DottaandPaperclip cfa5e0704e feat(mcp) [split 3/8]: add tool access policy core (#9558)

## Linked Issues or Issue Description

- Related parity reference: #9534
- Problem: Authorization, OAuth binding, secret projection, content
guards, and policy evaluation need a security-reviewable server
boundary.
- Proposed solution: Adds tool-access services/routes/tests plus the
runtime service dependencies directly imported by the core, without
registering the routes in the application.
- Alternatives considered: keeping #9534 as one 403-file review, or
rewriting the feature to manufacture seams; both were rejected in favor
of path extraction plus compile-driven boundary moves.
- Roadmap alignment: this advances the existing governed MCP/tool-access
work already represented by #9534; it does not introduce a separate
roadmap initiative.
- Stack position: base branch is `pap10341-split/02-schema-shared`.
- Merge policy: merge bottom-up, in order, only after the complete
eight-PR stack has been reviewed and the top-of-stack parity gate
remains empty.
- Requested review: SecurityEngineer for authz, OAuth, secrets, and
content guards; Greptile on every PR.

## What Changed

- Adds tool-access services/routes/tests plus the runtime service
dependencies directly imported by the core, without registering the
routes in the application.
- Keeps this PR below 100 changed files and independently typecheckable.
- Preserves the final tree from #9534 when combined with the other seven
stack levels.

## Verification

- `pnpm typecheck`
- Focused server Vitest run — 4 files, 143 tests passed

## Risks

- Authorization bugs could permit cross-company or over-broad tool
access; the PR remains inert until PR 4 wiring and requires dedicated
security review.
- Stack risk: merging out of order can expose incomplete layers;
mitigate by following the documented bottom-up merge policy.
- Parity risk: later edits to an intermediate branch can drift from
#9534; mitigate by re-running the empty top-of-stack diff before merge.

- Session and account cache cookies near the browser's per-cookie size
limit (for example with a long <code>cookiePrefix</code> or many cached
fields) are now split into chunks instead of being silently dropped by
the browser. A cache too large to fit even when chunked is skipped with
a warning rather than failing the request, so reads fall back to the
database.</p>

- Refresh-token rotation and token revocation, two-factor backup-code
regeneration, device-code claiming, and organization invitation
acceptance now work on Prisma. Concurrent or repeat requests in these
flows could previously return an error on Prisma instead of the expected
result.</p>
<p>On MongoDB servers older than 5.0, these flows and other guarded
value updates (rate-limit window resets, API-key refills) no longer fail
with an empty-update error.</p>
<p><code>@better-auth/core</code>: <code>incrementOne</code> now reports
a clear error when called with no <code>increment</code> and no
<code>set</code>.</p>

- <code>sendVerificationEmail</code> was invoked via
<code>runInBackgroundOrAwait</code>, which could defer work when
<code>advanced.backgroundTasks.handler</code> is configured (so the
handler could return <strong>200</strong> before the email callback
finished) and, in the default path, <strong>caught and logged errors
without rethrowing</strong>. User callbacks that throw
<code>APIError</code> (e.g. <strong>429</strong> from a rate limiter)
were therefore not reliably reflected in the HTTP response (<a
href="https://redirect.github.com/better-auth/better-auth/issues/8757">better-auth/better-auth#8757</a>).</p>
<p>Now we await <code>sendVerificationEmailFn</code> so failures surface
to the client with the correct status. The unauthenticated
<code>/send-verification-email</code> path enforces a constant-time
floor (500 ms) so that the response duration does not reveal whether the
email belongs to a real unverified user.</p>

## Thinking Path

Dale Stubblefield ad55af04cc fix: disable secure cookies for HTTP deployments
Fixes login failing silently on authenticated + private deployments
served over plain HTTP (e.g. Tailscale, LAN). Users can sign up and
sign in, but the session cookie is rejected by the browser so they
are immediately redirected back to the login page.

Better Auth defaults to __Secure- prefixed cookies with the Secure
flag when NODE_ENV=production. Browsers silently reject Secure cookies
on non-HTTPS origins. This detects when PAPERCLIP_PUBLIC_URL uses
http:// and sets useSecureCookies: false so session cookies work
without HTTPS.
