0f6f0a3fee fix(server): schedule issue monitors without heartbeat polling (#17863)
<!-- entire-trail-link-start -->
https://entire.com/gh/paperclipai/-/trails/56/changes/paperclip/56
<!-- entire-trail-link-end -->

## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Issue monitors save a future check on an existing issue.
> - The heartbeat timer still scans monitors on every tick, even when
none are due.
> - The saved deadline and claim already provide durable scheduling
state.
> - This pull request gives monitors their own deadline worker and
connects state changes to it.
> - Idle monitor work stops querying. Existing admission and recovery
rules remain in force.

## Linked Issues or Issue Description

Refs #17840, #16325. Related: #17171 addresses rearming a monitor after
its admitted run fails. This PR changes when monitor inspection runs and
follows the merged routine prerequisite.

**What existing behavior does this improve?**

Issue-monitor scheduling and idle database use.

**Current behavior**

Heartbeat ticks call the combined agent and monitor sweep. The existing
next-deadline query only retains future monitors. Polling covers overdue
leftovers, stale claims, and changes that make a saved monitor eligible.

**Proposed behavior**

Use the shared deadline worker. Retain overdue work with bounded
retries. Wake after committed changes to monitor state, issue
eligibility, company state, or native ownership. Keep legacy agent ticks
separate for the next slice.

## What Changed

- Add monitor-specific sweep and deadline methods to the existing
scheduling owner. Keep the manual combined method.
- Include overdue work and the strict five-minute claim fence. Exclude
active native owners until their state changes.
- Register monitors with the shared deadline worker. Heartbeat ticks
wake only the legacy agent timer worker.
- Connect direct issue-state writers and existing execution-control
notifications to monitor work. Checkout uses a short transaction around
its existing CAS. Provider work stays outside it.
- Add real database lifecycle tests and retain routine, startup,
admission, execution-control, and native-projection coverage.
- Record the obligation, producer inventory, recovery behavior, and
limits in `doc/plans/2026-10-27-issue-monitor-sleep-audit.md`.

## Verification

- **1,233 passing cases in 35 complete files**, no skips, on accepted
source `c24aadfc91e9394d73c799f9e05fc67202160557`. Coverage includes
real monitor scheduling/admission, native settlement, committed
producers, retained routines and all shared deadline consumer
integration suites.
- Database build, production server typechecking/build, module
boundaries, target migration order and Runner profile identity pass.
Twenty-four fresh PostgreSQL 17 schemas plus per-file fixtures applied
all 412 journal entries through index 413; every selected source hash
and temporary fixture import was restored exactly.
- Normal parent reconciliation preserves master's admitted-wake result
and scoped issue deletion while keeping committed monitor notifications.
All other paths match the automatic merge; packages remain byte-exact to
the qualified parent. The final routine-parent publication changes only
its audit.
- Historical failures and corrections remain in [the monitor
audit](doc/plans/2026-10-27-issue-monitor-sleep-audit.md). Old
delegated-Slack rate and Runner drain CI failures are retained; the
current inherited clock fixture and package source are byte-identical to
merged #17835. Fresh final-head CI remains required.
- No full local repository-suite, live provider, deployed suspend/wake,
external-alarm, production-load or fleet-savings claim. Final-head
CI/security, exact-head Greptile 5/5 and actionable Ponytail findings
remain separate merge gates.

## Risks

- A missing producer notification can strand a monitor after polling is
removed. The audit covers ordinary writes, direct status/assignment
writers, recovery, and native ownership changes.
- The writer model is process-local. Independent SQL writers and other
replicas need a supported notification or reconciliation contract.
- Overdue work uses the configured heartbeat interval as a retry floor
for the monitor worker. New notifications during that backoff wait for
its next pass. Normal state-change bursts coalesce for one second.
- Notification fanout can inspect monitors during other active work. An
unchanged idle subsystem has no recurring monitor scan.
- The legacy agent timer still polls. Cloud sleep proofs and external
alarm coverage are unchanged.

## Model Used

OpenAI Codex (GPT-6-based coding agent), using repository inspection,
ordinary Git merges, composition proofs, PostgreSQL fixtures and
complete local integration suites. No subagents were used in this
reconciliation.

4d0847c4ca fix(server): settle external conversations from committed work (#17772)
<!-- entire-trail-link-start -->
https://entire.com/gh/paperclipai/-/trails/6/changes/paperclip/6
<!-- entire-trail-link-end -->

## Thinking Path

> - Paperclip manages AI agents and the tasks that they execute.
> - External conversations must become idle after an authorized response
is published and all related work is complete.
> - Native replay and stranded-task recovery still discover this
settlement work through recurring scans.
> - A new deadline worker must preserve response ordering, monitor
handoffs, and unfinished work.
> - This pull request gives settlement one shared readiness predicate
and activates its worker from committed changes.
> - Existing rows provide recovery truth. No new table or per-feature
timer is added.
> - The benefit is a settlement worker that becomes quiet and resumes
when relevant work changes.

## Linked Issues or Issue Description

Refs #17758. This is slice 56. Slice 55 has merged. Related: #17606
(chat milestones), #17663 (bounded run reads), #17665 (monitor proof),
and #13809 (answered Slack conversation lifecycle). The provider
applicability remains Slack and Speko-owned conversations. This does not
extend Telegram settlement from #16559.

**What happened?**

Conversation settlement depended on periodic native replay and generic
task recovery. An exhausted monitor also cleared its schedule before
recovery admission. During that gap, an answered conversation could look
finished. Failed or deferred admission lost the monitor schedule.

**Expected behavior**

Relevant committed changes activate settlement. The worker uses the
owning service's evidence and becomes quiet when no settlement is owed.
Monitor recovery stays pending until admission, and restart does not
admit an already accepted recovery twice.

**Steps to reproduce**

1. Create an active external conversation with a successful run and an
authorized published response.
2. Keep execution, approval, a goal, or a monitor pending. Let the
settlement worker become quiet.
3. Resolve that prerequisite through its owning service. Verify that the
conversation settles without another periodic sweep.
4. Delay or fail exhausted-monitor recovery admission. Verify that the
monitor remains pending and the conversation cannot settle during the
handoff.

**Paperclip version or commit**

Accepted production source: `50f4432ed88537b8d522cf1dce24c33588db0bac`.
Parent #17758 merged as `8ac5a128e66fb672e88106412f76cf278599bfc4`. The
publication adopts that actual squash through a normal merge with an
unchanged complete child tree.

## What Changed

- Preserve quiet after coarse company-only maintenance hints. The shared
inbound-notice and response-availability helpers keep settlement enabled
by default. Only the milestone dispatcher suppresses their duplicate
settlement fanout. Its scoped issue-review signal and the explicit
company-authority signal still wake real work. Four database cases prove
both quiet and real company-wide producer delivery.
- Preserve source-generation checks for chat handoff,
task-before-comment locking, issue-scoped decision invalidation, and
current conversation event columns from master.
- Track initial external conversation settlement through shutdown
without blocking HTTP readiness on endpoint or task locks.

- Reuse the real queue's coalesced recovery receipt after restart,
without admitting the same monitor recovery again.

- Share the existing conversation readiness predicate between bounded
discovery and locked settlement. Preserve current provider policy,
run/publication proof, company boundaries, and endpoint-to-issue lock
order.
- Register a scoped deadline worker through the delivery coordinator.
Preserve startup, restore/worktree suppression, drain/resume, failure
recovery, and joined shutdown.
- Extend existing review, milestone, wake-notice, and finalization
signals. Add notifications at independent issue, comment, goal, session,
and wake-receipt unblocks.
- Keep monitor owner recovery pending until guarded admission. Reuse an
accepted receipt for the same monitor schedule after restart, even when
its limit reason changes; retain compatibility with legacy receipt keys.
Commit board escalation and its monitor clear together.
- Delegate periodic native settlement while preserving resource
callbacks. Keep direct/manual defaults and preserve generic recovery
ordering with a read-only readiness guard.
- Add the producer and lifecycle audit in
`doc/plans/2026-10-26-external-conversation-settlement-sleep.md` and
update the stack ledger.

## Verification

Accepted production source: `50f4432ed88537b8d522cf1dce24c33588db0bac`.
Publication head: `b256bb52647affd9f5279ea8c40f48bbe40f031f`.
The complete accepted child tree stays unchanged when adopting merged
parent #17758. This publication adds only the audit document.

- **3,129 distinct cases in 37 complete server files pass**, plus **18
selected chat-response cases** (3,147 selected cases). The other 1,796
chat cases were not run locally. Full settlement, queue, scheduling,
run-state, goals, interactions, task Retry, decision targets, status
reassessment, workspace, process recovery, and review consumers are
covered.
- **Preserved failure:** the first 35-file command passed 2,445 cases
and timed out one restored-work handoff at its existing 15-second limit.
That aggregate command failed. The unchanged full 89-case actions file
then passed on a fresh fixture; the affected case took 22 ms. Root cause
was not established. No timeout or assertion changed. The 34 other files
passed originally. Distinct counts exclude duplicate actions passes.
Fresh CI remains a separate gate.
- **Indirect wake regression:** the original unconditional baseline
failed two quiet checks. Guards alone left the chat case failing with
three candidate queries. The final controlled indirect baseline fails
one of four cases; the repair passes all four with the same test hash.
Positive controls preserve real company-only producers.
- **Source and fixture controls:** all 38 selected test files restore to
their exact committed hashes after fixture imports. Fresh PostgreSQL 17
fixtures use 413 migrations. All five manual merge resolutions preserve
current authority, locking, and event-column contracts.
- **Static checks pass:** database build; server typecheck and build;
module boundaries; migration order; Copilot profile identity. Runner
packages equal the accepted parent byte for byte. No new Runner/package
qualification is claimed.
- [Current integration and wake
audit](https://github.com/paperclipai/paperclip/blob/b256bb52647affd9f5279ea8c40f48bbe40f031f/doc/plans/2026-10-26-external-conversation-settlement-sleep.md#current-master-integration-and-indirect-wake-audit-2026-10-29).
Latest-head CI and exact-head Greptile 5/5 must pass before merge.

<details><summary>Earlier revision verification and preserved failure
history</summary>

Receiver failure fairness: a controlled 1,100 ms batch reproduced a
30-second delay for healthy due receivers. The worker now supplies its
existing per-item deadline through `nextRunAtOnError`. All 20 receiver
lifecycle tests pass on the owner and combined tip
`31c12f1d68ff4a51e06fc2be2abf2c2aee9f9d3a`; server production
typechecking passes on the owner. The failed receiver retains its own
backoff, and settled receivers return to zero recurring SQL. The
five-second assertion budget is unchanged. See [the owning
audit](https://github.com/paperclipai/paperclip/blob/83204a67880d980cfbea3771cee19a3cda0022b7/doc/plans/2026-10-25-chat-receiver-discovery-sleep-audit.md#healthy-receiver-progress-after-a-slow-failed-batch).
Current-head CI and review remain required.

Approved delegated Test-tab calls now evaluate caller and target
policies in the existing claim transaction. Real HTTP regressions first
showed a missing caller charge and shared-limit self-rejection. All 78
connection-management cases and 92 gateway cases now pass on the owning
branch, along with server production typechecking. All 78
connection-management cases also pass at combined tip
`d25f830db89c1623bff3547bfc68015c1c66e0cf`. The three new cases cover
shared charges, separate charges, rollback on denial, duplicate
approval, and rejection of a later authorized call. Intermediate
counter/identity fixture failures and a reused-database Slack collision
are recorded in [the owning
audit](https://github.com/paperclipai/paperclip/blob/ef9ae5767430d0093efc736ae0b303ba5afaae65/doc/idle-approved-tool-recovery.md#delegated-test-tab-admission-after-policy-changes).
The earlier combined checks below remain evidence for their stated
revisions. Current-head CI and review remain required.

Inherited voice-cleanup verification: the controlled slow-provider
regression and all 13 lifecycle tests passed on its owner and combined
tip `ab22d8ed0c09028315607414b83b375e5cc0a60a`. That repair changed only
the fixture clock advancement and its audit note.

Current head: `83204a67880d980cfbea3771cee19a3cda0022b7`. This remains a
single-duty PR stacked on #17758.

- Integrated pinned master `8548173fc9efdc0fa07644fb92dcc0b6851a95e5`
with normal merges. Every branch passed generated-schema checks and its
target-branch migration-order gate. Existing SQL is unchanged; the 25
stack migrations follow master’s new migration.
- Duty-specific producer coverage, lifecycle tests, and earlier failure
records:
[2026-10-24-recurring-server-sleep-stack.md](https://github.com/paperclipai/paperclip/blob/d6d8e4ffa6b330ebf4647aec997ee2dde6abc321/doc/plans/2026-10-24-recurring-server-sleep-stack.md),
[2026-10-26-external-conversation-settlement-sleep.md](https://github.com/paperclipai/paperclip/blob/d6d8e4ffa6b330ebf4647aec997ee2dde6abc321/doc/plans/2026-10-26-external-conversation-settlement-sleep.md).
- Combined tip `51e6cbe67979b55b340ea9f0f434c5b5c682ccd6`: **355
startup/cancellation/recovery/dispatch/preservation tests**, **75
connection-management tests**, and production server typechecking pass.
These combined checks do not mean every intermediate PR independently
ran the full repository suite.
- Real master-to-stack database upgrade passes: **379 unchanged prior
migration receipts plus 25 new migrations**, with the gateway
responsibility fixture preserved.
- Owning-branch checks: 84 runner installation/settlement cases plus
runner typechecking and generated TypeScript/Rust profile agreement; 253
GitHub review workflow cases. Stop-attribution and held-worker readiness
regressions failed before their fixes.
- Failure accounting: the initial combined run passed 349 cases and
failed six idle assertions in a reused preservation database containing
old cancelled runs. Repeating the combined command with a fresh
preservation database passed. Earlier invalid project selection, merge
import/type errors, and the interrupted full local suite are not counted
as passing checks.
- **Latest-head CI and review remain required.** Prior CI findings
include GitHub idle observation, route teardown, attention cleanup,
touch-picker E2E, workspace health, and voice-cleanup timing; local
passes do not erase those failures. No full local repository-suite,
deployed sleep, fleet cost, or live Copilot qualification claim is made.

</details>

## Risks

- Signals cover supported in-process writers. They do not supply change
capture for independent SQL or other replicas.
- Real company-wide changes can inspect active candidates. Coarse
maintenance hints do not activate settlement. Configured idle and
retained history must remain quiet.
- Monitor receipt binding and admission order must preserve company,
agent, issue, and exact schedule identity. Current parent handoff locks
and source-generation checks remain intact.
- The local restored-work timeout is retained above. A fresh full-file
pass does not establish its cause; latest-head CI must also pass.
- Generic recovery and Muse/resource duties remain separate slices. This
does not make the whole server idle. Existing Cloud sleep blockers
remain. No deployed sleep, database suspension, external alarm coverage,
fleet cost, or live provider qualification claim is made.

OpenAI Codex, GPT-6 family, with reasoning, code execution, PostgreSQL
tests, and GitHub tools. The exact runtime model identifier and
context-window size are not exposed in this session.

DottaandPaperclip 21137db805 fix: make Check now work for retained monitors on blocked tasks (#18061)
## Thinking Path

> - Paperclip lets people manage agents and inspect their work.
> - Task monitors schedule checks of work that is still pending.
> - A task can retain a monitor after it becomes blocked.
> - The UI still offered Check now, but the server rejected the task
status.
> - The UI also claimed that an automatic check would run on the next
tick.
> - This change admits an explicit human check through the normal
execution gates and shows that automatic checks are paused.
> - People can request the check without changing task status, and
skipped checks no longer report success.

## Linked Issues or Issue Description

**What happened?**

A blocked task with a retained monitor showed Check now and an overdue
countdown. Clicking the button failed because manual checks required an
in-progress or in-review task. The endpoint also returned success when
admission skipped a check.

**Expected behavior**

A human can request one check through normal admission. Automatic checks
stay paused while the task is blocked. A check that cannot start reports
a conflict.

**Steps to reproduce**

1. Schedule a monitor on an agent-owned task.
2. Let the task become blocked while it retains the schedule.
3. Open the task and click Check now.

Related: #14909 addresses scheduled-retry promotion. This PR addresses
retained task monitors.

**Paperclip version or commit**

Base: 7f10cc0585.

## What Changed

- Allow a human to check a retained monitor on a blocked task. Keep
agent and automatic checks limited to active task statuses.
- Keep the existing admission path and status guard. Do not change task
status to start a check.
- Return 409 when admission skips the check. Preserve the schedule when
a dependency or recovery hold prevents dispatch. Keep a successful
result when a new schedule replaces the consumed schedule after
admission.
- Show Automatic checks paused in the banner, composer strip and
properties. Hide Check now on backlog and todo tasks. Provider-quota
recovery stays paused on blocked tasks because it uses a direct retry
path; its monitor is retained until the task resumes.
- Add database, process, route and UI regression coverage, a Storybook
state, API metadata, and behavior documentation.

## Verification

- `pnpm -r typecheck` passed.
- `pnpm build` passed.
- Server scheduling, process admission and route suites: 94 tests
passed.
- UI monitor, properties and time-formatting suites: 142 tests passed.
- Additional admission checks passed: an explicit check starts a process
on a blocked task; an unresolved dependency prevents dispatch and
preserves the monitor.
- Token gates, map checks, Storybook catalog checks and 30 map/catalog
tests passed.
- The full local `pnpm test:run` attempt reported 1 timeout among 1,621
chat integration tests: “keeps an ambiguous github text part blocked
across restart and retries only that part and its tail” exceeded 15
seconds. The exact case passed in isolation on the final head (1 passed,
1,620 skipped). The remaining full-run groups were stopped to isolate
that failure. This does not establish a full local pass or explain the
initial timeout. Final-head CI is green, including the full chat lanes
and the successful single server-shard retry.
- Browser inspection passed for the blocked-task banner, composer strip
and properties row. Clicking Check now set both visible buttons to
disabled Checking state. The preview uses isolated fixtures.
- CI server shard 7/12 failed one decision-expiry deadline assertion.
The entire unchanged decision-expiry suite passed locally on the final
head (20/20). The single retry of that shard passed after the first
workflow attempt completed. The initial failure remains recorded here.
Neither that test nor its production path changed before retry; the
cause is not established by a passing retry.
- Greptile reviewed final head ed936f7433
at 5/5. Both prior review threads are resolved.
- The first CI chat job stopped after its runner received a shutdown
signal. The final head completed a fresh CI run.
- The process fixture uses a local no-op adapter. It proves admission
and execution, not a live provider's response. The Storybook state
simulates dispatch.

## Risks

- An explicit human check can start work on a blocked task with no
unresolved execution gate. This is intentional. Dependencies, recovery,
native finalization, ownership, pause and budgets retain their existing
checks.
- Skipped manual checks now return 409 instead of 200. The UI already
displays API errors in context.
- No schema change or migration.

## Model Used

OpenAI Codex, based on GPT-6. The session does not expose a more
specific served model ID or context-window size. Used repository tools,
code execution, and browser inspection.

DottaandPaperclip c6ee96a3df fix: stop idle MCP event polling (#17036)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - MCP task subscriptions and Dot mailboxes notify external clients
when work changes.
> - Their delivery workers queried SQL every two seconds, including when
no work existed.
> - Those queries prevent idle databases from becoming quiet across many
hosted instances.
> - Existing activity, mailbox, subscription, and delivery records
already preserve the work.
> - This pull request uses committed-write signals and saved retry
deadlines to drive delivery.
> - Idle subscriptions become quiet while startup, retry, drain, and
shutdown recovery remain supported.

## Linked Issues or Issue Description

Related sleep migrations: #15625, #17000, #17008. Searched open PRs and
issues for MCP event sleep/polling work; no duplicate found.

**What happened?**

Each public MCP and Dot event service started a two-second interval. It
checked settings, scanned subscriptions, created delivery receipts,
cleaned expired records, and attempted delivery even without changes.

**Expected behavior**

An empty or subscribed-but-idle service produces no recurring SQL or
network traffic. Committed events trigger prompt delivery. Only
unfinished work retains a retry deadline.

**Steps to reproduce**

Configure a public MCP origin and start the server. Observe recurring
settings queries with MCP disabled, or subscription/queue queries with
it enabled. No incoming task changes are required.

**Paperclip version or commit**

Initially inspected master at
`8b4ab65b1c3d863d282752e83307943720a5c85c`; rebased onto `c2daf906d9`
after master advanced. The only conflict was overlapping imports in the
heartbeat queue; both were preserved.

## What Changed

- Register separate MCP and Dot workers with the existing delivery
coordinator. Remove both unconditional intervals.
- Notify from committed activity, comment, mailbox, subscription, and
experimental-settings writes. Centralize comment insertion in one small
helper so normal callers do not manage recovery state.
- Preserve outer transaction ownership, uncertainty reconciliation, and
requests deferred during drain or standby.
- Restore delivery retry/abandoned-claim deadlines on startup. Scan
subscriptions in pages of 20; drain full source and delivery batches
without another external event. Scope subscription refreshes to their
subscription or shared Dot binding, issue activity to its task, comment
discovery to its company, and mailbox/assignment changes to their
binding. Retry deadlines do not rescan unchanged sources.
- Run expired-record cleanup opportunistically. Preserve on-use expiry
checks and the existing one-time automatic Dot challenge behavior.
- Add real PostgreSQL lifecycle coverage and guarded external test
database options for existing MCP/Dot integrations. Document producers
and the supported writer model.

## Verification

- Final head `e54fb34485921016ef988f7df9cca173a9ebdfc8`: all CI checks
passed (55 successful checks/statuses, one neutral review, two
intentionally skipped Storybook checks). Mergeable with no unresolved
review threads. Greptile: 5/5 on this exact commit; security scan
passed.
- All 284 affected tests passed in one local run after rebasing: 16
real-PostgreSQL lifecycle tests, 24 coordinator/scheduler/standby tests,
84 public MCP tests, 54 Dot runner tests, and 106 interaction-service
tests.
- Server TypeScript passed after refreshing generated Runner/plugin SDK
dependencies from master. Full workspace typecheck and build passed
before the rebase; final CI repeated typecheck and build successfully on
the rebased head.
- Coverage includes SQL/network quiet with configuration and history,
committed writes, outer commit/rollback, drain/standby, retry
restoration, reenable/unsubscribe, concurrent writes, shutdown,
source/delivery/subscription batch boundaries, Dot challenges and
reconnects, native broker fences, comment discovery, and
company-isolated scans.
- CI exposed an existing lock-order test whose insert proxy missed
nested transactions and waited indefinitely. The fixture now observes
nested inserts, preserves the lock assertion, and fails explicitly if
interception is missed. The complete 106-test interaction suite and the
formerly hanging CI shard pass. The warm-standby/settings test doubles
were also updated for the new coordinator/transaction boundaries.
- Earlier fixture failures (required timestamps, cleanup, external
database selection, async waits) were corrected. One existing OAuth
first-poll timing assertion and one E2E company-cleanup race passed on
single reruns; the final full CI run passed without a rerun. No OAuth or
company-deletion behavior was changed.
- The local full `pnpm test:run` attempt could not initialize embedded
PostgreSQL after five attempts. It was stopped; affected integrations
were verified with separate disposable PostgreSQL 17 databases, and
final CI supplied the full-suite result. Embedded-only local skips are
not counted as passes.

## Risks

- Notifications use one live server/database scope. Independent SQL
writers and separate live replicas do not trigger these process-local
workers. Startup recovers durable state; continuous multi-writer
discovery needs a notification/ingress contract.
- Idle physical cleanup waits for startup or another enabled sweep.
Subscription and authorization expiry remain enforced on use.
- Existing delivery claims, retry bounds, signatures, and receipt
deduplication remain in place. No new tables or migrations.
- Host sleep-safety checks remain conservative. MCP/Dot retry deadlines
are not yet covered by Paperclip Cloud's scheduled-wake contract. This
proves worker quiet, not deployed host suspension or fleet savings.

use. The runtime does not expose a more specific model version or

DottaandPaperclip 98d17da1e2 Fix native monitor dispatch and finalization recovery (#16975)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Native runs persist their results and workspace before they release
execution.
> - A task monitor must wait for that settlement before it starts the
next run.
> - The scheduler checked provider status but did not check unfinished
finalization.
> - A timer could dispatch too early, then its old finalizer could no
longer find the consumed schedule.
> - This pull request delays dispatch and reconciles prior delivery from
server records.
> - The task can continue without repeating the provider turn or
removing a newer run's ownership.

## Linked Issues or Issue Description

**What happened?**

A native run saved a yielded monitor result while workspace finalization
needed recovery. Its timer dispatched after the provider stopped. The
follow-up failed because the earlier run still held workspace recovery.
The old finalizer then retried `monitor_wait_authority_lost`
indefinitely.

**Expected behavior**

Timer dispatch waits for native finalization. If an older timer already
dispatched, the finalizer uses its saved scheduling receipt and admitted
successor to settle the old result. It preserves current task state and
ownership.

**Steps to reproduce**

1. Schedule a task monitor from a native run and save a yielded result.
2. Leave native finalization retryable after the provider reaches a
terminal state.
3. Let the monitor become due. Before this change, the scheduler
consumes it.
4. Restore workspace persistence and retry the old finalizer. Before
this change, the missing schedule leaves it retryable.

**Paperclip version or commit**

Reproduced against master `34be1b831bcaba070264ae8d214ceffe6a8f9eab`.

**Deployment mode**

Self-hosted server built from source, with PostgreSQL and the native
runner. Related recent recovery work: #16962 and #16963. No duplicate
repair found for this failure.

## What Changed

- Scheduled and manual monitor dispatch wait for unfinished native
finalization. If the monitor expires during workspace recovery, the
accepted result can settle into the persisted schedule, and the
scheduler then applies its existing expiry policy. New monitor-wait
results still require an unexpired, unexhausted schedule.
- A narrow reader joins the run's server-issued monitor receipt to its
durable wake and admitted successor.
- The finalizer and locked status commit verify that evidence and
preserve current task ownership. They do not create another wake.
- Monitor delivery participates in assessment identity, so recovery can
replace a prior error decision without violating the decision ledger's
uniqueness constraints.
- PostgreSQL regressions cover saved-result replay, active successors,
wrong task or agent, missing receipts, unadmitted wakes, ownership
races, and failed workspace persistence.
- The execution contract and verification maps describe the recovery
boundary.

## Verification

- Red: the saved monitor regression remained `retryable_failure` instead
of `committed` on unpatched code.
- Red: the scheduler admitted a wake while terminal-run finalization
remained retryable.
- Red/green: two PostgreSQL cases reproduce timeout during workspace
recovery for scheduled and manual checks. Before the review repair, both
remain `retryable_failure` with `monitor_wait_authority_lost`.
Afterward, both commit, apply board escalation once, preserve the saved
result, and enqueue zero provider wakes. New completion still rejects
the expired schedule.
- Typecheck: `pnpm -r typecheck` passed.
- Build: `pnpm build` passed.
- Maps: `node scripts/check-maps.mjs` passed.
- Review: Greptile 5/5 and Ponytail Paperclip Review 5/5 on the current
head, with no unresolved findings. Both reviews are static; execution
evidence is recorded separately above and below.
- Linux staging: the original repair passed 170 tests across monitor
integration, scheduling, status arbitration, and workspace finalization.
The expanded suite passes all 172 tests, including both new timeout
regressions.
- Live acceptance: deployed the original repair to the authorized Linux
ARM64 self-hosted instance. The stranded saved result finalized
automatically; the original task then completed through normal
admission, with its final answer persisted after browser reload. The
other original task was already complete and its external delivery was
verified without repeating it.
- The initial Mac regression run passed 68 tests, but three suites
failed during embedded PostgreSQL startup. A subsequent focused run
confirmed the host had exhausted its PostgreSQL semaphore allowance.
Linux verification uses isolated databases; the local failure logs
remain preserved.
- The local full test suite is not claimed as passed. It is blocked by
the same PostgreSQL startup limitation; full-suite CI passed on head
`8bc29d1b7783446f397b858e558083570a96bb66`
([run](https://github.com/paperclipai/paperclip/actions/runs/39447913466)).

## Risks

- A delivery match must not grant new execution authority. The lookup
requires the original run's server receipt and a matching company,
agent, task, wake, and admitted successor. The locked commit rejects
old-run checkout ownership and permits no status effects.
- Workspace persistence must succeed before settlement. Its existing
failure path remains effective.
- The scheduler waits while a native finalizer is unfinished. Workspace
recovery must settle before dispatch; elapsed monitor limits transfer to
the existing scheduler recovery policy instead of stranding the result.
Neither settlement path renews a deadline or changes budget, approval,
pause, or provider authority.
- No schema, migration, credential, or provider configuration change.

exact backend model ID and context-window size are not exposed in this

DottaandPaperclip b928c6698f fix(heartbeat): explain deferred monitor dispatch (#16891)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - One-shot task monitors request future work through the normal
heartbeat queue.
> - Queue admission can hold execution until recovery or ownership is
reconciled.
> - The scheduler retained those monitor schedules but recorded no
monitor diagnostic.
> - Agents could repeatedly reschedule the same blocked check without
seeing why it did not run.
> - This pull request records safe deferral and failure activity while
retaining all execution gates.
> - Agents and operators can inspect the actual blocker and use its
existing recovery path.

## Linked Issues or Issue Description

Related: #15556 handles transient monitor client errors. #14590 handles
parked monitor wakes. This change records the diagnosis when queue
admission defers or fails; it does not change their admission or monitor
consumption policies.

**What happened?**

A due monitor on a task with an unresolved execution-recovery hold was
claimed, passed to queue admission, and released without monitor
activity. The saved monitor continued to look scheduled. Unexpected
dispatch errors had the same audit gap. Repeated rearming could not
clear the recovery hold.

**Expected behavior**

Keep the monitor and the recovery hold. Record the reason that prevented
its wake. Do not execute uncertain work or consume an attempt. Limit
repeated identical diagnostics. If audit storage fails, release the
claim so a later tick can retry.

**Steps to reproduce**

1. Schedule a due monitor on an agent-owned task in progress.
2. Add an active recovery action for an interrupted legacy execution.
3. Run the monitor scheduler twice. No provider may start while the hold
remains.
4. Before this fix, neither attempt writes monitor deferral activity.
5. Resolve the hold through its existing recovery path. The saved
monitor can then request one wake.

**Paperclip version or commit**

Reproduced on master 5739a53f5f with real PostgreSQL and the assembled
heartbeat service.

**Deployment mode**

Built from source. This affects the server monitor scheduler.

## What Changed

- Pass the existing execution blocker cause to the monitor scheduler
through a server-owned diagnostic callback.
- Record issue.monitor_deferred for held admission and
issue.monitor_dispatch_failed for unexpected admission failures.
- Keep activity to known reason codes and allowlisted blocker causes. Do
not copy raw provider messages.
- Fence claim release and activity against replacement, reassignment,
completion and competing scheduler instances.
- Limit identical activity for one schedule and cause to one record per
five minutes.
- Keep claim release functional when diagnostic storage fails.
- Update execution documentation and the task verification maps.

## Verification

- Red: explains held monitor dispatch without replaying uncertain
execution failed because it expected one deferral activity row and
received none. The fixture used the real queue, persisted source run and
recovery action.
- Red: records bounded dispatch failure diagnostics while retaining the
schedule failed because no failure activity was recorded.
- Green: both regressions pass after the fix. The held-source test
proves no successor while held, no monitor attempt consumed, and one
wake after explicit hold resolution.
- Passed: all 43 tests in heartbeat/scheduling.test.ts and
issue-monitor-scheduler.test.ts, with at most two workers.
- Additional coverage uses real PostgreSQL for diagnostic rate limits,
replacement races and an audit-insert trigger failure. Audit failure
must not retain the monitor claim. Private error and unknown blocker
text do not reach activity.
- Passed: node scripts/check-maps.mjs and git diff --check.
- Greptile: 5/5 with no actionable issues on
1d0aec738c.
- Latest-head checks: 55 successful, two skipped, and one neutral. No
check remains pending or failed. The branch has no merge conflict.
Paperclip Review ended neutral because its agent did not return a
complete assessment; it is not counted as a validated review pass.
- The dependency added on deferral is one bounded recent activity query
and at most one audit insert. Successful admission adds no new database
read. Existing queue authority checks still decide admission.
- The process fixtures validate queue admission and persisted state.
They log workspace setup errors after admission; this report does not
claim a live provider turn succeeded or that an existing deployment was
repaired.
- Passed: pnpm --dir server exec tsc --noEmit. The wrapper typecheck was
stopped because it started a redundant cold native Runner build. Full
repository checks run in PR CI; no local full-suite or full-build pass
is claimed.

## Risks

- Audit work must not strand a monitor. The storage-failure regression
verifies that rollback is followed by a claim-fenced release.
- Recovery holds still require their current reconciliation path. This
change makes their effect visible and does not bypass ownership, budget,
pause, Stop or company checks.
- Activity can be stale after a later repair, so each entry includes the
checked time and the exact saved deadline. A replacement cannot inherit
an old dispatch diagnostic.

## Model Used

OpenAI Codex, GPT-6 family, with reasoning, tool use and code execution.
The exact runtime model suffix and context window are not exposed to
this session.

DottaandPaperclip 65965f0d3a refactor(heartbeat): extract scheduling and monitor recovery (#15717)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - The heartbeat service schedules and runs agent work.
> - Previous refactors moved queue admission, recovery, and run control
into separate modules.
> - Timer ticks, issue monitors, and session-goal recovery still occupy
more than 1,100 lines in the main service.
> - This pull request moves that scheduling code into one module with
explicit dependencies.
> - This makes scheduling easier to review and test before we split the
execution pipeline.

## Linked Issues or Issue Description

Refs #15685, the merged run-control extraction. A search found no
duplicate open scheduling extraction.

**What existing behavior does this improve?**

Maintenance and testing of heartbeat scheduling.

**Current behavior**

The main heartbeat service contains 10,473 lines. Scheduling shares a
file with the execution pipeline.

**Proposed behavior**

Move scheduling into `server/src/services/heartbeat/scheduling.ts`. Keep
the same public methods, admission rules, database claims, and execution
behavior. The main file falls to 9,330 lines.

**Reason and benefit**

Give timer ticks, issue-monitor recovery, and session-goal recovery one
clear module boundary.

**Breaking changes**

None intended. Existing heartbeat exports and method signatures remain
available.

## What Changed

- Add `createHeartbeatScheduling` with 11 explicit dependencies.
Construction does not query the database or start timers.
- Move timer ticks, issue-monitor dispatch and recovery, and
session-goal recovery into the new module.
- Keep queue admission, heartbeat policy parsing, and atomic timer
claims in `queue.ts`.
- Share the unchanged goal-control parser and issue-link formatter with
their existing callers.
- Add 20 focused tests. Document the module boundary in
`doc/DEVELOPING.md`.

## Verification

- 122 tests pass across 11 files: 20 new scheduler tests, 52 existing
scheduling integration tests, and 50 adjacent queue, goal,
native-selection, and task-drain tests.
- New database tests cover cross-instance monitor claims, replacement
schedules, deferred and failed admission, native ownership, archived
companies, timer claim loss, worktree cutoffs, agent eligibility, goal
recovery keys, pending-action deduplication, and invalid-action
persistence.
- AST comparison confirms 20 unchanged function bodies, unchanged
remaining service statements, unchanged `executeRun`, and all 140 public
exports preserved.
- Workspace typecheck (`pnpm -r typecheck`) and build (`pnpm build`)
pass. Greptile is 5/5 on commit
`89a1a3b3aa428949eba349af5c1618391f97a19c`. All CI checks pass on that
commit. The full test suite runs in CI; local verification was targeted.
- The first CI attempt hit a 15-second timeout in the first test of
unchanged `plugin-scoped-api-routes.test.ts`. All nine tests pass
locally in 6.6 seconds. That shard passed on its one retry without code
changes.
- Run the new tests with `pnpm exec vitest run
server/src/services/heartbeat/scheduling.test.ts --maxWorkers=1`.

## Risks

- Incorrect wiring could bypass suppression, duplicate a monitor wake,
or lose a replacement schedule. Database tests cover these boundaries
and existing integration tests still call the public heartbeat service.
- This PR moves the current behavior without changing scheduling policy,
schema, provider execution, or cancellation ownership.

## Model Used

OpenAI Codex, based on GPT-6, with reasoning, code editing, and command
execution. The session does not expose the exact model variant or
context window size.
