fix(chat): honor dismissal for all composer requests (#17218)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Task conversations show requests in the feed and above the message composer. > - Earlier fixes kept dismissed questions in history, but excluded completion reviews and other request kinds. > - Closing those forms left a pending badge, and the server could still demand a response. > - This pull request saves human dismissal and removes the request from composer reminders and ordinary completion gates. > - People can continue the conversation and reopen the request from history. ## Linked Issues or Issue Description Related work: #15229 and #15392. **What happened?** Dismissing a completion review closed its form but left it counted as pending. It could return after reload, block a retry, or cause the agent to demand another response. Raw runtime requests could also lose their only usable control when dismissed. **Expected behavior** Dismissal leaves an accessible request in the feed and clears the composer form and pending count. It persists after reload. It does not submit an answer, approve an action, or wake the agent. **Steps to reproduce** 1. Open a task with a pending completion review or another composer request. 2. Dismiss the request, then reload the task. 3. Check that the composer stays clear and the feed entry can reopen the request. ## What Changed - Add nullable dismissal fields and a human-only dismissal endpoint with company, private-task access, resolver, and locked review-policy checks. - Persist saved-request dismissal on the server. Keep browser persistence for raw runtime requests and existing question dismissals. - Keep confirmations, choices, item verdicts, suggested tasks, and runtime requests accessible in the feed. - Exclude dismissed ordinary requests from completion and retry gates. Tell agents not to repeat deferred requests. - Preserve tool, secret, connection, plan, company-policy, and configured review authorization gates. Expire deferred non-question requests when their task completes. - Preserve the selected form when another dismissal finishes saving. Keep reopened reviews visible through delayed comment refreshes. - Add a completion-review Storybook test drive and update design, API, database, and capability documentation. ## Verification - `pnpm -r typecheck`: passed. The final UI changes also passed the UI typecheck. - `pnpm build`: passed. The final UI changes also passed the UI production build. - Focused UI and transcript tests: 550 passed across nine files after merging the current base branch. - Focused database, completion, and prompt tests: 105 passed across three files, including two review-policy regressions that failed before the fix. The tests reject a review requester excluded by current policy, preserve the pending gate on denial, and permit an independent reviewer to defer the request. - Route regression suite: 81 passed, including denial before reading or mutating a private-task request. - `pnpm check:token-gates`: passed. - Map and Storybook structural checks: passed; 30 map/catalog script tests passed. - Browser: used the production task-thread components in the completion-review fixture. Dismissed, reloaded, reopened from the feed, and dismissed again. No pending badge or composer takeover remained. Story callbacks simulate API saves; separate server tests cover persistence and permissions. - `pnpm test:run`: stopped after the general-server group reported one failure in `workspace-runtime.test.ts`: "returns a bounded structured conflict and exposes only same-company workspace references". The same test passed in isolation (1 passed, 178 skipped). The cause of the full-suite failure is not established. The full PR CI suite subsequently passed on e243cbb001; its two authorization review findings are fixed in |

feat(cases): add persistent workflows with same-task agent handoffs (#16959)
## Thinking Path > - Paperclip manages AI agents and their work. > - An agent workflow needs persistent facts and a safe way to transfer work. > - Creating a task for each phase splits the objective, discussion, workspace, and review policy. > - A case can retain the facts while one primary task moves between agents. > - Transfers must survive process failure and preserve private access, budgets, and completion gates. > - This pull request adds durable case handoffs, native case tools, and read-only case pages. > - Prompts describe the workflow. Existing task controls decide when work can run and finish. ## Linked Issues or Issue Description **Problem or motivation** Prompt-driven agent teams need a persistent record that can move through triage, implementation, verification, and correction. Ordinary reassignment does not provide a durable instruction, safe release barrier, or recoverable delivery receipt for an agent handing off its own running task. **Proposed solution** Extend standalone cases with one primary task, a descriptive phase, retained access sources, and a handoff ledger. Apply assignments after source execution and workspace settlement. Deliver each transfer through the existing durable wake queue. Provide `create_case`, `read_case`, `update_case`, and `handoff_case` in the native runner. Creation requires kickoff and explicit workflow text. An explicit handoff to `done` closes the case when normal task completion is accepted. **Alternatives considered** Do not add a workflow engine, filesystem queue, configured state graph, or one task per phase. The workflow document combines transition prose with an optional Mermaid `stateDiagram-v2` map. The server does not execute the diagram. The Pipelines experiment removed on master stays removed. **Roadmap alignment** This extends Agent Reviews and Approvals, Enforced Outcomes, Bounded Run Recovery, and Activity Log attribution. Existing approval, review, budget, privacy, workspace, and responsible-user rules remain authoritative. Related work: [ordinary handoff wake recovery](https://github.com/paperclipai/paperclip/pull/15019). This feature adds a separate persisted case-handoff receipt and release barrier. ## What Changed - Add case routing fields, retained access sources, durable handoff history, delivery leases, and replay-safe migrations. - Keep the same primary task across agent phases. Preserve each returning agent's own session and the task's workspace. - Fence outgoing mutations, wait for provider and workspace release, and recover delivery without duplicate task comments or runs. Recheck current assignment permissions as the original caller before applying a delayed transfer. Scope a run Stop to that run’s own handoff intents, so repeated old Stop requests preserve newer work. Cancelling terminal intent without a source run also requires fenced current task ownership, including native retries and preparation with an old checkout. For Cloud callers, revalidate current stack membership with an uncached Cloud RPC and derive owner elevation from the current managed setting. - Keep case reads, documents, assets, Audit, and live events inside retained task/project permissions. - Add native case creation/read/update/handoff tools. Store required workflow text and initial documents on the first task. Page oversized case facts and referenced document bodies through the dedicated tool with bounded UTF-8 JSON, current routing versions, snapshot conflicts, and current source authorization on every page. - Add read-only case lists and detail pages, task-style list controls, compact dotted field tables, linked handoff tasks, and case filters in Audit. - Add native/legacy integration tests, migration/restart tests, browser coverage, and bounded live receipt fixtures. Extend the trusted sandbox condition to cover the new subscription fixture profile. - Hide obsolete paused-agent warnings on completed or cancelled task threads. - Synchronize the company-switch settings test with React effects after a CI failure. Preserve its security assertions and production remount behavior. ## Verification The branch directly includes master `9ef434fe79`. The required `ci / verify` and `ci / e2e` gates pass on head `765fccd67c`. [Current-head CI](https://github.com/paperclipai/paperclip/actions/runs/39464348614) passes typecheck, build, tests, and all eight browser shards. Security checks pass. [Greptile gives this exact head 5/5](https://github.com/paperclipai/paperclip/pull/16959#issuecomment-6291156549), with no unresolved review threads. The optional Paperclip Review check remains queued because its assigned agent is paused. It is not a required branch check. No check result was overridden. Local workspace typecheck and build pass after the master merge, along with database migration checks, token gates, map checks, Storybook catalog checks, and the Copilot profile identity check. Standard builds refresh Runner vendor output. Focused checks pass 2,008 API/paging/context tests and 7 strict case protocol tests. The final commit changes one test fixture only; its ten tests, UI typecheck, and token gates pass. No production source changed in that commit. On current head `765fccd67c`, all **194 critical workflow tests** executed and passed with no skipped tests in those files: - 38 Stop/ownership tests, including board completion intent, native retries, stale Stop, and preparation with an old checkout. - 2 actual legacy process/HTTP handoff tests, including scratch failure, a closed-stdio child that continues writing, and recovery through a fresh worker after the process group stops. - 5 migration tests, including SQL column metadata, preserved null authority context, and all six prior-numbering receipts. - 3 process-settlement and 5 scratch tests. - 88 handoff-service, 18 heartbeat handoff, and 21 native case-tool integration tests. - 7 bounded case/document-read projection tests and 7 strict protocol-contract tests. The current CI checkout was `fed65d49`, merging `765fccd67c` into master `9ef434fe79`. Its new document regressions reconstruct exact Unicode/escaped bodies and revisions through real native authority and authenticated HTTP with generic API tools disabled. They cover case-owned, primary-task, and linked-task documents; routing on every page; stale document snapshots; access loss and unlinking; unrelated/foreign/private sources; system documents; and quarantined content. Active-task document tools retain their existing behavior. These are current-head test results from the first CI attempt; the browser-only retry did not rerun these suites. The full `64fdd3403f` CI failed one separate settings test: after a company switch, it observed an unsettled API-key draft. The production component and test were unchanged from prior green source. Production switches remount both keyed settings layers; the test directly reused an unkeyed view and used a timer tick instead of waiting for React effects. The final commit replaces that fixture synchronization with React `act`. The exact secret-clearing assertions and all timeouts remain unchanged. The original assertion failure remains recorded; no unchanged retry was used. The narrow unchanged baseline and fixed fixture each pass 10 tests. The upstream result-paging merge preserves case capture namespaces and current asset privacy. Its API integration suite executed 39 of 41 tests successfully, with two skipped; the optional assigned-result PRP integration test was skipped. Neither skipped suite is counted in the 194 critical tests or as live provider proof. All 361 upstream migration entries and SQL bodies remain unchanged. Six case migrations follow master as 0363–0368 with identical SQL hashes. Official Drizzle generators produced the combined metadata and five retained snapshots. Generation reports no schema changes, and the snapshot drift test passes. The earlier `ce361ae5d0` CI passed on attempt 1, including its then-current 179 critical tests. Fresh review later identified unusable document references with generic API tools disabled; the bounded document reads and ten additional authenticated HTTP tests address that finding. The full local `pnpm test:run` attempt was cancelled after this Mac exhausted PostgreSQL semaphore capacity; it is **not** counted as a pass. Later database attempts also stopped before assertions or skipped, including one sandbox socket denial. Two original stale-Stop regressions did reproduce the bug against the old source. A REST serialization assertion passed after two preserved database-startup failures. No test timeout was increased. All 194 assertions listed above executed and passed in current-head CI. The complete 385-file, 3,963-test UI shard also passed after the fixture repair. A configured-provider receipt workflow passed 13 backend checks across triage → implementation → verification → correction → done, using one task and four work runs. The saved receipt is revision 2 with total 6 USD; case and task both closed. The original canonical attempt remains failed because its browser assertion confused the terminal destination with completed-task tooltips. The corrected full browser verifier passed on the same retained database and original runs, with no additional provider calls. The last clean-build browser readback is on `6a86c54a53`; later changes affect recovery, authorization, bounded reads, and settlement. Accounted work cost is a $1.010574 rate-card estimate; provider billing and setup-probe usage are unavailable. Copilot profile 49 passed the unchanged seven-fixture private protocol pack (46/46 checks, no retries) and the product file-edit canary after master's completion-replay change. All 60 bound implementation hashes remain unchanged on the current source. Historical profile-47/48 identities and results remain intact. See the [profile-49 compatibility record](https://github.com/paperclipai/paperclip/blob/codex/persistent-case-handoffs/doc/plans/2026-10-20-copilot-completion-recovery-compatibility-v49.json). This is bounded compatibility evidence, not a fresh full matrix, installed-package product test, or Daytona rollout. Preserved CI failures and retries: - On `20662930ac`, Slack managed setup failed a five-second success assertion after reload. Its test/application path was unchanged from a prior green head. One unchanged-source retry passed all 25 tests in that shard; no source or timeout changed. - On `9402b4f17a`, two CI workers received shutdown signals at 15:24:19 UTC. Neither log had an assertion failure before interruption. One unchanged-source retry of each passed all Runner static checks and all 32 tests in browser shard 6; both aggregates passed. The next head, `7b4374edaf`, passed all CI without retries. - On `295f289b5a`, serialized shard 9 timed out during its first device-login test, then the next test found an active login lease. Direct login/test paths were unchanged from a prior green head. Cold import plus a timed-out async body using the next test's mutable actor/store is the supported fixture-race explanation; no request trace proves that sequence. The original failure remains recorded. One unchanged-source retry passed all 294 tests in 19 files, including all 41 device-login tests. No login source or timeout changed. The subsequent `ce361ae5d0` CI passed without retries. - On `64fdd3403f`, one company-switch settings assertion observed an unsettled draft. The final test-only commit uses React `act` without changing assertions, timeouts, or production code. On `765fccd67c`, all ten settings tests and the complete 385-file, 3,963-test UI shard passed. - On `765fccd67c`, browser shard 3 completed the split-handoff assertions, then cleanup received HTTP 500 from run cancellation after a one-second PostgreSQL row-lock timeout. The extra chat run was reporting completed work while teardown began. Cleanup stopped before restoring a global experiment flag, causing a later assertion to fail. Relevant production and test paths are unchanged from an earlier green commit. Stop's lock order is consistent, and no nested self-lock was found; the exact PostgreSQL blocker was not recorded. One unchanged-shard retry passed. The original failure, trace, and downstream flag failure remain recorded. No Stop implementation, cleanup assertion, or timeout changed. The retry supports intermittent cleanup contention; it does not identify the original blocking transaction. ## Risks - Local process transfers require host-owned PID/group absence. Missing or uncertain termination evidence keeps delivery pending, independently of scratch setup. - Handoff admission and task cancellation share execution ownership. New regression tests cover temporary holds, late source writes, and uncertain provider termination. - Retained private sources use an intersection of existing permissions. This intentionally fails closed when a source is missing or an existing task cannot preserve that boundary. - Migrations add tables and indexes. They preserve master history and can replay over prior feature installs. Migration 0368 adds nullable internal authorization provenance with no default or authority backfill. Historical transfers still awaiting release without this context fail closed; an operator must cancel them and request a new handoff. - Delayed Cloud handoffs depend on the current Cloud membership RPC. An unavailable, ambiguous, expired, or mismatched response holds the handoff for retry; it does not reuse cached authority or grant access. - Bounded reads reload and reauthorize the selected projection per page. Underlying work grows with the data size and number of pages. Changed data or routing requires restarting from offset zero without the old snapshot. - Phase names and workflow diagrams remain advisory. They do not satisfy review or approval gates. - This is a large, cross-cutting PR. The maintainer requested one PR for the whole feature; the commits separate data, runtime, UI, and qualification work. ## Model Used OpenAI Codex, GPT-6 family, with tool use, code execution, and delegated code review. The exact deployment model ID and context-window size were not exposed in the session. Live fixture agents used the explicitly configured provider model recorded in their test evidence. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix(recovery): recover retained workspaces before saved messages (#16906)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - A stopped run can leave useful files in its original sandbox. > - The server keeps that sandbox when it cannot confirm file copyback. > - The task can then save new user input but offers no safe way to recover the files. > - A retry of the old agent turn can repeat work that already finished. > - This pull request recovers files through the existing workspace owner and delivers only saved human input. > - The user can recover a task without certifying infrastructure facts or losing completed work. ## Linked Issues or Issue Description Refs #16883 and #16835 for publication receipts and verified process termination. Related: #16478 clears stale holds after copyback already succeeded. This change performs the missing export and records its proof. #16901 manages the main GitHub response comment and does not perform workspace repair. **What happened?** A failed run retained its workspace. A later user message stayed saved behind a file-repair hold. The task asked the user to verify process termination and file recovery but offered no repair control. Sending a message could show an optimistic Working state and move the task to Todo before admission succeeded. **Expected behavior** The harness should recover the original files when it can prove ownership and termination. It should preserve newer edits and completed operations. Only waiting user input may start another turn. Missing evidence must keep the source held with a specific explanation. **Steps to reproduce** 1. Interrupt a sandbox run before workspace finalization succeeds. 2. Retain its exact allocation and save a user follow-up. 3. Observe the file-repair hold and missing end-user repair path. ## What Changed - Add source-bound background recovery for native and legacy runs. An accepted native result is not required. - Share the physical copyback lock and durable owner. Resume only the exact stopped allocation, export files, verify persistence and obtain a fresh stop receipt. - Persist retry limits and idempotent requests on the existing recovery record. Limit each server to two background exports. Add an indexed candidate scan and a separate cleanup obligation. - Add a task-editor recovery endpoint. Deny agents and viewers. Reject caller-supplied stop or repair claims. - Deliver waiting human input once through ordinary admission. Preserve pause, approval, budget and assignment gates. Require historical conversational-adapter evidence for legacy-to-Runner continuation. - Include exact GitHub publication receipts in continuation context. Preserve unknown effects and the original failed run. - Show observed recovery progress, a transient-failure retry action and specific service blockers. Keep saved messages visible. Suppress false startup feedback and implicit Todo promotion behind a file hold. Hide unavailable workspace links. - Update execution semantics, capability maps and production-component stories. ## Verification - Red: the original read model failed the new recovery-progress assertion; the other 23 checks in that file passed. - Passed: recovery regressions with 28 cases with real PostgreSQL, files, tar and Git operations. Lifecycle provider calls are controlled test boundaries. Coverage includes native/legacy export, concurrent owners, rollback after export, interrupted archive export with a new database connection, lost replies, historical discovery, durable retry limits, separate cleanup and saved-input delivery. The actual candidate query uses its partial index with 3,000 unrelated history rows. A first automatic repair can bind verified old-controller stop proof without an accepted native result. Review also reproduced and fixed export-only continuation and cleanup lost after a committed repair. Cleanup remains discoverable without waiting input and after task reconciliation. - Passed: two GitHub integration cases verify that continuation context carries the existing publication receipt and does not write another response. Company and task boundaries are checked. - Passed: 293 continuation, workspace-sync, platform-receipt and OpenAPI checks. After file repair, actual dispatch retains pause, cancellation, approval, real budget-policy and assignment gates. - Passed: 174 route checks, including editor admission, agent/viewer denial and rejection of fabricated repair claims. - Passed: 351 task detail, workspace properties and comment-reopen checks after updating the new read boundaries and asynchronous workspace-link expectations. Two new route cases keep held tasks Blocked when a comment is saved. - Passed: 265 task-thread and recovery-notice checks. The broader continuation suite also passed; its new legacy migration case first exposed a gap that the focused recovery suite now covers. - Passed after rebase: `pnpm -r typecheck` and `pnpm build`. The later master merge adds an unrelated permission migration; recovery migration generation and safety checks passed after the merge. - Passed: token gates, module boundaries, map structure and migration checks. The recovery index is migration 0360 after current master. - Browser: inspected the production recovery component and its retry-to-queued transition with fixture data. The first dev dependency load failed; a reload after optimization resolved it. This is UI evidence, not a live provider recovery claim. - Every test shard passed on final commit `bbbc09efb4`. The older local `pnpm test:run` was stopped after the complete CI test matrix passed. Local full typecheck and production build passed. The first CI build compiled and booted, then lost its runner during the clean public npm-install check. An isolated retry passed the build, production boot and clean public npm-install verification. - Greptile reviewed final commit `bbbc09efb4` at 5/5. All review threads are resolved. - The change is not deployed. Hosted recovery remains unverified until the served revision includes this change. ## Risks - Recovery crosses process, provider and persistence boundaries. Missing or mismatched ownership, stop, descriptor or file evidence keeps the hold. It never selects a replacement allocation or silently starts over. - File recovery does not certify external action outcomes or authorize replay. New input still uses normal execution admission. - Historical sources without sufficient durable evidence can remain blocked. Destructive reconstruction stays separate. - The index migration is idempotent. It changes no task data. Snapshot pruning follows repository policy. ## Model Used OpenAI Codex, `gpt-6.1-sol`, with code execution and browser tools. `gpt-6-astra` assisted earlier investigation. The runtime does not expose the context-window size. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(ui): refine dashboard cards, charts, and recent lists (#13269)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Operators use the dashboard and Live runs page to inspect current and recent runs. > - Large transcript cards take space and make it hard to compare run states. > - A compact card must show the agent, linked task, and time, with access to run details. > - This pull request applies the supplied card design to both views. > - Operators can scan more runs and open a run or task for details. ## Linked Issues or Issue Description **What existing behavior does this improve?** The agent run cards on the dashboard and Live runs page, plus dashboard charts and recent lists. **Subsystem affected** `ui/` — React board UI. **Current behavior** Both views show large cards with embedded transcripts. Queued and running cards share a live treatment. The dashboard chart grid leaves an empty column, recent lists use different row sizes, and some activity labels expose raw event names. **Proposed behavior** Both views use the same compact grid. Each card shows the existing initials avatar, the agent name, one linked task row, and a right-aligned timestamp. There is no status chip next to the agent name. The timestamp uses small Inter text and the same muted gray as dashboard metric descriptions. Recovery details remain in the task view. Run status remains in the header tooltip and accessible label, and run transcripts remain in the run detail view. The shared in-progress task icon is now an animated circular spinner across the app. **Reason and benefit** Operators can scan task states without scrolling through embedded output. The in-progress spinner keeps the shared task icon sizes, circular shape, and stroke width. It honors reduced-motion preferences. **Breaking changes** The dashboard and Live runs page no longer show transcripts inside cards. Open the agent header to inspect a run. There are no API or database changes. **Additional context** Related public work found during the duplicate search: - Refs #4317 — related run-state display concerns. This change does not change server state reconciliation. - Refs #11394 — related live-run cache updates. This change retains the existing data flow. - Refs #2118 — related navigation from agent cards. The card header here opens the specific run. ## What Changed - Apply compact cards to the dashboard and Live runs page, with status icons and theme tokens. - Use 16 × 16px status icons in the shared cards, including the missing-task clock. - Keep the existing circular initials avatar. Remove the status chip beside the agent name while preserving the run label for tooltips and accessibility. - Replace the shared in-progress task glyph with an animated open circle. Use the same 10-unit radius and 2-unit stroke as the other task glyphs, with a reduced-motion guard across all task status surfaces. This is the requested workflow-status indicator across the app, including between runs; live indicators separately report active execution. - Use the same Tailwind blue tokens as nav dots and Live labels for the progress icon: blue-400 in dark mode and blue-600 in light mode. Covered-blocked icons follow the same blue token. - Match the Tasks by Status chart's In Progress bar and legend to the spinner's theme-aware color token. - Distribute the three visible dashboard charts across equal-width desktop columns, preserving existing gaps, card padding, and page margins. Retain four columns when the optional priority chart is enabled. - Use the done task icon's `--status-task-icon-done` token for green bars in Run Activity, Tasks by Status, and Success Rate, including their legends. - Use the blocked task icon's `--status-task-icon-blocked` token for red bars and legends in Run Activity, Tasks by Status, Success Rate, and priority charts. - Align Recent Tasks live counts to the sidebar's right edge. Reveal the ellipsis over a fading row surface on hover, keyboard focus, and while its menu is open, without moving the label or count. - Keep the ellipsis backdrop synchronized with the row background during fade-out, using the actual sidebar surface in both themes to prevent a darker flash. - On coarse-pointer devices, reserve space for the always-visible ellipsis and disable its fade so the live count remains readable beside it. - Order desktop dashboard Recent Tasks as status icon, task title, agent avatar/name, 11px monospace task ID, then timestamp. In narrow cards, keep the ID right-aligned beside the truncated title and place the agent and timestamp on the second line. - Give both dashboard lists the same 48px desktop rows, 24px leading slots, 80px desktop task ID columns, and 64px right-aligned timestamp columns. Use an 8px icon-to-content gap matching the task detail heading. Narrow layouts use matching 76px rows with intrinsic-width IDs on the title line; activity timestamps stay below. - Order dashboard Recent Activity as actor avatar, actor name, verb, task title, 11px monospace task ID, then timestamp. Use direct verbs such as “Board read …” instead of “issue read marked”. Truncate titles to preserve IDs and timestamps; keep full names and titles in tooltips. - Use 12px task status icons in the top breadcrumb and Properties status row. Keep the main task title icon at 20px. - Reduce monospace task IDs to the 11px micro type token in breadcrumbs and shared run cards, aligning them to the adjacent task titles' text baseline while keeping status icons centered. - Show the task title and identifier in one bordered row. Keep missing-task links usable and show lookup failures. - Remove the recovery chip and place the timestamp below the task row. - Keep the link to all runs available when the dashboard has four or fewer runs. - Avoid transcript polling for compact cards. - Fix bundled Inter font URLs and update the design guide and Storybook fixtures. - Cover run navigation, task status, failed lookups, shared icon sizes and stroke, and reduced-motion-safe animation in tests. ## Verification - Latest review fixes: targeted SidebarRecentTasks, SidebarNavItem, StatusGlyph, StatusIcon, ActiveAgentsPanel, activity-format, ui-font-assets, and Dashboard suites — 65 tests pass, including read/unread verbs and reduced-motion-safe task animation. - `pnpm --filter @paperclipai/ui typecheck` — passes. - `pnpm check:token-gates` — all gates pass. - `git diff --check` — passes. - Related component suites passed during development: run cards, status glyphs, breadcrumbs, issue properties, charts, sidebar navigation, recent-task actions, and settings sidebar. - Full repository `pnpm -r typecheck` and `pnpm build` pass on the final commit using the temporary Rust toolchain. Storybook build passed earlier in preparation. - All CI gates pass on `c85e949b12b471d425d216caa609aee41dc58bde`: all general/workspace and serialized server test shards, all three browser shards, build/runner verification, typecheck/release registry, canary dry run, policy, and security checks. The duplicate local `pnpm test:run` was stopped after CI completed the same test suites; it is not claimed as a completed local pass. The two Storybook jobs are skipped by the configured draft-PR workflow. - Greptile reviewed the final commit at 5/5 with no actionable findings and zero unresolved review threads. - Browser measurements at 1654px, 860px, and 390px confirm matching 48px desktop and 76px narrow rows (plus 1px dividers), right-aligned IDs on the title line, 64px timestamp slots, and no horizontal overflow. Narrow Recent Tasks rows retain readable agent names below the title. Activity reads as actor, verb, title, ID, time. Additional reference details can expand an activity row. - Verified three equal-width desktop charts with 16px gaps and aligned outer edges. Chart blues, greens, and reds resolve to the matching task-icon tokens. - Verified real queued, running, succeeded, failed, cancelled, and timed-out runs in the dev dashboard. Checked compact cards, task/run links, timestamps, light/dark themes, and narrow layout. - To inspect: open Dashboard, then Live runs. Both pages use the same small cards. Open an agent header for run output, or the task row for task details. - Verified the updated spinner on the real dashboard cards, Recent tasks, and task list. Checked actual SVG circle radius, rendered size, and stroke width. Confirmed the spinner, nav dots, and Live labels resolve to the identical blue-400 color in the dark-mode preview; light mode uses their shared blue-600 token. - Verified 12px computed width and height in the task breadcrumb and Properties pane, the restored 20px main task icon, and 11px task IDs with baseline alignment in the breadcrumb and shared dashboard/Live runs cards. - Verified the chart bar and legend resolve to the spinner color; Recent Tasks counts align with Dashboard's count. Checked the fading ellipsis overlay with keyboard focus and its open-menu state, then dismissed the menu. ## Risks - Operators must open run details to read output that was previously embedded. - Run outcomes are available in the header tooltip and run details instead of a visible status chip. An in-progress task icon reflects task status, independently of an individual run's outcome. - Long names and task titles truncate within the compact layout. Full labels remain available through links and tooltips. - Font loading changes affect all UI text that uses the bundled Inter font. The files remain served from the existing public fonts directory. - No schema, API, adapter, or execution-policy changes. ## Model Used OpenAI Codex, GPT-6 Astra (`gpt-6-astra`), with extended reasoning, repository tools, code execution, and browser verification. feat(server): log an activity row for each queued-comment queue mutation (#13159)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - The server records actions that change issues and their queued comments > - The queued-comment edit, reorder, and discard routes changed queue state without activity rows > - Operators could not inspect these queue mutations in the activity feed > - This pull request adds one identifier-only activity row for each successful queue mutation > - The benefit is a durable audit trail with no comment text in the activity log ## Linked Issues or Issue Description **What existing behavior does this improve?** The queued-comment edit, reorder, and discard routes now record their successful mutations in the activity feed. **Subsystem affected** Cross-cutting (server and ui). **Current behavior** The three queue mutation routes change queued comments but do not write an activity row. The activity feed has no label for these actions. **Proposed behavior** Each successful route writes one activity row with the actor fields, entity fields, queue identifiers, and queue revision. The discard row also includes the cancelled run identifier. The activity feed shows a label for each action. **Reason and benefit** Operators need a durable record of queue changes. Identifier-only details support audit and troubleshooting without storing comment text. **Breaking changes** None. The routes keep their existing response and authorization behavior. **Additional context** Each mutation writes its activity row on the same locked transaction that applies the mutation, so the two commit or roll back together. The route publishes the live activity event only after that transaction commits. The separate comment-cancel route opts out of this write and keeps its existing single activity row. ## What Changed - Add activity rows for queued-comment edit, reorder, and discard mutations. - Include queue identifiers, revisions, ordered comment identifiers, and cancelled run identifiers as applicable. - Add activity-feed labels for the three new actions. - Add route and activity-format tests for the new behavior. - Write each activity row on the same transaction as the mutation it records, through a new port method that the adapter implements. - Keep the comment-delete route opted out of that write, so a cancellation does not log two rows. ## Verification - [x] `npx vitest run server/src/__tests__/issue-queued-comments-routes.test.ts` passes. - [x] `npx vitest run ui/src/lib/activity-format.test.ts` passes. - [x] `pnpm --filter @paperclipai/server typecheck` exits 0. - [x] `pnpm --filter @paperclipai/ui typecheck` exits 0. - [x] `node scripts/check-module-boundaries.mjs` passes. - [x] The full CI suite is green. ## Risks Low risk. The change adds activity rows after successful mutations and does not change route responses, authorization, or stored comment text. ## Model Used OpenAI Codex, GPT-5 Codex. The model used repository inspection, Git operations, and command execution. The context window and reasoning mode are not exposed by this runtime. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes: #` / `Refs: #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(apps): unify permissions and action testing (#12802)
## Thinking Path > - Paperclip is the control plane for companies that use AI agents. > - Apps give humans and agents controlled access to external services. > - The existing app detail flow split permissions, tests, setup, and activity across separate pages. > - The split made access rules harder to understand and made reconnect work hard to find. > - New write actions also defaulted to Ask first, which did not match the intended connection policy. > - This pull request combines permission control and action testing, removes the setup page, and moves connection activity into Audit. > - The benefit is one clear place to configure, test, reconnect, and review each app. ## Linked Issues or Issue Description **What existing behavior does this improve?** The installed app Permissions, Test, Setup, and Activity views. **Subsystem affected** Cross-cutting. This change updates the React UI, shared app defaults, server permission behavior, tests, smoke scripts, and connection documentation. **Current behavior** App access and action testing use separate pages. The app detail view also links to a setup page after installation. Connection activity uses a separate tab. New write actions default to Ask first. **Proposed behavior** Permissions uses the connection access language from the initial flow. It includes searchable Read and Write sections, a three-state permission control, and a Test dialog for each action. Reconnect appears below a Needs attention header on Permissions and Review. Old Setup and Test links redirect to Permissions. Old Activity links redirect to the filtered company Audit feed. New write actions default to Allowed. **Reason and benefit** A person can understand and test app access without moving between several pages. Reconnect work stays visible where the person reviews the connection. Audit events use one consistent feed and filter model. New connections have the intended default policy. **Breaking changes** The Setup, Test, and app Activity tabs are removed. Existing deep links redirect to their replacement pages. Existing saved action permissions do not change. Only defaults for new write actions change. **Additional context** This builds on the managed app connection work in #12728. A search found no duplicate open pull request or issue. ## What Changed - Combined action testing with Permissions. - Added searchable Read and Write action groups. - Added Off, Ask first, and Allowed controls with tooltips. - Added an action Test dialog with agent selection, arguments, and formatted results. - Removed the installed-app Setup and Activity tabs. - Added reconnect guidance to Permissions and Review when a connection needs attention. - Routed connection activity into the company Audit feed and preserved the Apps & tools filter in streamlined Audit. - Moved connection removal to the Connectors-page management menu. - Made new write actions default to Allowed across connection creation paths. - Updated regression tests, browser suites, smoke scripts, and connection documentation. ## Verification - `pnpm check:token-gates` - `pnpm exec vitest run packages/shared/src/app-definitions.test.ts server/src/__tests__/generic-mcp-connection.test.ts server/src/__tests__/tool-access-service.test.ts ui/src/components/AppConnectionSidebar.test.tsx ui/src/pages/apps/AppDetail.test.tsx ui/src/pages/apps/AppNotConnected.test.tsx ui/src/pages/apps/AppsConnect.test.tsx ui/src/pages/apps/Browse.test.tsx ui/src/pages/apps/Connections.test.tsx ui/src/pages/apps/composio-services.test.ts ui/src/pages/audit/AuditFeed.test.tsx ui/src/pages/tools/PasteConfigTab.test.tsx` (517 tests passed) - `pnpm exec vitest run ui/src/pages/apps/app-detail/TestPanel.test.tsx ui/src/pages/audit/AuditHub.test.tsx ui/src/pages/audit/AuditFeed.test.tsx ui/src/pages/apps/AppDetail.test.tsx ui/src/pages/apps/Browse.test.tsx` (96 tests passed) - Targeted Playwright verification for connection removal, rename on Permissions, inline action testing, and Smoke Lab Audit evidence (5 flows passed) - `pnpm -r typecheck` - `pnpm build` - `pnpm test:run` completed with 5,755 passing tests and 20 unrelated macOS harness failures. The failures use `/tmp` versus `/private/tmp`, invalid ports above 65535, and workspace fixtures outside this change. ## Risks - Low migration risk. This change has no database migration. - Old app-detail URLs depend on redirect compatibility. - New connections grant write actions by default. Finalization remains configure-authorized and audited, Ask first and Off remain available per action, and existing connections keep their saved policy. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected - check the roadmap first. See `CONTRIBUTING.md`. ## Model Used OpenAI Codex, exact model ID `gpt-5`. The client does not expose the context-window size. The model used reasoning, repository tools, code execution, and browser verification. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(ui): complete the task workspace (#12640)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - The task page is the main place where people guide and review agent work. > - Native runner events already project into the existing task chat on the lower stack. > - The larger task workspace must support those events without breaking direct adapters. > - Legacy questions, final replies, empty transcripts, and classic controls must keep their behavior. > - This pull request completes the provider-neutral task workspace experience. > - The benefit is one coherent task surface for native and direct execution paths. ## Linked Issues or Issue Description Refs #12639. Refs #12617. Refs #12352. **Subsystem affected** Task chat, task detail, side panels, interaction forms, and transcript presentation. **Problem or motivation** The task page does not provide one complete workspace for live activity, plans, questions, queued guidance, files, and documents. Earlier native UI work also exposed compatibility risks in legacy question and reply paths. **Proposed solution** Add the task workspace components and provider-neutral protocol presentation. Keep runner-only controls behind runtime facts. Preserve all direct-adapter composer, transcript, interaction, and finalization behavior. **Alternatives considered** A separate runner page would duplicate task behavior. Replacing legacy transcript logic would create unnecessary adapter regressions. **Roadmap alignment** This work supports the unified task experience and the Cloud and Sandbox agents milestone. ## Stack - Base PR: #12639. - Lower PR: #12638. - This PR contains only its 119-file UI delta against `runner/remote-wss-transport`. - The next stack PR adds administrator and observability controls. ## What Changed - Added a reusable task workspace side panel for files and documents. - Added provider-neutral cards for tools, plans, questions, protocol activity, and progress. - Added queued guidance and richer composer state. - Added compact and expanded interaction presentation. - Added live activity, thinking, usage, recovery, and final reply presentation. - Added document annotations and task deep links. - Added bounded question validation and response handling. - Preserved native runner event projection from master. - Preserved legacy channel-less replies and explicit final reply precedence. - Preserved direct-adapter and classic-interface controls. - Did not change server execution selection, Rust code, migrations, workflows, or `pnpm-lock.yaml`. ## Verification - GitHub Actions will run UI tests, repository tests, typecheck, build, browser tests, security, and policy gates. - Tests cover active, settled, empty-transcript, interaction, queued-message, plan, question, file, document, and classic-interface states. - Compatibility tests cover direct adapters and native runner projection together. - Local tests were not run. The requested verification policy uses GitHub Actions for this series. - `git diff --check runner/remote-wss-transport...HEAD` passes. - The delta contains 119 UI-only files. ## Risks - This is a large task-page change. - Most files are new focused components and tests. - Shared transcript code keeps the lower native projection and legacy direct-adapter fallbacks. - Runner-only controls use adapter and runtime facts. - No execution path or rollout flag changes in this PR. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used OpenAI Codex with GPT-5.6. feat(ui): badge the review policy when it constrains approval (#10938)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Agents move their work into review, and a reviewer must then give a verdict on it > - By default anyone with write access can give that verdict, including the agent that did the work > - The server can constrain that default per issue with a `reviewPolicy` column, but no screen showed the value > - A reviewer could therefore press Approve on a review that the server refuses, and get a 403 > - This pull request shows the policy as a badge on the two surfaces where a person gives a verdict > - It also makes an agent verdict read as a verdict in the activity timeline > - The benefit is that a reviewer sees who can approve before they try ## Linked Issues or Issue Description No public GitHub issue exists for this change. The description below follows `.github/ISSUE_TEMPLATE/enhancement.yml`. **What existing behavior does this improve?** The issue review flow. A reviewer cannot see the approval constraint on an issue before they give a verdict. **Subsystem affected** Web UI (`ui/`), with one supporting change in the server attention service. **Current behavior** The server stores an optional approval constraint for each issue in a `reviewPolicy` column. The column has three meaningful states: the default (`NULL` or `anyone`), `not_creator`, and `human_only`. The server enforces the constraint when it receives a verdict. No screen shows the value. Two problems follow: 1. A reviewer presses Approve on a review that the server refuses. The server answers 403, and the reason is not visible on the card. 2. An agent that accepts or rejects a review renders in the activity timeline as the raw action id, for example "issue thread interaction accepted". A person who reads the timeline cannot tell that a verdict was given. **Proposed behavior** Show the constraint as a read-only badge on the two surfaces where a person gives a verdict. Show no pixels for the default state, because the default is what every issue already does. Make an agent verdict read as a verdict in the timeline. Only agents set the column today, so this change adds no control to set it. **Reason and benefit** A reviewer sees the constraint before they act. This prevents the 403, and it removes the need to explain the 403 afterwards. The timeline also becomes complete, because it now shows agent verdicts and human verdicts in the same way. **Breaking changes** None. The change adds a badge and changes copy. It adds no column, no endpoint, and no request. **Additional context** The server-side column and the verdict enforcement landed earlier in #10931. This pull request is the user interface for that column. The default state stays unchanged on screen, so the badge appears on a small number of issues. ## What Changed - **A read-only "Approvals" row** in the issue Execution properties. The row renders *only* for a constrained policy: "Anyone else" (`not_creator`) or "Human only" (`human_only`). A `NULL` or `anyone` column adds no row, so the panel is untouched on the overwhelming majority of issues. - **The same badge on the stalled-review card** in `/decisions`, above the three review verbs. A reviewer now sees the constraint before they press Approve. The condition is the same, so the default card is unchanged. - **Agent verdicts read as verdicts in the activity timeline.** An agent that accepted or rejected a review request previously rendered the raw action id ("issue thread interaction accepted"). It now reads "approved the request". A stalled-review decision names the verb that the actor chose. - **A cleared policy reports as "anyone", not "none",** in the field-change receipt. The `reviewPolicy` column is nullable by default, so an absent value is a real setting rather than a missing one. - **All copy comes from `ui/src/lib/review-policy.ts`.** Its badge lookup returns `null` for the default. This makes "no pixels for the default" one enforced decision instead of a condition repeated at each call site. It also keeps the badge, the activity line, and the receipt reading alike. - **The server attention service carries the policy** on the review attention subject, so the stalled-review card can read it. ## Verification Automated tests: - `ui/src/lib/review-policy.test.ts` — the default returns no badge, however the column spells it (`null`, `undefined`, `"anyone"`). An unrecognised policy from the wire shows nothing rather than leaking an enum value. - `ui/src/components/AttentionQueueRow.test.tsx` — no badge on the default card, and the verbs still render. Suppression of the badge must not suppress the card. - `ui/src/components/IssueProperties.test.tsx` — no Approvals row on the default. The constrained row contains no `button`, so nothing there can PATCH. - `server/src/__tests__/attention-service.test.ts` — the review attention subject carries the policy, and subjects built from narrower selects do not claim one. Run them with: ```sh pnpm vitest run ui/src/lib/review-policy.test.ts \ ui/src/components/AttentionQueueRow.test.tsx \ ui/src/components/IssueProperties.test.tsx \ server/src/__tests__/attention-service.test.ts ``` Manual steps: 1. Open an issue that has no `reviewPolicy`. Confirm that the Execution properties panel shows no Approvals row. 2. Set the column to `not_creator`. Reload the issue. Confirm that the Approvals row reads "Anyone else", and that the row has no control. 3. Move that issue into review. Open `/decisions`. Confirm that the stalled review card shows the same badge above the review verbs. 4. Let an agent approve the review. Confirm that the activity timeline reads "approved the request" and not "issue thread interaction accepted". Screenshots were captured at 1440x900 and 390x844, in light mode and dark mode, with the three policy states side by side. The leftmost column in each capture is the default. It carries no badge and no extra row. ## Risks Low risk. - The change is additive on screen. Every new surface is behind a constrained policy, so the default path renders exactly as before. - The badge is read-only. It has no control and sends no request, and a test asserts that the row contains no `button`. - An unknown policy value from the wire renders nothing. It does not render the raw enum. - No migration, no schema change, and no endpoint change. ## Model Used Claude Opus 5 (Anthropic), model id `claude-opus-5[1m]`, 1M context window, with extended thinking and tool use enabled. feat(issues): explain cross-task agent writes with attribution, audit receipts, and actionable denials (#10843)
## Thinking Path
> - Paperclip is the open source app people use to manage AI agents for
work
> - Agents write to tasks they do not own. They comment, they change
fields, and the control plane now permits this by default for
standard-trust agents on any task they can read
> - This makes a task thread ambiguous. A reader sees a comment from an
agent that is not the assignee, but no surface says whose authority that
write rode
> - The same gap applies to field edits. The activity stream named the
verb, but it did not show the before value, the after value, or the
reason the write was permitted
> - The remaining refusals are also opaque. An agent that hits a wall
receives a 403 with no boundary name, no actor who can act, and no
sanctioned path. One real incident spent a full detour to find the
workaround
> - This pull request adds the three surfaces that make open cross-task
writes legible: an attribution chip, a field-level audit receipt, and an
actionable denial contract shared by the API and the UI
> - The benefit is that a reader can answer "who did this, on whose
authority, and was it allowed?" on the task itself, and a blocked writer
is told what to do next
## Linked Issues or Issue Description
No public issue exists for this work, so the enhancement is described
here.
**What existing behavior does this improve?**
Cross-task agent writes are permitted, but they are not explained. A
task thread can hold comments from agents that are not the assignee, and
the activity stream can hold field changes made by those agents. Neither
surface names the responsible user behind the write. When a write is
refused, the error text does not name the boundary or the way forward.
**Subsystem affected**
Issue detail UI (comment thread and activity stream), the issue write
authorization responses in the server, and the shared copy contract that
both consume.
**Current behavior**
- An agent comment on a task the agent does not own looks the same as an
assignee comment.
- An `issue.updated` activity row states the verb only. It does not show
the field-level before and after values, the responsible user, or the
authorization reason.
- A refused write returns a short message such as an ownership error.
The message does not state which rule fired, who is able to perform the
action, or which alternative path is sanctioned.
**Proposed behavior**
- An agent comment on a task the agent does not own carries a chip that
reads "for {user}". The chip names the responsible user. Its tooltip
states that the author is not the assignee and cannot exceed that user's
permissions.
- Each `issue.updated` row shows a receipt: the changed fields with
before and after values, the responsible user, and the authorization
reason. This applies to board edits as well as agent edits.
- Each refusal states three things: the boundary that fired, who is able
to act, and the sanctioned path. The API error body and the in-app
notice use the same words, because both read one shared contract.
Related pull requests, found by searching this repository:
- Refs #10837 — merged. It added the default-open cross-task write rule,
the comment attribution data, and the per-run containment cap that this
pull request makes visible.
- Refs #10114 — open. It proposes a narrower authorization change in the
same area.
- Refs #7998 — open. It proposes append-only cross-assignee comments as
an alternative to opening writes.
## What Changed
- Adds `packages/shared/src/issue-write-denial.ts`. This is one copy
contract for eight ways an issue write can be refused: not visible,
responsible-user ceiling, responsible user unavailable, excluded actor
class, assignee run lock, per-run cross-task cap, missing run context,
and rejected attribution. Each entry names the boundary, who can act,
and the sanctioned path.
- Maps server authorization decisions onto that contract in
`server/src/routes/issues.ts` and
`server/src/services/cross-issue-influence-limit.ts`. The flattened
`error` string carries all three obligations, and `details.code` lets
the UI render the same words. The two cap codes keep the names they
already ship under.
- Adds `CommentAttributionChip`. It renders "for {user}" beside the
author name on agent comments where the author is not the assignee. It
renders nothing when no responsible user is recorded, so older rows stay
clean. It is wired into both `IssueChatThread` and the flagged
`TaskChatThread` redesign.
- Adds `IssueFieldChangeReceipt`. It renders the change receipt under
`issue.updated` rows in the activity stream. Ids resolve to agent and
user names where the directory is loaded. Server-truncated text is
labelled as a preview, so the receipt never implies that it shows a
whole value.
- Adds `IssueWriteDenialNotice`. It renders the shared copy in the app,
keyed off the denial events the server logs on a task.
- Adds a public `/ux-lab/cross-issue-collaboration` page. It renders all
three surfaces and their edge cases for review without a seeded thread.
This follows the existing `ux-lab` pages.
## Verification
Automated, all green:
```
pnpm --filter @paperclipai/shared exec vitest run src/issue-write-denial.test.ts # 17 tests
pnpm --filter @paperclipai/ui exec vitest run src/components/IssueWriteDenialNotice.test.tsx \
src/components/IssueFieldChangeReceipt.test.tsx src/components/CommentAttributionChip.test.tsx \
src/lib/issue-change-receipt.test.ts src/lib/comment-attribution.test.ts # 46 tests
pnpm --filter @paperclipai/server exec vitest run src/__tests__/cross-issue-influence-limit.test.ts \
src/__tests__/low-trust-red-team-routes.test.ts # 98 tests
```
`tsc --noEmit` passes for the shared, ui, and server packages.
Manual, in a browser:
1. Start the UI only: `pnpm --filter @paperclipai/ui exec vite`.
2. Open `/ux-lab/cross-issue-collaboration`. No session is needed,
because `ux-lab` routes are public.
3. All three surfaces were captured at 1440x900 in light mode and dark
mode, and at 390x844. The page reported no errors.
4. The chip tooltip was opened by a hover and by a keyboard focus.
Rendering the page found defects that the tests had missed. Three copy
and contrast defects were fixed, and two of them are now pinned by a
test. A design review then found three layout defects, which are also
fixed: the denial notice orphaned its label when a value wrapped, the
receipt icon wrapped onto its own line at narrow widths, and the chip
tooltip was reachable by hover only.
## Risks
Low risk, and additive.
- Every new surface renders nothing when its data is absent. Comments
without a recorded responsible user show no chip, and activity events
without a receipt show no receipt, so existing rows do not change.
- No migration is included. The data these surfaces read already ships.
- The wire values of the two per-run cap denial codes are unchanged.
Only the human-readable text changes, plus six codes that had no
`details.code` before.
- The denial copy is read by agents as well as people. If wording must
change later, one shared module is the only place to change it.
- Roadmap check: this extends the completed "Activity log & action
attribution" area rather than duplicating planned core work.

feat(audit): agent audit UI — company page + per-agent tab (#9744)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Operators need an audit record of agent actions across tasks, comments, documents, approvals, and runs > - The permission-gated audit read API provides that record, but operators cannot inspect it in the product > - A readable UI must preserve company boundaries, server-side permission decisions, and redaction > - Audit exports must also be safe to open in spreadsheet software and must record the export itself > - This pull request adds company and per-agent audit views plus a guarded CSV export > - The benefit is a searchable, filterable, and reviewable agent action history with direct links back to work ## Linked Issues or Issue Description **Feature.** This change adds the frontend and CSV export for the agent action audit log. Refs #9731 and #9735. - Problem: agent actions are recorded, but operators have no readable product surface to inspect or export them. - Solution: add a company audit page and a per-agent Audit tab that use the permission-gated audit API. - Alternative: build a separate plugin-only surface. This was rejected because the existing permission model already supports a unified, server-authoritative view. This pull request targets the audit epic branch, which contains the merged #9735 audit API. ## What Changed - Added a company Audit page and sidebar entry. - Added a per-agent Audit tab with a fixed agent filter. - Added filters for agent, responsible user, action domain, entity type, and date range. - Added task and run links, responsible-user context, cursor pagination, and readable action text. - Added a permission-denied Enterprise card for callers without `audit:view_agent_actions`. - Added a CSV export that is permission-gated, capped, self-audited, CSV-escaped, and protected against spreadsheet formula injection. - Preserved the merged audit API cursor validation, redaction, and sub-millisecond pagination behavior. ## Verification - `pnpm exec vitest run ui/src/pages/audit/AuditFeed.test.tsx` — 6 passed. - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/agent-action-audit-routes.test.ts` — 8 passed with embedded PostgreSQL. - `pnpm -r typecheck` — passed across all workspaces. - `pnpm build` — passed across all workspaces. - `pnpm test:run` — all completed shards passed except one environment-sensitive CLI assertion caused by injected static AWS credential variables; the exact test passes 8/8 with those variables unset. - Manual Chromium QA exercised the populated feed, active filters, permission-denied card, per-agent tab, and CSV export. ## Screenshots and Manual QA - [All audit states exercised in Chromium](https://github.com/paperclipai/paperclip/pull/9744#issuecomment-4998997001) - [Detailed browser report and per-agent tab root cause](https://github.com/paperclipai/paperclip/pull/9744#issuecomment-4998771061) The per-agent redirect defect found during QA is fixed in this branch. ## Risks Low to moderate risk. The UI and export route are additive and use the existing company-scoped permission gate. The main risks are large exports and spreadsheet interpretation. The export is capped at 10,000 rows, records truncation accurately, and prefixes formula-like cells as text. There are no schema changes or migrations. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. feat(secrets): add user-specific runtime secrets (#8825)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Agent runs often need provider credentials, API tokens, and other environment-bound secrets. > - Company-level secrets work for shared credentials, but they do not model values that should differ by human operator. > - Without a user-scoped model, a run can dispatch without knowing whether the responsible human has supplied the needed value. > - Paperclip also needs run attribution to make those user-scoped runtime checks deterministic and auditable. > - This pull request adds user-specific secret definitions, per-user values, environment bindings, responsible-user attribution, and runtime resolution gates. > - The benefit is that teams can define the secret once, let each user provide their own value, and block runs before dispatch when required user secrets or active definitions are unavailable. ## Linked Issues or Issue Description Refs #224 Refs #6057 This PR implements user-specific secret support as a core secret-management capability rather than a one-off adapter setting. It is related to existing public work on company secrets UI and runtime secret refs, but is distinct because the value is owned by the responsible user and resolved at run dispatch time. Related PR search before opening found existing secrets work such as #1550, #8256, #8614, #8634, and #8647; none of those add the full user-secret definition/value/runtime gate covered here. ## What Changed - Added user-secret definitions and per-user "My secrets" values, keeping stored values out of access metadata. - Added `user_secret_ref` environment bindings and UI affordances to pick them alongside existing secret refs. - Added responsible-user runtime resolution so user-secret refs resolve against the human responsible for the run. - Added pre-dispatch missing-secret gates so runs fail before adapter dispatch when required user values are absent or definitions are inactive. - Added low-trust allowlist hardening for user-secret runtime access. - Added issue, routine, run, and agent API key responsible-user attribution and fail-closed dispatch behavior when attribution cannot be resolved. - Added denial-copy mapping so responsible-user authorization failures surface as actionable run outcomes instead of opaque setup failures. - Added OpenAPI documentation for the user-secret routes. - Rebases cleanly on current `master`; migrations were renumbered incrementally as `0128_user_specific_secrets`, `0129_agent_api_key_responsible_user`, and `0130_run_responsible_user_invariant` after upstream `0126`/`0127` migrations. - Removed previously committed local design screenshots so the PR contains code/docs/tests only. ## Verification - PASS: PR head `2527febd106bcf3ca264ca0da7fca491084192d6` is based on `paperclipai/paperclip:master`. - PASS: `git diff --check` - PASS: `git diff --name-only public/master...HEAD | rg '^(pnpm-lock\\.yaml|\\.github/workflows/|screenshots/)' || true` produced no files. - PASS: migration journal audit confirmed unique indexes through `130` with tail entries `0126_issue_comment_derived_attribution`, `0127_environment_custom_images_instance_scoped`, `0128_user_specific_secrets`, `0129_agent_api_key_responsible_user`, and `0130_run_responsible_user_invariant`. - PASS: `pnpm --filter @paperclipai/ui typecheck` - PASS: `pnpm --filter @paperclipai/server typecheck` - PASS: `pnpm --filter @paperclipai/server exec vitest run src/__tests__/heartbeat-responsible-user-invariant.test.ts` - PASS: `pnpm --filter @paperclipai/server exec vitest run src/__tests__/heartbeat-active-run-output-watchdog.test.ts src/__tests__/heartbeat-stale-queue-invalidation.test.ts src/__tests__/heartbeat-workspace-finalize-branch.test.ts src/__tests__/issue-monitor-scheduler.test.ts` - PASS: `pnpm --filter @paperclipai/server exec vitest run src/__tests__/heartbeat-comment-wake-batching.test.ts src/__tests__/heartbeat-retry-scheduling.test.ts src/__tests__/heartbeat-accepted-plan-workspace-refresh.test.ts src/__tests__/heartbeat-plugin-environment.test.ts` - PASS: `pnpm --filter @paperclipai/server exec vitest run src/__tests__/low-trust-red-team-routes.test.ts` - PASS: `pnpm --filter @paperclipai/server exec vitest run src/__tests__/secrets-service.test.ts` (55 tests) - PASS: `pnpm vitest run server/src/__tests__/secrets-routes.test.ts server/src/__tests__/secrets-service.test.ts` (89 tests after final Greptile cleanup fixes) - PASS: `pnpm --filter @paperclipai/server exec vitest run src/__tests__/heartbeat-issue-liveness-escalation.test.ts` (17 tests after the final rebase CI fix) - PASS: focused server Vitest batches covering heartbeat recovery, project env, plugin env, routines, low-trust, pipelines, monitors, watchdog, and stale queue paths. - PASS: GitHub checks are green on `2527febd106bcf3ca264ca0da7fca491084192d6`, including Typecheck + Release Registry, Build, General tests, serialized server suites, e2e, Canary Dry Run, verify, security checks, and Greptile Review. - PASS: Greptile Review completed successfully on `2527febd106bcf3ca264ca0da7fca491084192d6` with Confidence Score 5/5, and GraphQL review-thread audit returned zero unresolved non-outdated threads. ## Risks - Runtime behavior now depends on a run having a correct responsible user; missing or incorrect responsibility assignment can block runs before adapter dispatch. - `user_secret_ref` bindings intentionally expose metadata without values, but UI/API callers may need to handle the new binding kind explicitly. - External secret providers and IAM policies are not automatically provisioned by this PR; operators still need to configure provider-side access for non-local vaults. - The PR is broad across db/shared/server/UI/runtime paths, so release validation should include both API and UI secret workflows before merge. - The migration renumbering is intentionally incremental after upstream migrations; the branch migrations use guarded column/table/index/constraint creation so users who tested the older draft numbering should not hit duplicate DDL for the existing objects. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. [codex] Add clear-error agent action (#7695)
## Thinking Path > - Paperclip is the open source control plane people use to manage AI agents for work. > - Agent runtime state is surfaced in both the server API and the board UI so operators can tell whether an agent is idle, running, paused, or in error. > - When an agent is already in `error`, the existing pause/resume action slot is not useful because there is no running work to pause. > - Operators need a direct, audited recovery path that clears the stale error state only for agents in the same company. > - This pull request adds a company-scoped clear-error mutation, exposes the shared API contract, and wires the board action cluster to show Clear error in the pause/resume slot for errored agents. > - The benefit is that operators can recover CEO/CTO-style errored agents without resorting to database edits or unrelated session reset actions. ## Linked Issues or Issue Description Refs #4021 Paperclip issue: PAP-10515 — right now the CEO and CTO agents are in error state, but there is no way to clear the error; they appear otherwise fine. ## What Changed - Added shared constants, API path, and agent status type support for a company-scoped clear-error action. - Added the server service and route to clear an agent from `error` back to `idle`, with company access enforcement and activity logging. - Added OpenAPI/docs coverage for the clear-error endpoint. - Added backend coverage for service behavior and cross-tenant authorization. - Updated the board agent action cluster to show a red-tinted Clear error button only when `agent.status === "error"`. - Updated agent properties to show a red active last-error indicator only while the agent is currently errored. - Added UI component tests for the error-state action and the non-error pause/resume behavior. ## Verification Local: - `pnpm exec vitest run server/src/__tests__/agents-service-clear-error.test.ts server/src/__tests__/agent-cross-tenant-authz-routes.test.ts ui/src/components/AgentActionButtons.test.tsx` - `pnpm --filter @paperclipai/ui typecheck` - `pnpm exec vitest run server/src/__tests__/openapi-routes.test.ts` PR checks: - Main Paperclip workflow is green on `a7378e584d50594e7bd507a1a02985bfaaa5abf8`. - Greptile is 5/5 with no files requiring special attention and no new comments on the latest review. - `commitperclip PR Review` is still red because its security-gate step canceled after filing a draft advisory; the linked `security-review` check is neutral and says the draft advisory is not a merge block. Visual artifact: - ## Risks Low to medium risk. The mutation is intentionally narrow, but reviewers should check that clearing `lastError`/`lastRunError` and returning to `idle` is the desired recovery semantics for every adapter state. The remaining red check is from the external commitperclip security-review workflow, not from the code/test workflow for this PR. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. PAP-10430: split Issue-to-Task copy migration (#7651)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - The board UI is the operator surface where users create, assign, monitor, and review work items. > - The product language is moving toward "tasks" for user-facing work items while the internal API and database still use "issues". > - PR #7543 bundled this copy migration with broader information-architecture work, which made the branch too large for Greptile review. > - This pull request peels the Issue-to-Task copy migration into a smaller, independently reviewable change. > - The benefit is clearer user-facing terminology, less agent confusion via the Paperclip skill note, and a smaller PR that Greptile can review. ## Linked Issues or Issue Description Refs #7645 Refs #7543 Refs PAP-10430 This PR was split out of #7543 so the Issue-to-Task copy migration can be reviewed separately and the remaining IA PR can fall under Greptile's file limit. ## What Changed - Preserves Scott Tong's original `PAP-57` copy-only commit, with author and co-author credit intact, to rename user-facing "Issues" copy to "Tasks" across the UI while keeping routes/API/internal symbols as `issue`. - Updates onboarding and release-smoke browser selectors from `Create & Open Issue` to `Create & Open Task`. - Adds a terminology note to `skills/paperclip/SKILL.md` clarifying that task and issue refer to the same Paperclip work item. - Resolves the only cherry-pick conflict by keeping current search artifacts support and changing visible search copy to "tasks". ## Verification - `pnpm --filter @paperclipai/ui build` passed. - `NODE_ENV=test pnpm exec vitest run ui/src/components/IssuesList.test.tsx ui/src/components/Sidebar.test.tsx ui/src/components/NewIssueDialog.test.tsx ui/src/pages/IssueDetail.test.tsx` passed: 4 files, 66 tests. - `git diff --check origin/master...HEAD` passed. - Diff is 80 files, below Greptile's 100-file limit. - Before/after UI copy examples: "Issues" -> "Tasks", "New Issue" -> "New Task", "Create & Open Issue" -> "Create & Open Task". ## Risks - Medium copy-risk: this intentionally changes user-facing terminology broadly while keeping internal issue identifiers and routes unchanged. - Some docs and APIs still say `issue`; the skill note clarifies this so agents do not treat task and issue as separate entities. - Browser-level visual validation is expected from CI because this local container is missing usable browser dependencies. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used Scott Tong authored the original `PAP-57` copy migration, assisted by Claude Opus 4.8 and Paperclip agents per the preserved commit metadata. fix: Stop archived companies from waking agents (#7478)
## Thinking Path
> - Paperclip orchestrates AI agents for zero-human companies
> - Each agent has a heartbeat scheduler that wakes it on timers and on
events; every wake spawns an adapter (Claude / Codex / …) run that bills
the operator's subscription
> - When an operator archives a company, the agents inside it remain in
invokable states — the heartbeat scheduler never consults company status
— so timers keep firing and event-driven wakes (comments, mentions,
blockers-resolved, etc.) keep cascading
> - On real deployments this silently drains the operator's
subscription: idle archived companies wake their CEOs hourly, plus any
cross-company event cascade
> - This pull request enforces "archived ⇒ never spawns a run" as a
structural invariant by guarding the wake path AND cascading agent state
on archive/reactivate
> - The benefit is that archived companies stop billing the operator,
and the UI/queue stays consistent with the invariant
## What Changed
- `server/src/services/heartbeat.ts`:
- `enqueueWakeup()` loads the company and short-circuits when status is
not `active`. Background sources (timer, automation, events) write a
`company.inactive` skipped wake and return `null`; explicit user invokes
throw a `conflict` so the UI surfaces the real reason.
- `tickTimers()` joins agents to active companies so the scheduler does
not iterate archived-company agents at all (no skip-row noise).
- `server/src/services/companies.ts`:
- `archive(id, actor?)` pauses runnable agents with `pauseReason =
"company_archived"` inside the transaction (preserving
`pending_approval`, `terminated`, and agents paused for unrelated
reasons), then cancels `queued`/`running` heartbeat runs after the
transaction commits.
- `update(id, data, actor?)` reverses the cascade only for agents whose
`pauseReason === "company_archived"` on the `archived → active`
transition; manually-paused agents stay paused.
- Both methods emit activity-log entries (`company.archived` with
`agentsPaused` + `runsCancelled`, `company.reactivated` with
`agentsRestored`) so the audit trail fires regardless of caller.
- `packages/shared/src/constants.ts` + `server/src/services/budgets.ts`:
add `company_archived` to the legal `PauseReason` union so the
restorable marker is a first-class value.
-
`packages/db/src/migrations/0094_backfill_archived_company_agent_pauses.sql`:
backfill so existing archived-company agents become `paused /
company_archived` (excludes `pending_approval`).
- `ui/src/lib/activity-format.ts`: add the `company.reactivated` label.
## Verification
- `npx vitest run src/__tests__/companies-service.test.ts` — archive
cascade, reactivate cascade, and activity-log entries (with counts) all
pass.
- `npx vitest run
src/__tests__/heartbeat-archived-company-guard.test.ts` — timer +
on-demand + event-wake paths all blocked for archived companies;
`company.inactive` skipped-wake row written; user-initiated wakes throw
`conflict`.
- `pnpm typecheck` — clean.
- Manual repro from the bug description: archive a company, wait an
interval / post a comment on one of its issues, observe zero new
heartbeat runs.
## Risks
- Migration `0094` is a single bulk UPDATE on `agents` joined to
archived `companies`. On large deployments it briefly holds row locks on
archived-company agent rows; should be quick because the predicate is
narrow (`status NOT IN (paused, terminated, pending_approval)` and
`companies.status = 'archived'`).
- New `pauseReason` value (`company_archived`) is opaque to older
clients that only know the previous union. Acceptable because the union
is read as plain text and the contract is sync'd in the same change.
- Behavior change for users: invoking an agent in an archived company
now fails with a conflict instead of silently spawning a run. Intended.

## Related Work
Fixes #1348 (`[Bug] Archived companies still running heartbeats and
consuming tokens`).
Prior attempts and parallel work in this area:
- #1365 and #1429 by @hungdqdesign (March 2026) — both closed without
merging. Same three-layer shape (`tickTimers` / `enqueueWakeup` /
`resumeQueuedRuns` + archive-route cancellation) targeting #1348. Credit
for first publicly proposing the wake-path-guard approach.
- #5865 by @stubbi (May 2026, open) — adds the same `companies.status !=
'archived'` joins to `tickTimers`, `enqueueWakeup`, `resumeQueuedRuns`,
**and** routines `tickScheduledTriggers`, bundled with plugin-table
tenant isolation (`plugin_entities` / `plugin_job_runs` / `plugin_logs`
/ `plugin_webhook_deliveries` get a `companyId` FK with `ON DELETE
CASCADE`). This PR is narrower — it does not touch routines or plugin
tables — but adds the **archive cascade** (pause agents with
`pauseReason = "company_archived"`), the **reactivate reverse**
(un-pause only that subset), the **`company_archived` pause-reason
marker**, and a **backfill migration** for pre-existing archived
companies, which #5865 does not include. Happy to coordinate sequencing
or rebase if #5865 lands first.

## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies, so planning approvals and child-issue fan-out are part of the core control-plane loop. > - Accepted plans are supposed to be a safe bridge from planning into execution, especially when agents wake from review decisions and reuse isolated workspaces. > - The duplicate-subtask incident showed that an accepted plan revision could be interpreted more than once across overlapping runs, which broke the single-source-of-truth model for issue decomposition. > - Fixing that required tightening the backend contract first: accepted-plan decomposition needs an exact-once fingerprint, durable claim state, and retry-safe child creation. > - Once that backend behavior existed, the board still needed visibility into what happened, so the issue detail view needed a dedicated decomposition section instead of forcing operators to reconstruct child creation from raw activity. > - This pull request adds the exact-once decomposition primitive, hardens wake routing and regressions around the incident, and surfaces decomposition state in the UI so future incidents are both prevented and easier to inspect. ## What Changed - Added accepted-plan decomposition semantics to `doc/execution-semantics.md`, including the exact-once fingerprint, durable claim/result expectations, and retry/resume behavior. - Added persistent accepted-plan decomposition claims in the backend, including schema, shared types/validators, service logic, and issue routes for creating and listing decomposition state. - Hardened heartbeat routing so an accepted-plan continuation stays scoped to the relevant planning issue instead of opportunistically re-decomposing another accepted issue on the same assignee. - Added regression coverage for the original failure modes: concurrent same-parent retries, cross-issue accepted-plan isolation, and partial child recreation under the same fingerprint. - Added the `Plan decomposition` issue-detail section plus supporting API/query-key/activity formatting updates so operators can see revision status, owner, child counts, and the linked child issues directly in the UI. - Included the small follow-up UI fix so the decomposition section still renders when the issue work mode is no longer `planning`. ## Verification - `pnpm --filter @paperclipai/server typecheck` - `pnpm --filter @paperclipai/ui typecheck` - `pnpm --filter @paperclipai/db typecheck` - `pnpm exec vitest run server/src/__tests__/issues-service.test.ts` - `pnpm exec vitest run server/src/__tests__/issues-service.test.ts -t "lists persisted decompositions with child issue summaries"` - `pnpm exec vitest run server/src/__tests__/issues-service.test.ts -t "accepted plan decomposition" server/src/__tests__/heartbeat-accepted-plan-workspace-refresh.test.ts server/src/__tests__/heartbeat-context-summary.test.ts` - Manual UI path: create a planning issue without an isolated execution workspace, add a `plan` document, accept the `request_confirmation`, let Paperclip create child issues, then reopen the parent issue detail page and confirm the `Plan decomposition` section shows the accepted revision, status, idempotent-claim badge, and child links. - Separate follow-up bug noted during manual UI validation: accepting a plan on an issue whose run never records `workspace_finalize` is tracked in `PAPA-445` and is not part of this PR’s fix scope. ## Risks - This adds a new migration and a large Drizzle snapshot update; reviewers should confirm the schema shape and generated metadata match the intended decomposition table. - The exact-once claim changes sit on the accepted-plan fan-out path, so regressions there could block legitimate child creation or mis-handle retries if the claim state machine is wrong. - The new UI only appears when decomposition records exist; reviewers should use the manual verification path above rather than expecting existing issues on a stale local instance to show the section automatically. - `PAPA-445` remains an open follow-up for the `workspace_finalize` accept gate when a planning handoff never records finalize; that bug can interfere with reproducing the UI flow on isolated workspaces but does not change the correctness of the exact-once decomposition feature itself. > Checked `ROADMAP.md`: this PR is a bug fix / control-plane hardening change for accepted-plan decomposition, not a new uncoordinated roadmap feature. ## Model Used - OpenAI Codex via Paperclip `codex_local` (GPT-5-based coding agent; exact backend model ID/context window not exposed in the run context), with repository tool use, shell execution, and code-editing capabilities. <img width="806" height="1069" alt="Screenshot 2026-05-27 at 11 05 48 PM" src="https://github.com/user-attachments/assets/5b00b670-96cd-4470-b0a3-581743bcae28" /> ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

[codex] Roll up May 17 branch changes (#6210)
## Thinking Path > - Paperclip is the control plane for autonomous AI companies, so agent work needs visible ownership, recovery, and operator controls. > - This local branch had accumulated several related control-plane reliability and operator-experience fixes across recovery actions, watchdog folding, model-profile defaults, mentions, markdown editing, plugin launchers, and small UI polish. > - The branch needed to be converted into a PR against the current `origin/master` without losing dirty work or including lockfile/workflow churn. > - The safest standalone shape is a single rollup PR because the recovery/server/UI files overlap heavily across the local commits and splitting would create avoidable conflicts. > - This pull request replays the local branch onto latest `origin/master`, preserves the uncommitted work as logical commits, and adds a Zod 4 validator compatibility fix found during verification. > - The benefit is that the May 17 local branch can be reviewed and merged as one coherent, conflict-free branch under the 100-file Greptile limit. ## What Changed - Rebased the local May 17 branch work onto current `origin/master` in a dedicated worktree. - Preserved and committed previously dirty changes for recovery retry handling, plugin/sidebar launcher polish, and `.herenow` ignores. - Added recovery-action behavior for returning source issues to `todo` when retrying source-scoped recovery. - Included the existing local recovery/liveness/watchdog fold, Codex cheap-profile, markdown/mention, duplicate-agent, and UI polish commits from the branch. - Normalized shared validator `z.record(...)` schemas to explicit string-key records for Zod 4 compatibility. - Confirmed the PR has no `pnpm-lock.yaml` or `.github/workflows/*` changes and stays below the 100-file Greptile limit. ## Verification - `pnpm install --frozen-lockfile --ignore-scripts` - `npm run install` in `node_modules/.pnpm/sqlite3@5.1.7/node_modules/sqlite3` to build the local native sqlite3 binding after installing with scripts disabled - `pnpm exec vitest run packages/shared/src/validators/issue.test.ts packages/shared/src/project-mentions.test.ts packages/adapter-utils/src/server-utils.test.ts server/src/__tests__/heartbeat-model-profile.test.ts server/src/__tests__/issue-recovery-actions.test.ts server/src/__tests__/issue-agent-mutation-ownership-routes.test.ts server/src/__tests__/heartbeat-active-run-output-watchdog.test.ts server/src/__tests__/plugin-local-folders.test.ts ui/src/components/IssueRecoveryActionCard.test.tsx ui/src/components/Sidebar.test.tsx ui/src/components/SidebarAccountMenu.test.tsx ui/src/components/IssueProperties.test.tsx ui/src/components/MarkdownEditor.test.tsx ui/src/components/MarkdownBody.test.tsx ui/src/lib/duplicate-agent-payload.test.ts ui/src/pages/Routines.test.tsx` - First pass: 13 files passed with 201 passing tests; 3 server files failed before sqlite3 native binding was built. - After rebuilding sqlite3: `server/src/__tests__/heartbeat-model-profile.test.ts`, `server/src/__tests__/issue-recovery-actions.test.ts`, and `server/src/__tests__/heartbeat-active-run-output-watchdog.test.ts` passed/loaded; embedded Postgres tests were skipped by the local host guard. - `pnpm --filter @paperclipai/shared typecheck` - `pnpm --filter @paperclipai/adapter-utils typecheck` - `pnpm --filter @paperclipai/server typecheck` - `pnpm --filter @paperclipai/ui typecheck` ## Risks - Medium risk: this is a broad rollup PR across recovery semantics, server tests, shared validators, and UI surfaces. - Some embedded Postgres tests skipped locally due the host guard, so CI should provide the stronger database-backed signal. - UI changes were covered by component tests, but no browser screenshot was captured in this PR creation pass. - This branch may overlap with existing recovery/liveness PR work; merge this PR independently or restack/close overlapping branches rather than merging duplicate implementations together. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI Codex, GPT-5-based coding agent, tool-enabled local repository and GitHub workflow, medium reasoning effort. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [ ] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

[codex] Add issue document locking (#6009)
## Thinking Path > - Paperclip orchestrates AI-agent companies through company-scoped issues, comments, and issue documents. > - Issue documents are the durable place where plans, handoffs, and other work artifacts are revised over time. > - Some documents need to be preserved as operator-approved snapshots while agents continue working on the same issue. > - Without document locking, a later board or agent write can overwrite the document key that reviewers expected to remain stable. > - This pull request adds board-managed issue document locks and makes agent writes to locked keys create a derived document instead of mutating the locked document. > - The benefit is safer document handoffs: approved or frozen issue documents stay immutable until the board explicitly unlocks them. ## What Changed - Added `locked_at`, `locked_by_agent_id`, and `locked_by_user_id` document fields plus migration `0085_tranquil_the_executioner.sql`. - Added document lock/unlock service behavior, route endpoints, activity events, and locked-document write protections. - Made agent document writes to locked keys create a new derived key such as `plan-2` rather than overwriting the locked document. - Surfaced lock state through shared issue document types, UI API methods, document header lock controls, and activity formatting. - Added server and UI tests for lock/unlock behavior, locked document immutability, and UI action visibility. - Updated `doc/SPEC-implementation.md` with the V1 document lock contract and endpoints. ## Verification - `git rebase public-gh/master` completed cleanly after committing the branch changes. - `git diff --check` passed before commit. - `pnpm run preflight:workspace-links && pnpm exec vitest run server/src/__tests__/documents-service.test.ts server/src/__tests__/issue-agent-mutation-ownership-routes.test.ts ui/src/components/IssueDocumentsSection.test.tsx ui/src/components/IssueContinuationHandoff.test.tsx ui/src/lib/document-revisions.test.ts` passed: 5 files, 32 tests. ## Risks - Medium risk because this changes the document persistence contract and adds a migration. - The migration uses `ADD COLUMN IF NOT EXISTS` and guarded foreign-key creation so it remains safe for users who may have already applied an earlier copy of the migration. - Locked documents intentionally reject board edits/deletes/restores until unlocked; any existing workflows that expected direct overwrite need to unlock first. - Agent writes to locked keys now create derived documents, which may create extra issue documents when agents retry locked writes. ## Model Used - OpenAI Codex coding agent based on GPT-5, with tool use and local code execution in the Paperclip worktree.

[codex] Add source-scoped recovery actions (#5599)
## Thinking Path > - Paperclip is a control plane for autonomous AI companies, where work must end with a clear disposition rather than ambiguous agent liveness. > - Recovery currently detects stalled or missing-next-step issues, but source issue recovery can become split across child recovery issues, blockers, and comments. > - That makes it harder for operators and agents to see who owns recovery and what exact action is needed on the original issue. > - Source-scoped recovery actions give the original issue a first-class active recovery state with owner, evidence, wake policy, and resolution outcome. > - This pull request adds the recovery-action data model, backend reconciliation and resolution APIs, and board UI indicators/actions. > - The benefit is clearer stalled-work recovery without losing source issue context or relying on comments as the liveness path. ## What Changed - Added the `issue_recovery_actions` schema, shared types/constants/validators, and an idempotent `0084_issue_recovery_actions` migration ordered after current `master` migrations. - Updated stranded/missing-disposition recovery to create source-scoped recovery actions, wake the recovery owner on the source issue, and avoid locking the source issue for recovery-action wakes. - Added API support for reading active recovery actions on issue detail/list surfaces and resolving them with restored, blocked, cancelled, or false-positive outcomes. - Require blocked recovery resolutions to have an unresolved first-class blocker, and removed the UI shortcut that could mark recovery blocked without a blocker selection path. - Surfaced recovery indicators/actions in the issue UI, blocker notices, active run panels, issue rows, and Storybook coverage. - Updated docs and focused tests for recovery semantics, ownership, races, stale comments, and UI behavior. ## Verification - `pnpm exec vitest run server/src/__tests__/issue-recovery-actions.test.ts server/src/__tests__/heartbeat-process-recovery.test.ts ui/src/components/IssueRecoveryActionCard.test.tsx ui/src/components/IssueBlockedNotice.test.tsx ui/src/api/issues.test.ts` — 5 files, 72 tests passed. - `pnpm --filter @paperclipai/shared typecheck` — passed. - `pnpm --filter @paperclipai/db typecheck` — passed, including migration numbering check. - `pnpm --filter @paperclipai/server typecheck` — passed. - `pnpm --filter @paperclipai/ui typecheck` — passed. - Follow-up verification after blocker-resolution guard: `pnpm exec vitest run server/src/__tests__/issue-recovery-actions.test.ts ui/src/components/IssueRecoveryActionCard.test.tsx ui/src/api/issues.test.ts` — 3 files, 27 tests passed. - Follow-up `pnpm --filter @paperclipai/server typecheck` — passed. - Follow-up `pnpm --filter @paperclipai/ui typecheck` — passed. - UI states are available in `ui/storybook/stories/source-issue-recovery.stories.tsx`; screenshot capture helper is `scripts/screenshot-recovery-card.cjs`. ## Risks - Medium: recovery behavior changes from child recovery issue ownership toward source-scoped actions, so operators may see stalled-work state in new places. - Migration risk is mitigated by using the next migration slot after `master` and making the table/constraints/index creation idempotent for anyone who previously applied the old branch-local `0082_dizzy_master_mold` migration. - Existing child recovery issue paths are still guarded for already-created recovery issues, but new source-scoped flows should be watched in CI and Greptile review. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI Codex, GPT-5 coding agent, tool use enabled for shell, Git, GitHub, and local test execution. Context window not exposed by the runtime. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

Add recovery handoff system notices (#5289)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies. > - Agent runs can end productively while the source issue still lacks a durable final disposition. > - That leaves the control plane unsure whether to resume, escalate, or close the work. > - Issue comments also need a presentation contract so system-authored recovery notices can render as first-class thread messages without overloading normal comments. > - This pull request adds successful-run handoff recovery, comment presentation metadata, and system notice rendering. > - The benefit is stricter task liveness with clearer operator-facing recovery state. ## What Changed - Added successful-run handoff decisions, wake payloads, escalation behavior, and recovery tests. - Added issue comment presentation metadata with migration `0078_white_darwin.sql` and shared/server/company portability support. - Rendered recovery/system notices in issue chat with dedicated UI components, fixtures, tests, and storybook/lab coverage. - Included the current recovery model-profile hint patch so automatic recovery follow-ups use the cheap profile. ## Verification - `pnpm install --frozen-lockfile` - `pnpm exec vitest run server/src/services/recovery/successful-run-handoff.test.ts ui/src/components/SystemNotice.test.tsx ui/src/lib/system-notice-comment.test.ts ui/src/components/IssueChatThreadSystemNotice.test.tsx` ## Risks - Migration-bearing PR: merge this before any other branch that might later add a migration. - The branch touches both recovery services and issue-thread rendering, so review should pay attention to recovery wake idempotency and comment metadata compatibility. ## Model Used - OpenAI GPT-5 Codex via Paperclip `codex_local` adapter, with shell/git/GitHub CLI tool use.

[codex] Add issue monitor liveness controls (#4988)
## Thinking Path > - Paperclip is a control plane for autonomous AI companies where work must stay observable, governable, and recoverable. > - The task/heartbeat subsystem owns agent execution continuity, issue state transitions, and visible recovery behavior. > - Waiting on an external service is not the same as being blocked when the assignee still owns a future check. > - The gap was that agents had no first-class one-shot monitor state for external-service waits, so recovery could look stalled or require ad hoc comments. > - This pull request adds bounded issue monitors that can wake the owner, clear exhausted waits, and produce explicit recovery behavior. > - It also surfaces monitor status in the board UI and documents when to use monitors versus `blocked`. > - The benefit is clearer liveness semantics for asynchronous waits without weakening single-assignee task ownership. ## What Changed - Added issue monitor fields, shared types, validators, constants, and an idempotent `0075` migration for scheduled monitor state. - Added server-side monitor scheduling, dispatch, recovery bounds, activity logging, and external-ref redaction. - Added board/agent route coverage for monitor permissions and child monitor scheduling. - Added issue detail/property UI for monitor state, a monitor activity card, and Storybook stories for review surfaces. - Documented monitor semantics and recovery policy behavior in `doc/execution-semantics.md`. - Addressed Greptile review feedback by preserving monitor state in skipped-stage builders and making board monitor saves send `scheduledBy: "board"`. ## Verification - `pnpm install --frozen-lockfile` - `pnpm run preflight:workspace-links && pnpm exec vitest run server/src/__tests__/issue-execution-policy-routes.test.ts server/src/__tests__/issue-execution-policy.test.ts server/src/__tests__/issue-monitor-scheduler.test.ts server/src/__tests__/recovery-classifiers.test.ts ui/src/components/IssueMonitorActivityCard.test.tsx ui/src/components/IssueProperties.test.tsx ui/src/lib/activity-format.test.ts` - First run passed 5 files and failed to collect 2 server suites because the worktree was missing the optional `acpx/runtime` dependency. - After `pnpm install --frozen-lockfile`, reran the 2 failed suites successfully. - `pnpm exec vitest run server/src/__tests__/issue-monitor-scheduler.test.ts server/src/__tests__/recovery-classifiers.test.ts` - `pnpm --filter @paperclipai/shared typecheck && pnpm --filter @paperclipai/db typecheck && pnpm --filter @paperclipai/server typecheck && pnpm --filter @paperclipai/ui typecheck` - `pnpm exec vitest run server/src/__tests__/issue-execution-policy.test.ts ui/src/components/IssueProperties.test.tsx` - `pnpm --filter @paperclipai/server typecheck && pnpm --filter @paperclipai/ui typecheck` - `pnpm exec vitest run ui/src/components/IssueMonitorActivityCard.test.tsx ui/src/components/IssueProperties.test.tsx` - `pnpm --filter @paperclipai/ui typecheck` - Storybook screenshot captured from `http://127.0.0.1:6006/iframe.html?viewMode=story&id=product-issue-monitor-surfaces--monitor-surfaces` with Playwright. ## Screenshots ## Risks - Medium: this changes heartbeat recovery behavior for scheduled external-service waits, so regressions could affect wake timing or recovery issue creation. - Migration risk is reduced by using `IF NOT EXISTS` for the new issue monitor columns and index. - External monitor references are treated as secret-adjacent and are intentionally omitted from visible activity/wake payloads. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. feat: implement multi-user access and invite flows (#3784)
## Thinking Path > - Paperclip is the control plane for autonomous AI companies. > - V1 needs to stay local-first while also supporting shared, authenticated deployments. > - Human operators need real identities, company membership, invite flows, profile surfaces, and company-scoped access controls. > - Agents and operators also need the existing issue, inbox, workspace, approval, and plugin flows to keep working under those authenticated boundaries. > - This branch accumulated the multi-user implementation, follow-up QA fixes, workspace/runtime refinements, invite UX improvements, release-branch conflict resolution, and review hardening. > - This pull request consolidates that branch onto the current `master` branch as a single reviewable PR. > - The benefit is a complete multi-user implementation path with tests and docs carried forward without dropping existing branch work. ## What Changed - Added authenticated human-user access surfaces: auth/session routes, company user directory, profile settings, company access/member management, join requests, and invite management. - Added invite creation, invite landing, onboarding, logo/branding, invite grants, deduped join requests, and authenticated multi-user E2E coverage. - Tightened company-scoped and instance-admin authorization across board, plugin, adapter, access, issue, and workspace routes. - Added profile-image URL validation hardening, avatar preservation on name-only profile updates, and join-request uniqueness migration cleanup for pending human requests. - Added an atomic member role/status/grants update path so Company Access saves no longer leave partially updated permissions. - Improved issue chat, inbox, assignee identity rendering, sidebar/account/company navigation, workspace routing, and execution workspace reuse behavior for multi-user operation. - Added and updated server/UI tests covering auth, invites, membership, issue workspace inheritance, plugin authz, inbox/chat behavior, and multi-user flows. - Merged current `public-gh/master` into this branch, resolved all conflicts, and verified no `pnpm-lock.yaml` change is included in this PR diff. ## Verification - `pnpm exec vitest run server/src/__tests__/issues-service.test.ts ui/src/components/IssueChatThread.test.tsx ui/src/pages/Inbox.test.tsx` - `pnpm run preflight:workspace-links && pnpm exec vitest run server/src/__tests__/plugin-routes-authz.test.ts` - `pnpm exec vitest run server/src/__tests__/plugin-routes-authz.test.ts server/src/__tests__/workspace-runtime-service-authz.test.ts server/src/__tests__/access-validators.test.ts` - `pnpm exec vitest run server/src/__tests__/authz-company-access.test.ts server/src/__tests__/routines-routes.test.ts server/src/__tests__/sidebar-preferences-routes.test.ts server/src/__tests__/approval-routes-idempotency.test.ts server/src/__tests__/openclaw-invite-prompt-route.test.ts server/src/__tests__/agent-cross-tenant-authz-routes.test.ts server/src/__tests__/routines-e2e.test.ts` - `pnpm exec vitest run server/src/__tests__/auth-routes.test.ts ui/src/pages/CompanyAccess.test.tsx` - `pnpm --filter @paperclipai/shared typecheck && pnpm --filter @paperclipai/db typecheck && pnpm --filter @paperclipai/server typecheck` - `pnpm --filter @paperclipai/shared typecheck && pnpm --filter @paperclipai/server typecheck` - `pnpm --filter @paperclipai/ui typecheck` - `pnpm db:generate` - `npx playwright test --config tests/e2e/playwright.config.ts --list` - Confirmed branch has no uncommitted changes and is `0` commits behind `public-gh/master` before PR creation. - Confirmed no `pnpm-lock.yaml` change is staged or present in the PR diff. ## Risks - High review surface area: this PR contains the accumulated multi-user branch plus follow-up fixes, so reviewers should focus especially on company-boundary enforcement and authenticated-vs-local deployment behavior. - UI behavior changed across invites, inbox, issue chat, access settings, and sidebar navigation; no browser screenshots are included in this branch-consolidation PR. - Plugin install, upgrade, and lifecycle/config mutations now require instance-admin access, which is intentional but may change expectations for non-admin board users. - A join-request dedupe migration rejects duplicate pending human requests before creating unique indexes; deployments with unusual historical duplicates should review the migration behavior. - Company member role/status/grant saves now use a new combined endpoint; older separate endpoints remain for compatibility. - Full production build was not run locally in this heartbeat; CI should cover the full matrix. ## Model Used - OpenAI Codex coding agent, GPT-5-based model, CLI/tool-use environment. Exact deployed model identifier and context window were not exposed by the runtime. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge Note on screenshots: this is a branch-consolidation PR for an already-developed multi-user branch, and no browser screenshots were captured during this heartbeat. --------- Co-authored-by: dotta <dotta@example.com> Co-authored-by: Paperclip <noreply@paperclip.ing> Co-authored-by: Claude Opus 4.6 <noreply@anthropic.com> |

[codex] Improve issue detail and issue-list UX (#3678)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies > - A core part of that is the operator experience around reading issue state, agent chat, and sub-task structure > - The current branch had a long run of issue-detail and issue-list UX fixes that all improve how humans follow and steer active work > - Those changes mostly live in the UI/chat surface and should be reviewed together instead of mixed with workspace/runtime work > - This pull request packages the issue-detail, chat, markdown, and sub-issue list improvements into one standalone change > - The benefit is a cleaner, less jumpy, more reliable issue workflow on desktop and mobile without coupling it to unrelated server/runtime refactors ## What Changed - Stabilized issue chat runtime wiring, optimistic comment handling, queued-comment cancellation, and composer anchoring during live updates - Fixed several issue-detail rendering and navigation regressions including placeholder bleed, local polling scope, mobile inbox-to-issue transitions, and visible refresh resets - Improved markdown and rich-content handling with advisory image normalization, editor fallback behavior, touch mention recovery, and `issue:` quicklook links - Refined sub-issue behavior with parent-derived defaults, current-user inheritance fixes, empty-state cleanup, and a reusable issue-list presentation for sub-issues - Added targeted UI tests for the new issue-detail, chat scroll/message, placeholder-data, markdown, and issue-list behaviors ## Verification - `pnpm vitest run ui/src/components/IssueChatThread.test.tsx ui/src/components/MarkdownEditor.test.tsx ui/src/components/IssuesList.test.tsx ui/src/context/LiveUpdatesProvider.test.tsx ui/src/lib/issue-chat-messages.test.ts ui/src/lib/issue-chat-scroll.test.ts ui/src/lib/issue-detail-subissues.test.ts ui/src/lib/query-placeholder-data.test.tsx ui/src/hooks/usePaperclipIssueRuntime.test.tsx` ## Risks - Medium: this branch touches the highest-traffic issue-detail UI paths, so regressions would show up as chat/thread or sub-issue UX glitches - The changes are UI-heavy and would benefit from reviewer screenshots or a quick manual browser pass before merge ## Model Used - OpenAI Codex coding agent (GPT-5-class runtime in Codex CLI; exact deployed model ID is not exposed in this environment), reasoning enabled, tool use and local code execution enabled ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [ ] If this change affects the UI, I have included before/after screenshots - [ ] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |
