no secrets or private URLs. - Earlier checks cover approval retries, rollback, late verification results, policy holds, deletion rollback, concurrent company deletion, and queue admission during setup. - Before the rebase, CI passed on `00aac7646` and Greptile Apex rated it 5/5 with no open comments. CI and a new Apex review are pending for the rebased commit. The branch has no merge conflicts. - The full local build and native Dot suite were not run. The host has no Cargo. No live provider test was run. ## Risks - Background policy recovery takes more than one minute when more than 100 agents are eligible. Explicit company and budget updates still reconcile immediately. - The restricted configuration integration keeps an agent reference and its dependent records in one transaction. It cannot change a budget or invoke a lifecycle transition. A periodic scan starts pending setup after that transaction commits. - Secret approval and binding use separate transactions. A failed binding can leave an approved secret with no agent binding. - New approved hires remain unavailable until setup and verification complete. Existing clients see `paused` during these steps. This PR does not change the UI. - Migration `0320_daily_onslaught.sql` maps existing statuses and pause reasons. It does not create external resources or claim that an existing harness passed verification. - Use a coordinated server update for this migration. Older server versions can write legacy status without the new lifecycle checks. - A failed onboarding or invitation request can leave a committed hire. A retry reuses that hire and completes the remaining records. - Deletion requires completed termination. Company deletion can require another request while cleanup is pending. - Required plugins must make repeated calls safe and reject older revisions. A disabled or removed required plugin blocks completion until it is restored. - A new hire can run a second harness test after the existing UI test. Some adapters can charge for this test. - No live provider test was run. Provider implementation and resource backfill remain separate work. ## Model Used OpenAI GPT-6 through Codex, with reasoning, code tools, and local test execution. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have described the issue in this PR with the feature request field labels - [x] I have not referenced internal or instance-local issues or links - [x] My branch name describes the change - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation - [x] I have considered and documented risks - [x] Targeted local tests pass - [x] All Paperclip CI gates are green on the new commit - [x] Greptile Apex is 5/5 with no open comments on the new commit - [x] I will address review comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat: add personal primary agents (#15470)
Add a personal primary agent per company and user. Initialize it from the first human-created agent, expose profile-only switching with confirmation, and use it after recent choices for task and Chat defaults. Persist authenticated preferences, preserve lifecycle and membership rules, keep selections out of shared audit events, and document the API contract. Include the reviewed Storybook surfaces and regression coverage for concurrent choices, onboarding, cross-device updates, and browser journeys. Co-Authored-By: Paperclip <noreply@paperclip.ing> |

Add project and agent resource lifecycle hooks (#15280)
## Thinking Path > - Paperclip manages agents and projects for work. > - Plugins need reliable lifecycle hooks for these resources. > - In-process notifications can disappear during a restart. > - A lifecycle record must commit with the resource change. > - This pull request adds a generic lifecycle journal without provider calls. > - A later plugin delivery layer can use these records without losing lifecycle changes. ## Linked Issues or Issue Description **Problem or motivation** Resource provisioning needs durable hooks for agent hiring, pause, resume, termination, and project creation. Hooks must cover shared service callers, budget actions, and approval paths. Capture should work for all plugins and deployment modes. **Proposed solution** Record content-free events in the resource transaction. Deduplicate creation and unchanged status. Preserve each pause/resume cycle. Commit approval, activation, and creation together. Commit termination and API-key revocation together. **Alternatives considered** Event subscriptions alone cannot survive process failure. Cloud-only capture would exclude other plugins. Provider calls inside tenant transactions would couple resource creation to external services. **Roadmap alignment** This is lifecycle infrastructure for the existing plugin system. It adds no provider, adapter, UI, or plugin read API. Repository mutations and backfill remain separate work. Searches found no duplicate lifecycle-journal PR. ## What Changed - Add a company-scoped lifecycle journal and an additive migration. - Record hired-agent and project creation from shared services. - Record agent pause, resume, and termination, including generic updates and budget actions. - Lock agent state changes to suppress concurrent duplicate hooks. - Make hire approval, rejection, and termination transactions atomic with their lifecycle records. - Document capture, ordering, future per-plugin acknowledgments, and migration scope. ## Verification - Full workspace typecheck: `pnpm -r typecheck` passed. - Production build: `pnpm build` passed. - Focused agent, project, approval, budget, and built-in regressions: 102 tests passed across 9 suites. - Final lifecycle journal check: 14 tests passed. It covers repeated/concurrent transitions, rollback, key revocation, self-hosted capture, and company boundaries. - Review regression: 32 lifecycle and approval tests passed, including rejected-hire rollback and retry; server typecheck passed after the fix. - The initial local `pnpm test:run` overlapped the rejection fix and reported the new rollback regression against the earlier service code. A fresh lifecycle run passed all 14 tests. Fresh full local shards were stopped once the complete CI test matrix passed on the final commit. - `git diff --check` and a local secret/PII scan passed. - Greptile: 5/5 on `3ee3903f8e1a171ea3ba2bea9caa2b5766a3f807`, with the review thread resolved. - [Complete CI passed](https://github.com/paperclipai/paperclip/actions/runs/37374227222) on `3ee3903f8e1a171ea3ba2bea9caa2b5766a3f807`: general server/chat/workspace tests, serialized server suites, runner checks, build, typecheck, canary, and all end-to-end shards. The requester waived CI during the Actions outage, but the workflow subsequently completed successfully. ## Risks - The migration creates an empty table. It does not scan or backfill existing resources. - Apply the normal database migration before running this server version. Event-write failure intentionally rolls back the resource change. - Pending hires cannot bypass approval through pause or resume. - Capture works on all deployments. Plugin delivery, retention, retries, and provider actions remain separate work. No plugin can read this journal through a new API in this PR. - Future delivery must enforce company scope, track acknowledgments per plugin, and preserve resource order. A global sequence cursor can skip uncommitted transactions. - A termination hook does not authorize deleting persistent volumes. ## Model Used OpenAI Codex, based on GPT-6, with reasoning, repository inspection, code execution, and tool use. The exact deployment model ID and context window are not exposed in this session. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(ui): integrate agent personas across the app (#13171)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - A stable agent persona is useful only when the same identity appears across the app. > - Lists, task messages, selectors, and activity feeds need inexpensive static avatars. > - Onboarding and agent headers need a larger character with expressions and pointer tracking. > - This pull request connects the persona foundation to those existing views and preserves onboarding draft assignments. > - Full-page stories and Linux checks make the placements and performance contract reviewable. ## Linked Issues or Issue Description **Problem or motivation** Agents need a stable visual identity in lists, tasks, onboarding, and configuration. External tools also need an image URL for that identity. **Proposed solution** Assign each agent a permanent palette from a fixed ClipLab character library. Store the assignment on the agent. Render and cache preset PNG URLs on demand. Use static images in dense views and one animated character in larger placements. **Alternatives considered** A generated image bundle requires a separate asset build. A live renderer in every avatar adds unnecessary work in large lists. Arbitrary uploaded images do not provide the requested shared character system. **Roadmap alignment** This improves agent identity across existing control-plane views. It preserves agent permissions, company boundaries, and status labels. ROADMAP.md has no separate ClipLab persona milestone. Related approaches: #2422 adds configurable image URLs and DiceBear generation; #5578 adds optional uploaded avatars. This work uses a fixed, versioned character library and preset URLs. ## What Changed - Replace agent icons with static persona images across lists, the sidebar, org charts, tasks, comments, selectors, activity, and dashboard views. - Put one animated character in the agent header. Let it follow the pointer across the page, with reduced-motion and touch fallbacks. - Add larger padded characters to agent creation. Keep the palette stable across draft refreshes and connection retries, then reveal it after success. - Pass appearance through shared projections rather than fetching each agent separately. - Add real full-page Storybook examples for the agent list, overview, task, dashboard, new-agent dialog, and connection page. - Add Linux screenshot, clipping, density, and 500-avatar performance checks. ## Verification - `pnpm -r typecheck`, `pnpm build`, and token gates pass on the rebased tree. Persona lifecycle tests pass. - The rebased feature passes 38 Linux screenshot/performance checks, including both display densities, corner pointer positions, and the no-WebGL/no-live-download contract for 500 avatars. - The final Linux persona suite passes all 38 visual, lifecycle, density, and full-page checks using the standard Storybook configuration and real on-demand avatar endpoint. - Final local focused verification: 45 avatar/native-recovery tests pass; UI identity/routine tests, typecheck/build, token gates, and Storybook build pass. - Current-head CI passes: full workspace/server tests, all serialized server groups, typecheck/release checks, build, canary validation, and end-to-end shards. The build passed after retrying a native-runner concurrency-test failure; its three targeted cases also pass locally. - Manual inspection covered stable identities in the app, header placement, full-page mouse tracking, onboarding size, and task/dashboard placements. ### Screenshots Linux captures use synthetic Storybook fixtures. Full-page captures use reduced motion. The live character, mouse tracking, and disposal are checked separately. <details> <summary>Agent overview with the character in its header</summary> <img src="https://raw.githubusercontent.com/paperclipai/paperclip/8c68f42b268ada22b67d79d0fe1bb0a2f84ec25c/screenshots/full-page-agent-overview.png" width="900" alt="Agent overview with the character in its header" /> </details> <details> <summary>Task messages and assignee identity</summary> <img src="https://raw.githubusercontent.com/paperclipai/paperclip/8c68f42b268ada22b67d79d0fe1bb0a2f84ec25c/screenshots/full-page-task.png" width="900" alt="Task messages and assignee identity" /> </details> <details> <summary>Larger onboarding character with room for expressions</summary> <img src="https://raw.githubusercontent.com/paperclipai/paperclip/8c68f42b268ada22b67d79d0fe1bb0a2f84ec25c/screenshots/full-page-meet-your-next-agent.png" width="900" alt="Larger onboarding character with room for expressions" /> </details> <details> <summary>Dashboard agent activity</summary> <img src="https://raw.githubusercontent.com/paperclipai/paperclip/8c68f42b268ada22b67d79d0fe1bb0a2f84ec25c/screenshots/full-page-company-dashboard.png" width="900" alt="Dashboard agent activity" /> </details> ## Risks - This PR depends on #13170, the persona foundation. Merge the foundation first, then retarget this PR to master. - Many placements change from icons to character silhouettes. Human avatars and authoritative agent status labels retain their existing behavior. - Only one character can render live per view. Reduced motion, hidden/offscreen content, touch input, and renderer failures use the defined fallbacks. - The full-page stories use fixture data. They do not contact a real company or complete real provider sign-in. ## Model Used OpenAI Codex, GPT-6 family. The exact model identifier and context window are not exposed in this session. fix: prevent duplicate built-in agents and self-heal reconciliation (#10223)
## Thinking Path
> - Paperclip is the open source app people use to manage AI agents for
work.
> - Every company is auto-provisioned a set of built-in agents (e.g. the
Summarizer), and a startup reconciler keeps that set correct across
every company on boot.
> - Provisioning marks these agents with
`metadata.paperclipBuiltInAgent.key`, but nothing in the database
enforced one active agent per `(company, key)` —
`provision()`/`ensure()` did a check-then-insert with no guard.
> - Two concurrent server processes (e.g. a `tsx watch` double-boot)
could both read "no summarizer exists" and both insert, leaving a
company with duplicate built-in agents plus paired orphan pending
`hire_agent` approvals.
> - That data blemish then became a recurring outage: `findSingleAgent`
throws on >1 marked row, and because the throw escaped
`reconcileBuiltInAgentsOnStartup`'s sequential loop, **every company
after the affected one was silently skipped** on each boot — no
auto-provisioning, no default grants — until manual DB surgery.
> - This pull request closes the race at the database level and makes
reconciliation self-healing and fault-isolated.
> - The benefit is that concurrent provisioning can no longer create
duplicates, and even pre-existing duplicates are resolved automatically
instead of bricking startup reconciliation for unrelated companies.
## Linked Issues or Issue Description
- [x] I searched the GitHub PR list (open and recently closed) for
similar PRs and confirmed this is not a duplicate.
No public GitHub issue exists; describing the bug in-PR (bug-report
shape):
**What happened**
A dev instance booted with two concurrent server processes. Both ran
built-in agent provisioning for the same company at the same time, and
the check-then-insert in `provision()`/`ensure()`
(`server/src/services/built-in-agents.ts`) let both writers see "no
summarizer exists" and each create one — the company ended up with two
identical Summarizer agents (identical `paperclipBuiltInAgent` markers)
plus two paired pending `hire_agent` approvals.
From then on, **every** server boot logged:
```
ERROR: startup reconciliation of built-in agents failed
Multiple built-in agents found for summarizer (built_in_agent_duplicate_instance)
```
because `findSingleAgent` throws on >1 marked row rather than resolving
the duplicate. Worse, `reconcileBuiltInAgentsOnStartup` loops companies
sequentially and the throw escaped the loop, so every company *after*
the affected one was silently skipped on every boot.
**Expected behavior**
1. Concurrent provisioning must not create duplicate built-in agents
(there was no DB uniqueness constraint on the marker key per company).
2. Reconciliation should be resilient: if duplicates exist anyway,
self-heal (keep the oldest row, terminate the newer dupe, cancel its
orphan pending `hire_agent` approval), and never let one bad company
abort reconciliation for the rest.
**Steps to reproduce**
- Race two `provision(companyId, "summarizer")` calls for a company with
board approval for new agents enabled (or simulate a double-boot); both
insert.
- Restart the server → startup reconciliation error fires, companies
later in the loop are never reconciled.
## What Changed
**Part 1 — stop creating duplicates**
- Migration `0192_built_in_agent_unique_marker` adds a **partial unique
index** on `(company_id, metadata->'paperclipBuiltInAgent'->>'key')`
where the marker exists and `status != 'terminated'`. It first resolves
any pre-existing duplicates (keep oldest by `created_at`, terminate
newer dupes, cancel their orphan pending `hire_agent` approvals, revoke
their API keys) so the index can be created on already-affected
instances.
- `provision()`/`ensure()` now catch the losing race's `23505` unique
violation (walking the driver's wrapped cause chain) and re-resolve to
the winning row instead of surfacing the error.
**Part 2 — resilient reconciliation**
- `findSingleAgent` self-heals: keeps the oldest marked row, terminates
the newer duplicates, and cancels each one's orphan pending `hire_agent`
approval (idempotent) instead of throwing.
- `reconcileBuiltInAgentsOnStartup` isolates per-company failures in
both loops so one bad company can't abort reconciliation for the rest;
it surfaces a `companyFailures` count in the startup log.
- Adds `approvalService.cancel()` for system-initiated cancellation of
an orphan approval.
## Verification
- `pnpm --filter @paperclipai/db run check:migrations` → numbering +
safety checks pass.
- `packages/db` migration test (real embedded Postgres) — seeds
pre-index duplicate state, runs the migration, asserts dupes resolved +
index enforced: **1 passed**.
- `server` `built-in-agents.test.ts` — self-heal, concurrent races
(plain and board-gated), and startup
self-heal-without-aborting-later-companies: **34 passed**.
```
pnpm --filter @paperclipai/db exec vitest run src/built-in-agent-unique-marker-migration.test.ts
pnpm --filter @paperclipai/server exec vitest run src/__tests__/built-in-agents.test.ts
```
## Risks
- **Migration safety**: the migration mutates data (terminates duplicate
rows, cancels their orphan pending approvals, revokes their API keys)
before creating the index. It keeps the oldest row per `(company, key)`
and only touches non-terminated marked rows; the destructive step is
covered by the migration test and the safety-check baseline. On a clean
instance it is a no-op cleanup followed by `CREATE UNIQUE INDEX IF NOT
EXISTS`.
- Otherwise low risk: the unique index is partial (excludes terminated
rows, so re-provisioning after a termination stays possible), and the
conflict handling degrades gracefully to re-resolving the existing
winner.

thinking, with tool use.

[codex] Add built-in agents and Reflection Coach bundle (#9206)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Operators need first-party agent capabilities for repeatable company work, not just manually created one-off agents. > - Built-in agents need to behave like normal company-scoped agents while preserving approval gates, permissions, budgets, and audit trails. > - Reflection and coaching work also needs bundled instructions, skill content, and a routine so the feature can be installed and reset predictably. > - The API, database, UI, portability, and tests all need to agree on the built-in lifecycle from not provisioned through setup, approval, ready, paused, and reset. > - This pull request adds built-in agent provisioning and the Reflection Coach bundle end-to-end. > - The benefit is a safer first-party path for Paperclip-managed agents without bypassing the same governance model used for operator-created agents. ## Linked Issues or Issue Description No public GitHub issue was found for this exact built-in agent and Reflection Coach bundle work. Problem/motivation: - Paperclip did not have a first-party built-in agent lifecycle for product-owned agents. - Bundled agent resources such as default instructions, skills, and routines needed managed ownership and reset semantics. - Approval-gated companies needed built-in setup to preserve requested adapter, budget, manager, and permission state through board approval. - The board UI needed clear built-in badges, setup affordances, readiness state, and bundle status without exposing secrets. Proposed solution: - Add a company-scoped built-in agent registry, provisioning/reset/reconcile/status APIs, and Reflection Coach bundled resources. - Track bundled managed resources in the database with idempotent migration behavior. - Reuse existing agent approval, authorization, budget, and activity-log paths instead of creating a bypass. - Add UI setup, badges, gates, bundle panels, and route coverage for built-in agents. Duplicate search: - Searched GitHub PRs for `built-in agents Reflection Coach repo:paperclipai/paperclip`; only this PR was returned. - Searched GitHub issues for the same query; no public issues were returned. ## What Changed - Added built-in agent definitions, lifecycle state derivation, provisioning, reset, reconcile, status, and routine-control routes. - Added the `built_in_managed_resources` migration and schema exports for bundled instructions, skill, and routine ownership. - Added the Reflection Coach built-in bundle with default instructions, skill catalog content, routine template, default permissions, and managed-resource drift handling. - Added approval-aware provisioning behavior that preserves requested adapter config, budgets, manager assignment, and built-in permissions through hire approval. - Added authorization and mutation gates for built-in agent and skill changes, including consented Reflection Coach change paths. - Added UI surfaces for built-in agent setup, roster/detail badges, readiness gates, bundle status, routine controls, and route filtering. - Added company import/export and validator coverage for built-in managed resources and low-trust/red-team presets. - Addressed Greptile follow-ups for pending approval reconciliation, consent-gate error propagation, config-read authorization fallback, approval-path manager preservation, and non-model adapter provisioning. ## Verification Local verification: - `git diff --check public/master..HEAD` passed. - `pnpm check:token-gates` passed with all gates clean. - `pnpm exec vitest run ui/src/components/ConfigureBuiltInAgentModal.test.tsx` passed: 1 file, 4 tests. - `pnpm exec vitest run ui/src/components/EntityRow.test.tsx ui/src/pages/Agents.test.tsx ui/src/components/BuiltInAgentGate.test.tsx ui/src/components/ConfigureBuiltInAgentModal.test.tsx ui/src/components/BuiltInBundlePanel.test.tsx ui/src/pages/InstanceExperimentalSettings.test.tsx ui/src/pages/Routines.test.tsx` passed: 7 files, 64 tests. - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/built-in-agents.test.ts src/__tests__/authorization-service.test.ts src/__tests__/company-skills-routes.test.ts` passed: 3 files, 91 tests. - `pnpm --filter @paperclipai/db check:migrations` passed. - `pnpm -r typecheck` passed after the rebase; `pnpm --filter ui typecheck` passed after the final UI review fix. Remote verification on latest head `1c61f693a4ec881d739022b0e75a8ca8bf8c2cd8`: - Merge state: `CLEAN`. - Greptile: `5/5`, zero unresolved Greptile threads. - PR check rollup: all checks successful, neutral, or skipped as expected. - Passing gates include Build, Typecheck + Release Registry, all server shards, all workspace shards, all serialized server suites, e2e, Canary Dry Run, policy, review, verify, Socket, Superagent, and Snyk. ## Risks - This adds a new managed-resource table and migration; the migration uses idempotent create/add/index guards and passed migration safety checks. - Built-in agent provisioning touches approval and authorization paths; tests cover pending approval preservation, stale retry rejection, consent gates, and config-read fallback behavior. - Reflection Coach creates managed instructions, skill, and routine resources; drift/reset behavior is covered by service tests and redacted API responses. - Non-model adapter setup now provisions a `needs_setup` built-in row before command/endpoint fields are complete; this matches the server lifecycle and is covered by the setup modal regression test. ## Model Used OpenAI Codex coding agent based on GPT-5. Exact hosted model ID, context-window size, and reasoning-mode labels are not exposed in this runtime; tool use, shell execution, GitHub CLI/API access, and local code editing were enabled. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix(agents): clear inbox hire approval when approving/terminating from detail page (#8340)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Hiring a new agent creates a `hire_agent` approval record; until it is resolved, the agent sits in `pending_approval` and the open approval surfaces an "Approve/Reject" card in the inbox > - There are two places to act on that hire: the inbox approval card and the agent detail page's Approve/Terminate buttons > - The agent detail page only flipped the agent to `idle` via `activatePendingApproval`, never resolving the linked approval record > - So after approving (or terminating) from the detail page, the approval stayed `pending` and the inbox kept showing a stale "Approve/Reject" card for an agent that was already decided > - This pull request routes the detail-page approve through the shared `approvalsSvc.approve()` (which resolves the approval and runs activation, budget policy, and the hire-approved notification), and rejects the linked approval when a still-pending agent is terminated > - The benefit is a single source of truth: deciding a hire in one place clears it everywhere, so the inbox no longer asks you to approve an agent you already approved ## Linked Issues or Issue Description No public GitHub issue exists for this; describing in-PR (bug report). **What happened:** Create a new agent, then approve it from the agent detail page. The agent activates, but the inbox still shows a "Hire Agent" item with Approve/Reject buttons for it. **Expected:** Once a hire is approved or rejected anywhere in the UI, it should be resolved everywhere — the inbox should not re-ask you to approve an agent that is already decided. **Root cause:** `POST /agents/:id/approve` called `activatePendingApproval`, which only changes the agent's status. The linked `hire_agent` approval row stayed `pending`. The inbox lists approvals filtered to unresolved statuses, so the card persisted. Terminating a pending agent from the detail page had the mirror problem for the "reject" half. Related (not duplicates): #215 (join-request inbox badge), #1815 (approval detail button loading text). ## What Changed - Added `approvalService.findOpenHireApprovalForAgent(companyId, agentId)` to locate the open `hire_agent` approval for an agent. The company/type/open-status **and** `payload->>'agentId'` predicates all run in SQL (jsonb operator), so the DB returns only the relevant row instead of filtering in JS (`server/src/services/approvals.ts`). - `POST /agents/:id/approve` resolves the linked approval through the shared `approvalsSvc.approve()` — running activation, budget-policy upsert, and the hire-approved notification as one path — and falls back to direct `activatePendingApproval` only when no open approval exists (legacy agents created before approvals were tracked) (`server/src/routes/agents.ts`). - `POST /agents/:id/terminate` now branches the same way: when a still-`pending_approval` agent has an open hire approval, it delegates to `approvalsSvc.reject()` (which resolves the approval **and** terminates the agent internally) and re-reads the agent, otherwise it terminates directly. This avoids terminating the agent twice (`reject()` already calls `agentsSvc.terminate()`) (`server/src/routes/agents.ts`). - The `agent.approved` activity log records the resolved `approvalId` (or `null` on the fallback path) for traceability. ## Verification - `pnpm --filter @paperclipai/server typecheck` — clean. - Targeted server tests pass (57 tests): `npx vitest run src/__tests__/approvals-service.test.ts src/__tests__/agent-permissions-routes.test.ts` - `findOpenHireApprovalForAgent` returns the row the SQL filter yields / returns null when none matches. - Approving from the detail page resolves the linked approval via `approvalsSvc.approve()` and does not double-activate; legacy fallback still calls `activatePendingApproval`. - Terminating a still-pending agent with an open approval calls `approvalsSvc.reject()` and does **not** call `agentsSvc.terminate()` a second time; terminating with no open approval terminates directly. - Manual: create an agent → inbox shows the hire card → approve from the agent detail page → inbox card is gone and the agent is active. Same for terminate-while-pending clearing the card. ## Risks Low risk, server-only, no schema or migration changes. The fallback to `activatePendingApproval` preserves existing behavior for agents with no tracked approval record, so legacy agents still activate. The shared approval path is the same one the inbox card already uses, so approve/terminate-from-detail-page now match approve/reject-from-inbox exactly. ## Model Used Claude Opus 4.8 (claude-opus-4-8), extended thinking with tool use, via Claude Code. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots (server-only change, no UI diff) - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups (in progress) - [x] I will address all Greptile and reviewer comments before requesting merge |
