Add governed secret alias confirmation cards (#11486)

The agent cannot safely discover the internal secret identifier. The existing proposal is also easy for the operator to miss because it only appears in Settings. A generic confirmation can record acceptance without executing the binding. **Proposed solution** Let an agent create a binding proposal from one of its existing config paths. Mint a server-owned, human-only confirmation card on the checked-out issue. Recheck the operator's target-agent permission under the proposal row lock. Execute the existing proposal transaction after card acceptance. Store an `executed` or `failed` result on the card. Render the complete lifecycle in the issue thread and attention resolver. **Alternatives considered** A new alias subsystem would duplicate proposal quotas, expiry, authorization, and binding synchronization. A text-only issue comment would not provide a governed action or an execution result. An agent-supplied card payload would permit metadata smuggling. This change uses the existing proposal transaction and a server-owned payload instead. **Roadmap alignment** This change extends the completed "Secrets Manager with per-agent access" roadmap item. This left valid agent confirmations pending. Task watchdogs could see the same problem without board identity. **Expected behavior** Every interaction kind must use one resolver policy contract. The contract must support `anyone`, `not_creator`, and `human_only`. It must also apply all normal governance controls. **Steps to reproduce** 1. **What happened?**
On deployments where PostgreSQL is network-attached (managed providers,
pooled endpoints), the attention feed endpoint is slow:
`attentionService.list()` awaits ~15–20 queries strictly in sequence, so
a 70ms round trip turns into more than one second of pure network wait
per call. Separately, connecting through a transaction-mode pooler
(pgbouncer, Supavisor port 6543, Neon `-pooler` hosts) requires
disabling prepared statements, and the only documented way was to
hand-edit `packages/db/src/client.ts` — which `doc/DATABASE.md` itself
tells operators not to do.
**Expected behavior**
The DB client is configurable from the environment (prepared statements,
pool size, timeouts) with driver defaults when unset, and hot read paths
do not multiply network latency by issuing independent queries
sequentially.
**Steps to reproduce**
1. Run the server with `DATABASE_URL` pointing at a Postgres instance
with ~70ms round-trip latency.
2. Open the attention feed (`GET /companies/:companyId/attention`) and
measure response time — it exceeds one second even with little data.
3. Try to connect through a transaction-mode pooler: there is no
supported configuration to disable prepared statements.
## What Changed
- `packages/db/src/client.ts`: `createDb` accepts a
`DatabaseClientOptions` argument and reads optional env config —
`DATABASE_PREPARED_STATEMENTS`, `DATABASE_POOL_MAX`,
`DATABASE_IDLE_TIMEOUT_SECONDS`, `DATABASE_CONNECT_TIMEOUT_SECONDS`.
When nothing is set, no option is passed to the driver and behavior is
identical to the previous bare `postgres(url)`.
- `packages/db/src/client-options.test.ts` (new): env parsing and
driver-option mapping tests, including malformed-value rejection.
- `server/src/services/attention.ts`: the independent related-data
lookups in each feed section now run under `Promise.all` (issue
summary/image/plan-document maps, decision bundle titles, blocked-issue
maps, the newer-runs scan). Section order, item assembly, and query
shapes are unchanged.
- `doc/DATABASE.md` and `docs/deploy/database.md`: the edit-source
pooling instruction is replaced with the env toggle, plus a short
client-tuning reference.
## Verification
- `pnpm --filter @paperclipai/db exec vitest run
src/client-options.test.ts` — 6 tests pass.

- Low risk for self-hosted installs: with no env vars set,
`postgres(url, {})` receives an empty options object, which postgres.js
treats the same as no options — driver defaults throughout.
- The `Promise.all` batches only group queries that had no data
dependency on each other; on the transaction call path the driver still
executes them one at a time on the reserved connection, so transactional
semantics are unchanged.
- Malformed env values now fail fast at startup with a clear message
instead of being silently ignored; this is intentional and only affects
operators who set the new variables.

The token travels in an env variable; it never appears in argv, URLs, or on disk. Only `https://github.com` remotes are authenticated; everything else keeps ambient behavior. The provider is a single factory seam so a future brokered credential source can replace it without touching call sites. **Alternatives considered** - A GitHub OAuth "connect your account" flow: heavier product surface, needs app registration and callback custody; out of scope for a server credential and better served by a dedicated connector later. ## Linked Issues or Issue Description
No external GitHub issue is linked. Describing the problem inline
following the bug-report template:
**What happened:** `claude_local` agents flipped between `idle` and
`error` on every heartbeat because the persisted session jsonl carried a
synthetic UUID `previous_message_id` (from `<synthetic>` assistant
placeholders injected after rate-limit/max-turn/upstream errors).
Anthropic's API rejected every `--resume` with a 400:
``diagnostics.previous_message_id: must be the `id` from a prior
/v1/messages response (starts with `msg_`)``.
**Expected behavior:** When the persisted session is poisoned and
unrecoverable, the adapter should rotate to a fresh session — the same
fallback path already used for unknown-session errors — and stop
re-persisting the poisoned `session_id`.
**Actual behavior:** The session-rotation fallback only matched the
"unknown session" pattern, so the poisoned session was `--resume`-d
forever. The result event of the failing run still carried `session_id`,
which was being persisted into `agentTaskSessions`, so every subsequent
continuation reloaded the same poisoned id and hit the same 400.
**Reproduction:** Inject any flow that causes Claude Code to emit a
`<synthetic>` placeholder (rate-limit, max-turn exhaustion, transient
upstream failure). The next `--resume` will fail with the 400 and the
agent will not self-recover.
**Scope of fix:** Add a `previous_message_id` 400 detector; route it
through the existing unknown-session fallback; drop the poisoned
`sessionId` and emit `clearSession: true` so the heartbeat service wipes
the persisted row; best-effort delete the local poisoned `.jsonl`.
## What Changed
Two commits:
1. **`adapter-claude-local: auto-rotate session on previous_message_id
400 (synthetic-msg poisoning)`** — detector + execute-time rotation
2. **`adapter-claude-local: guard against persisting poisoned
sessionId`** — validate-before-persist + `clearSession`
Combined diff:
- `parse.ts`: new `isClaudePoisonedPreviousMessageIdError(parsed)`
matching ``/diagnostics\.previous_message_id.*starts with `msg_`/i``
against `parsed.result` and `extractClaudeErrorMessages(parsed)`
- `parse.ts`: `isClaudeTransientUpstreamError()` excludes the new error
from transient classification so it isn't masked as retryable upstream
noise
- `execute.ts`: expand the resume-fallback branch so it triggers on both
`isClaudeUnknownSessionError` and the new
`isClaudePoisonedPreviousMessageIdError`, with a distinct log line
(`"returned a poisoned message-id"` vs `"is unavailable"`)
- `execute.ts`: for local (non-remote) execution targets, best-effort
delete the poisoned `~/.claude/projects/.../{sessionId}.jsonl` before
retrying so the file can't be accidentally resumed by an out-of-band
caller. The `fs.unlink` and follow-up log call are in separate try/catch
blocks so a closed log stream cannot mask a successful unlink (and vice
versa)
- `execute.ts` / `toAdapterResult`: when a result carries the poisoned
400, **drop** `sessionId`/`sessionParams`/`sessionDisplayId` (return
`null`) and emit `clearSession: true` so the heartbeat service's
`resolveNextSessionState` wipes the persisted row. The result also
surfaces `errorCode: "claude_poisoned_previous_message_id"` for
observability
- `docs/adapters/claude-local.md`: runbook entry — symptom,
auto-recovery flow, on-call checklist
- Tests:
- 4 new `parse.test.ts` cases covering positive detection in `result`
and `errors[]`, negative cases, and non-transient classification

poisoned error → sessionId dropped + `clearSession: true`; (b) recovery
retry also reports the poisoned error → same guards apply; (c)
session-rotation success on retry

Both suites green locally. This patch is also currently running as a
hot-patch over the published `2026.513.0` adapter on the reporting
deployment — sessions that previously looped indefinitely now
self-recover on the first heartbeat after the 400 surfaces.
## Risks
- Low risk. The detector is conservative (regex over `result` +
`errors[]` only) and the rotation reuses the existing unknown-session
fallback path
- The local-only `fs.unlink` of the poisoned `.jsonl` is wrapped in
`try/catch` and ignored on failure — strictly an optimization; the
server-side session clear is the authoritative reset
- Remote execution targets (`executionTargetIsRemote`) skip the disk
cleanup because the file lives on a remote host that we can't safely
reach from the adapter
- The `clearSession: true` + nulled session fields path is a no-op on
healthy runs; it only fires when the new detector matches, so existing
successful continuations are unaffected
- No DB schema changes, no public API changes, no new dependencies

build(docker): bundle Gemini CLI in image for gemini_local adapter (#7693)

Add pause and resume actions to sidebar agents (#4616)
