fix(runner): restore local session and task integrity (#12721)
## Thinking Path > - Paperclip is the control plane for agents that perform work. > - Paperclip Runner connects durable provider sessions to individual task runs through PRP. > - Provider continuity and per-run authority are different lifetimes. > - The existing implementation mixed those lifetimes and lost event metadata between provider frames, runnerd, persistence, API sanitization, and the task thread. > - That caused failed continuation, missing progress and Plans, duplicate replies, hidden failures, and unsafe recovery. > - This repair gives every heartbeat fresh authority, preserves qualified provider-session continuity, and restores one lossless presentation path without changing direct adapters. ## Linked Issues or Issue Description **What happened?** A second native heartbeat could reuse tickets, leases, command receipts, sequence state, and run identity from the first heartbeat. Provider phase and item identity could be lost before the UI read them. Redaction could corrupt protocol discriminators while still missing malformed credential tails. The task thread could fold progress into the final response, hide failures, or show more than one final answer. Native Codex also exposed approval modes that do not yet have a durable approval bridge. **Expected behavior** Each heartbeat uses a new PRP authority epoch. Codex and OpenCode preserve exact qualified provider sessions; ACPX emits an explicit continuity event when its qualified process-replacement policy is used. Every accepted provider event is presented, classified as internal, or surfaced as unsupported. The task page shows chronological progress, reasoning summaries, activity, Plans, interactions, terminal failures, and exactly one final reply. Direct adapters retain their existing path. **Steps to reproduce** 1. Enable the unified experimental Paperclip Runner setting. 2. Create a local native Codex, OpenCode, ACPX Claude, or ACPX Codex agent. 3. Run response, Plan, structured-question/resume, restart, cancellation, and failure scenarios. 4. Reload the task while active, waiting, failed, and settled. 5. On the old implementation, observe stale run authority, missing classifications, incomplete output, or duplicated/folded replies. **Paperclip version or commit** The repair is based directly on `master` at `87d05e194b643810d16d20612115acd01d735d43`. **Deployment mode** Local development with the embedded database. Related work: Refs #12616, #12646, #12666, #12685, and #12700. ## What Changed - Rotates PRP control-plane, outbox, ticket, lease, command, receipt, and sequence authority for each heartbeat while carrying forward only a validated provider-session identity. - Reads `control-plane-state.json`, validates both durable schemas and lifecycle values, resumes coherent current runs, archives qualified settled authority, and quarantines malformed or mismatched scoped state without moving ambiguous live legacy state. - Preserves Codex provider phase and stable item identities so commentary remains progress and only `final_answer` becomes final. - Adds raw OpenCode HTTP/SSE boundary coverage and canonical reasoning lifecycle mapping. - Makes ACPX normalization lossless for visible reasoning, tool lifecycle metadata, stable bounded identities, Plan revisions, structured requests, failures, and qualified process replacement. Only the compatible terminal assistant message is promoted as final. - Applies schema-aware redaction before generic JWT-shaped detection and scans every diagnostic string leaf. Malformed raw/escaped quoted credential tails are redacted in both server and durable Rust state. - Restores snapshot-style chronological task presentation, expandable tool activity, inline Plan cards, visible waiting/resume/cancel/failure states, and exactly one final answer. - Makes `never` the only qualified native Codex permission mode and rejects unsupported persisted native modes with remediation. OpenCode and ACPX policies remain intact. - Keeps the unified experimental Runner setting as the only enablement flag. Onboarding and direct Codex, Claude, and OpenCode stay on their legacy execution/finalization paths. - Adds cross-language goldens, authority/recovery/fault coverage, exact response/count assertions, and native plus legacy acceptance scenarios. ## Verification - Pull-request GitHub Actions run Rust formatting/tests, TypeScript checks, server/UI tests, builds, protocol drift checks, browser E2E, and security scans. - A separate workflow-only validation ref is pinned directly on this PR head and runs the 35-cell paid local matrix: three core scenarios plus structured-question resume and restart/resume for native Codex, native OpenCode, ACPX Claude, ACPX Codex, and direct Codex/Claude/OpenCode. Run: https://github.com/paperclipai/paperclip/actions/runs/33682434315 - Acceptance requires exact single visible replies, monotonic sequences, matching envelope discriminators, one semantic terminal, one run terminal, no unresolved interaction, no duplicate mutation, no secret leakage, provider continuity, and zero native rows for direct adapters. - Per maintainer direction, tests are running in GitHub Actions rather than on the slower local host. Only formatters and static diff checks were run locally. ## Risks - Recovery from old or partial filesystem state is sensitive. The repair fails closed, preserves active or unverifiable authority, and quarantines only state whose scoped ownership is safe to move. - Provider event formats can change. Closed validators and boundary goldens turn new or malformed events into visible diagnostics instead of silent drops. - Shared task presentation could affect direct adapters. Runtime-fact gating plus the direct-adapter matrix protect the existing path. - Managed and remote providers are not qualified here. Shared code continues to compile and fail safely, but live qualification is deferred. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used OpenAI Codex based on GPT-5. feat(runner): add Codex-native application integration (#12591)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - The runner package is useful only when the application can start, observe, and recover a native Codex run safely. > - Existing direct adapters must keep their current execution and finalization paths. > - The application boundary therefore needs additive persistence, authorization, coordination, and recovery behind an explicit experimental adapter. > - This pull request adds that Codex-only boundary without activating generalized providers, remote environments, or the later task/SDK surfaces. ## Linked Issues or Issue Description **Subsystem affected** Shared contracts, database persistence, adapter utilities, server native-runtime services, and the experimental Paperclip Runner adapter. **Problem or motivation** The already-landed runner package has a qualified Codex path, but the application needs durable native-run state, guarded runtime selection, authenticated coordination, tool security, finalization, and recovery before the experimental adapter can be exercised safely. **Proposed solution** Add a Codex-only `paperclip_runner` application path behind the existing default-off native-runner setting. Bind native state and coordination to company/run identity, preserve persisted-run recovery, and leave every direct adapter on its existing legacy execution path. **Alternatives considered** The earlier stack boundary introduced a generalized executor and remote-environment lifecycle here. That made this PR depend on implementations in higher PRs and changed reusable sandbox behavior globally. Those pieces are now deferred together to #12592. **Roadmap alignment** ROADMAP.md does not list a conflicting native-runner integration project. This change adds the application boundary for the existing Runner architecture. ## What Changed - Added native run/result/finalization/provider-trace persistence, shared validators, and idempotent migration/replay coverage. - Added guarded Codex-only runtime selection, authenticated PRP coordination, recovery, finalization, and interaction services. - Added run/company-bound tool-gateway authorization, credential redaction, SSRF protections, and replay-safe behavior. - Added the explicit `paperclip_runner` adapter behind the default-off rollout setting. - Preserved legacy answered-question wake projection and direct-adapter execution/finalization paths. - Hardened cancellation so only owned in-memory child processes are signaled; persisted recycled PIDs/process groups are never trusted. - Retained the narrow Claude ACPX isolated-context security follow-up discovered after #12590. - Deferred the generalized executor, provider ingress, remote lifecycle, SDK/lab/eval work, release-process changes, and lockfile. ## Verification - Changed-file delta against `master`: 133 files. - GitHub Actions is the authoritative verification environment for this PR. - Full CI, security, and Greptile review will run on this lowest unmerged stack PR. - Local tests/build/typecheck were not run because this checkout is resource constrained. - Static diff/reference checks pass, and `pnpm-lock.yaml` is unchanged. ## Risks - This touches central heartbeat and agent-route code, so legacy compatibility is the primary risk. - Runtime selection remains Codex-only and explicit; direct Codex, Claude, OpenCode, process, HTTP, and plugin adapters remain on their existing paths. - Fresh native starts fail closed while the rollout flag is off; persisted native records remain readable and recoverable. - Cancellation, company/run binding, tool calls, status decisions, and completion writes are guarded or replay-safe. > For core feature work, check [ROADMAP.md](ROADMAP.md) first and discuss it in #dev before opening the PR. Feature PRs that overlap with planned core work may need to be redirected. ## Model Used OpenAI Codex, GPT-5.6, with repository tools, code execution, and parallel agent review. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either linked existing issues or described the issue in-PR following the relevant issue template - [x] I have not referenced internal or instance-local Paperclip issues or links - [x] My branch name describes the change and contains no internal Paperclip ticket id - [ ] I have run tests locally and they pass — GitHub Actions is authoritative for this resource-constrained checkout - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented risks above - [ ] All Paperclip CI and security gates are green - [ ] Greptile is 5/5 with no open actionable findings - [x] I will address all Greptile and reviewer comments before merge ## Stack - Position: 3 of 5 overall; lowest of 3 currently unmerged - Base: `master` - Previous: [#12590](https://github.com/paperclipai/paperclip/pull/12590), qualified Claude ACPX runtime — merged - Next: [#12592](https://github.com/paperclipai/paperclip/pull/12592), generalized Codex executor, task experience, and developer SDKs --------- Co-authored-by: Dev Agent <dev@paperclip.ing> |

feat(connections): add self-serve intent runtime (#12345)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Agents need a governed way to request app connections during issue work. > - The catalog now describes the available providers and setup methods. > - A request must become a durable, company-scoped intent before an operator acts on it. > - This pull request adds that intent runtime across server, agent, CLI, and shared contracts. > - The benefit is a safe bridge from agent need to operator-approved setup. ## Linked Issues or Issue Description Refs #11965 This is stack 7 of 11. It depends on stack 6 and replaces another reviewable part of #11965. ## What Changed - Add connection intent types, validation, service logic, and routes. - Add agent runtime tools and CLI support for connection requests. - Add issue-thread interaction support for connection intents. - Add runtime, route, adapter, and contract tests. - Hold the final resolved-continuation row lock through asynchronous adapter preparation until an actual process spawn, so parking or reassignment cannot cross that boundary. - Report Hermes Gateway's first remote run request through the shared dispatch hook so the resolved-intent lock is released at the true dispatch boundary. - Revalidate the addressed user's live non-viewer membership and connection-management authority for every intent mutation, including OAuth completion. ## Verification - `pnpm --filter @paperclipai/server typecheck` - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/tool-access-service.test.ts` - Result: 176 tests passed. - `pnpm build` - `pnpm --filter @paperclipai/server exec vitest run src/__tests__/heartbeat-stale-queue-invalidation.test.ts` (32 passed; includes non-process dispatch lock-release coverage) - `pnpm exec vitest run --project @paperclipai/server server/src/__tests__/connection-intents-service.test.ts -t "addressed-user mutation"` (1 passed) - `pnpm exec vitest run --project @paperclipai/server server/src/__tests__/tool-access-service.test.ts -t "binds OAuth callback completion to the initiating board session"` (1 passed) - `pnpm --filter @paperclipai/hermes-paperclip-adapter test -- src/gateway/server/execute.test.ts` (23 passed; includes dispatch-hook ordering and exactly-once coverage) - `pnpm --filter @paperclipai/hermes-paperclip-adapter typecheck` ## Risks - A malformed intent could create an unusable operator request. - Validators and company checks reject invalid or cross-company requests. - The final continuation gate holds the issue row lock through adapter preparation until process or remote dispatch; later operator changes use the normal active-run interruption path. - The change does not add a database migration. > I checked `ROADMAP.md`. This stack continues the existing app connection work from #11965 and does not duplicate another planned item. ## Model Used OpenAI Codex, GPT-5. fix(adapters): restore Paperclip skill for legacy runners (#12225)
<!-- Write all pull request text in Simplified Technical English (ASD-STE100): short sentences, one instruction per sentence, simple approved vocabulary, and the active voice. --> ## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Legacy local adapters run agents that use the Paperclip skill for the control-plane workflow. > - PR #7029 removed the required-skill fallback and made runtime skill selection depend only on stored preferences. > - No migration or runtime fallback replaced that behavior for existing agents or non-CEO agents. > - PR #12138 added core skills to new CEOs, and PR #12147 added Claude skill discovery. These changes did not mount the operational skill for all legacy agents. > - This pull request makes the operational skill a legacy adapter runtime invariant. It keeps all other skills configurable. > - The native runner stays unchanged because its protocol supplies the control-plane contract. > - The benefit is that new and existing legacy agents can always operate through Paperclip. ## Linked Issues or Issue Description Refs #7029 Refs #12138 Refs #12147 **What happened?** A skill-capable legacy local agent could start without `paperclipai/paperclip/paperclip`. This happened when the agent had no stored skill preference. An explicit empty preference also removed the skill. The agent then reported that the Paperclip skill was not available. **Expected behavior** Every skill-capable legacy local adapter must mount the Paperclip operational skill when the runtime inventory contains it. Optional skills must remain configurable. The native runner must keep its current protocol-based behavior. **Steps to reproduce** 1. Create a non-CEO `codex_local` agent without `paperclipSkillSync` preferences. 2. Start a legacy heartbeat. 3. Inspect the managed `CODEX_HOME/skills` directory. 4. Observe that the Paperclip skill is absent before this change. **Paperclip version or commit** The problem reproduces on `master` before this pull request. PR #7029 introduced the configured-only selection behavior. **Deployment mode** Local development and self-hosted legacy local adapters. ## What Changed - Added a shared legacy skill resolver that always selects the canonical Paperclip operational skill when it is available. - Applied the resolver to direct adapter execution, ACPX execution, skill snapshots, and persistent skill sync. - Added Hermes skill materialization at sync and run boundaries. - Aligned Cursor, Gemini, and OpenCode execution-time injection with the configured child `HOME`. - Made Hermes stop execution when another installation blocks the required operational skill. - Kept optional skills controlled by `paperclipSkillSync.desiredSkills`. - Kept `paperclip_runner` on the configurable-only resolver. - Added regression coverage for missing preferences, empty preferences, each skill-capable legacy adapter, ACPX, Hermes, and native runner isolation. - Documented the legacy runtime invariant. ## Verification - `pnpm -r typecheck` passed on the pushed commit. - `pnpm build` passed on the pushed commit. - The adapter utility regression suites passed: 236 tests. - The changed server adapter suites passed: 48 tests across 12 files. - The OpenCode adapter suite passed: 8 tests. - The Hermes adapter suite passed: 7 tests. - `git diff --check` passed. - `pnpm test:run` is not clean on this macOS host. The command reported failures in unchanged workspace and filesystem suites. An isolated rerun of `company-skills.test.ts` and `company-skills-service.test.ts` reproduced 11 failures because macOS resolved `/var/...` paths as `/private/var/...`. The changed adapter suites pass independently. ## Risks - This change deliberately makes the operational skill non-removable for skill-capable legacy local adapters. - Existing agents receive the skill on their next list, sync, or run boundary. No database migration is required. - The resolver does not create a skill when the runtime inventory does not contain the canonical entry. - Hermes aborts a run if another installation occupies the required operational skill target. - Hermes removes only an undesired Paperclip-owned symlink that still points to the known Paperclip source. - The native runner does not receive the legacy default. The exact serving model ID and context window were not exposed. The agent used reasoning, tool use, and code execution. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix(server): surface skill materialization failures instead of dropping the skill (#12146)
## Thinking Path
> - Paperclip is the open source app people use to manage AI agents for
work
> - Runtime skill listing materializes each company skill's files before
handing them to the agent's adapter
> - A materialization failure was swallowed with catch-to-null, and the
skill silently vanished from the runtime while the library still showed
it installed
> - Operators saw "installed", agents saw nothing, and nobody saw the
cause; on claude-local a missing desired skill could even crash the
prompt-bundle hasher
> - This pull request turns both failure paths into structured "missing"
entries with the real error and makes every adapter skip unmountable
entries explicitly
> - The benefit is that a broken skill shows up as broken, with its
cause, instead of not existing
## Linked Issues or Issue Description
**What happened?**
A company skill whose runtime files fail to materialize (deleted source,
missing stored SKILL.md copy, failed version snapshot) disappears from
`listRuntimeSkillEntries` with no trace. Agent skill snapshots report a
generic "not available" with no cause. On claude-local, a desired skill
whose source path does not exist reaches the prompt-bundle hasher, whose
`fs.lstat` throws and can fail the whole run.
**Expected behavior**
The skill appears with `sourceStatus: "missing"` and a `missingDetail`
carrying the underlying error, snapshots and the UI show it as broken,
and adapters skip it at mount time with a logged warning instead of
crashing or dangling-symlinking.
**Steps to reproduce**
Install a local-path skill referenced by an agent, delete its source
directory contents so the stored SKILL.md copy cannot be recovered, and
start a run: before this change the skill vanishes from the runtime set
silently; on claude-local a pinned-but-unmaterializable version can fail
bundle preparation.
## What Changed
- `server/src/services/company-skills.ts` `resolveRuntimeSkillSource`:
both `.catch(() => null)` sites (version snapshot, runtime
materialization) now return the structured `{status: "missing", source,
detail}` shape the deliberate missing branch already used, with the
underlying error message in `detail`.
- `packages/adapter-utils/src/server-utils.ts`:
`isPaperclipSkillSourceMissing` is exported with a doc comment.
- `packages/adapters/claude-local/src/server/execute.ts`: missing
desired skills are filtered out of the prompt bundle and each one logs a
`[paperclip] Warning` with its detail to the run output.
- `cursor-local`, `gemini-local`, `kimi-local`, `opencode-local`,
`pi-local` `execute.ts`: mount loops (and the cursor/gemini injection
calls) skip missing entries instead of symlinking a nonexistent path.
## Verification
- `cd server && npx vitest run
src/__tests__/company-skills-service.test.ts` — new test pins the
missing-with-cause entry for a failed materialization. Nine pre-existing
project-workspace tests in this file fail on my machine at clean
`master` too (environment-specific); their count is unchanged by this
PR.
- `cd server && npx vitest run
src/__tests__/heartbeat-runtime-skills.test.ts
src/__tests__/claude-local-skill-sync.test.ts
src/__tests__/cursor-local-skill-sync.test.ts
src/__tests__/cursor-local-skill-injection.test.ts
src/__tests__/gemini-local-skill-sync.test.ts` — 12 tests pass.
- `cd packages/adapters/claude-local && npx vitest run` — 244 passed, 1
skipped.
- `pnpm run typecheck` clean in server, adapter-utils, and all six
touched adapters.
## Risks
- Runtime skill entry lists grow by the previously dropped entries (now
flagged missing). All shipped consumers either intersect with desired
sets, already handle `sourceStatus: "missing"`, or now skip missing
entries at mount time. The snapshot layer already understood the missing
shape via the `materializeMissing: false` path, so downstream contracts
are unchanged.

fix: stop teaching agents to curl literal {id} route templates (#12061)
## Thinking Path
> - Paperclip is the open source app people use to manage AI agents for
work
> - Adapters inject prompt text that teaches agents how to call the
Paperclip API, including copy-pasteable curl examples
> - Some of those URLs contained brace placeholders like
`/api/issues/{id}/checkout`
> - Agents paste such lines verbatim; the placeholder reaches the server
as `/api/issues/%7Bid%7D` and 404s, and request logs show agents doing
exactly that
> - The acpx engine's API note already avoids this by using
`$PAPERCLIP_TASK_ID`, and its test pins `/api/issues/{id}` out of the
prompt
> - This pull request applies the same standard to the gemini adapter,
the shared prompt template, and the openclaw gateway workflow
> - The benefit is that agents stop burning turns on placeholder 404s
and doc examples stay safe to execute as written
## Linked Issues or Issue Description
No public issue exists for this defect. The description below follows
the bug report template.
**What happened?**
Server request logs show agents issuing `GET /api/issues/%7Bid%7D` — the
literal, percent-encoded text `{id}` — which 404s. The source is adapter
prompt text: the gemini adapter's API note embeds a curl example with
`/api/issues/{id}/checkout` in the URL, the shared agent prompt template
mentions `/api/issues/{issueId}` endpoints, and the harness checkout
notice names `/api/issues/{id}/checkout`. Models copy these strings into
real requests.
**Expected behavior**
URL paths in prompt text must carry environment variables or real ids,
never brace placeholders, in every string an agent might execute
verbatim. Where a placeholder is unavoidable, the prompt must state
explicitly that the literal text must never be sent.
**Steps to reproduce**
1. Give an agent the gemini adapter's API access note.
2. Watch it call `curl ...
"$PAPERCLIP_API_URL/api/issues/{id}/checkout"` as written.
3. The server logs `POST /api/issues/%7Bid%7D/checkout 404`.
## What Changed
- gemini-local's API note curl example now uses `$PAPERCLIP_TASK_ID` and
tells the agent to substitute a real issue id when that variable is
absent — the same convention as the acpx engine's API note.
- The shared agent prompt template (`server-utils.ts`) uses
`$PAPERCLIP_TASK_ID` in its interaction-creation and resume-endpoint
mentions, and the harness checkout notice names `POST
/api/issues/$PAPERCLIP_TASK_ID/checkout`.
- openclaw-gateway's endpoint workflow keeps its `{issueId}`
placeholders — they are defined by its "determine issueId" step — but
now states explicitly that the literal text must never be sent in a URL.
- `server-utils.test.ts` pins the new form and adds negative pins that
keep `/api/issues/{id}` and `/api/issues/{issueId}` out of the shared
prompt template, mirroring the existing acpx-engine negative pin.
- The `confirmation:{issueId}:plan:{revisionId}` idempotency-key
template is untouched: it is a value-construction pattern, not a URL.
## Verification
- `npx vitest run packages/adapter-utils/src/server-utils.test.ts
packages/adapters/gemini-local packages/adapters/openclaw-gateway` — 152
passed. The single failure (`pre-selects gemini-api-key auth in the
managed HOME for sandbox execution`) is a pre-existing
environment-specific failure on the development machine, unrelated to
prompt text; CI is authoritative for it.
- `pnpm --filter @paperclipai/adapter-utils --filter
@paperclipai/adapter-gemini-local --filter
@paperclipai/adapter-openclaw-gateway typecheck`.
## Risks
- Low risk: prompt-text and test changes only; no runtime logic changes.
- Agents that memorized the old example strings keep working — the
routes are unchanged, only the placeholder text in prompts is.
## Model Used
- Claude Fable 5 (Anthropic), model id `claude-fable-5`, extended
thinking enabled, agentic tool use via Claude Code (CLI harness), 200k
context window.

fix(adapter-utils): extend the duplex fail-closed run disposition to the CLI lane (#11966)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Paperclip runs agents through adapter execution lanes > - Duplex adapters can lose their control channel before a process completes > - The ACP lane already fails closed, but the CLI lane can report false success > - This pull request applies the same completion rule to the CLI lane and shares the loss code > - The benefit is consistent failure reporting when a duplex channel closes during a run ## Linked Issues or Issue Description **What happened?** A CLI-lane duplex run can lose its control channel before clean process completion. The run can then report `succeeded` with exit code 0 and no error code. **Expected behavior** The execution target must fail closed when the channel dies before clean completion. It must return exit code 1, the typed `duplex_channel_lost` error code, and a short stderr note. **Steps to reproduce** 1. Start a duplex adapter run through the CLI execution lane. 2. Close the duplex control channel before the process completes cleanly. 3. Inspect the run result and error code. **Paperclip version or commit** Commit `5e01523d4eb6df4a20a0bddd05374c9c42225203`. **Deployment mode** Built from source. **Installation method** Built from source with pnpm. **Agent adapter(s) involved** Claude Code, Codex, Cursor, Gemini, Kimi, OpenCode, and Pi local adapters. **Database mode** Not database-related. ## What Changed - Add an optional `errorCode` field to `RunProcessResult`. - Add a one-read completion seam to the execution target process options. - Fail closed when a duplex channel dies before clean process completion. - Add `settleRunDisposition()` to atomically read and mark orderly completion. - Share the typed duplex loss error code across the ACP and CLI lanes. - Mark non-success terminal results as orderly completion before teardown. - Wire the seam through the seven duplex adapters. - Add regression tests for channel loss, clean completion, and non-clean terminal results. ## Verification - `npx vitest run packages/adapter-utils/src/execution-target-sandbox.test.ts` — 118 passed. - `npx vitest run packages/adapter-utils/src/acpx-engine/execute.test.ts -t "sandbox duplex run-disposition seam"` — 4 passed. - The author confirmed a clean type-check for `@paperclipai/adapter-utils` and the seven duplex adapter packages. - Pre-existing environment failures remain outside this change. They include `EACCES mkdir '/srv/paperclip'` and remote file-size setup failures. ## Risks The change alters terminal status for CLI duplex runs that lose control before clean completion. The typed error code and stderr note keep the failure visible. The broker marks failed, cancelled, and timed-out results as orderly completion to prevent false loss events during teardown. ## Model Used OpenAI Codex, GPT-5, tool use and code execution, with the standard GPT-5 context window. The model assisted with the implementation and test work. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes: #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(annotations): include issue document annotations in agent review context (#11332)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Reviewers annotate plans and issue documents with inline comments, and assigned agents act on that feedback > - The server already builds a bounded review context from open plan annotations and includes it in agent wake payloads > - Non-plan issue documents did not get the same treatment: their open annotation threads never reached the agent, and the properties pane did not surface their annotations > - This pull request extends the review-context path and the properties-pane UI to issue documents, at parity with plans > - The benefit is that agent feedback on any issue document reaches the assigned agent, not only feedback on the plan ## Linked Issues or Issue Description **What existing behavior does this improve?** The review-context pipeline that delivers inline annotation feedback to assigned agents, and the properties pane that surfaces those annotations to reviewers. **Subsystem affected** The server review-context path (`server/src/services/plan-review-context.ts`, wake payload assembly in `server/src/services/heartbeat.ts`, `server/src/routes/issues.ts`), shared wake-payload types (`packages/shared`, `packages/adapter-utils`), and the issue properties pane (`ui/src/components/issue-properties/`). **Current behavior** A reviewer can annotate any issue document, not only the plan. The agent wake payload includes open annotation threads for the plan document only. Feedback left on other issue documents is invisible to the assigned agent. In the properties pane, the Artifacts tab also gives no way to see or open a document's annotations. **Proposed behavior** Add `buildDocumentReviewContext` beside the existing plan builder. It collects open annotation threads for all non-plan issue documents, applies the same thread, comment, and character budgets across documents, and reports truncation. Include the result as a new `documentReviewContext` field in agent wake payloads and in the issue wake-context route. Keep the plan context on its legacy builder and field so plan-only wakes stay byte-for-byte compatible. Render the new context in the adapter wake-payload text, and surface annotation counts and the annotation panel for documents in the properties pane's Plans and Artifacts tabs. **Reason and benefit** The floating annotation popover and persistent highlight UI landed earlier; this change completes the loop so agent feedback on any issue document reaches the assigned agent, not only feedback on the plan. **Breaking changes** None. The wake payload gains a new optional `documentReviewContext` field; the existing plan context field and its legacy builder are unchanged, so plan-only wakes stay byte-for-byte compatible. ## What Changed - Add `buildDocumentReviewContext` in `server/src/services/plan-review-context.ts`: bounded review context (shared thread/comment/character budgets, per-document legacy limits) over all non-plan issue documents - Include `documentReviewContext` in agent wake payloads (`server/src/services/heartbeat.ts`) and in the issue wake-context response (`server/src/routes/issues.ts`) - Add shared `DocumentReviewContext` / `DocumentReviewContextDocument` types in `packages/shared` - Normalize and render the new context in adapter wake-payload text (`packages/adapter-utils/src/server-utils.ts`), with tests - Show a `DocumentAnnotationsCountChip` and the annotation panel for documents in the properties pane Plans and Artifacts tabs, with tests - Extend server document-annotations service tests to cover the new context builder ## Verification - Run `npx vitest run packages/adapter-utils/src/server-utils.test.ts server/src/__tests__/document-annotations-service.test.ts` from the repo root — 104 tests pass - Run `TZ=UTC npx vitest run ui/src/components/issue-properties/IssuePropertiesDocumentAnnotations.test.tsx ui/src/components/IssueProperties.test.tsx ui/src/components/IssueDocumentAnnotations.test.tsx ui/src/components/DocumentAnnotationPopover.test.tsx` from the repo root — 75 tests pass (one pre-existing monitor-row case asserts UTC timestamps, so use `TZ=UTC` locally; CI runs in UTC) - `pnpm run typecheck` in `server/` passes - Manual: annotate a non-plan issue document, then wake the assigned agent with a comment — the wake payload lists the open document annotation threads; the Artifacts tab shows the annotation count chip and opens the panel ## Risks - The wake payload gains a new optional `documentReviewContext` field; consumers that ignore unknown fields are unaffected, and the plan context field is unchanged - The context is new input to agent wakes; shared budgets (same limits as the plan context) bound token cost across all documents - Low UI risk: the properties-pane changes reuse the existing annotation components > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - Claude (Anthropic), model ID `claude-fable-5` (Claude Fable 5), with extended thinking and agentic tool use (Claude Code harness) ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix(adapter-utils): let explicit PAPERCLIP_API_URL override the derived runtime URL in run env (#10339)
## Thinking Path
> - Paperclip orchestrates AI agents for zero-human companies
> - Every agent run gets a run-scoped bridge into the Paperclip API
through the injected `PAPERCLIP_API_URL` / `PAPERCLIP_API_KEY` env vars,
built by `buildPaperclipEnv` in
`packages/adapter-utils/src/server-utils.ts`
> - `buildPaperclipEnv` resolves that URL as `PAPERCLIP_RUNTIME_API_URL
?? PAPERCLIP_API_URL ?? http://<listen-host>:<port>`, and the server
always exports `PAPERCLIP_RUNTIME_API_URL` derived from
`authPublicBaseUrl` at boot
> - When `authPublicBaseUrl` points at an address that is not reachable
from inside the runtime container (e.g. a VPN/tailnet-only address used
to keep the web UI off the public internet), every local run receives a
dead API URL (`curl` exit 7) and agents only survive by hand-rolling a
localhost fallback
> - An operator-set `PAPERCLIP_API_URL` is the documented escape hatch —
`docs/deploy/environment-variables.md` states the server "preserves the
value" when set externally and that the run-level var "inherits the
server-level value" — but the run env builder inverts the precedence, so
the override never actually reaches runs
> - This pull request swaps the precedence in `buildPaperclipEnv` so an
explicit `PAPERCLIP_API_URL` wins over the derived runtime URL, aligning
the behavior with the documented contract
> - The benefit is that operators with split-horizon topologies (public
auth URL != container-reachable URL) can point agent runs at a reachable
endpoint with one env var, with zero behavior change for deployments
that do not set it
## Underlying Issue
No pre-existing public issue covers this, so per CONTRIBUTING ("Link
Issues or Describe Them In-PR") here are the `bug_report.yml` fields
inline:
- **What happened:** with `PAPERCLIP_AUTH_PUBLIC_BASE_URL` on a
tailnet-only address and `PAPERCLIP_API_URL=http://localhost:3100`
explicitly set in the server environment, every agent run still received
`PAPERCLIP_API_URL=http://100.x.y.z:3100` (the derived,
container-unreachable URL); `curl` from inside the run exits 7 and
agents can only reach the API by hand-rolling a localhost fallback
- **Expected behavior:** the run env inherits the operator-configured
`PAPERCLIP_API_URL`, as documented in
`docs/deploy/environment-variables.md` ("preserves the value", run-level
var "inherits the server-level value")
- **Steps to reproduce:** (1) set `PAPERCLIP_AUTH_PUBLIC_BASE_URL` to an
address not reachable from inside the server container, (2) set
`PAPERCLIP_API_URL=http://localhost:3100` in the server env, (3) trigger
any agent run and inspect the spawned process env: it carries the
derived URL, not the override
- **Version/commit:** reproduced on the `91e58acb` image (2026-07-19);
the precedence is unchanged on current `master` (`a3b293e`)
- **Deployment mode:** single-host Docker Compose, local adapters
(`claude_local`/`codex_local`), web UI exposed via VPN/tailnet only
## Related PRs (dedup search)
Several in-flight PRs touch the same pain point (runs receiving an
unreachable injected API URL) — linked for reviewer context; none of
them honors the documented explicit override, and the older ones appear
stale:
- #9916 — reworks `PAPERCLIP_RUNTIME_API_URL` derivation and port
preservation (server side); complementary, does not change run-env
precedence
- #8130 — honors a pre-set `PAPERCLIP_RUNTIME_API_URL` (server side); a
complementary escape hatch via the runtime var instead of the documented
`PAPERCLIP_API_URL` override
- #8025 — heuristic: prefer loopback when the runtime bind is loopback
(no activity since Jun 12)
- #5692 — heuristic loopback-safe URL inside `buildPaperclipEnv` (no
activity since May 14)
- #4877 — broader same-host injection rework across 10 files (no
activity since May 2)
- #4794 — always forces loopback for spawned agents (no activity since
Apr 30; would break split-horizon setups where a reachable non-loopback
URL is intended)
This PR intentionally takes the Path-1 route from CONTRIBUTING: the
smallest possible change (swap two lines so the documented operator
override wins) plus regression tests, rather than a new heuristic.
## What Changed
- `packages/adapter-utils/src/server-utils.ts`: `buildPaperclipEnv` now
resolves the injected URL as `PAPERCLIP_API_URL ??
PAPERCLIP_RUNTIME_API_URL ?? http://<listen-host>:<port>` (explicit
override first), with a short comment explaining why
- `packages/adapter-utils/src/server-utils.test.ts`: three new tests
covering the override precedence, the derived-URL fallback, and the
listen-host default (including the `0.0.0.0` to `localhost` mapping)
- `server/src/__tests__/paperclip-env.test.ts`: updated the expectation
that encoded the old runtime-URL-first precedence and added the
symmetric fallback case (runtime URL used when no explicit override is
set)
- No docs changes needed: `docs/deploy/environment-variables.md` already
describes the fixed behavior
## Verification
- `vitest run` on the new `buildPaperclipEnv` tests in
`packages/adapter-utils`: 3/3 pass
- `vitest run` on `server/src/__tests__/paperclip-env.test.ts` after the
expectation update: 5/5 pass (the first CI run correctly flagged the one
test that encoded the old precedence)
- Reproduced and verified on a production deployment (single-host
Docker, `PAPERCLIP_AUTH_PUBLIC_BASE_URL` on a tailnet-only address):
- Before: freshly spawned runs received
`PAPERCLIP_API_URL=http://100.x.y.z:3100` (verified in the spawned
process `/proc/<pid>/environ`); `curl` to it from inside the container
exits 7
- After (with `PAPERCLIP_API_URL=http://localhost:3100` in the compose
environment): a fresh run received `http://localhost:3100`, and `curl
$PAPERCLIP_API_URL/api/agents/me` with the run-scoped key returned HTTP
200; the run finished `succeeded` with usage telemetry recorded
## Risks
- Low. Behavior changes only for deployments that explicitly set
`PAPERCLIP_API_URL`; when unset (the default),
`PAPERCLIP_RUNTIME_API_URL` is used exactly as before
- The sandbox callback bridge (`execution-target.ts`) is intentionally
untouched: remote sandboxes genuinely need the publicly reachable URL,
and its `input.hostApiUrl || PAPERCLIP_RUNTIME_API_URL || ...` chain
still provides it

thinking + agentic tool use via Claude Code, operating over SSH against
the affected deployment

feat(settings): add experimental Simplified English Interactions flag (#10934)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Agents ask humans for decisions through interaction blocks: plan confirmations, structured questions, suggested tasks, and checkbox prompts > - Each agent writes these decision prompts in its own style, so operators can get long or unclear text at the exact moment they must decide > - There is no instance-level control that makes agents use a controlled language for these decision points only > - This pull request adds an experimental setting that tells agents to write all user-interaction content in ASD-STE100 Simplified Technical English, with the context the user needs and the effect of each choice > - The benefit is faster, clearer human decisions, with agent thinking and normal responses unchanged ## Linked Issues or Issue Description Refs #10410 (optional `/simplified-english` skill in the skills catalog; this PR adds the instance-level toggle for interactions). **Problem or motivation** Agent-posted user interactions (plan confirmations, structured questions, suggested-task proposals, checkbox prompts) are written in each agent's default style. Operators who want fast, unambiguous decisions have no way to ask agents to use a controlled language for exactly those decision points. **Proposed solution** Add an experimental instance setting, `enableSimplifiedEnglishInteractions` ("Simplified English Interactions"). When it is on, the server sets `simplifiedEnglishInteractions: true` in the heartbeat wake payload. The shared wake-prompt renderer, used by every adapter, then emits a directive: write all user-interaction content in ASD-STE100 Simplified Technical English, state what information the user needs to decide, and state what happens for each choice. The directive applies to interaction content only. Thinking, comments, documents, and other responses keep their usual style. **Alternatives considered** Per-agent instructions work today, but someone must maintain them on every agent. An instance-level toggle applies uniformly and turns off in one place. Server-side rewriting of interaction payloads was rejected: post-hoc translation is lossy and cannot add the decision context that only the agent has. **Roadmap alignment** Extends the experimental settings surface with another opt-in agent-behavior refinement, consistent with existing prompt-side flags. ## What Changed - Added `enableSimplifiedEnglishInteractions` to the experimental instance-settings zod schema, mirror type, and feature catalog (default off, tier preference) in `packages/shared`. - Server `instance-settings.ts` normalizes the flag on both read branches; `heartbeat.ts` reads it once and passes `simplifiedEnglishInteractions` into `buildPaperclipWakePayload`. - Shared adapter renderer (`packages/adapter-utils/src/server-utils.ts`): added the field to `PaperclipWakePayload`, normalization, and an `- interaction language (experimental): ...` directive emitted in both fresh and resume prompt lanes, so one injection point covers all adapters. - UI: new experimental settings card "Simplified English Interactions" in `ui/src/pages/InstanceExperimentalSettings.tsx`, in alphabetical card order; fixtures updated. - Tests: renderer coverage for flag on/off in both lanes, plus schema/catalog/UI fixture updates. ## Verification - From the repo root: `node_modules/.bin/vitest run packages/adapter-utils` (88/88), `packages/shared` validators (25/25), server instance-settings + heartbeat suites (47/47 and 70/70 consumer tests), `ui` settings tests (32/32). - Typecheck is clean in all four touched packages. - Manual check: turn the flag on in Settings → Experimental, wake an agent, and confirm the wake prompt contains the interaction-language directive; turn it off and confirm the directive is absent. ## Risks - Low risk: the flag defaults to off, and the only behavior change is one extra directive line in the wake prompt when an operator turns it on. - The directive is advisory to the agent; models can still deviate from STE. No data or API shape changes; no migration. ## Model Used - Claude Fable 5 (Anthropic, model ID `claude-fable-5`), extended thinking, agentic tool use via Claude Agent SDK. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(observability): add granular OpenTelemetry spans for sandbox startup and execution (#10758)
## Thinking Path > - Paperclip coordinates AI agent work and records execution data. > - The sandbox startup path and the host-to-sandbox execution path need clearer OpenTelemetry spans. > - The trace data now shows wait time, critical path data, and bounded labels. > - This pull request adds bounded spans and closed attribute helpers for sandbox startup and execution. > - The result is better trace data with low cardinality and no secret leakage. ## Linked Issues or Issue Description - No public GitHub issue exists for this branch. - Related PR: #10536 - This PR extends the sandbox OpenTelemetry work with exec spans, root timing, and bounded labels. ## What Changed - Added a closed span-attribute contract for sandbox startup data. - Added a bounded label helper for command and region values. - Threaded the active step context into the execution path. - Added a `sandbox.exec` span with true timestamps and bounded attributes. - Added skipped-step and root-span timing data with low-cardinality context. - Moved handshake sub-times and bridge batch data onto spans. ## Verification - `pnpm --filter @paperclipai/adapter-utils run typecheck` - `pnpm --filter @paperclipai/server run typecheck` - `pnpm --filter @paperclipai/adapter-utils exec vitest run` - `pnpm --filter @paperclipai/server exec vitest run environment-execution-target` - `git log --oneline origin/master..origin/feat/sandbox-startup-otel-spans` - `git diff origin/master...origin/feat/sandbox-startup-otel-spans --stat` ## Risks - Span attribute rules could still miss a future field if new code skips the shared helper. - Low-cardinality labels may hide some detail, but that is the intended tradeoff. - Tracing stays fail-open, so a missing tracer still hides data rather than stopping work. ## Model Used - OpenAI Codex, GPT-5, tool use enabled. The runtime did not expose the context window size. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge The current head still has one failing required check: `e2e`. fix(adapter-utils): correct confirmation wake semantics (#10588)
<!-- Write all pull request text in Simplified Technical English (ASD-STE100): short sentences, one instruction per sentence, simple approved vocabulary, and the active voice. --> ## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Local agent wakes include a default execution contract > - That contract tells agents how issue-thread continuation policies behave > - The current text says `wake_assignee` resumes a confirmation only after acceptance > - The server actually wakes for every non-expired resolution and reserves acceptance-only behavior for `wake_assignee_on_accept` > - This pull request makes the default prompt match the server contract and strengthens the recovery follow-up regression case > - The benefit is that agents choose the correct continuation policy and recovery tests cover normalized agent name keys ## Linked Issues or Issue Description Related work: Refs #5473, Refs #5060, and Refs #10562. **What happened?** The default local-agent prompt described `wake_assignee` as acceptance-only for `request_confirmation`. This conflicts with the server. The server wakes on every non-expired resolution. A recovery follow-up test also used an already-normalized execution agent name key, so it did not exercise the normalization seam. **Expected behavior** The prompt must state that `wake_assignee` resumes after acceptance or rejection. It must direct acceptance-only flows to `wake_assignee_on_accept`. The recovery regression must use a display-style agent name key and prove that the follow-up path still works after normalization. **Steps to reproduce** 1. Read the default local-agent prompt in `packages/adapter-utils/src/server-utils.ts`. 2. Compare its confirmation continuation text with `queueResolvedInteractionContinuationWakeup` in `server/src/routes/issues.ts`. 3. Observe that the prompt gives acceptance-only semantics to `wake_assignee`. 4. Inspect the recovery hand-back test and observe that its execution name key is already normalized. **Paperclip version or commit** `7301fae942` **Deployment mode** Local dev. The prompt and test behavior are not deployment-specific. ## What Changed - Corrected the default agent prompt for `wake_assignee` and `wake_assignee_on_accept`. - Added focused prompt assertions for both the required and obsolete text. - Changed the recovery follow-up fixture to use a display-style agent name key. ## Verification - `pnpm exec vitest run packages/adapter-utils/src/server-utils.test.ts -t 'keeps the default local-agent prompt action-oriented'` passed: 1 test. - `pnpm exec vitest run server/src/__tests__/heartbeat-comment-wake-batching.test.ts -t 'defers recovery hand-back wakes until the resolving run exits'` passed: 1 test. - `pnpm --filter @paperclipai/adapter-utils typecheck` passed. - `pnpm --filter @paperclipai/server typecheck` passed. - `git diff --check origin/master...HEAD` passed. ## Risks - Low risk. The production change updates prompt text only. - Agents that followed the old text may now choose `wake_assignee_on_accept` for acceptance-only flows. - The server test change only broadens an existing regression fixture. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. feat: sync @-mentioned projects into remote sandboxes (confined sandbox transport, flag ON) (#10564)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - The run layer must move project context into the sandbox that executes the agent > - Local sandboxes already stage referenced projects for @-mentions > - Remote confined sandboxes dropped the whole referenced set, so the agent lost needed files and paths > - This pull request keeps the confined sandbox transport aligned with the local behavior for referenced projects > - It does this behind a remote-only flag that defaults on, while SSH keeps the old drop-only path > - The benefit is that remote runs can read the same referenced project context that local runs already provide ## Linked Issues or Issue Description **Subsystem affected** Cross-cutting. This change touches server orchestration, sandbox transport, and observability. **Problem or motivation** A run can @-mention another project. Local targets stage each referenced project and give the agent a path. Remote confined sandboxes dropped the full referenced set, so the agent could not read those project files or paths. **Proposed solution** Enable referenced-project sync for the confined sandbox transport behind `PAPERCLIP_MULTI_PROJECT_WORKSPACE_SYNC_REMOTE`, which defaults on. Keep the SSH transport out of scope and keep it dropping referenced projects. Repoint each referenced workspace hint at its staged `project-<projectId>` sandbox directory. Publish `PAPERCLIP_WORKSPACES_JSON` on the confined sandbox lane. Count each per-project remote staging failure as a `staging` failure in the requested-vs-synced metrics. **Alternatives considered** Keep the remote path drop-only. That keeps the gap open. Move the change into SSH too. That expands scope beyond the target transport and adds risk. **Roadmap alignment** No matching item in `ROADMAP.md` showed up in this review. **Additional context** The change lands in three commits. The first commit opens the gate for the confined sandbox transport. The second commit repoints the workspace hints and publishes the workspace map. The third commit records per-project staging failure data. ## What Changed - Opened remote referenced-project sync for the confined sandbox transport behind `PAPERCLIP_MULTI_PROJECT_WORKSPACE_SYNC_REMOTE`. - Repointed referenced workspace hints to the staged `project-<projectId>` sandbox directories and published `PAPERCLIP_WORKSPACES_JSON`. - Counted per-project remote staging failures as first-class `staging` failures in the requested-vs-synced observability. ## Verification - The pushed ref `refs/heads/feat/sync-referenced-projects-remote-sandbox` resolves to the authorized submit SHA. - `git log --oneline origin/master..origin/feat/sync-referenced-projects-remote-sandbox` shows exactly the three expected commits. - The handoff reports server typecheck clean, adapter-utils typecheck clean, and the listed unit tests passing. - The handoff also reports no open review comments and no Greptile score yet. ## Risks - The change touches authorization and sandbox path handling, so regressions could block remote runs or expose the wrong project context. - The new flag defaults on, so any bug in the remote path affects normal remote use. - SSH stays out of scope, so the two transport paths must remain distinct. ## Model Used OpenAI GPT-5 via Codex. Tool use enabled. Context window not reported in this run. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes: #` / `Refs: #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [x] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat: make recovery updates quieter (#10542)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - The recovery subsystem restores work after an agent run stops or loses state > - Recovery notices currently use the same visual weight as normal work comments > - Recovery agents can also post long narratives that obscure the useful hand-off > - The server must identify recovery output because agents cannot set presentation controls > - This pull request adds compact recovery notices, structured action references, and brief recovery prompts > - The benefit is a quieter issue thread that still keeps recovery state inspectable ## Linked Issues or Issue Description **Subsystem affected** Cross-cutting: `server/`, `packages/shared`, and `packages/adapter-utils`. **Problem or motivation** Recovery notices and recovery-run comments can dominate an issue thread. Operators must scan routine recovery narration before they find the work hand-off. **Proposed solution** Give routine recovery output a compact system-notice presentation. Derive the presentation on the server so agents cannot hide arbitrary comments. Keep the successful missing-state summary fully visible because that comment is the recovery deliverable. **Alternatives considered** The UI could detect recovery text. That approach is fragile and does not provide structured action references. Agents could also set presentation directly, but that would weaken the current board-only security boundary. **Roadmap alignment** This change refines the completed “Self-healing runs & automatic recovery” and “Enforced Outcomes” roadmap areas. It does not add a competing roadmap capability. **Additional context** The scope covers shared comment validation, server recovery notices, agent-comment derivation, and recovery prompt text. No database migration is needed because presentation data already uses JSON. ## What Changed - Add the `compact` issue-comment presentation density to shared constants, types, and validation. - Give recovery escalation, waiting, and in-place notices compact titles and structured recovery-action metadata. - Use recovery-action metadata for notice deduplication, with the legacy text marker as a compatibility fallback. - Derive compact presentation for comments from recovery-scoped runs while preserving the board-only presentation boundary. - Keep successful missing-state recovery summaries fully visible. - Ask recovery participants to record outcomes in `resolutionNote` and keep source-issue comments brief. - Add shared, route, service, and prompt tests for the new behavior and exceptions. ## Verification - `pnpm -r typecheck` - Focused Vitest coverage: 320 tests passed across shared validators, adapter prompts, issue comments, recovery actions, and heartbeat recovery. - Full server phase: 292 files passed, 3,094 tests passed, and 2 tests skipped. - Full UI phase: 386 files passed and 3,182 tests passed. - `pnpm build` - Known master baseline: `cli/src/__tests__/secrets.test.ts` expects `pass`, but the current implementation returns `warn` when strict secret mode is disabled for Postgres. This branch does not change CLI secrets code. ## Risks - Low migration risk. The presentation column is JSON and needs no database migration. - Recovery-run detection depends on the persisted run context snapshot. - Structured metadata becomes the primary deduplication key. The existing body marker remains as a fallback for older comments. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI Codex with `gpt-5.6-sol`. The runtime did not expose the context-window size. The model used agentic reasoning, repository tools, code execution, and test execution. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> Co-authored-by: Claude Opus 4.8 <noreply@anthropic.com> |

perf(heartbeat): reuse task sessions for issue-scoped timer wakes and bound control-plane write retries (#10350)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Agents make progress in heartbeats: the server wakes an agent session, it does a slice of work on an issue, records a disposition, and exits > - Benchmarking identical coding tasks run as Paperclip-orchestrated agent pairs vs invoking the same agent harness directly measured a 1.8–2.2× wall-clock slowdown for the Paperclip pairs, dominated by per-heartbeat orchestration overhead rather than model time > - Two contributors stood out: (1) since PF-4 (#4838) every `heartbeat_timer` wake starts a brand-new task session, so continuation work on a specific issue repays the full session-start and re-orientation cost on every heartbeat; (2) in degraded environments agents burn many tool calls retrying the same failing control-plane write before giving up > - This pull request reuses the task session for issue-scoped timer wakes (keeping the PF-4 fresh-session rule only for unscoped exploratory wakes, which were the original context-bloat case) and adds a bounded-retry rule to the wake prompt and core skill: after 2 consecutive failures of the same control-plane write, stop retrying it for the rest of the heartbeat and rely on the adapter/runtime status channel > - The benefit is materially less wall-clock and token overhead per heartbeat while preserving the context-bloat protection PF-4 was added for ## Linked Issues or Issue Description Refs #4838 (merged PF-4 change whose reset rule this refines), Refs #5287, Refs #1907 (related timer-heartbeat session work). No public GitHub issue exists for the slowdown itself; bug-report fields: - **What happened:** Agent pairs orchestrated through Paperclip heartbeats complete identical task sets 1.8–2.2× slower (wall-clock) than the same harness invoked directly. Profiling attributed the gap to per-heartbeat orchestration overhead: every timer wake discards the task session (full session start + re-orientation), and in degraded environments agents repeatedly retry the same failing control-plane write. - **Expected behavior:** Heartbeat orchestration should add minimal wall-clock overhead on top of the underlying harness; issue-scoped continuation work should not pay a fresh-session tax each interval. - **Steps to reproduce:** Run a fixed benchmark task set once through Paperclip issue heartbeats and once via direct harness invocation with the same model/config; compare wall-clock totals. - **Version/commit:** master @ |

Deduplicate wake-payload issue descriptions and compact resume deltas (#10216)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Heartbeat wake payloads and the task-context markdown are the two channels that deliver an issue's brief into an agent's prompt > - #10151 fixed wake-prompt-only adapter lanes waking without the issue description by adding it to the structured wake payload > - That left the description delivered twice per prompt on lanes that also inject the task-context markdown, and re-delivered in full on every resume wake, permanently bloating persistent-session context > - This pull request makes the task markdown the single description carrier on lanes that use it, and omits the description from non-assignment resume deltas on all lanes while keeping it for assignment-shaped and recovery wakes > - The benefit is that every lane receives the brief exactly once when it needs it, and long-lived sessions stop re-paying the full brief in tokens on every wake ## Linked Issues or Issue Description Refs #10151 Related prior work: #2883, #8402 (earlier description-delivery attempts referenced by #10151). I searched the PR list for open work on wake-payload description handling and found none besides the merged #10151. **Bug:** After #10151, adapters that inject the `Paperclip task context` markdown (ACPX engine lanes, claude-local CLI, hermes server and gateway) receive the issue description twice in a single prompt — once in the wake prompt's `Issue description:` block and once in the task markdown. Separately, resume deltas re-send the full description (up to 12k characters) on every wake even though the persistent session already received it. **Expected behavior:** The description appears exactly once per prompt on every lane, and resume deltas only carry it when the resuming session may not have seen the brief (assignment-shaped or recovery wakes), leaving an explicit fetch breadcrumb otherwise. **Reproduction:** Wake a claude-local or ACPX agent on an issue with a description and inspect the assembled prompt: the description text appears in both the wake-payload block and the task-context block. Wake the same session again via a comment: the full description is present again in the resume delta. **Affected version:** Current `master` (with #10151 merged). **Deployment mode:** Adapter-backed heartbeat execution, local and sandboxed lanes. ## What Changed - `renderPaperclipWakePrompt` accepts `suppressIssueDescription`; the four task-markdown lanes pass it so the task markdown stays the single, uncapped description carrier there. - Non-assignment resume deltas omit the description and emit `- issue description: omitted from this resume delta; fetch the issue if you need the latest brief`. Assignment-shaped reasons (`issue_assigned`, `issue_reopened_via_comment`, `issue_recovery_action_restored`, `issue_tree_restored`) and recovery wakes still deliver the full brief. - `buildPaperclipTaskMarkdown` gains `includeDescription`; the server now also publishes `context.paperclipTaskMarkdownCompact` (description stripped, directives and wake comment kept), and the new `selectPaperclipTaskMarkdown` helper picks the right variant under the same resume rules, falling back to the full markdown when no compact variant exists (version skew safety). - The wake prompt's description block now carries the same user-authored trust framing the task markdown already had. ## Verification - `npx vitest run packages/adapter-utils/src/server-utils.test.ts packages/adapters/claude-local/src/server/acp.test.ts packages/adapters/codex-local/src/server/acp.test.ts server/src/__tests__/heartbeat-context-summary.test.ts` — 137 tests passed, including new coverage for suppression, resume omission plus breadcrumb, assignment-shaped resume inclusion, compact-variant building, variant selection, and an end-to-end ACPX prompt-assembly test asserting the description appears exactly once on fresh wakes and not at all on comment resumes. - `npx vitest run` in `packages/adapters/hermes` — 59 tests passed, including a gateway execute-level test asserting the brief is sent exactly once on fresh runs and not re-sent on stable-session resumes. - `tsc --noEmit` in `packages/adapter-utils`, `packages/adapters/claude-local`, `packages/adapters/hermes` — clean; `server` matches the `master` baseline exactly (pre-existing plugin-sdk resolution errors only, none in touched files). - Pre-existing failures confirmed identical on clean `master`: claude-local `execute.remote.test.ts` / `test.probe.test.ts`, adapter-utils `mcp-isolation.integration.test.ts` (requires a newer local Claude CLI). ## Risks - Behavioral shift, prompt-only: a resumed session woken by a comment on an issue it never handled (rare — assignment wakes normally precede comment wakes) would not get the inline description; the breadcrumb plus the standard issue-fetch path covers it. - Additive context key (`paperclipTaskMarkdownCompact`); older adapters ignore it and newer adapters fall back to the full markdown when it is absent, so mixed-version deployments degrade to current behavior. - No schema, migration, or API changes; the structured wake-payload JSON shape is unchanged. - Known follow-up deliberately out of scope: openclaw embeds the raw wake-payload JSON (which still contains the description) in prompt text for machine parsing. The hermes-gateway lane is handled: it detects stable-session resumes (issue/agent session-key strategy plus a stored prior session id), compacts the task markdown, and omits the description from its prompt-embedded JSON copy. > This is a focused correctness/efficiency fix to existing wake plumbing and does not overlap with planned roadmap feature work. ## Model Used - Anthropic Claude Fable 5 (`claude-fable-5`), extended thinking enabled, with repository tool use, shell execution, and local test execution via Claude Code. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [ ] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details (execution-workspace branch, same convention as merged #10202) - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes (code-level docs; no user-facing docs affected) - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge 🤖 Generated with [Claude Code](https://claude.com/claude-code) --------- Co-authored-by: Claude Fable 5 <noreply@anthropic.com> |

Fix issue descriptions in structured wake payloads (#10151)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Heartbeat wake payloads provide the scoped task context an agent needs before it can act safely > - Assignment wakes already loaded the issue description for task markdown, but the structured wake-payload builder dropped it > - Agents reading `PAPERCLIP_WAKE_PAYLOAD_JSON` could therefore see a missing brief while also being told no fallback fetch was needed > - Long descriptions also need a bounded representation so wake environments and prompts remain safe > - This pull request carries the description through the server and adapter contract, and marks truncated descriptions as requiring fallback fetch > - The benefit is that agents receive the actual brief instead of inventing requirements from the title ## Linked Issues or Issue Description Fixes: #5844 Fixes: #2882 Related prior attempts: #2883 and #8402. This change adds focused regression coverage and enforces the missing long-description fallback invariant. **Bug:** Issue-assignment wake payloads omitted the issue description from the structured payload even when the issue had a populated description. **Expected behavior:** The structured wake payload includes the issue description. If the description must be truncated for payload size, `fallbackFetchNeeded` is `true`. **Reproduction:** Assign an issue with a description to an agent and inspect `PAPERCLIP_WAKE_PAYLOAD_JSON`; before this change, `issue.description` was absent while `fallbackFetchNeeded` could remain `false`. **Affected version:** Reproduced on current `master` before this patch. **Deployment mode:** Adapter-backed heartbeat execution, including local Codex agents. ## What Changed - Include `issues.description` in the server wake-payload query and supplied issue summaries. - Bound inline descriptions at 12,000 characters and force fallback fetch when truncation occurs. - Preserve and render description metadata through shared adapter normalization and prompt rendering. - Add focused tests for long-description fallback and exact brief-string rendering. ## Verification - `pnpm exec vitest run server/src/__tests__/heartbeat-agent-session-message.test.ts packages/adapter-utils/src/server-utils.test.ts` — 81 tests passed. - `pnpm --filter @paperclipai/adapter-utils typecheck` — passed. - `pnpm --filter @paperclipai/server typecheck` — passed. - `git diff --check` — passed. ## Risks - Low risk: the payload shape is additive. - Very long descriptions are truncated at 12,000 characters; the payload explicitly requests a fallback fetch for the full brief. - Prompt size increases by the issue-description length for scoped wakes, bounded by the same limit. > This is a focused correctness fix and does not overlap with planned roadmap feature work. ## Model Used - OpenAI GPT-5.4 via Codex CLI, with reasoning, repository tool use, shell execution, and test execution. The runtime did not expose a context-window size. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix: preserve control plane access in Codex sandbox (#10152)
## Thinking Path > - Paperclip is the open source control plane people use to manage AI agents for work > - Local coding adapters can be confined with filesystem and network sandbox policies > - Codex confinement proxied only user-allowlisted hosts, so it denied Paperclip's own run API and managed MCP endpoints > - The same proxy returned untyped plaintext denials, which MCP clients could treat as a fatal unexpected content type > - Host-only `PAPERCLIPAI_CMD` values could also leak into agents even when the referenced checkout module did not exist > - This pull request gives the sandbox an explicit trusted-URL channel for Paperclip endpoints, returns structured JSON errors, and removes the inherited host CLI pointer > - The benefit is confined Codex agents retain control-plane access without broadening the operator's external network allowlist or crashing on policy denials ## Linked Issues or Issue Description Refs #3802 Related foundation: #9504 ### What happened? A `codex_local` agent configured with `networkScope: "allowlist"` and an external-only allowlist could not reach its own Paperclip API or Paperclip-managed MCP endpoints. The sandbox proxy returned a `403` plaintext response without `Content-Type`, and an inherited `PAPERCLIPAI_CMD` could point at a missing checkout-local CLI module. ### Expected behavior Paperclip's run-scoped API and managed MCP endpoints remain reachable regardless of the user external allowlist. Policy denials are valid structured JSON responses with an explicit media type, and host-only CLI pointers are not inherited by agent processes. ### Steps to reproduce 1. Configure a `codex_local` agent with `networkScope: "allowlist"` and `networkAllowlist: ["api.openai.com"]`. 2. Run the agent and request its Paperclip issue API or a Paperclip-managed MCP endpoint. 3. Observe the sandbox proxy deny the request with an untyped plaintext `403` response. ### Environment - Paperclip commit: `f49a3f99` originally exhibited the defect; fix is based on current `master`. - Deployment: local source build on Linux. - Adapter: Codex. - Database: not database-related. - Access context: agent bearer/run-scoped credentials. ## What Changed - Added internal trusted URL rules to the local network allowlist proxy and supplied the Codex run API plus managed MCP endpoints. - Returned JSON error envelopes with `Content-Type` and `Content-Length` for HTTP and CONNECT policy denials. - Removed inherited `PAPERCLIPAI_CMD` from child process environments while preserving explicitly constructed runtime variables. - Added focused proxy and environment sanitizer regression tests. ## Verification - `pnpm exec vitest run packages/adapter-utils/src/local-process-sandbox.test.ts packages/adapter-utils/src/server-utils-env.test.ts --reporter=verbose` - 2 test files passed; 8 tests passed; 4 platform-dependent tests skipped. - `git diff --check` - Package typecheck was attempted; it reaches unrelated current-`master` type drift in untouched files (`spawnCwd` in `adapter-utils`, and staged-runtime ACP types in `codex-local`). ## Risks - Low risk: trusted access is restricted to exact HTTP(S) hostname and port pairs derived from Paperclip-provided URLs. - Invalid or non-HTTP trusted URL values are ignored rather than broadening access. - Denial response bodies change from plaintext to structured JSON; status codes remain unchanged. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI Codex coding agent (exact model ID and context window are not exposed to this runtime), reasoning and terminal/tool execution enabled. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix: deliver plugin agent session turns and replies (#10137)
## Thinking Path > - Paperclip manages agent execution through heartbeat runs and adapter-specific sessions > - Plugins can open an agent session and send a conversational message through the host service > - The host previously stored that message only in opaque wake payload metadata, so local adapters never saw it in their CLI prompt > - The host also forwarded run log chunks but did not expose the persisted final assistant text as the session reply > - This pull request defines both sides of the session contract in the shared wake renderer and terminal run event > - The benefit is that local adapters receive the actual conversational turn and plugins receive one canonical final reply ## Linked Issues or Issue Description Related context: Refs #629 and Refs #2880 describe adjacent `claude_local` final-text visibility failures. They concern issue comments rather than plugin agent sessions, but exercise the same need for a canonical persisted run summary. Companion consumer change: paperclipai/paperclip-gateway#3. Bug description: - **Observed:** calling the plugin host's `agents.sessions.sendMessage()` with `prompt: "hello"` woke a `claude_local` agent, but the generated CLI prompt omitted `hello`. On completion, the session emitted log chunks and a generic `Run completed` done event, so callers could not reliably recover the assistant reply. - **Expected:** the prompt becomes the user-supplied conversational turn for that agent session, and the successful terminal event carries the run's canonical final user-facing assistant text. - **Reproduction:** create a plugin agent session for a local adapter, call `sendMessage()` with a non-empty prompt, inspect the adapter prompt and terminal session event. - **Affected baseline:** `b517b887a` on `master`, local trusted deployment with plugin host services and `claude_local`; `codex_local` shared the wake-rendering gap because both use the common Paperclip wake prompt renderer. ## What Changed - Added a typed `agentMessage` wake payload rendered by the shared adapter prompt path used by `claude_local`, `codex_local`, and other local adapters. - Labeled session content as user-supplied and explicitly non-authoritative: it cannot expand authorization, permissions, task scope, or company boundaries. - Preserved ordinary heartbeat behavior by omitting the section when no agent-session message exists. - Added canonical `finalText` to terminal heartbeat status events from the already-persisted run summary/result/message. - Defined successful `AgentSessionEvent.message` as the canonical final user-facing reply (or `null`) and forwarded it on the terminal `done` event. - Added host, wake-renderer, normal-heartbeat, and terminal-reply regression coverage. ## Verification - `pnpm exec vitest run packages/adapter-utils/src/server-utils.test.ts server/src/__tests__/heartbeat-agent-session-message.test.ts server/src/__tests__/heartbeat-run-status-payload.test.ts server/src/__tests__/plugin-agent-sessions.test.ts server/src/__tests__/heartbeat-run-summary.test.ts` — 87 passed. - `pnpm -r typecheck` — passed across all 31 workspaces. - `pnpm build` — passed. - `pnpm test:run` — 2,860 passed, 1 skipped, 3 unrelated failures: two existing macOS temp-path alias assertions (`/tmp` vs `/private/tmp`) in workspace branch-containment tests and one reproducible auto-port runtime-service adoption failure. The same three failures reproduce when the two files run alone; none touch this change. - Live Slack verification intentionally remains operator-gated because it requires rebuilding/restarting the host. ## Risks - User-controlled chat text now reaches the model prompt, which is an intentional prompt-injection surface. The renderer labels it as untrusted conversational content, while the existing plugin/session company checks and caller authorization remain unchanged. - `finalText` is added to company-scoped heartbeat status events. fix(runtime): scope PAPERCLIP_ env-binding strip to reserved keys (#9974)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Agent runs get their environment from user/adapter/project/routine env bindings resolved by the server heartbeat, plus `PAPERCLIP_*` runtime vars (identity, wake, workspace, API access) injected by the harness > - The heartbeat stripped **every** `PAPERCLIP_`-prefixed binding before resolution, so legitimately user-named keys (e.g. cloud provider token bindings like `PAPERCLIP_CLOUD_PROD_PROVIDER_RAILWAY_*`) were silently dropped and never reached the run env > - At the same time, several adapters honored an explicitly configured `PAPERCLIP_API_KEY` over the harness-minted run token, which is exactly the one key config must never control > - This pull request replaces the blanket prefix strip with a precise three-rule policy: never accept `PAPERCLIP_API_KEY` from config, always let harness-assigned runtime vars win, and let every other `PAPERCLIP_*`-named user binding flow through > - The benefit is that user secrets with a `PAPERCLIP_`-style name work like any other binding, while runtime identity and API credentials stay fully harness-controlled ## Linked Issues or Issue Description **Bug description** (no public issue exists): - **What happened:** Env bindings whose key starts with `PAPERCLIP_` (e.g. a cloud provider token a user deliberately named `PAPERCLIP_CLOUD_PROD_PROVIDER_RAILWAY_TOKEN`) were silently stripped by the server before secret resolution, so the spawned agent never received them. No error, no access event — the variable just never appeared. - **Expected behavior:** A user-named `PAPERCLIP_*` binding should reach the run env unless the harness itself uses that key. Only `PAPERCLIP_API_KEY` should be categorically rejected, and harness-assigned runtime vars (`PAPERCLIP_RUN_ID`, `PAPERCLIP_AGENT_ID`, wake/workspace vars, …) should always win over config. - **Steps to reproduce:** Configure an agent/project env binding named `PAPERCLIP_<ANYTHING>` (plain or secret_ref), run a heartbeat, and inspect the spawned process env — the key is absent. - **Deployment mode:** local server, any local adapter. Related prior PRs (different, save-time/API-layer blanket-ban approach; this PR supersedes that direction with a runtime allow-except-reserved policy): Refs #8239, Refs #8439. ## What Changed - `server/src/services/heartbeat.ts`: the pre-resolution strip now removes only `PAPERCLIP_API_KEY` (hard denylist) instead of every `PAPERCLIP_`-prefixed binding; other `PAPERCLIP_*` keys flow into binding resolution. Low-trust inline-sensitive-env checks now also cover those keys. - `packages/adapter-utils/src/server-utils.ts`: new `isForbiddenConfigEnvKey()` helper; the shared `refreshPaperclipWorkspaceEnvForExecution` merge drops `PAPERCLIP_API_KEY` from config and keeps harness-assigned `PAPERCLIP_*` keys authoritative. - `packages/adapter-utils/src/acpx-engine/execute.ts`: removed the explicit-`PAPERCLIP_API_KEY`-from-config allowance; the run token (`authToken`) is now always applied; config `PAPERCLIP_API_KEY` is ignored. - All local adapters (`claude-local`, `codex-local`, `cursor-local`, `gemini-local`, `grok-local`, `opencode-local`, `pi-local`) plus `cursor-cloud`, `hermes`, and the server `process` adapter: removed `hasExplicitApiKey`-style allowances so the harness token always wins, and guarded the remaining unguarded env-merge loops (claude-local inline loop, process adapter) with the same policy. - Tests updated/added: heartbeat binding-strip test now asserts the three-rule policy; adapter-utils merge tests assert the `PAPERCLIP_API_KEY` ban and `PAPERCLIP_*` pass-through; acpx engine tests moved credential fixtures to `authToken` and assert config `PAPERCLIP_API_KEY` is ignored while other `PAPERCLIP_*` config keys forward and still bust the session fingerprint on rotation. ## Verification - `pnpm vitest run packages/adapter-utils/src/server-utils.test.ts packages/adapter-utils/src/acpx-engine/execute.test.ts` — 127 passed - `pnpm vitest run server/src/__tests__/heartbeat-project-env.test.ts server/src/__tests__/heartbeat-local-environment.test.ts server/src/__tests__/claude-local-execute.test.ts server/src/__tests__/codex-local-execute.test.ts server/src/__tests__/cursor-local-execute.test.ts server/src/__tests__/gemini-local-execute.test.ts` — 68 passed - Adapter package execute suites and the server tests touching API-key fixtures (`heartbeat-run-log`, `redaction`, `effective-run-config-fingerprints`, `agent-permissions-routes`) — green. Three pre-existing sandbox/SSH fixture failures reproduce identically on clean `master` on this host and are unrelated. - `pnpm --filter <pkg> typecheck` for server, adapter-utils, and all nine touched adapter packages — all pass. ## Risks - Behavioral change: a deployment that relied on configuring a static `PAPERCLIP_API_KEY` in adapter config env loses that override — by design; the harness-minted run token is now the only source. When no run token exists, no API key is injected at all. - `PAPERCLIP_*`-named user bindings now reach binding resolution and run envs; a key that collides with a harness runtime var is still discarded at merge time, so runtime identity/wake/workspace vars cannot be spoofed. - Low risk otherwise: no migrations, no API surface changes. ## Model Used - Claude Fable 5 (`claude-fable-5`, Anthropic Claude 5 family, Mythos-class tier), extended thinking enabled, agentic tool use (file edits, shell, test runner) via Claude Agent SDK. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge 🤖 Generated with [Claude Code](https://claude.com/claude-code) --------- Co-authored-by: Paperclip <noreply@paperclip.ing> Co-authored-by: Claude Fable 5 <noreply@anthropic.com> |

fix(recovery): route recovery by failure cause (#9634)
## Thinking Path > - Paperclip is the open source control plane people use to coordinate AI-agent companies and their work. > - Its recovery subsystem detects stranded issue execution and decides whether to retry, escalate, or request operator intervention. > - The existing recovery path used a mostly generic owner ladder and generic execution contract, so transient failures could wake a manager who then performed the deliverable instead of repairing and returning the task. > - Provider quota failures also entered the same takeover path even when the correct action was to wait for capacity and retry the original assignee. > - Recovery actions already retain the source owner and evidence needed to choose a cause-specific route, render a scoped contract, and measure whether work was handed back. > - This pull request adds a cause-keyed recovery playbook, propagates its contract through every built-in adapter, and makes resolved recovery actions return work to the original owner by default. > - The benefit is bounded self-recovery that preserves task ownership, avoids needless management takeover, and makes recovery outcomes observable. ## Linked Issues or Issue Description No matching public GitHub issue was found. Related recovery work was reviewed but is not duplicated here: #9630 restores bounded recovery continuations, #8807 changes one assignee-ranking case, and #9404 records runtime-failure transition evidence. This change instead introduces cause-specific routing and recovery contracts across the recovery lifecycle. ### What happened? When an issue became stranded, recovery generally selected an owner through the same fallback ladder and rendered the normal execution contract. That made the recovery wake look like ordinary deliverable work, even when the correct action was to retry the original agent, repair its runtime, or wait for a provider quota reset. ### Expected behavior Recovery should select a response by failure cause, tell the recipient to recover rather than complete the deliverable, suppress takeover wakes for provider quota waits, and return repaired work to its original assignee unless the recovery owner explicitly completes it. ### Actual behavior Recovery could escalate transient failures to management, omit the cause-specific next action from the wake, and leave the recovery owner assigned after the runtime problem was resolved. ### Impact The generic path creates avoidable management work, ownership churn, and budget consumption while obscuring whether recovery successfully returned work to the responsible agent. ## What Changed - Added cause-keyed routing for process loss, missing disposition, provider quota limits, Codex output inactivity, workspace validation failures, and fallback recovery causes. - Added recovery-scoped wake rendering that replaces the generic execution contract with the failure summary, original assignee, attempt count, next action, and cause-specific playbook instruction. - Propagated the structured recovery contract through all built-in adapter execution paths, including Hermes local and gateway adapters. - Added provider-quota wait monitoring so capacity failures schedule the original assignee instead of enqueueing a takeover wake. - Added hand-back behavior and `handed_back` / `owner_completed` outcome accounting when recovery actions are resolved. - Added focused routing, renderer, quota-monitor, and hand-back regression coverage plus implementation-spec documentation. ## Verification - `pnpm exec vitest run packages/adapter-utils/src/server-utils.test.ts server/src/__tests__/heartbeat-process-recovery.test.ts server/src/__tests__/heartbeat-workspace-branch-containment.test.ts server/src/__tests__/issue-recovery-actions.test.ts` - 4 test files passed; 194 tests passed. - Targeted `pnpm --filter ... typecheck` across `@paperclipai/adapter-utils`, `@paperclipai/shared`, `@paperclipai/server`, `@paperclipai/ui`, and all nine changed adapter packages. - 13 affected workspace packages passed typecheck. - `pnpm check:token-gates` - All UI token gates passed. ## Risks - Recovery routing behavior changes for stranded work, so an incorrectly classified cause could select a different recipient than before; fallback causes retain the existing management ladder. - Provider quota detection depends on structured failure evidence and conservative text matching; unmatched failures continue through fallback recovery. - Adapter prompt plumbing changes across built-ins, covered by shared renderer tests and compile-time call signatures. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. fix(adapters): forward resolved adapter env to local agents, keep runtime env authoritative (#9617)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Local coding adapters (Claude, Codex) run each agent heartbeat in a spawned child process; Paperclip resolves adapter-configured env — including `secret_ref` bindings — into the env for that process > - The resolved adapter env was not being forwarded reliably: config env could overwrite Paperclip's own runtime vars, and a warm/resumable ACP session could keep serving stale env because its fingerprint ignored the resolved env > - This mattered because a configured API key or other secret could be silently absent from the agent shell, and a resumed session would never pick up an updated value — while a config binding could also override runtime identity/wake vars > - This pull request keeps Paperclip-managed `PAPERCLIP_*` runtime env authoritative over config, and folds a stable hash of the applied adapter env into the session fingerprint so an env change forces a fresh launch > - The benefit is that adapter-configured env (plain values and resolved secrets) reliably reaches the agent process, updates are picked up on the next launch, and runtime identity can never be clobbered by config ## Linked Issues or Issue Description No public GitHub issue exists, so the underlying bug is described inline following the bug-report template. **What happened** Env keys configured on a local adapter (plain values and `secret_ref` bindings, resolved server-side into plain strings) did not reliably reach the spawned agent process. Two distinct gaps: (1) when merging config env into the process env, a config key in the reserved `PAPERCLIP_*` namespace could overwrite a Paperclip-managed runtime variable (identity, wake, workspace, API access); (2) a warm-handle / resumable ACP session computed its reuse fingerprint from `secretManifestHash` only, which misses plain-value edits and same-version secret rotations — so a resumed session kept serving stale env and never re-launched with updated values. **Expected behavior** Non-`PAPERCLIP_*` adapter env (plain + resolved secret values) is forwarded to the agent process; a change to any applied forwarded value invalidates a warm/resumable session so the next launch sources the latest env; configured `PAPERCLIP_*` entries can never override Paperclip runtime env, while an explicitly configured `PAPERCLIP_API_KEY` (stable per-run config) is still honored and its rotation also busts the session. **Steps to reproduce** Configure an adapter with an env key (e.g. a `secret_ref` API key) and a resumable ACP session. On resume, the updated env value is not sourced; separately, a `PAPERCLIP_*` config key overrides the runtime value. **Deployment mode** Local adapters (Claude / Codex) via the shared adapter-utils execution path. ## What Changed - `packages/adapter-utils/src/server-utils.ts`: add `isPaperclipRuntimeEnvKey` and, in `refreshPaperclipWorkspaceEnvForExecution` (used by all local adapters), skip a `PAPERCLIP_*` config key when Paperclip has already assigned it this run; all other keys still forward. - `packages/adapter-utils/src/acpx-engine/execute.ts`: apply the same `PAPERCLIP_*` non-override rule (via the shared helper) when merging config env, capture the applied config env in `resolvedAdapterEnv`, and fold a stable `adapterEnvHash` of it into the session fingerprint so an env change forces a fresh launch. Per-wake `PAPERCLIP_*` runtime vars are assigned earlier and never enter that map, so they stay out of the hash; stable configured `PAPERCLIP_*` values (e.g. an explicit `PAPERCLIP_API_KEY`) are included so rotating one busts the session. - Added unit/integration tests for plain + secret forwarding, `PAPERCLIP_*` non-override, the explicit-API-key path, fingerprint refresh-on-env-change vs. stable-across-wakes, and rotation of a configured `PAPERCLIP_API_KEY`. ## Verification - `npx vitest run packages/adapter-utils/src/acpx-engine/execute.test.ts packages/adapter-utils/src/server-utils.test.ts` → 2 files, 111 tests passing (includes the new cases). - Tests assert: forwarded plain/secret values appear in the spawned wrapper `.env`; a `PAPERCLIP_*` config key does not override the runtime value; changing an applied forwarded env value (including a rotated `PAPERCLIP_API_KEY`) changes `configFingerprint`, while a new wake with the same config env keeps it stable. ## Risks Low risk. Behavior change is limited to (a) config env no longer overriding `PAPERCLIP_*` runtime vars — a security-positive tightening — and (b) a resumable session re-launching when its applied config env changes, which is the intended fix. Per-wake `PAPERCLIP_*` churn is deliberately excluded from the fingerprint so normal sessions still resume across heartbeats. Existing sessions get a new fingerprint once on first deploy (the added `adapterEnvHash` field), which is expected. No secret values are logged (key-name redaction in the adapter plus manifest-driven redaction of `meta.env`). ## Model Used Claude Opus 4.8 (Anthropic), model id `claude-opus-4-8`, extended reasoning with tool use. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [ ] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(adapters): confine local coding processes (#9504)
## Thinking Path > - Paperclip is the open source control plane people use to manage AI agents for work. > - Local coding adapters currently spawn their CLI processes directly on the Paperclip host. > - CLI-native approval and sandbox flags do not provide a reliable host filesystem or network boundary. > - An agent can therefore inspect unrelated host files or fetch external material when an operator needs stronger isolation. > - The confinement must stay opt-in so existing local adapter behavior does not change unexpectedly. > - This pull request adds a shared Linux Bubblewrap spawn layer for workspace filesystem and deny/allowlist network scopes. > - The benefit is enforceable defense in depth around Codex and Claude local runs while preserving explicit provider connectivity. ## Linked Issues or Issue Description ### What happened? `codex_local` and `claude_local` processes could read arbitrary host paths and make unrestricted outbound network requests because Paperclip did not impose a spawn-level boundary. ### Expected behavior Operators can opt into a workspace-only filesystem view and either deny network egress or allow exact provider/API hosts, independently of CLI approval flags. ### Steps to reproduce 1. Run current `master` on Linux and configure a Codex or Claude local adapter. 2. Ask the agent to read a canary file outside its active workspace. 3. Ask the agent to `curl` a public host. 4. Observe that both operations succeed without a Paperclip-level confinement option. ### Environment - Paperclip commit: `c36f1a4af` / current `master` base. - Deployment mode: Linux local dev or self-hosted server. - Installation: built from source. - Adapters: Codex and Claude Code. - Database: not related. ## What Changed - Added a shared Bubblewrap process wrapper with opt-in `filesystemScope: "workspace"`, managed/extra path mounts, private `/tmp`, and Linux-only validation. - Added `networkScope: "deny" | "allowlist"`; both use a private network namespace, while allowlist mode exposes an exact-host HTTP(S) proxy over a Unix-socket bridge. - Wired Codex and Claude local CLI execution through the wrapper and forced scoped auto runs onto the CLI lane because ACP processes are not covered. - Added unit and gated Bubblewrap canaries for outside-file denial, workspace writes, direct network denial, allowlisted forwarding, and rejected destinations. - Documented both scopes, provider allowlist examples, Bubblewrap requirements, and default-off behavior. ## Verification - `pnpm exec vitest run packages/adapter-utils/src/local-process-sandbox.test.ts packages/adapters/codex-local/src/server/acp.test.ts packages/adapters/claude-local/src/server/acp.test.ts` — 34 passed, 4 gated Bubblewrap tests skipped by default. - `pnpm --filter @paperclipai/adapter-utils typecheck` - `pnpm --filter @paperclipai/adapter-codex-local typecheck` - `pnpm --filter @paperclipai/adapter-claude-local typecheck` - `pnpm --filter @paperclipai/adapter-utils build` - `pnpm --filter @paperclipai/adapter-codex-local build` - `pnpm --filter @paperclipai/adapter-claude-local build` - Attempted the gated tests with a vendored Bubblewrap binary; this container blocks unprivileged namespace setup (`setting up uid map: Permission denied` / loopback `RTM_NEWADDR: Operation not permitted`), so kernel-level execution remains for CI or a namespace-enabled Linux host. ## Risks - Bubblewrap must be installed and unprivileged user/mount/network namespaces must be enabled on the host; scoped runs fail clearly if the prerequisite is missing. - Allowlist mode depends on the coding CLI honoring standard `HTTP_PROXY` / `HTTPS_PROXY` variables; custom providers must list every required exact hostname and port. - Exact-host allowlists intentionally reject wildcards, which is safer but may require operators to enumerate multi-host provider setups. - No behavior changes unless an operator enables `filesystemScope` or `networkScope`. > This aligns with the ROADMAP direction toward safer remote and sandboxed agent environments and does not duplicate an open PR or issue found in the repository search. ## Model Used - OpenAI GPT-5.5 (`gpt-5.5`) via Codex CLI, with reasoning, repository tool use, shell execution, code editing, and test execution. The serving context-window size is not exposed to the agent. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

fix(adapters): inject execution contract once per fresh heartbeat (#9469)
## Thinking Path > - Paperclip coordinates AI-agent work through repeated heartbeat runs. > - Adapter prompts combine a default heartbeat template with scoped wake context. > - Fresh heartbeats received the same execution contract from both layers, wasting prompt tokens and obscuring which layer owns the contract. > - Resume deltas and template-less adapters do not share that composition path, so removing the wake-payload copy unconditionally would drop required guidance. > - Empty comment batches also emitted instructions and metadata that only matter when comments exist. > - This pull request makes execution-contract inclusion explicit by prompt path, preserves OpenClaw gateway behavior, and suppresses no-op comment boilerplate. > - The benefit is one contract per heartbeat path and roughly 300 fewer prompt tokens on a fresh zero-comment wake. ## Linked Issues or Issue Description - Fixes #9221 - Refs #9200 - Refs #7634 ## What Changed - Stop emitting the execution-contract paragraph from fresh scoped wake payloads because the default heartbeat template already contains the full contract. - Keep the contract in resume deltas, and add `includeExecutionContract` for adapters that do not render the default heartbeat template. - Opt `openclaw-gateway` into wake-payload contract rendering so template-less gateway runs retain the guidance. - Omit comment-batch acknowledgement/fetch guidance and empty `pending comments` / `latest comment id` metadata when a fresh wake has no pending comments. - Add regression and acceptance coverage proving composed fresh prompts contain `Execution contract` exactly once while resume and template-less paths retain it. Measured effect: the fresh zero-comment wake block drops from 1,840 to 855 characters (about 300 tokens saved per fresh heartbeat; about 220 on comment wakes), and the composed fresh prompt contains `Execution contract` once instead of twice. ## Verification - `npx vitest run packages/adapter-utils/src/server-utils.test.ts` — 63 passed - `npx vitest run server/src/__tests__/codex-local-execute.test.ts` — 13 passed - `npx vitest run server/src/__tests__/heartbeat-comment-wake-batching.test.ts server/src/__tests__/openclaw-gateway-adapter.test.ts server/src/__tests__/low-trust-red-team-routes.test.ts` — 27 passed - `pnpm --filter @paperclipai/adapter-utils typecheck` — passed - `pnpm --filter @paperclipai/adapter-openclaw-gateway typecheck` — passed ## Risks - Low risk: prompt text and adapter composition only; no database or API migration. - The main compatibility risk is a template-less adapter losing the contract. The explicit option and OpenClaw gateway regression coverage protect the known template-less path. - External adapters that call `renderPaperclipWakePrompt` directly can opt into `includeExecutionContract: true` when they do not render the default template. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. fix(adapter-utils): runChildProcess escalates to SIGKILL on liveness, not child.killed (#8598)
## Thinking Path
> - Paperclip is the open source app people use to manage AI agents for
work
> - Those agents run as child processes spawned by
`@paperclipai/adapter-utils`'s `runChildProcess`, which arms a
parent-side wall-clock timer at `timeoutSec` to bound a hung run
> - At the deadline `runChildProcess` sends SIGTERM, then after a grace
window escalates to SIGKILL — the SIGKILL backstop is what turns a
wedged child into a dead PID so the scheduler can reclaim and retry it
> - On the **direct-child fallback** path (`signalRunningProcess`, used
on win32 and whenever process-group signaling is unavailable or throws)
the escalation was gated on `!child.killed`
> - But Node sets `ChildProcess.killed` to `true` the instant a signal
is *successfully sent*, not when the process exits — so once the earlier
SIGTERM has been sent, `child.killed` is already `true`, the
`!child.killed` guard is `false`, and the SIGKILL escalation never runs
> - A child that ignores SIGTERM (e.g. a graceful-shutdown handler
wedged on a socket) is therefore never force-killed, outlives its
deadline, and for an unattended scheduler sits running forever with no
terminal state
> - This PR gates the fallback escalation on real liveness (`exitCode
=== null && signalCode === null`), so SIGKILL fires precisely while the
child is still alive
> - The benefit is the hard timeout actually guarantees termination
(except true uninterruptible D-state) on every platform/configuration,
not just where the process-group path is available
## Linked Issues or Issue Description
No existing public issue — describing the bug inline, following
`.github/ISSUE_TEMPLATE/bug_report.yml`:
### What happened?
When `@paperclipai/adapter-utils`'s `runChildProcess` reaches
`timeoutSec` and the spawned child ignores SIGTERM, the SIGKILL
escalation on the **direct-child fallback** path
(`signalRunningProcess`, taken on win32 or whenever `process.kill(-pgid,
…)` is unavailable or throws) never fires, so the child outlives its
deadline indefinitely. Root cause: the escalation is gated on
`!running.child.killed`, and `ChildProcess.killed` reflects only that a
signal was *successfully sent* (per the Node docs it "does not indicate
that the child process has been terminated"). After the deadline
SIGTERM, `child.killed` is already `true`, so `!child.killed` is `false`
and the follow-up SIGKILL is suppressed.
### Expected behavior
After the grace window, a child that is still alive is force-killed with
SIGKILL regardless of whether SIGTERM was already sent — the hard
timeout should guarantee termination (except true uninterruptible
D-state) on every platform/configuration.
### Steps to reproduce
1. Spawn a child that installs a no-op `SIGTERM` handler and never exits
(e.g. `process.on('SIGTERM', () => {}); setInterval(() => {}, 1000)`).
2. Drive it through the direct-child fallback, i.e.
`signalRunningProcess({ child, processGroupId: null }, …)` (the path
used on win32 / when group signaling is unavailable).
3. Send SIGTERM (the child swallows it; `child.killed` becomes `true`),
then send SIGKILL.
4. On the pre-fix `!child.killed` guard the SIGKILL call is a no-op and
the PID survives past its deadline. Covered by the new regression test
in this PR.
### Paperclip version or commit
Reproduces on `master` (the `signalRunningProcess` fallback). Also
present in published `@paperclipai/adapter-utils` (e.g. `2026.325.0`),
where the same `!child.killed` guard sits on the single direct-child
escalation path.
_Searched the open PR list for duplicates/related work on
`runChildProcess` / `signalRunningProcess` / SIGKILL escalation; found
none._
## What Changed
- `packages/adapter-utils/src/server-utils.ts`: in
`signalRunningProcess`, replace the direct-child fallback guard
`!running.child.killed` with `running.child.exitCode === null &&
running.child.signalCode === null` (real liveness). The process-group
path is unchanged.
- `packages/adapter-utils/src/server-utils.ts`: `export`
`signalRunningProcess` so the fallback branch can be unit-tested
directly.
- `packages/adapter-utils/src/server-utils.test.ts`: add a companion
regression test (POSIX-only, like the sibling timeout tests) that forces
the fallback (`processGroupId: null`) — sends SIGTERM (child swallows
it, `child.killed` becomes `true`), asserts the child is still alive,
then sends SIGKILL and asserts the PID dies. Also keeps the end-to-end
`runChildProcess` SIGTERM-ignoring test.
## Verification
```
npx vitest run packages/adapter-utils/src/server-utils.test.ts # 52 passed
npx tsc --noEmit # clean
```
- **Regression proof:** reverting the guard to `!running.child.killed`
makes the new fallback test fail (`waitForPidExit` → false; the child
survives); the liveness guard makes it pass. This addresses the prior
review note that the existing test only exercised the process-group path
(which already escalated correctly on POSIX) and never reached the
changed branch.
## Risks
Low. A one-line guard change scoped to the direct-child fallback; the
process-group path is untouched. SIGKILL is only sent when
`exitCode`/`signalCode` are both still `null`, i.e. the process is
provably alive, so the change cannot signal an already-reaped/recycled
PID. New tests are POSIX-only and `skipIf(win32)`, consistent with the
sibling timeout tests in this file.
## Model Used
Anthropic **Claude Opus 4.8**, driven via the Cursor agent (extended
reasoning + tool use, large context). Diff, tests, and the regression
proof above were produced and run by the agent; reviewed by a human
before pushing.

Enforce durable external-wait liveness (#9373)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - The heartbeat/recovery subsystem decides whether an agent run has a durable continuation path after the process stops. > - External waits need stricter semantics than local background watchers: a killed local process is not durable, while a first-class blocker/monitor/scheduled wake is. > - Without that distinction, recovery can repeatedly treat adapter-failed continuations as live work and obscure the real reason a task stopped. > - This pull request adds explicit durable external-wait liveness handling and documents the expected execution semantics. > - It also improves operator-visible recovery evidence so invalid external-wait paths explain why they were rejected. > - The benefit is clearer recovery behavior, fewer duplicate continuation recoveries, and a safer contract for monitor-backed external waits. ## Linked Issues or Issue Description - Refs #5978 - Related PRs: #4988, #7495, #8502 ## What Changed - Added durable external-wait liveness classification so local/background watchers are not accepted as durable live paths after the owning process exits. - Preserved first-class blocker/monitor/scheduled wake paths as valid external-wait continuations. - Added backend regression coverage for killed watcher failure, monitor-backed durable wait resumption, normal completion, blocker behavior, and no duplicate recovery. - Added adapter utility coverage for terminal cleanup behavior used by local process adapters. - Surfaced invalid external-wait recovery evidence in the recovery action card and run ledger. - Updated execution semantics documentation and the V1 implementation contract. ## Verification - `pnpm check:token-gates` passed. - `pnpm -r typecheck` passed. - `node scripts/run-vitest-stable.mjs --mode general --group general-server` equivalent lane passed in CI-clean env: 238 files, 2164 tests passed, 1 skipped. - `node scripts/run-vitest-stable.mjs --mode general --group general-workspaces-a` passed in fully Paperclip-env-clean env: UI 305 files / 2430 tests; CLI 43 files / 230 tests. - `node scripts/run-vitest-stable.mjs --mode general --group general-workspaces-b` passed in fully Paperclip-env-clean env: shared/db/adapters/plugin packages all green. - `node scripts/run-vitest-stable.mjs --mode serialized` passed in fully Paperclip-env-clean env: 107 serialized server suites green, including 84/84 heartbeat-process-recovery tests. - `pnpm build` passed in fully Paperclip-env-clean env. Notes: running `pnpm test:run` directly inside the Paperclip heartbeat environment exposed local harness env contamination in existing tests (`PAPERCLIP_CONFIG`, `PAPERCLIP_DB_BACKUP_DIR`, and `PAPERCLIP_WORKTREE_START_POINT`). Re-running the same lanes with inherited `PAPERCLIP_*` and port env removed produced the CI-equivalent green results above. ## Risks - Medium behavioral risk: this changes recovery classification for stopped local external-wait processes, so adapters relying on unmanaged background watchers must use blockers, monitors, scheduled wakes, or explicit durable handoff instead. - Low UI risk: recovery-card copy changes are covered by component tests and Storybook screenshot QA. - No database migration is included. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI Codex, GPT-5-based coding agent, tool-enabled terminal/code execution. Exact context-window metadata was not exposed in the runtime. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> Co-authored-by: Claude Opus 4.8 <noreply@anthropic.com> |

fix(prompt): render exact branch name with backtick-safe fence in wake branch guard (#9326)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Agents run heartbeats inside execution workspaces pinned to a specific git branch; the wake prompt now carries a one-time "stay on this branch" guard (#9319) > - Greptile's final review round on #9319 landed after the PR merged: the guard sanitized the branch name by stripping backticks, which mutates the ref, so the prompt could tell the agent to stay on a branch name that does not exist > - A guard that names the wrong branch defeats its purpose and still leaves the workspace contract breakable > - This pull request keeps the pinned ref name exact and instead escapes it at render time with a backtick fence longer than any backtick run inside the name (standard Markdown inline-code escaping) > - The benefit is the guard always names the real branch while a hostile ref name still cannot close the code span or inject prompt text ## Linked Issues or Issue Description Refs #9319 — follow-up addressing the final Greptile review round that arrived after that PR merged. ## What Changed - `normalizePaperclipWakeExecutionWorkspace` no longer strips backticks from the branch name; it removes only control characters (illegal in git ref names, and the newline route into the prompt), trims, and caps length. - Added a `markdownInlineCode` helper that wraps a value in an inline-code span whose backtick fence is one longer than the longest backtick run in the value, and used it when rendering the branch guard line. - Updated the hostile-branch-name test to assert the exact ref is preserved and fenced, and control characters are removed. ## Verification - `pnpm vitest run packages/adapter-utils/src/server-utils.test.ts` — 58 tests pass, including the updated hostile-branch-name case. - `npx tsc --noEmit` in `packages/adapter-utils` — clean. - Manual: render a wake payload with `branchName: "evil` + backtick + `name"` and confirm the guard line reads ``` `` evil`name `` ``` and the span does not break. ## Risks - Low risk: prompt-rendering-only change; the normalized payload shape is unchanged. Branch names containing backticks (extremely rare) now render exactly instead of mutated. ## Model Used - Claude Fable 5 (`claude-fable-5`, Anthropic) with extended thinking and tool use, via Claude Code / Paperclip agent harness. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge 🤖 Generated with [Claude Code](https://claude.com/claude-code) Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(prompt): one-time execution-workspace branch guard in wake prompt (PAP-13326) (#9319)
## Summary
When a task runs in a branch-pinned execution workspace, agents
sometimes switch or rename the workspace branch, which breaks the
worktree contract. This adds a short, one-time prompt hint telling the
agent to stay on the pinned branch.
- **heartbeat.ts**: after the execution workspace is resolved, attach
`executionWorkspace: { branchName }` to the wake payload (only when a
branch pin exists — agent-home runs without a branch are untouched).
- **server-utils.ts (adapter-utils)**: normalize the new payload field
and render one bullet in `renderPaperclipWakePrompt`:
> `- execution workspace branch: you are running in an execution
workspace on branch \`<name>\`. Do not switch, rename, or re-point this
branch; keep all commits on it.`
- The hint renders **only on non-resumed sessions** — resume-delta
prompts skip it, so it appears the first time an issue's session starts,
not on every turn, and it never pollutes the issue thread. One renderer
change covers every adapter (claude, codex, cursor, gemini, grok,
opencode, pi, hermes, acpx engine) with zero per-adapter edits.
## Tests
- `server-utils.test.ts`: branch guard renders on first prompt, absent
on resumed-session prompts, absent when no branch is pinned; payload
round-trips through `stringifyPaperclipWakePayload`.
- `heartbeat-workspace-branch-containment.test.ts`: the finalize-path
adapter mock now asserts the wake payload the adapter receives carries
the branch pin matching `context.paperclipWorkspace.branchName`
(end-to-end heartbeat wiring, embedded postgres). All 6 pass.
- Full `server-utils` (56) and acpx-engine execute (34) suites green;
adapter-utils typechecks clean; no new server tsc errors.

[codex] Include checkbox selections in continuation wakes (#8893)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Issue-thread interactions are the subsystem that lets board users answer structured prompts and resume agent work > - Checkbox confirmations capture a selected subset of known options, then wake the assignee through continuation context > - The wake context previously carried generic interaction metadata, but not the accepted checkbox option ids or option labels > - That meant the resumed agent could be woken after a checkbox confirmation without seeing the board's selected options in the turn context > - This pull request carries accepted checkbox selections through the interaction continuation wake snapshot and renders them into the adapter wake prompt > - The benefit is that agents can act on checkbox-confirmation selections without refetching or guessing the user's choices ## Linked Issues or Issue Description No public GitHub issue exists for this bug. Searched public issues and PRs for checkbox confirmation / continuation selection duplicates and found no matching issue or PR. ### Pre-submission checklist - [x] I have searched existing open and closed issues and this is not a duplicate. - [x] I am on the latest released version of Paperclip or can reproduce on `master`. - [x] I have confirmed the error originates in Paperclip itself, not in my agent adapter, API provider, or local configuration. ### What happened? When a board user accepted a `request_checkbox_confirmation` interaction, the assignee continuation wake included generic interaction metadata but did not include the accepted checkbox selections. The resumed agent turn therefore had no in-prompt access to the selected option ids or option labels/descriptions. ### Expected behavior When a `request_checkbox_confirmation` interaction is accepted, the resumed agent wake should include the checkbox prompt, selected option ids, and selected option labels/descriptions so the agent can act on the selected subset directly. ### Steps to reproduce 1. Create an issue-thread `request_checkbox_confirmation` interaction with multiple options and `continuationPolicy: "wake_assignee"`. 2. Accept the interaction with one or more selected options. 3. Inspect the continuation wake payload/prompt received by the assignee. 4. Observe that the selected checkbox options are missing from the wake context before this fix. ### Paperclip version or commit Reproduced against the pre-fix code path on `master`; this PR head is `9d17e70bce373e4850117f30c015c973c4b61789`. ### Deployment mode Local dev (pnpm dev) / built from source. ### Installation method Built from source (pnpm dev / pnpm build). ### Agent adapter(s) involved Not adapter-specific (core bug). The Codex/local adapter path exposed the missing wake context, but the missing field was in core interaction continuation payload construction. ### Database mode Embedded PGlite or external Postgres; the bug is not database-mode specific. ### Access context Both. Board users resolve the checkbox interaction, and agent bearer-key wakes consume the continuation context. ### Node.js version `v22.22.2` ### Operating system Linux workspace. ### Relevant logs or output No runtime exception is required to reproduce this. The failure mode is missing `checkboxSelection` data in the resolved interaction continuation wake payload. ### Relevant config Not config-related. ### Additional context Root cause: accepted checkbox interaction results were not extracted into the continuation wake context, and adapter wake payload normalization/rendering had no typed `checkboxSelection` field. ### Privacy checklist - [x] I have reviewed all pasted output for PII (usernames, file paths, API keys, tokens, company names) and redacted where necessary. ## What Changed - Added checkbox selection extraction for accepted `request_checkbox_confirmation` interactions and stored it in interaction continuation wake context. - Included checkbox selection context in heartbeat wake payload construction. - Added adapter-utils normalization and wake prompt rendering for checkbox prompt, selected ids, and selected option details. - Added regression coverage for route continuation context, heartbeat payload summaries, and adapter wake prompt rendering. ## Verification - `pnpm exec vitest run packages/adapter-utils/src/server-utils.test.ts server/src/__tests__/heartbeat-context-summary.test.ts server/src/__tests__/issue-thread-interaction-routes.test.ts` - `git diff --check origin/master...HEAD` - `rg -n "checkbox|confirmation|interaction|wake|continuation" ROADMAP.md` - `gh pr list --state all --search "checkbox continuation selection repo:paperclipai/paperclip" --json number, title, state, url, headRefName --limit 20` - `gh issue list --state all --search "checkbox confirmation options repo:paperclipai/paperclip" --json number, title, state, url --limit 20` ## Risks Low risk. The new payload field is additive, only populated for accepted checkbox confirmations, and existing continuation fields are preserved. The main compatibility risk is downstream code assuming an exact wake payload shape; adapter normalization treats the new field as optional. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used OpenAI Codex coding agent based on GPT-5, with shell/tool execution in this workspace.

[codex] Preserve plan review context in agent wakes (#8649)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Planning work relies on issue documents, request-confirmation interactions, and inline plan annotations > - Agents can be woken after a plan comment, annotation, or confirmation decision > - The wake payload needs enough plan-review context for the agent to act on the specific feedback instead of losing the thread and falling back to broad refetches > - This pull request adds bounded plan-review context to wake payloads and heartbeat context > - It also teaches the adapter wake prompt renderer to surface those open plan annotations and interaction results directly > - The benefit is that agents can continue plan review and plan acceptance flows with the relevant comments in hand while keeping wake payloads bounded and company-scoped ## Linked Issues or Issue Description No matching public GitHub issue was found. ### Subsystem affected Cross-cutting: `server/`, `packages/shared`, and `packages/adapter-utils`. ### Problem or motivation Plan-review continuations can wake an agent after a plan comment, inline annotation, or request-confirmation decision without enough inline context about the open plan annotations or accepted/rejected confirmation target. That makes scoped wakes less reliable because the agent may need to refetch broad issue history before it can tell what feedback should be incorporated. ### Proposed solution Include bounded, company-scoped plan review context in wake payloads and heartbeat context. The context includes open `plan` annotation threads, recent annotation comments, truncation metadata, and plan-confirmation interaction target/result details. Render that information in the adapter wake prompt so agents see the relevant plan-review feedback immediately. ### Alternatives considered Relying on agents to fetch the full issue thread after every plan-review wake was rejected because it is slower, harder to audit, and easier to mishandle when the wake is meant to be scoped to a specific comment, annotation, or interaction result. ### Roadmap alignment This supports the roadmap areas for Agent Reviews and Approvals, Deep Planning, and Enforced Outcomes by making plan approval continuations explicit and actionable. ### Additional context The implementation keeps payload size bounded with per-thread, per-comment, and total-body limits. Resolved annotation threads are intentionally omitted so the wake focuses on feedback still needing action. ## What Changed - Added shared `PlanReviewContext` types for plan annotation threads, comments, interaction targets, and continuation results. - Added server-side plan review context assembly for open `plan` annotation threads with bounded thread/comment/body limits. - Included plan review context in heartbeat context and scoped wake payloads for planning, annotation, comment, and plan-confirmation interaction wakes. - Rendered plan annotation deltas, open plan comments, interaction results, and accepted target revisions in adapter wake prompts. - Added focused regression coverage for scoped plan review context, wake prompt rendering, annotation filtering, and safe standard-mode annotation wakes. - Addressed Greptile feedback by bounding the plan-comment DB fetch and removing unused plan review context input fields. ## Verification - `pnpm run preflight:workspace-links` - `pnpm exec vitest run --project @paperclipai/adapter-utils packages/adapter-utils/src/server-utils.test.ts` - `pnpm exec vitest run --project @paperclipai/server --no-file-parallelism --maxWorkers=1 server/src/__tests__/document-annotations-service.test.ts server/src/__tests__/issue-thread-interaction-routes.test.ts server/src/__tests__/issues-goal-context-routes.test.ts` - `pnpm --filter @paperclipai/shared typecheck` - `pnpm --filter @paperclipai/adapter-utils typecheck` - `pnpm --filter @paperclipai/server typecheck` - `git diff --check public-gh/master...HEAD` - GitHub PR checks are green on `36d0ac6a5dce27b9d62e201bf6d9829170c5974e`n- Rebased onto current `paperclipai/paperclip:master` and confirmed GitHub reports the PR as mergeable - Greptile Review completed successfully after 2 comments were addressed and resolved; 0 unresolved review threads remain ## Risks - Medium: wake payloads now include additional plan-review data, so limits and truncation behavior need to stay conservative as annotation volume grows. - Low migration risk: no database schema or migration changes. - Low repository hygiene risk: this PR does not touch `pnpm-lock.yaml`, `.github/workflows`, or media assets. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used OpenAI Codex using `gpt-5` as a coding agent with shell/tool execution. Reasoning mode and exact context window were not exposed by the runtime. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] All Paperclip CI gates are green - [x] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

feat(skills): remove bundled paperclip-dev skill and retire required skill attribute (#7029)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work > - Local adapters (Claude, Codex, Cursor, Gemini, Grok, OpenCode, Pi, ACPX) ship bundled "skills" — opinionated Markdown prompt bundles materialized into the agent's runtime > - One of those bundled skills, `paperclip-dev`, existed to let agents develop Paperclip itself; it has now moved to its own external repo and no longer belongs in the core tree > - The adapter skill model also carried a `required` / `requiredReason` attribute plus a `paperclip_required` `AdapterSkillOrigin` variant, all of which only existed to mark bundled skills as non-optional in the UI and adapter sync logic > - With `paperclip-dev` gone, no bundled skill is "required" anymore, and the type / runtime surface for `required` is dead weight — but it is computed at request time and never persisted, so a clean removal is safe (no compatibility shim needed) > - This pull request deletes `skills/paperclip-dev/` and removes every trace of the `required` / `requiredReason` field and the `paperclip_required` origin across shared types, validators, adapter-utils, all eight local adapters, server routes, the company-skills service, the UI, the storybook fixtures, and the test suite > - The benefit is a smaller, simpler adapter-skill surface: one origin (`company_managed`) for managed bundled skills, `resolvePaperclipDesiredSkillNames` collapses to "just the configured desired set", and the AgentDetail skills tab no longer renders a "Required by Paperclip" section that no longer applies ## Linked Issues or Issue Description <!-- No existing public GitHub issue; describing the underlying work inline (feature_request template fields). --> **Summary** Remove the bundled `paperclip-dev` skill (now maintained in its own external repo) and retire the `required` / `requiredReason` skill attribute and the `paperclip_required` skill origin, which only existed to support it. **Problem or motivation** `paperclip-dev` is the only bundled skill that was ever marked "required". Now that it lives in a separate repository, shipping it inside the core tree is wrong, and the entire `required` surface (a type field, a validator field, a synthesized `paperclip_required` origin, UI "Required by Paperclip" section, and required-skill merging in the desired-skills calculation) becomes dead weight. The `required` value is computed at request time and never persisted, so it can be removed cleanly without a migration or compatibility shim. **Proposed solution** Delete `skills/paperclip-dev/`, drop the `required` / `requiredReason` fields and `paperclip_required` origin everywhere they are produced or consumed, collapse managed-skill origin to a single `company_managed` value, and simplify `resolvePaperclipDesiredSkillNames` to return only the configured desired set. **Alternatives considered** Keeping the `required` attribute as a no-op for forward compatibility — rejected because it is request-time only (nothing persists it), so leaving it in place is pure dead surface area with no callers. **Roadmap alignment** Internal cleanup / dead-code removal that simplifies the adapter-skill surface; it does not introduce or duplicate any planned core feature in ROADMAP.md. ## What Changed - Deleted bundled `skills/paperclip-dev/` (moved to a separate repo). - Dropped `required`, `requiredReason`, and the `paperclip_required` origin from `packages/shared/src/types/adapter-skills.ts`, `packages/shared/src/validators/adapter-skills.ts`, and `packages/adapter-utils/src/types.ts`. - In `packages/adapter-utils/src/server-utils.ts`: removed `readSkillRequired()`; dropped `required`/`requiredReason` from `listPaperclipSkillEntries()`, `normalizeConfiguredPaperclipRuntimeSkills()`, `buildPersistentSkillSnapshot()`, and `PaperclipSkillEntry`; collapsed `buildManagedSkillOrigin()` to always return `company_managed`; simplified `resolvePaperclipDesiredSkillNames()` to return only the configured desired set (signature preserved so adapter call sites are untouched). - Walked all eight local adapters (`acpx-local`, `claude-local`, `codex-local`, `cursor-local`, `gemini-local`, `grok-local`, `opencode-local`, `pi-local`) and removed every remaining `requiredReason` / `paperclip_required` reference. - `server/src/services/company-skills.ts`: dropped the `required = sourceKind === "paperclip_bundled"` synthesis when listing runtime skill entries. - `server/src/routes/agents.ts`: removed required-skill merging from the desired-skills calculation in the persist-config path and the unsupported-snapshot path (keeping the current version-aware `desiredSkillEntries` structure). - `ui/src/pages/AgentDetail.tsx`: dropped required-based filters, the required tooltip, and the entire "Required by Paperclip" section from the agent skills tab; storybook fixtures in `ui/storybook/stories/acpx-local.stories.tsx` cleaned up to match. - Tests: deleted the `required: false` case in `paperclip-skill-utils.test.ts` and the "keeps required bundled skills installed" case in every `*-local-skill-sync.test.ts`; `acpx-local-execute.test.ts`, `cursor-local-execute.test.ts`, `cursor-local-skill-sync.test.ts`, `agent-skills-routes.test.ts`, and `packages/adapter-utils/src/server-utils.test.ts` were updated to drop removed fields and map `origin: "paperclip_required"` → `"company_managed"`. - `server/src/adapters/registry.ts`: two `as unknown as ServerAdapterModule["..."]` casts on `hermesListSkills` / `hermesSyncSkills` (matching the existing `executeHermesLocal` pattern). `hermes-paperclip-adapter@0.2.0` still depends on the published `@paperclipai/adapter-utils` which keeps the retired `paperclip_required` variant; the cast bridges the workspace-vs-published type mismatch at the registry seam and can drop once hermes upgrades. ## Verification Run from the workspace root: ```sh grep -rn "skills/paperclip-dev" . grep -rn "paperclip_required" --include="*.ts" --include="*.tsx" . grep -rn "requiredReason" --include="*.ts" --include="*.tsx" . pnpm -w typecheck pnpm --filter @paperclipai/server exec vitest run paperclip-skill-utils pnpm --filter @paperclipai/server exec vitest run skill-sync ``` The first three greps return only the explanatory comment in `server/src/adapters/registry.ts` (no live `paperclip_required` / `requiredReason` usage) and zero `skills/paperclip-dev` source hits. Locally: - `pnpm -w typecheck` → all packages this PR touches pass (adapter-utils, shared, server, ui, cli, and the cursor/gemini/opencode/pi adapters). - Affected vitest suites pass: `paperclip-skill-utils`, `server-utils`, all eight `*-local-skill-sync`, `agent-skills-routes`, and the `acpx`/`cursor`/`pi` execute suites. ## Risks - Behavioral shift in the agent skills UI: the "Required by Paperclip" section disappears. No bundled skill is required anymore, so this only affects environments that previously surfaced `paperclip-dev` as a forced-on row; those installs will see the skill move into the regular "company-managed" list (and be uninstalled on next sync unless explicitly listed as desired). - Existing agents may still have the string `"paperclip-dev"` in their persisted `desiredSkills`. That entry is inert (no source for it to install from); a one-time DB cleanup is out of scope. Low risk. - Hermes adapter type bridge: two casts in `registry.ts` paper over a type-only divergence between the workspace `@paperclipai/adapter-utils` and the published version still pinned by `hermes-paperclip-adapter@0.2.0`. Runtime behavior is unaffected because the retired `paperclip_required` value is no longer produced by anything in this tree. The casts can be removed once hermes upgrades its dependency. ## Model Used - Provider: Anthropic - Model: Claude Opus 4.7 (`claude-opus-4-7`) - Capability: agent tool use via Paperclip's `claude_local` adapter ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have not referenced internal/instance-local Paperclip issues or links (only public GitHub `#NNN` / `github.com/paperclipai/paperclip` URLs) - [ ] My branch name describes the change (e.g. `docs/...`, `fix/...`) and contains no internal Paperclip ticket id or instance-derived details - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge Co-authored-by: Paperclip <noreply@paperclip.ing> |

[codex] feat(watchdog): add task watchdog control plane (#8339)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - The task lifecycle and recovery subsystems decide when agent work is still productive, stalled, or ready for review. > - Existing recovery paths can observe stopped or incomplete work, but there was no first-class per-task watchdog model with scoped review permissions. > - Watchdog follow-ups also need strict boundaries so recovery/status-only runs cannot mutate approvals or perform deliverable work. > - This pull request adds the task watchdog data model, API/service layer, scheduler/review flow, adapter wake context, UI configuration surfaces, and docs. > - The branch has been rebased onto current `paperclipai/paperclip` `master`; the watchdog migration is now ordered after master's latest migrations as `0104_issue_watchdogs`. > - The benefit is a more explicit task-review loop that preserves Paperclip's single-assignee and governance invariants while making stalled work easier to route. ## Linked Issues or Issue Description No linked GitHub issue. Paperclip task: [PAP-11275](/PAP/issues/PAP-11275). ## Problem or motivation Task recovery needs a first-class watchdog path that can inspect stopped work and create scoped follow-ups without bypassing normal task ownership. Board/UI users need a way to configure watchdogs on tasks and see watchdog-related live work. Recovery/status-only runs must remain limited to status reporting and must not create approvals, link approvals, or submit approval comments. ## Proposed solution Add a task-watchdog data model, scheduler/classifier, scoped mutation guard, adapter wake context, API/UI configuration surfaces, and documentation so watchdog agents can review stopped task subtrees under explicit boundaries. ## Alternatives considered Reuse the existing recovery-action flow only. That would keep stopped-work detection implicit, make per-task watchdog assignment harder to expose in the UI, and would not provide a durable scoped-review issue for stalled task trees. ## Roadmap alignment This is Paperclip control-plane lifecycle infrastructure for task execution and recovery. I checked `ROADMAP.md`; this PR does not duplicate an existing planned core item. ## What Changed - Added issue watchdog schema, migration, shared contracts, validators, CRUD API, and service support. - Added task watchdog scheduler/classifier behavior, scoped mutation enforcement, adapter wake context, and default watchdog mandate guidance. - Added UI surfaces for configuring watchdogs on new/existing tasks, viewing watchdog activity, and exposing the experimental setting. - Added docs for the user-facing task watchdog workflow and implementation semantics. - Gated new-task watchdog setup behind `enableTaskWatchdogs` and blocked cheap status-only recovery runs from approval mutations. - Rebased onto current `master` and renumbered the idempotent watchdog migration from the branch-local `0102_issue_watchdogs` slot to `0104_issue_watchdogs`. - Addressed Greptile feedback by loading watchdog classifier input with a recursive subtree query and centralizing the watchdog origin-kind constant. - Added and updated focused server/UI tests for watchdog routes, scheduler/classifier behavior, scope boundaries, live task visibility, settings, and new issue dialog behavior. ## Verification - `pnpm vitest run server/src/__tests__/task-watchdogs-scheduler.test.ts server/src/__tests__/task-watchdogs-classifier.test.ts` - `pnpm vitest run server/src/__tests__/approval-routes-idempotency.test.ts server/src/__tests__/issue-agent-mutation-ownership-routes.test.ts` - `pnpm vitest run ui/src/components/NewIssueDialog.test.tsx` - `pnpm --filter @paperclipai/server typecheck` - `git diff --check` - Verified the PR diff does not include `pnpm-lock.yaml` or `.github/workflows`. ## Risks - Medium risk: this introduces a new task lifecycle surface touching DB schema, server routes/services, adapter wake context, and UI task configuration. - Watchdog scheduling behavior depends on the new experimental setting and runtime context checks behaving consistently across local and production agents. - The watchdog migration is idempotent (`IF NOT EXISTS` / duplicate-object guards) so users who tried the previous branch-local migration number should not get duplicate-object failures. - CI and the second Greptile pass are pending after the latest review-fix push. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used OpenAI Codex, GPT-5-class coding agent in the Paperclip workspace. Exact runtime model id and context window were not exposed to the agent; tool use and local command execution were enabled. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have searched GitHub for duplicate or related PRs and linked them above - [x] I have either (a) linked existing issues with `Fixes: #` / `Closes #` / `Refs #` OR (b) described the issue in-PR following the relevant issue template - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots — N/A per Paperclip task instruction: do not add screenshots/images to this PR unless they are specifically part of the work. - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [ ] All Paperclip CI gates are green - [ ] Greptile is 5/5 with no open P2s, recommendations, or follow-ups - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> Co-authored-by: Claude Opus 4.8 <noreply@anthropic.com> |

Build the Skills Store (#7990)
## Thinking Path > - Paperclip is the open source app people use to manage AI agents for work. > - Agents increasingly depend on reusable skills, so the control plane needs a first-class way to browse, inspect, install, version, and attach those skills. > - The old skills surface was mostly operational plumbing; it did not give operators a store-like discovery flow, canonical detail URLs, rich source/version context, or creation paths. > - The backend also needed stronger contracts around company skill metadata, versions, install counts, runtime materialization, and adapter skill preferences. > - This pull request builds the Skills Store foundation across DB, shared contracts, server routes/services, UI, and Storybook. > - The benefit is a more inspectable, operator-friendly skill workflow that still preserves company-scoped control-plane boundaries and agent runtime behavior. ## Linked Issues or Issue Description No GitHub issue exists for this Paperclip work item. Paperclip task refs: PAP-10846 and PAP-10921. Feature request: Paperclip operators need a single Skills Store experience where company skills can be discovered, inspected, created, versioned, installed, and attached to agents without relying on scattered operational screens or implicit runtime state. Related PR search: - Searched GitHub for `Skills Store`, `company skills`, and `skill detail`. - Found several open skills-related PRs such as #7809 and #4409, but no duplicate PR for this end-to-end Skills Store branch. ## What Changed - Added the Skills Store backend foundation: company skill schema fields, migrations, shared types/validators, and expanded server skill routes/services. - Added skill discovery, category navigation, canonical skill detail routes, tabs, source attribution, version snapshots/diffs, install count backfill, and creation flows. - Updated agent skill preference handling so version selections survive runtime mention injection and runtime skill materialization honors pinned versions. - Preserved unversioned skill assignments as live/current selections instead of silently pinning them to the current version at assignment time. - Added focused regression coverage for company skill routes/services, route helpers, UI behavior, skill version diffs, and runtime skill version pins. - Added Storybook coverage for Skills Store discovery/detail states and updated the main layout navigation. - Addressed Greptile findings around version creation races, soft-deleted comments, fork metadata scoping, GitHub skill directory fallback, runtime snapshot materialization, shared runtime skill-selection helpers, and version-assignment semantics. ## Verification - `pnpm exec vitest run server/src/__tests__/heartbeat-project-env.test.ts server/src/__tests__/heartbeat-runtime-skills.test.ts` - `pnpm exec vitest run packages/shared/src/validators/company-skill.test.ts` - `pnpm exec vitest run server/src/__tests__/company-portability.test.ts server/src/__tests__/company-skills-service.test.ts` - `pnpm exec vitest run cli/src/__tests__/company-import-export-e2e.test.ts` - `pnpm exec vitest run server/src/__tests__/agent-skills-routes.test.ts` - `pnpm exec vitest run server/src/__tests__/openapi-routes.test.ts` - `pnpm exec vitest run server/src/__tests__/company-skills-service.test.ts server/src/__tests__/company-skills-routes.test.ts server/src/__tests__/heartbeat-runtime-skills.test.ts` - `pnpm exec vitest run server/src/__tests__/company-skills-service.test.ts` - `pnpm exec vitest run ui/src/components/IssueProperties.test.tsx -t "edits existing custom assignee model options from the properties pane"` - `pnpm --filter @paperclipai/server typecheck` - GitHub checks are green on `0823957a2`: Build, Canary Dry Run, General tests, Typecheck + Release Registry, serialized server suites, e2e, policy/review, Socket, Snyk, and aggregate `verify`. - Greptile Review succeeded on `0823957a2` with `40 files reviewed, 0 comments added`; GitHub unresolved review threads: 0. Not run in this heartbeat: - Browser screenshot capture for the UI changes. This PR intentionally omits screenshots per the Paperclip task direction not to add design screenshots/images. ## Risks - Broad feature branch touching DB, shared contracts, server, and UI; reviewers should still scan merge conflicts carefully if `master` moves again before landing. - Skill version/runtime behavior is sensitive: pinned skill versions must stay pinned while default selections should continue following the current version. - UI polish should get normal reviewer/browser attention before merge because this PR includes a large Skills Store surface and screenshots were intentionally omitted. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. [codex] Add skills CLI and catalog management (#6782)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies through company-scoped control-plane workflows. > - Agents need reusable, inspectable skills that can be installed, reset, audited, exported, and assigned without bespoke local setup. > - The existing skill truth model needed cleanup so bundled skills, optional catalog skills, runtime skills, and adapter-provided skills have clear provenance. > - Operators also need a practical CLI and board UI for discovering and managing company skills. > - This pull request adds the skills CLI, packaged skills catalog, company skills APIs, and catalog-aware board UI. > - The benefit is a more reusable Paperclip company setup where skills are portable, auditable, and easier for operators and agents to manage. ## What Changed - Added `paperclipai skills` CLI commands and coverage for catalog listing, installing, resetting, and inspecting company skills. - Added a packaged `@paperclipai/skills-catalog` workspace with bundled and optional skill content plus validation/build tests. - Added shared company-skill types and validators used across CLI, server, and UI contracts. - Added server catalog APIs/services for company skill catalog operations, reset semantics, audit behavior, and portability provenance. - Updated adapter skill handling so runtime/catalog provenance remains explicit across local adapters. - Added board UI support for browsing and managing catalog-backed company skills. - Updated docs for the skills CLI/catalog flow and the company skills Paperclip skill reference. - Rebased the branch onto current `paperclipai/paperclip:master`; no `pnpm-lock.yaml`, `.github/workflows`, or migration files are included in the final PR diff. ## Verification - Passed: `pnpm run preflight:workspace-links && pnpm exec vitest run cli/src/__tests__/skills.test.ts packages/skills-catalog/src/catalog-builder.test.ts packages/skills-catalog/src/shipped-catalog.test.ts packages/shared/src/validators/company-skill.test.ts packages/adapter-utils/src/server-utils.test.ts packages/plugins/create-paperclip-plugin/src/entrypoints.test.ts server/src/__tests__/company-skills-catalog-service.test.ts server/src/__tests__/company-skills-routes.test.ts server/src/__tests__/company-portability.test.ts`. - Passed: `pnpm exec vitest run server/src/__tests__/workspace-runtime.test.ts -t "default branch|origin/master|symbolic-ref"`. - Attempted: full `server/src/__tests__/workspace-runtime.test.ts`. Four provisioning tests failed while seeding an isolated worktree database from the local Paperclip instance because the local plugin schema dump contains a duplicate-column foreign key (`plugin_content_machine_18a7bc327b.content_case_signals`). The default-branch tests touched by the rebase conflict passed in the focused run above. - Checked final diff: no `pnpm-lock.yaml`, no `.github/workflows`, and no migration-file changes relative to `master`. ## Risks - Medium: this is a broad skills/catalog change touching CLI, server APIs, shared contracts, adapter skill sync, and UI. - Catalog validation and reset semantics need careful reviewer attention because they affect reusable company setup and portability. - No database migrations are included in this PR, so there is no migration ordering/idempotency risk in the final diff. - No lockfile is included by design; dependency resolution will be handled by the repository lockfile workflow. ## Model Used - OpenAI Codex coding agent based on GPT-5, running in Paperclip via the `codex_local` adapter with shell, git, GitHub CLI, and code-editing tool access. Exact hosted model build/context-window metadata is not exposed in this runtime. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run targeted tests locally and documented the local workspace-runtime seed failure above - [x] I have added or updated tests where applicable - [x] If this change affects the UI, screenshots were intentionally omitted per PAP-10124 instructions; UI behavior is covered by tests and reviewer inspection - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

[codex] Add LLM Wiki plugin host support (#5597)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies. > - The plugin system needs host contracts and runtime support before large plugins can integrate cleanly. > - The source branch mixed the LLM Wiki package with supporting host/runtime work, managed plugin skills, root-level storage spaces, and a bookmarks reference plugin. > - [PAP-9173](/PAP/issues/PAP-9173) asked for the current branch to be split by file boundary: plugin package separately from everything else. > - [PAP-9188](/PAP/issues/PAP-9188) clarified that LLM Wiki may have plugin-local spaces, but Paperclip core should not reorganize top-level local storage into spaces. > - Follow-up review clarified that the bookmarks example should not ship in this PR either. > - This pull request contains the non-`packages/plugins/plugin-llm-wiki/` host/runtime work, keeps runtime state under the selected Paperclip instance root, and no longer includes the bookmarks example. ## What Changed - Added/updated plugin host contracts, SDK types, worker RPC plumbing, managed plugin skill support, and related server tests. - Removed the bookmarks example plugin package and its bundled-example/workspace references. - Removed the root-level local spaces CLI/migration surface and restored instance-root runtime defaults for config, db, logs, storage, secrets, workspaces, projects, and adapter homes. - Replaced shared root `space-paths` helpers with `home-paths` helpers for core runtime storage. - Tightened stranded recovery unique-conflict detection so concurrent recovery scans reuse the raced recovery issue when Postgres errors are wrapped. - Kept `packages/plugins/plugin-llm-wiki/` out of this PR diff; plugin-local spaces remain in the stacked plugin-only PR. ## Verification - `pnpm exec vitest run cli/src/__tests__/data-dir.test.ts cli/src/__tests__/home-paths.test.ts cli/src/__tests__/onboard.test.ts packages/shared/src/home-paths.test.ts packages/db/src/runtime-config.test.ts server/src/__tests__/agent-instructions-service.test.ts server/src/__tests__/claude-local-execute.test.ts server/src/__tests__/codex-local-execute.test.ts` - `pnpm exec vitest run packages/db/src/runtime-config.test.ts` - `pnpm exec vitest run server/src/__tests__/plugin-routes-authz.test.ts` - `pnpm --filter @paperclipai/server typecheck` - `pnpm exec vitest run server/src/__tests__/heartbeat-process-recovery.test.ts -t "reuses the raced stranded recovery issue"` skipped locally because embedded Postgres did not initialize on this macOS temp host; the code path was typechecked and is covered by Linux CI. - Boundary check: no core references remain for `PAPERCLIP_SPACE_ID`, `spaces migrate-default`, `@paperclipai/shared/space-paths`, `registerSpacesCommands`, or the removed bookmarks example. - Previous PR head `4f23e034` had green GitHub checks: `verify`, all four serialized server shards, `e2e`, `Canary Dry Run`, `policy`, Snyk, and `Greptile Review`. Current head `582f466d` is re-running checks after the bookmarks deletion. ## Risks - Plugin host changes touch shared runtime paths, so regressions would most likely appear in adapter startup, plugin loading, or local dev path defaults. - Removing the bookmarks example also removes one demonstration of plugin database namespaces plus local-folder persistence; remaining plugin examples still cover bundled example discovery and plugin host flows. - The plugin package itself is intentionally deferred to the stacked plugin-only PR, where LLM Wiki plugin-local spaces live. - Existing installs that tested the transient root-level spaces CLI should stop using it; this PR intentionally removes that unsupported migration surface before merge. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI GPT-5 Codex via Codex CLI, tool use and local code execution enabled; context window not exposed. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass, except where noted above for host-specific embedded Postgres initialization - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge Stacked follow-up: PR #5592 contains only `packages/plugins/plugin-llm-wiki/` and targets this branch. --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

Stabilize runtime probes and Codex env tests (#5445)
## Thinking Path
> - Paperclip orchestrates AI agents for zero-human companies
> - Adapters expose a Test action that probes the configured runtime —
install, resolvability, hello — to give operators a fast yes/no on
whether an environment is healthy
> - The Codex test path was running its hello probe directly without
going through the managed-runtime preparation that production runs use,
so a healthy production setup could still report a probe failure
> - The plugin worker manager wasn't surfacing terminated workers
cleanly, leaving the runtime probe waiting on a dead worker until the
request timed out
> - This pull request routes the Codex test probe through
`prepareAdapterExecutionTargetRuntime` (so it sees the same managed
Codex home production sees), exposes `commandCwd` on
`createCommandManagedRuntimeClient` so callers can target a per-probe
directory without leaking the workspace `remoteCwd`, and propagates
plugin-worker termination as a usable error instead of a hang
> - The benefit is the Codex Test action mirrors production behavior
end-to-end, and probes against a terminated plugin worker fail fast
instead of timing out
## What Changed
- `packages/adapter-utils/src/command-managed-runtime.ts`: rename the
`remoteCwd` knob to `commandCwd` so callers can target a per-probe
directory without inheriting the workspace cwd; matching test coverage
in `command-managed-runtime.test.ts`
- `packages/adapter-utils/src/sandbox-callback-bridge.{ts,test.ts}`:
small fixes to keep callback bridge stop semantics deterministic
- `packages/adapters/codex-local/src/server/test.ts`: thread the Codex
hello probe through `prepareAdapterExecutionTargetRuntime` +
`prepareManagedCodexHome` so the probe sees the same managed home
production sees; new `test.remote.test.ts` covers the remote probe path
- `packages/adapters/cursor-local/src/server/execute.ts`: small
probe-side cleanup that aligns with the new commandCwd contract
- `server/src/services/plugin-worker-manager.ts`: surface plugin-worker
termination as a structured error so callers fail fast; new
`plugin-worker-terminated.cjs` fixture and
`plugin-worker-manager.test.ts` cases pin the behavior
## Verification
- `pnpm vitest run --no-coverage --project @paperclipai/adapter-utils
--project @paperclipai/adapter-codex-local --project
@paperclipai/adapter-cursor-local --project @paperclipai/server` —
1749/1750 passing (1 unrelated skip)
- `pnpm typecheck` clean
## Risks
Low–medium. The `remoteCwd → commandCwd` rename is a parameter renaming
on an internal helper used only by adapter test/execute paths in this
repo. The plugin-worker-terminated path was previously a hang; failing
fast may surface latent timeouts as explicit termination errors in
callers that already expected them.

Harden remote workspace sync and restore flows (#5444)
## Thinking Path
> - Paperclip orchestrates AI agents for zero-human companies
> - When an agent runs against a remote target, Paperclip syncs the
workspace out to the remote at run start and restores changes back to
the local workspace at run end
> - The previous restore flow naïvely overwrote local files with
whatever the remote returned, so files that the remote run never touched
but had timestamp/mode drift could be needlessly rewritten — and a
single static `refs/paperclip/ssh-sync/imported` ref made concurrent SSH
workspace exports race on the same git ref
> - This pull request adds a `workspace-restore-merge` module that diffs
a pre-run snapshot against the post-run remote state and only writes
back files the remote actually changed; SSH workspace exports now use a
per-import unique ref so concurrent runs can't trample each other
> - Every adapter's execute path threads the snapshot through
`prepareAdapterExecutionTargetRuntime` so the merge has the baseline it
needs
> - The benefit is workspace restores no longer churn untouched files,
and concurrent SSH runs no longer collide on the import ref
## What Changed
- `packages/adapter-utils/src/workspace-restore-merge.{ts,test.ts}`: new
module — directory snapshot (kind/mode/sha256/symlink target) plus
snapshot-aware merge that writes only the files the remote changed
- `packages/adapter-utils/src/ssh.ts`: SSH workspace export uses a
per-import unique ref (`refs/paperclip/ssh-sync/imported/<uuid>`);
restore goes through the new merge helper; `ssh-fixture.test.ts` covers
the unique-ref + merge paths
- `packages/adapter-utils/src/sandbox-managed-runtime.ts` +
`remote-managed-runtime.ts`: thread the snapshot/merge through the
sandbox and SSH paths
- `packages/adapter-utils/src/server-utils.{ts,test.ts}` +
`execution-target.ts`: helpers for capturing the pre-run snapshot;
`prepareAdapterExecutionTargetRuntime` gains required `runId` and
optional `workspaceRemoteDir`, and returns the realized
`workspaceRemoteDir`
- Each adapter's `execute.ts` (acpx, claude, codex, cursor, gemini,
opencode, pi) takes the snapshot at run start and passes it through to
the runtime restore
- Remote execute test mocks updated to match the new
`prepareWorkspaceForSshExecution` return shape and the per-run
`${managedRemoteWorkspace}` cwd subdirectory
## Verification
- `pnpm vitest run --no-coverage --project @paperclipai/adapter-utils
--project @paperclipai/adapter-acpx-local --project
@paperclipai/adapter-claude-local --project
@paperclipai/adapter-codex-local --project
@paperclipai/adapter-cursor-local --project
@paperclipai/adapter-gemini-local --project
@paperclipai/adapter-opencode-local --project
@paperclipai/adapter-pi-local` — 196/196 passing
- `pnpm typecheck` clean across the workspace
## Risks
Medium. The restore path now writes a strict subset of what it
previously did — files the remote did not touch are no longer rewritten.
If any flow was relying on a touch-without-content-change being copied
back (timestamp or permission propagation only), that behavior is now
skipped. Snapshot capture adds an O(N-files-in-workspace) hash pass at
run start; the cost is bounded by the existing exclude list. The `runId`
parameter on `prepareAdapterExecutionTargetRuntime` is now required —
every in-tree caller is updated; out-of-tree adapter authors need to
pass it.

Harden control-plane safety and issue identifiers (#5292)
## Thinking Path > - Paperclip relies on issue identifiers, execution policies, and agent heartbeat rules to keep autonomous work auditable. > - Safety checks need to reject ambiguous agent handoffs, and identifier parsing needs to support Cloud tenant prefixes. > - Agent instructions also need to make final-disposition rules explicit so work does not stall in vague states. > - This pull request isolates backend correctness and governance hardening from the UI and recovery-system-notice branches. > - The benefit is safer in-review transitions, better identifier compatibility, and clearer agent operating contracts. ## What Changed - Fixed run-aware confirmation ordering and interrupted-run state cleanup. - Added Cloud tenant identity bootstrap and alphanumeric issue identifier support across shared parsing and server routes. - Guarded agent-authored `in_review` updates unless a real review path exists. - Tightened heartbeat disposition instructions in adapter utilities/default AGENTS/Paperclip skill. ## Verification - `pnpm install --frozen-lockfile` - `pnpm exec vitest run packages/shared/src/issue-references.test.ts server/src/__tests__/issue-identifier-routes.test.ts server/src/__tests__/issue-execution-policy-routes.test.ts packages/adapter-utils/src/server-utils.test.ts` initially had the first execution-policy test hit Vitest's 5s timeout under the parallel bundle while the rest passed. - `pnpm exec vitest run server/src/__tests__/issue-execution-policy-routes.test.ts --testTimeout=20000` passed with 10/10 tests. - Follow-up: `pnpm run typecheck:build-gaps` passed. - Follow-up: `pnpm --filter @paperclipai/ui typecheck` passed. - Follow-up: `pnpm vitest run server/src/__tests__/issue-comment-reopen-routes.test.ts server/src/__tests__/company-portability.test.ts server/src/__tests__/costs-service.test.ts` passed. - Follow-up: `pnpm vitest run ui/src/context/LiveUpdatesProvider.test.ts ui/src/lib/issue-chat-messages.test.ts ui/src/lib/issue-reference.test.ts ui/src/lib/issue-timeline-events.test.ts` passed. ## Risks - Medium control-plane risk: in-review update validation changes agent behavior. The error message is explicit and tests cover allowed review paths. ## Model Used - OpenAI GPT-5 Codex via Paperclip `codex_local` adapter, with shell/git/GitHub CLI tool use. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

Add planning mode for issue work (#5353)
## Thinking Path > - Paperclip is a control plane for autonomous AI companies. > - Issues are the core unit of work, and issue comments are how board users and agents coordinate execution. > - Some issue conversations need to produce plans and approvals instead of immediate implementation work. > - The existing issue contract did not distinguish standard execution comments from planning-oriented issue work. > - This pull request adds an issue work-mode contract and board UI affordances for standard vs planning mode. > - The benefit is that planning-mode issues can be created, displayed, discussed, and carried through agent heartbeat context without losing the normal issue workflow. ## What Changed - Added `standard` / `planning` issue work-mode contracts across DB, shared validators/types, server issue flows, plugin protocol, and adapter heartbeat payloads. - Added an idempotent `0081_optimal_dormammu` migration for `issues.work_mode`, ordered after current `public-gh/master` migrations. - Updated heartbeat/context summaries and issue-thread interaction behavior so planning work mode is preserved when creating suggested follow-up issues. - Added UI support for planning-mode issue creation, issue rows, detail composer styling, and composer work-mode toggles. - Added focused server/shared/UI tests plus a Playwright visual verification spec for planning-mode surfaces. - Rebased the branch onto current `public-gh/master` and added durable planning-mode screenshots under `doc/assets/pap-3368/`. ## Verification - `pnpm --filter @paperclipai/db run check:migrations` - `pnpm exec vitest run --project @paperclipai/shared packages/shared/src/validators/issue.test.ts` - `pnpm exec vitest run --project @paperclipai/server server/src/__tests__/heartbeat-context-summary.test.ts server/src/__tests__/issue-thread-interactions-service.test.ts server/src/__tests__/issues-goal-context-routes.test.ts --pool=forks --poolOptions.forks.isolate=true` - `pnpm exec vitest run --project @paperclipai/ui ui/src/components/IssueChatThread.test.tsx ui/src/components/NewIssueDialog.test.tsx ui/src/components/IssueRow.test.tsx ui/src/pages/IssueDetail.test.tsx` - `pnpm exec vitest run --project @paperclipai/adapter-utils packages/adapter-utils/src/server-utils.test.ts` - `PAPERCLIP_E2E_SKIP_LLM=true npx playwright test --config tests/e2e/playwright.config.ts tests/e2e/planning-mode-visual-verification.spec.ts` ## Screenshots Desktop planning detail: Desktop planning row: Desktop staged standard toggle: Mobile planning detail: Mobile planning row: ## Risks - Medium migration risk: this adds a non-null issue column. The migration uses `ADD COLUMN IF NOT EXISTS` so installations that applied an older branch-local migration number can still apply the final numbered migration safely. - Medium contract risk: issue payloads, plugin payloads, and adapter heartbeat payloads now include work mode; compatibility is handled by defaulting missing values to `standard`. - UI risk is moderate because composer controls changed; focused component tests and visual e2e coverage exercise standard vs planning display and toggle behavior. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Sanitize remote execution envs at the boundary (#5325)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies > - Adapters spawn CLIs against local, SSH, and sandbox targets, threading a runtime env through `runAdapterExecutionTargetProcess` and the SSH/sandbox runners > - Host identity vars (HOME, TMPDIR, XDG_*, NVM_DIR, PATH) routinely leak into the env we send to remote targets — sometimes via test probes, sometimes via runtime config — and break sandboxed/SSH'd CLIs whose own profiles set those values correctly > - The sanitization logic existed but lived alongside other helpers in `server-utils.ts` and was applied piecemeal at adapter callsites, so it was easy to bypass > - This pull request lifts the sanitization into a standalone `remote-execution-env.ts`, applies it at the SSH and sandbox runtime boundary so every remote spawn goes through it, and removes the duplicated callsite-level filtering > - The benefit is identity-bound host env stops leaking across SSH/sandbox transports regardless of which adapter calls in ## What Changed - `packages/adapter-utils/src/remote-execution-env.ts`: new module — single source of truth for which env keys are identity-bound and how to strip them when the value matches the host's value - `packages/adapter-utils/src/server-utils.ts`: remove the inline sanitization (now in `remote-execution-env.ts`) - `packages/adapter-utils/src/execution-target.ts`: apply sanitization at the sandbox runtime boundary - `packages/adapter-utils/src/ssh.ts`: apply sanitization at the SSH spawn boundary - `packages/adapters/opencode-local/src/server/test.ts`: drop now-redundant callsite filtering - `packages/adapters/pi-local/src/server/test.ts`: drop now-redundant callsite filtering - New tests `execution-target.test.ts` and `execution-target-sandbox.test.ts` cover the sanitizer flow at both transports, including positive cases (host-shaped path stripped) and explicit-override preservation ## Verification - `pnpm vitest run --no-coverage --project @paperclipai/adapter-utils --project @paperclipai/adapter-opencode-local --project @paperclipai/adapter-pi-local` - `pnpm typecheck` clean ## Risks Low–medium. The sanitization is now applied at one layer (boundary) instead of N (callsites), so behavior is more consistent. Any adapter that previously relied on a leaked host var landing on the remote shell would now see it stripped — but those reliances were what this change exists to fix. ## Model Used Claude Opus 4.7 (1M context) ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable — new tests at both transports - [x] If this change affects the UI, I have included before/after screenshots — N/A (no UI) - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge |

Strip inherited host shell env from SSH remote execution (#5142)
## Thinking Path
> - Paperclip orchestrates AI agents for zero-human companies
> - Agents executing on remote SSH hosts receive an env map built from
the host
> process's env plus per-run additions like `PAPERCLIP_API_KEY`,
> `PAPERCLIP_RUN_ID`, etc.
> - The env map currently includes inherited host vars by default,
including
> identity-bound ones like `PATH`, `HOME`, `USER`, `NVM_DIR`, `XDG_*` —
> variables whose values are meaningful only on the host they came from
> - Sending the host's `PATH` (containing host-only directories like a
local
> nvm install path) to a remote SSH box overrides the remote's actual
`PATH`
> and breaks command resolution. Same hazard for `HOME` (commands
looking for
> config files end up in a non-existent dir), `USER` (writes go to the
wrong
> path), etc.
> - This PR adds `sanitizeSshRemoteEnv()` that drops inherited
identity-bound
> vars when their value matches the host process's value. Explicitly-set
> values pass through untouched, so callers that genuinely want to
override
> remote `PATH` etc. still can — but accidental leakage from
> `process.env` is filtered.
> - The benefit is that SSH remote execution stops corrupting the remote
> shell's environment with host-shaped paths, so commands resolve
correctly
> against the remote PATH and config files land in the remote `HOME`
## What Changed
- New `sanitizeSshRemoteEnv(env, inheritedEnv = process.env)` in
`packages/adapter-utils/src/server-utils.ts`. The identity-bound key set
is:
- `PATH`, `HOME`, `PWD`, `SHELL`, `USER`, `LOGNAME`
- `NVM_DIR`, `TMPDIR`, `TMP`, `TEMP`
- `XDG_CONFIG_HOME`, `XDG_CACHE_HOME`, `XDG_DATA_HOME`,
`XDG_STATE_HOME`, `XDG_RUNTIME_DIR`
For any key in this set, the entry is dropped iff the env value equals
the
inherited (host process) value. Other keys pass through unchanged.
- `readEnvValueCaseInsensitive(...)` helper handles Windows-style
case-insensitive env var lookups.
- Wired into `resolveSpawnTarget(...)` for the SSH transport. Sandbox
and local
paths are unaffected.
- Tests added in `server-utils.test.ts` (~50 lines) covering: matching
keys
filtered, mismatched keys preserved, non-identity keys passed through,
case
insensitivity.
## Verification
- `pnpm --filter @paperclipai/adapter-utils test -- server-utils`
- Manual QA: run any adapter against an SSH-backed environment, confirm
remote command resolution works (e.g. `node`, `npm`, the adapter's CLI)
and
config files land in the remote user's `HOME`. Compare to the prior
behaviour
by transiently re-introducing the inherited `PATH` and watching commands
fail with `command not found`.
## Risks
- Behavioural shift: SSH remote execution previously passed inherited
host env
vars verbatim. Code that relied on that (e.g. a remote command somehow
expecting the host's `PATH`) will see different behaviour. None of the
adapter code in this repo has such a dependency.
- Edge case: if a caller explicitly sets `PATH` to the same value as the
host's
`PATH` (literally — same exact string), the sanitizer drops it as a
leak.
In practice no caller constructs the env this way.
- Windows host: case-insensitive lookup handles `Path` vs `PATH`
correctly.
Tested.

Fix remote workspace environment shaping (#5118)
> **Stacked PR (part 5 of 7).** Depends on: - PR #5114 - PR #5115 - PR #5116 - PR #5117 > Diff against `master` includes commits from earlier PRs in the stack — the new commit in this PR is the topmost one. ## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies > - Agents run with a Paperclip-shaped environment (`PAPERCLIP_WORKSPACE_CWD`, > worktree path, `PAPERCLIP_WORKSPACES_JSON` hints) so the CLI can locate the > correct project tree > - SSH testing reproduced a real failure: a Codex SSH run wrote to > `/tmp/paperclip-env-matrix-...` (the *host* path) instead of the realized > remote workspace at `/home/<user>/paperclip-env-matrix-ssh-claude/...` > because the adapter injected `PAPERCLIP_WORKSPACE_CWD=/tmp/...` into the > remote env > - Code review on the initial codex-only fix asked to roll the same approach > into every other SSH-capable adapter (claude, acpx, cursor, opencode, gemini, > pi) via a shared helper rather than duplicating per-adapter > - This PR adds `shapePaperclipWorkspaceEnvForExecution` in adapter-utils that, > when the execution target is remote: replaces local cwd with the realized > execution cwd, nulls out worktree path (which has no remote meaning), and > rewrites/strips `cwd` entries in workspace hints based on what was actually > synced. Every adapter calls it before invoking the remote runner > - The benefit is that remote runs see the realized remote workspace, host-local > paths stop leaking into remote env, and the rule is unit-tested in one place ## What Changed - Added `shapePaperclipWorkspaceEnvForExecution` to `packages/adapter-utils/src/server-utils.ts` with full unit coverage (`server-utils.test.ts`) - Each of acpx-local, claude-local, codex-local, cursor-local, gemini-local, opencode-local, pi-local now calls the new shaper before issuing the remote command and feeds the shaped values into `applyPaperclipWorkspaceEnv` - Per-adapter `execute.remote.test.ts` files extended to cover the new shaping behaviour: localhost paths replaced with remote cwd, foreign-cwd hints stripped, worktree path nulled out for remote targets - `acpx-local/src/server/execute.test.ts` extended with shaping coverage ## Verification - `pnpm test -- server-utils execute.remote` - `pnpm --filter @paperclipai/adapter-acpx-local test` - Manual QA reproducing the original failure: 1. Provision an E2B sandbox environment for the Paperclip QA company 2. Assign an issue to a remote-targeted claude-local agent and confirm the run starts in the correct remote cwd (no `/Users/...` path leakage in the run logs) 3. Repeat for opencode-local and pi-local ## Risks - Behavioural shift: hints whose `cwd` doesn't match the workspace cwd are now stripped on remote targets. If any adapter relied on a leaked local hint cwd, it will see a missing `cwd` instead. Reviewed all current callers — none do. - Adds a small per-run cost (path resolve + string normalisation) on every remote execution. Negligible. - Worktree path is now nulled out on remote (it has no meaning there). Adapters that previously read the value defensively will continue to work. ## Model Used - OpenAI GPT-5.4 (reasoning effort: high) via Codex CLI - Provider: OpenAI - Used to author the code changes in this PR ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [ ] If this change affects the UI, I have included before/after screenshots — N/A - [ ] I have updated relevant documentation to reflect my changes — N/A - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge |

Add ACPX local adapter runtime (#4893)
## Thinking Path > - Paperclip orchestrates AI-agent companies through a control plane that can start, supervise, and recover agent runs. > - Local adapters are the bridge between Paperclip issues and concrete agent runtimes such as Claude, Codex, and other ACP-compatible tools. > - The roadmap calls out broader “bring your own agent” and claw-style agent support, and ACPX gives Paperclip one path to normalize multiple ACP agents behind a single adapter. > - The branch needed to become one reviewable PR against current `paperclipai/paperclip:master`, without carrying stale base conflicts or generated lockfile churn. > - This pull request adds an experimental built-in `acpx_local` adapter, integrates it through the server/CLI/UI adapter surfaces, and adds regression coverage for runtime execution, skill sync, stream parsing, diagnostics, and log redaction. > - The benefit is that Paperclip can run Claude/Codex/custom ACP agents through ACPX while keeping operator configuration, skills, logging, and transcript rendering inside the existing adapter model. ## What Changed - Added `@paperclipai/adapter-acpx-local` with server execution, config schema, ACPX session handling, CLI formatting, UI config helpers, and stdout parsing. - Registered `acpx_local` across CLI, server, shared constants, UI adapter metadata, adapter capabilities, and agent creation/editing surfaces. - Added ACPX runtime execution support with persistent sessions, local-agent JWT environment handling, skill snapshots, runtime skill materialization, and isolation/security regressions. - Added ACPX adapter diagnostics and marked the adapter experimental in the UI. - Added command/env secret redaction for resolved command metadata in adapter-utils, server event storage, and the Agent Detail invocation UI. - Added Storybook coverage for ACPX config, transcript rendering, and skill states, plus PR screenshots under `docs/pr-screenshots/pap-2944/`. - Rebased the branch onto current `public-gh/master`; `pnpm-lock.yaml` is intentionally not included and there are no migration/schema changes. ## Verification - `pnpm exec vitest run packages/adapters/acpx-local/src/server/execute.test.ts packages/adapters/acpx-local/src/server/test.test.ts packages/adapters/acpx-local/src/cli/format-event.test.ts packages/adapters/acpx-local/src/ui/parse-stdout.test.ts packages/adapter-utils/src/server-utils.test.ts server/src/__tests__/redaction.test.ts server/src/__tests__/acpx-local-execute.test.ts server/src/__tests__/acpx-local-skill-sync.test.ts server/src/__tests__/acpx-local-adapter-environment.test.ts server/src/__tests__/adapter-routes.test.ts server/src/__tests__/agent-skills-routes.test.ts ui/src/adapters/metadata.test.ts` — 12 files, 87 tests passed. - `pnpm --filter @paperclipai/adapter-acpx-local typecheck` — passed. - `pnpm --filter @paperclipai/server typecheck` — passed. - `pnpm --filter @paperclipai/ui typecheck` — passed. - Confirmed PR diff does not include `pnpm-lock.yaml`, database schema files, or migrations. Screenshots: ## Risks - Medium risk: this introduces a new built-in adapter package and touches runtime execution, adapter registration, agent config, skills, and transcript rendering. - ACPX and ACP agent behavior can vary by installed tool versions; the adapter is marked experimental to set operator expectations. - `pnpm-lock.yaml` is excluded per repository PR policy, so dependency lock refresh must be handled by the repo’s automation or maintainers. - No database migration risk: no schema or migration files changed. > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Fix CEO AGENT_HOME paths and centralize workspace env propagation (#4551)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies. > - The local adapter layer is responsible for turning Paperclip runtime context into the environment seen by the child agent process. > - The CEO onboarding bundle tells the agent where to read and write its persistent memory and fact files. > - That bundle was using `./memory/...` and `./life/...`, which only works when the process cwd happens to equal the agent home directory. > - At the same time, six local adapters each duplicated the same workspace-env propagation logic, including `AGENT_HOME`, which makes this contract easy to drift. > - This pull request fixes the CEO instructions to use `$AGENT_HOME/...` and centralizes workspace-env propagation in one shared helper with shared tests. > - The benefit is a real bug fix for agent memory paths plus a single tested contract that makes future built-in adapter work less likely to forget `AGENT_HOME`. ## What Changed - Updated `server/src/onboarding-assets/ceo/HEARTBEAT.md` to use `$AGENT_HOME/memory/...` and `$AGENT_HOME/life/...` instead of cwd-relative `./memory/...` and `./life/...`. - Added `applyPaperclipWorkspaceEnv(...)` in `packages/adapter-utils/src/server-utils.ts` to centralize `PAPERCLIP_WORKSPACE_*` and `AGENT_HOME` propagation. - Added shared helper coverage in `packages/adapter-utils/src/server-utils.test.ts` for both populated and skip-empty cases. - Switched the built-in local adapters (`claude_local`, `codex_local`, `cursor_local`, `gemini_local`, `opencode_local`, `pi_local`) over to the shared helper instead of inline env assignment blocks. ## Verification - `pnpm install` - `pnpm exec vitest run packages/adapter-utils/src/server-utils.test.ts packages/adapters/claude-local/src/server/execute.remote.test.ts packages/adapters/codex-local/src/server/execute.remote.test.ts packages/adapters/cursor-local/src/server/execute.remote.test.ts packages/adapters/gemini-local/src/server/execute.remote.test.ts packages/adapters/opencode-local/src/server/execute.remote.test.ts packages/adapters/pi-local/src/server/execute.remote.test.ts` - Result: 7 test files passed, 31 tests passed, 0 failures. ## Risks - Low risk. - The only behavioral surface is the shared env propagation refactor across six adapters; if the helper diverged from prior semantics, an adapter could miss a workspace env var. - The shared helper test plus the affected adapter execute tests reduce that risk, and the helper preserves the prior "set only non-empty strings" behavior. ## Model Used - OpenAI Codex via Paperclip `codex_local` agent runtime; tool-assisted coding workflow with shell execution, file patching, git operations, and API interaction. The exact backend model identifier and context window are not surfaced by this local runtime. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [x] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge |

feat: add paperclip-dev skill with optional bundled skill support (#3854)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies > - Agents working on the Paperclip codebase itself need guidance on dev workflows: server lifecycle, worktrees, builds, database ops, diagnostics > - There was no bundled skill covering these workflows — agents had to figure it out from scratch each time > - Additionally, not every skill should be force-installed on every agent — a dev-focused skill should be opt-in > - This PR adds a `paperclip-dev` skill with `required: false` frontmatter so it ships with Paperclip but isn't auto-installed > - The skill's PR section references canonical files (`.github/PULL_REQUEST_TEMPLATE.md`, `CONTRIBUTING.md`) instead of duplicating their content, with gated instructions that force agents to read those files before creating any PR > - The benefit is that developers (human or agent) can opt in to structured dev guidance without polluting the default agent skill set or creating drift between duplicated docs ## What Changed - Added `skills/paperclip-dev/SKILL.md` covering server management, worktree lifecycle, builds, database ops, diagnostics, agent operations, and common mistakes - The Pull Requests section uses gated, reference-based instructions — agents MUST read `.github/PULL_REQUEST_TEMPLATE.md` and `CONTRIBUTING.md` before running `gh pr create`, with a brief checklist of required section names (no content duplication) - Updated `packages/adapter-utils/src/server-utils.ts` to respect `required: false` frontmatter — optional skills are bundled but not auto-installed on agents - Added test in `server/src/__tests__/paperclip-skill-utils.test.ts` verifying that optional skills are excluded from the default install set ## Verification ```bash # Run tests pnpm test # Manual verification: create a fresh worktree without seeding npx paperclipai worktree:make test-optional-skill --no-seed cd ~/paperclip-test-optional-skill eval "$(npx paperclipai worktree env)" npx paperclipai run # Verify paperclip-dev appears in company skill library but is NOT auto-assigned # Call listPaperclipSkillEntries() — paperclip-dev should show required: false # Call resolvePaperclipDesiredSkillNames() — paperclip-dev should NOT be in the default set # Cleanup npx paperclipai worktree:cleanup test-optional-skill ``` ## Risks - Low risk. The `required` field defaults to `true` when absent, so all existing skills behave identically. Only the new `paperclip-dev` skill sets `required: false`. ## Model Used Claude Opus 4.6 (`claude-opus-4-6`) via Claude Code, with tool use and extended context. ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [ ] If this change affects the UI, I have included before/after screenshots - [x] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

[codex] Improve transient recovery and Codex model refresh (#4383)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies > - Adapter execution and retry classification decide whether agent work pauses, retries, or recovers automatically > - Transient provider failures need to be classified precisely so Paperclip does not convert retryable upstream conditions into false hard failures > - At the same time, operators need an up-to-date model list for Codex-backed agents and prompts should nudge agents toward targeted verification instead of repo-wide sweeps > - This pull request tightens transient recovery classification for Claude and Codex, updates the agent prompt guidance, and adds Codex model refresh support end-to-end > - The benefit is better automatic retry behavior plus fresher operator-facing model configuration ## What Changed - added Codex usage-limit retry-window parsing and Claude extra-usage transient classification - normalized the heartbeat transient-recovery contract across adapter executions and heartbeat scheduling - documented that deferred comment wakes only reopen completed issues for human/comment-reopen interactions, while system follow-ups leave closed work closed - updated adapter-utils prompt guidance to prefer targeted verification - added Codex model refresh support in the server route, registry, shared types, and agent config form - added adapter/server tests covering the new parsing, retry scheduling, and model-refresh behavior ## Verification - `pnpm exec vitest run --project @paperclipai/adapter-utils packages/adapter-utils/src/server-utils.test.ts` - `pnpm exec vitest run --project @paperclipai/adapter-claude-local packages/adapters/claude-local/src/server/parse.test.ts` - `pnpm exec vitest run --project @paperclipai/adapter-codex-local packages/adapters/codex-local/src/server/parse.test.ts` - `pnpm exec vitest run --project @paperclipai/server server/src/__tests__/adapter-model-refresh-routes.test.ts server/src/__tests__/adapter-models.test.ts server/src/__tests__/claude-local-execute.test.ts server/src/__tests__/codex-local-execute.test.ts server/src/__tests__/heartbeat-process-recovery.test.ts server/src/__tests__/heartbeat-retry-scheduling.test.ts` ## Risks - Moderate behavior risk: retry classification affects whether runs auto-recover or block, so mistakes here could either suppress needed retries or over-retry real failures - Low workflow risk: deferred comment wake reopening is intentionally scoped to human/comment-reopen interactions so system follow-ups do not revive completed issues unexpectedly > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first. See `CONTRIBUTING.md`. ## Model Used - OpenAI Codex GPT-5-based coding agent with tool use and code execution in the Codex CLI environment ## Checklist - [x] I have included a thinking path that traces from project context to this change - [x] I have specified the model used (with version and capability details) - [x] I have checked ROADMAP.md and confirmed this PR does not duplicate planned core work - [x] I have run tests locally and they pass - [x] I have added or updated tests where applicable - [ ] If this change affects the UI, I have included before/after screenshots - [ ] I have updated relevant documentation to reflect my changes - [x] I have considered and documented any risks above - [x] I will address all Greptile and reviewer comments before requesting merge --------- Co-authored-by: Paperclip <noreply@paperclip.ing> |

[codex] Improve issue thread review flow (#4381)
## Thinking Path > - Paperclip orchestrates AI agents for zero-human companies > - Issue detail is where operators coordinate review, approvals, and follow-up work with active runs > - That thread UI needs to surface blockers, descendants, review handoffs, and reply ergonomics clearly enough for humans to guide agent work > - Several small gaps in the issue-thread flow were making review and navigation clunkier than necessary > - This pull request improves the reply composer, descendant/blocker presentation, interaction folding, and review-request handoff plumbing together as one cohesive issue-thread workflow slice > - The benefit is a cleaner operator review loop without changing the broader task model ## What Changed - restored and refined the floating reply composer behavior in the issue thread - folded expired confirmation interactions and improved post-submit thread scrolling behavior - surfaced descendant issue context and inline blocker/paused-assignee notices on the issue detail view - tightened large-board first paint behavior in `IssuesList` - added loose review-request handoffs through the issue execution-policy/update path and covered them with tests ## Verification - `pnpm vitest run ui/src/pages/IssueDetail.test.tsx` - `pnpm vitest run server/src/__tests__/issues-service.test.ts server/src/__tests__/issue-execution-policy.test.ts` - `pnpm exec vitest run --project @paperclipai/ui ui/src/components/IssueChatThread.test.tsx ui/src/components/IssueProperties.test.tsx ui/src/components/IssuesList.test.tsx ui/src/lib/issue-tree.test.ts ui/src/api/issues.test.ts` - `pnpm exec vitest run --project @paperclipai/adapter-utils packages/adapter-utils/src/server-utils.test.ts` - `pnpm exec vitest run --project @paperclipai/server server/src/__tests__/issue-comment-reopen-routes.test.ts -t "coerces executor handoff patches into workflow-controlled review wakes|wakes the return assignee with execution_changes_requested"` - `pnpm exec vitest run --project @paperclipai/server server/src/__tests__/issue-execution-policy.test.ts server/src/__tests__/issues-service.test.ts` ## Visual Evidence - UI layout changes are covered by the focused issue-thread component and issue-detail tests listed above. Browser screenshots were not attachable from this automated greploop environment, so reviewers should use the running preview for final visual confirmation. ## Risks - Moderate UI-flow risk: these changes touch the issue detail experience in multiple spots, so regressions would most likely show up as thread-layout quirks or incorrect review-handoff behavior > For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and discuss it in `#dev` before opening the PR. Feature PRs that overlap with planned core work may need to be redirected — check the roadmap first.
