DottaandPaperclip 27270e9bf8 fix(server): wake interrupted retry cancellation from committed intent (#17667)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - A parked scheduled retry has no executor to finish an interrupted
Stop request.
> - Its saved cancellation fence already records the work that remains.
> - The orphan reaper currently scans for that work on every heartbeat,
including idle periods.
> - This PR registers the existing records with the shared delivery
scheduler and wakes recovery after committed writes.
> - Recovery keeps the existing cancellation guards and settlement
behavior, then becomes quiet.

## Linked Issues or Issue Description

Refs #17666, the merged preceding PR in the recurring-server-work
migration stack. This PR targets master. Related cancellation work found
in the dedup search: #12301 and #7340. This PR changes the trigger for
interrupted parked-retry cancellation; it does not replace their
cancellation or retry policies.

**What happened?**

The periodic orphan reaper queries scheduled retries with cancellation
fences even when no cancellation needs recovery. Its first-20-row scan
can also stop behind malformed evidence.

**Expected behavior**

Committed cancellation intent wakes bounded recovery. Failed work
retains a retry. Startup reconstructs durable work. Empty or settled
work performs no recurring SQL for this duty.

**Steps to reproduce**

1. Start a server with heartbeat scheduling enabled and no interrupted
retry cancellations.
2. Observe the orphan reaper repeatedly query for cancellation fences.
3. Cancel a scheduled retry and fail the final status write after its
fence commits. Recovery must complete the existing Stop request without
running a provider or dropping its metadata.

**Paperclip version or commit**

acceptance `66107ae0a7ad85fd0476f91c4f8f98ae74a7a048`.

**Deployment mode**

Self-hosted server with the existing single-process transaction signal
model. Local evidence uses PostgreSQL 17.

## What Changed

- Add one domain-owned recovery worker over existing heartbeat run rows.
No new table. It delegates to run control and preserves original
cancellation, budget, and operator metadata.
- Signal cancellation fences, lifecycle transitions, dispatch
deferral/promotion/suppression, relevant settings changes, and deletion
within their owning transactions. Wrap only the ordinary database fence
in a short transaction; provider calls stay outside it.
- Keep startup/manual recovery compatible and remove this duty from the
periodic orphan sweep. Other reaper duties remain for separate PRs.
- Extend the shared scoped worker to retain pending work with a
memory-only admission retry while its owner is paused. Without pending
work, it schedules nothing. Existing GitHub consumers retain their
behavior.
- Add a concurrent idempotent partial index and lifecycle/producer
tests. Enable guarded disposable PostgreSQL URLs in affected test
harnesses.
- Reconstruct one hundred candidates per scope per scheduler pass.
Resume from the saved scope cursor and retain fair twenty-item execution
batches. Manual recovery starts cancellation before reading the full
backlog.
- Require the recovered browser reply to be saved before testing reload.
Keep all original assertions and time limits.
- Record the full producer audit, lifecycle limits, and failed attempts
in `doc/plans/2026-10-25-scheduled-retry-cancellation-sleep-audit.md`.

## Verification

Published head: `405e533394b26215e22d0452e9b0207d3f784a79`. Source
acceptance: `a35fa43b98e2bd378d4a920d2775ce243654838b`. The final commit
changes the audit only.

- All 363 backend cases in ten complete owner and consumer files pass
without skips. This includes actual Stop/retry producers, transaction
settlement, interrupted-write recovery, pause/resume, shutdown, fair
backlog processing and quiet after settlement. Seven separate fresh
PostgreSQL 17 databases prevent fixture interference.
- The two embedded-only backend fixtures temporarily use separate
external databases on this IPC-exhausted host. Their fixture imports and
native client teardown are restored byte for byte afterward. Assertions
and timing limits are unchanged. CI must still pass with the original
embedded fixtures.
- Two paging regressions fail on the prior worker and pass on the fix.
They cover cancellation before full reconstruction, a malformed first
page, fair batches, a newly committed intent behind the cursor, and no
later idle SQL. Per-candidate admission reads remain deliberate safety
checks.
- The repeated CI reload failure is reproduced with a temporary
twelve-second fixture settlement delay. The trace retains the same
five-second missing-reply assertion, even though a cleanup race masked
it in the top-level error. Requiring exactly one saved reply from the
resumed run passes under the identical delay. The fault is removed. All
six browser journeys pass on the final source in 40.5 seconds, with zero
retries and the original 120-second case and five-second reload limits.
- Browser acceptance uses the canonical CLI/server/static-UI path and a
fresh external database. Only fixture database-URI setup is temporarily
adapted and then restored. This is not an unmodified embedded
CI-environment reproduction. The six cases include the controlled case;
repeated runs are not added to the distinct count.
- Actual database build, server typecheck/build, module boundaries and
exact-current migration order pass. The database source tree is
unchanged from the verified 0405-to0406 upgrade: all 404 prior receipts
and 300 companies stay intact, exactly one receipt is added, replay adds
none, and the index is valid on upgraded and fresh databases. The five
snapshots remain 0402 through 0406.
- All earlier failures remain recorded in the audit. Prior head
`48b3fcb83f305616df02a4337a42ec0867bb4b80` had exact-head Greptile 5/5
but failed the repeated browser assertion. It was not retried blindly.
Its completed Ponytail review's backlog concern and old migration
paragraph are addressed here. Old scores do not satisfy new-head gates.
- Current-head CI passes. Exact-head Greptile and Ponytail both give
5/5, with no remaining actionable findings. These checks do not prove
deployed instance or database suspension.

## Risks

- Only supported in-process writers produce local transaction signals.
Independent SQL writers or other replicas need a separate ingress
contract. Startup reconstructs saved obligations.
- A real failure retains bounded retries. Unknown commit outcomes retain
the shared intent hold. Neither is discarded to report quiet.
- Per-owner drain uses a memory-only admission recheck. The shared
helper change affects existing consumers, which have focused regression
coverage.
- The partial index is concurrent and idempotent; it adds index storage.
Each scoped load reads at most one hundred rows. The shared helper
retains discovered pending IDs between passes, so a large unresolved
backlog still uses memory.
- Cloud alarms, idle-safety exemptions, and other reaper duties remain
separate. Merge requires passing current-head CI and review gates.

## Model Used

OpenAI Codex, GPT-6 family, with reasoning, shell tools, and local
PostgreSQL tests. The runtime does not expose a more precise model
identifier or context-window size for this turn.

Devin FoleyandPaperclip 7ebf98eafd fix(recovery): avoid duplicate terminal run status notifications (#17614)
Retain pending status deliveries when Native or legacy terminal recovery projects the same state. Preserve completion time on unchanged Native terminal status. Real state, metadata, error, explicit patch, and retained-file changes still notify. The publisher acknowledgement, accepted results, retry deadlines, recovery holds, ownership checks, and workspace cleanup remain intact.

Correct the queued Retry browser fixture race by releasing its old-process guard at the exact Retry request, requiring a successful response, and retaining fixture cleanup after failure.

Validated by 159 focused tests, 653 wider Native recovery tests, six real Chromium/PostgreSQL browser cases, five queued Retry repeats, build/typecheck, exact-head Apex 5/5, and green required CI. The original detached-button CI timeout is documented in the PR. The additional full local repository Vitest command is still running and is not claimed complete.

DottaandPaperclip d33b40a5c3 fix: recover saved work and provide reliable task Retry (#16965)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - The control plane must retain work when an execution or workspace
export fails.
> - Retained-workspace holds can leave saved input waiting with no
usable Retry action.
> - Users cannot verify process ownership or repair provider
allocations.
> - This pull request gives system recovery a durable budget and gives
each deliberate Retry a durable intent.
> - Safe recovery can preserve files, settle an accepted result, or
prepare an independent workspace.
> - One task state keeps infrastructure diagnostics out of the ordinary
conversation while preserving real permission and approval gates.

## Linked Issues or Issue Description

**What happened?**

A task with a retained workspace can show recovery, failed-run and
saved-message warnings at the same time. Historical runs without native
workspace descriptors cannot use the native restore path. Old recovery
metadata can prevent a new attempt even when an independent workspace
would be safe.

**Expected behavior**

Paperclip owns bounded infrastructure retries and file preservation. A
user can request another attempt with ordinary Retry. Thinking appears
immediately. The service preserves completed effects and current user
controls. It does not claim that starting fresh recovered the old files.

**Steps to reproduce**

1. Retain a failed run and its workspace after export or process-stop
failure.
2. Save a follow-up while the recovery hold is active.
3. Open the task and try to continue without infrastructure credentials
or operator certification.

Refs #16906, #16835, #16883. This extends their recovery and receipt
contracts. Related work: #16962 and #16963.

## What Changed

- Add task-level Retry with company-scoped authorization, all-status
idempotency and durable admission before physical work. Keep exact-run
Retry compatible.
- Add typed recovery phases, finite persisted backoff, historical
adoption and durable local failure diagnostics.
- Preserve native workspaces or verified historical snapshots. Admit an
independent fresh workspace only after execution containment. Keep
old-source preservation and cleanup obligations separate.
- Carry completed publication and API receipts forward. Fence uncertain
mutation retries across replacement runs. Preserve connected-chat
identity and authorization.
- Re-enter normal dispatch for saved input. Reuse accepted results
through the existing finalizer.
- Use one task interruption projection across the thread, composer and
task list. Show optimistic Thinking, ordinary Retry and genuine user
gates.
- Add PostgreSQL, filesystem, API, browser-fixture and controlled
provider coverage. Document the behavior and update the feature/access
maps.

## Verification

- All GitHub Actions CI jobs pass on
`6d8c6d27d50586d8548b31d03b47f6d73a0ee11d`: typecheck, all
server/chat/workspace and serialized suites, Runner checks, eight
browser shards, canary and build. The required merge gate passes.
- Greptile reviewed the final commit at 5/5, with no actionable findings
or unresolved review threads. Its two earlier P1 findings are fixed.
- Full recursive typecheck and build pass locally on the final commit.
All 16 task Retry cases pass against isolated real PostgreSQL. The
unrelated-success case and both stale Retry cases fail on the previous
code and pass after the fix.
- The corrected gateway fixture passes its uncertain-mutation and
independent-read case. Four real HTTP API cases pass for definite
403/422 rejection, completed-effect reuse, and uncertain-effect fences
across deliberate and automatic retries. These focused gateway/API
checks used a temporary database lifecycle harness on an existing
healthy local PostgreSQL process. The standard repository helper is
unchanged.
- Earlier qualification covers 291 task UI cases, saved-file copyback
and ancestry, current Agent Chat ownership races, nine chat Retry API
browser cases, and six assembled browser recovery fixtures. A real
API/PostgreSQL/native-runner journey showed Thinking in 25 ms despite a
delayed response, admitted one successor in a separate workspace,
retained original files, and survived reload.
- The controlled Daytona test interrupts export, joins the controller,
restarts it in a separate process, and verifies one agent start, one
external sentinel operation, retained file contents and no premature
cleanup. It does not establish abrupt-kill or live LLM/GitHub
acceptance.
- The full local `pnpm test:run` was attempted and was not green: an
OAuth row-lock timing assertion failed, and a later suite exhausted
macOS PostgreSQL semaphores. The ten OAuth timing cases passed on rerun
against a separate database. Later embedded database initialization
failures remain recorded; the final CI matrix passes.
- Security scanning passed on production-code commit
`63953edf069b111c6becc3144dffa6deb52f68b5`. Its only successor changes
one test-fixture identity line. The final scan is neutral/inconclusive
because its pinned merge-base checkout failed or timed out. Both rerun
API requests returned 404, so no fresh successful scan is claimed.
Socket and Snyk checks pass on the final head.
- The optional Paperclip Review remains queued behind an earlier run in
workspace finalization. Its prior completed assessment was incomplete
because a comparison tool was blocked by the platform filter. It is not
counted as a completed code review. GitHub does not require this check
for merge.
- Hosted acceptance remains pending deployment. Local and CI results do
not establish recovery of a hosted task or its retained files.

## Risks

- Recovery changes span admission, physical ownership and dispatch.
Incorrect evidence could permit overlapping work. Existing physical
locks and authoritative stop requirements remain in place.
- The migration adds concurrent indexes. It requires the existing
top-level migration runner and retries incomplete index creation safely.
- A fresh attempt can complete while old-source preservation remains
unresolved. The service retains that obligation and does not report
those files as recovered.
- Unknown tool mutations remain blocked until their authoritative
receipt owner resolves them. Generic upstream tools have no universal
result-query protocol.
- Recovery and diagnostics stay inside the stack. No Cloud incident
endpoint, notification worker or new outbound reporting channel is
included.

## Model Used

OpenAI Codex, GPT-6 family. The runtime does not expose a more specific
model version or context-window size. The model used reasoning, shell
execution, source editing and browser tools. No model subagents were
used for this implementation.

DottaandPaperclip 45b67f1d93 feat: default new Apple Silicon Claude agents to Paperclip Runner (#15993)
## Thinking Path

> - Paperclip lets users choose an agent harness and account.
> - The server resolves the runner for new agents.
> - Claude still needs the same ordinary native-default journey as
Codex.
> - Existing agents must retain their runner, model and account.
> - This PR defaults new Apple Silicon Claude agents to Paperclip Runner
with Opus 5.5.
> - Users can connect and work without a runner override.

## Linked Issues or Issue Description

Refs #15683. This PR depends on #15994 and precedes #15997. Related
scope: #15422, #13978 and #15224.

## What Changed

- Resolve new Apple Silicon Claude agents to `paperclip_runner`,
`provider: acpx`, `acpxAgent: claude`. Keep explicit Legacy, models,
provider routing and saved execution.
- Use `claude-opus-5-5` for ordinary Claude defaults and retain explicit
overrides.
- Reuse existing API-key/subscription connections, credential storage
and checks. Generalize the shared Advanced runner control with Claude
branding.
- Carry configured effort into the shared v9 contract. Preserve
compatible permissions, instructions, skills, timeouts and session
behavior. Require Legacy for unsupported settings.
- Qualify a new immutable managed Claude target at explicit Connect
through the existing permission-gated probe. Use fresh scoped discovery
for test and hire. Discard stale results and require an explicit Connect
after settings change.
- Retain merged Codex onboarding recovery, auth preparation,
import/effort corrections and Grok Bot gates.
- Check the Claude first-task wizard's saved default directly. Update
configuration/runtime documentation and exact Opus 5.5 accounting.

- Align hire fixtures with the merged host-environment restriction.
Native agent-authenticated hires cannot inject host env settings; both
creation endpoints verify that no extra agent is saved.

## Verification

- Updated onto master `9179d8dcc3e908c217fde4f63c9d08ccf8132eeb` through
#15994. Native input v9 carries effort while preserving v7 home modes
and v8 Muse. Codex defaults remain unchanged.
- Focused local verification passed 285 server tests, 323 UI tests, 124
setup/registry tests, and two v9 permission-pin transport cases. Direct
server/UI TypeScript checks passed. An initial 30-second hook timeout
was resolved by serial verification with a longer hook budget: all 122
credential/permission cases and 232 snapshot/environment cases pass.
Direct server TypeScript checking also passes.
- Ordinary web creation with the experiment disabled saved native
Claude, Opus 5.5 and High effort without runner/model overrides. The
actual product task and follow-up both succeeded on integrated source
`b2c9fe43f9cd3bc5dd9d22552da0148845ba71a4` with the cloud-built daemon
from `04e0202ca81387b17cbe7fd094a8338648356fe9`. The final master
reconciliation also changes shared runtime source. Historical proof
retains its source identity; a final-source cloud qualification is
running before refreshing the preview daemon.
- Both actual browser downloads were checked: v1 has three sections/six
checkboxes (451 bytes); v2 preserves the original body and adds two
Rollback checks (612 bytes). The original attachment stayed
byte-identical. The follow-up records session resume and the same
provider session. Total estimated API usage: $0.518432800, with no
pending/unpriced usage in the successful fixture.
- Fixed the live-discovered setup incompatibility: native API-key
validation no longer depends on the global Legacy Claude CLI. The
existing provider endpoint verifies the key without a paid prompt;
subscription/custom routing probes remain intact.
- Fixed security review's native-hire credential redirection:
agent-supplied URLs, proxy/launch settings and other host environment
controls are rejected before inheritance. Native credential references
remain supported and partial edits use the saved harness. Added
create/hire/update regression coverage.
- Current-head cloud CI is green. All three stack PRs have fresh
Greptile 5/5 reviews and zero unresolved review threads. Required
typecheck, test and build gates ran in cloud CI; Docker work stayed
remote. Earlier failed checks remain in their original runs.
- Docker and native release builds ran in cloud infrastructure. An
inadvertent local Rust prerequisite was stopped; no local Docker build
was run.

- The final master merge preserves task-loading, permission,
skill-retention and OpenRouter changes. Pi uses a fresh combined profile
27, preserving both historical profile-24 declarations; Copilot remains
profile 22. Focused protocol checks passed 155 cases (one skipped), the
updated Pi attestation check passed 18 (one skipped), and 91
evaluation/catalog checks passed.

- Latest-application manual continuation passed in 37.942 seconds on
`695e5c005a85bddc8711d3fcddcaf6b7e1fb64d0`, with High effort, native
Opus 5.5, Anthropic API attribution, the same resumed Claude session, an
independently checked 635-byte file and persisted Done status. It uses
the earlier verified Mac daemon; the preceding-source package
qualification passed at `dca62c7aeb4b45f0dd91a8e18d386a16a2658ef8`. That
package run predates the small TypeScript startup fix and is not
relabeled as qualification for it.
- Upgrade limitation: changed compiled sidecar identity rejected old
durable launch state and recovery hit session-open bounds before
eventually resuming. That attempt was operator-cancelled and retained;
the successful manual continuation does not qualify seamless upgrades.

- Local latency profile, Opus 5.5/High unchanged: three alternating
real-ACP loopback samples per variant reduce median provider-ready time
from 5.081s to 4.034s. The matched real API task startup improves from
5.533s to 4.348s, but total reply time changes from 17.933s to 20.221s
because agent work varies. This establishes a startup saving of about
one second, not a consistent total-reply speedup. A real candidate
follow-up passed in 20.146s and resumed the same saved session. The
loopback fixture makes no external model requests; failed initial probes
remain retained.
- The startup change passes 107 focused runtime/effort/lease tests, 53
profile checks (one skipped), TypeScript checking and the
TypeScript-only runtime build. Native hire fixture checks pass 70 tests;
their first local run retained a socket-hangup failure and stale
OpenCode expectation, both accounted for before the passing rerun.
Broader required checks run in cloud CI.

## Risks

- Missing packages, authentication rejection, unsupported settings and
unavailable models must fail explicitly. Existing agents retain saved
execution; explicit runner changes use existing revision/session
invalidation.
- Subscription follow-up remains unverified; remaining live acceptance
uses API keys per human steering. Historical accounting holds and the
separate $50 ceiling remain intact.
- The subscription acceptance limit remains explicit for review. No
Claude database migration is introduced.

## Model Used

OpenAI Codex, GPT-6 family, with reasoning, repository tools, code
execution and browser testing. The exact serving model ID and
context-window size are not exposed in this session.

DottaandPaperclip 88b7bbb5e2 Stabilize task lifecycle browser fixtures (#15878)
## Thinking Path

> - Paperclip manages agents and their tasks.
> - Browser and runner tests check task recovery and user actions.
> - Some fixtures act before a prior request or run has settled.
> - Those races can report a product failure before the intended
assertion runs.
> - This PR binds each check to the exact request, run, or geometry
snapshot it needs.
> - The tests retain their existing budgets and negative assertions.

remains pinned to its original commit; current proof files stay with the
UI PR.

This is the test-only prerequisite extracted from the experimental Boat
work. It contains five existing test-file changes. It does not add the
Boat adapter or change task admission.

## What Changed

- Wait for the exact signoff fixture run and each replacement run to
succeed before applying the next stage decision.
- Wait for the inbox Retry response for the exact failed run before
navigating away.
- Compare task planning time with the first run that actually started,
instead of assuming the first list item started.
- Read the touch target, container, and scroll position in one browser
evaluation. Keep strict containment and native gesture checks.
- Bind the native recovery fixture to its disposable fake Codex
executable so installed provider binaries cannot override it.

## Verification

Draft branch: `codex/boat-fixture-prerequisite`, commit
`e318850909c0777565b43644a4d8114c6006da60`. Base: master
`e733bddc45e00efb1819fffa8677f7b0d07802c5`. Five changed files.

The isolated fixture-plus-runtime stack passed 24 browser cases. Six
legacy recovery cases could not reach their assertions because the
fixture requires embedded PostgreSQL metadata. A normal embedded
PostgreSQL attempt then failed during cluster setup on this host. These
are not six passing cases. No test seam, timeout increase, or
product-database fixture was used.

The fixture-only commit has not yet had a separate complete local
browser run. All six legacy cases subsequently passed on this exact
fixture commit in PR CI browser shard 8 (20/20 total, job114470999277).
The original local setup failures remain recorded. The full-stack
typecheck/build results belong to the assembled fixture-plus-runtime
tree and are not claimed as independent qualification of this smaller
commit.

Review the exact run/request filters and the unchanged rejection
assertions. Run the four changed Playwright specs and the
native-session-resume suite through the repository's normal fixture
setup. At 13:03:24 UTC, this exact head had 53 successful checks and two
skips, with no pending checks or failures. All eight browser shards
passed. Greptile is 5/5 on the exact head with no unresolved threads.

[Exact-head legacy browser
proof](https://github.com/paperclipai/paperclip/blob/9eacca2c5321bb63f061eef4bb775b6951d5846d/doc/plans/assets/2026-10-10-boat/evidence/prerequisite-legacy-browser-ci-proof.json).

Feature acceptance stays in the downstream report. Deployment29 verified
`05f3725af838b5a074707df1d6ed1e193bdfe49e` at 12:23:23.944 UTC. Native
Claude cold execution passed at 12:39:57.782 after 14m56s of first-use
preparation. Warm continuation passed at 12:42:09.943 with the same
owner, boot and session identities. Normal retirement and archive
followed. The ordinary saved post-idle turn then failed: its primary
attempt reported the closed `LINK_COUNT` module fence at
`turn_ownership_verified` (3,711ms), and same-run recovery settled
failed at 12:52:09 with identity/authority errors. Natural archive
completed at 12:52:21. Numeric link counts and their direction are not
yet proven. A narrower failure-only diagnostic and a bounded read-only
probe are under review; neither establishes a fix for this live failure.
Prior staging27/28 failures remain preserved. Local complete lifecycle
passes and staging Codex/computer/HMR passes remain separate
source-bound evidence. No session, quarantine, provider stop or
migration history was reset. These live observations do not qualify this
prerequisite by themselves.

## Risks

- Waiting for the wrong run could hide a task race. The fixtures match
task, agent, and replacement-run identity before proceeding.
- A geometry helper could weaken touch coverage. This change preserves
containment and gesture assertions and only makes their inputs one
snapshot.
- The six legacy browser cases passed in exact-head CI; their local
PostgreSQL setup limitation remains recorded. No live Boat acceptance
result follows from this test-only change.

Documentation impact: no product command or behavior changes. This
description records fixture scope and validation limits.

## Model Used

OpenAI Codex (GPT-6 family) assisted with code, test execution, and
review. The host does not expose the exact primary model ID or context
window. The work used tools for source inspection and local execution.
Prior architecture work used `gpt-6-astra`; an independent design review
used `gpt-6.1-sol`.

Nicky LeachandPaperclip 2dd9811e8b Own agent hiring and termination in a lifecycle module (#15631)
## Thinking Path

> - Paperclip manages AI agents and their work.
> - Agent creation, approval, pause, and termination change whether an
agent can work.
> - These changes currently use separate service and database paths.
> - Some agents need setup after their record exists and before their
harness can run.
> - This PR adds one module that owns those state changes and their
transactions.
> - The module records progress and retries incomplete work after a
server restart.

## Linked Issues or Issue Description

**Problem or motivation**

A hire can become active before its execution environment is ready.
Direct agent state writes also bypass one common lifecycle boundary.
Termination does not have a durable step for resource cleanup.

**Proposed solution**

Use agent lifecycle commands for hiring, approval, pause, resume, and
termination. Keep proposed hires pending until approval. Prepare each
approved hire, test its saved harness configuration, and then permit
work. Wait for required plugin work before completing a step.

**Alternatives considered**

A general database commit hook would expose transaction details to
callers. A timer alone would delay each new hire. This module owns each
command transaction and starts its worker after commit. A periodic scan
recovers work after a process stops.

**Roadmap alignment**

This change adds the lifecycle boundary for the existing agent and
plugin systems. It does not add a user interface or a provider
implementation. It replaces the lifecycle delivery approach in #15391.
Related proposals include #11475 and #13099; this change does not add
their external executor protocols.

## What Changed

- Add durable approval, preparation, verification, pause, resume, stop,
and cleanup states.
- Keep agent creation, lifecycle state, configuration writes, and agent
record deletion behind `modules/agent-lifecycle`. Keep validation,
credential rules, reads, onboarding, invitations, general approvals, and
company data deletion in their services.
- Commit a hire before the remaining onboarding or invitation work.
Serialize requests with the existing dedicated database connection
helper. Reuse the hire after a failed attempt.
- Keep accepted work in the run queue during setup. Require execution
and cleanup evidence before completing pause or termination.
- Retain the original credential owner when approving migrated hires.
- Start lifecycle work after each command commits. Use database leases
and revision checks for retries and late results.
- Add a generic required plugin method and capability. Store the
required plugin IDs for each agent. Track host and plugin completion
separately.
- Keep harness verification and credential rules in services. Supply
their results to lifecycle through explicit ports. Reject failed logins
and incomplete probes.
- Remove reverse imports from lifecycle adapters into services. Share
record queries and validation below both layers.
- Use one hire decision path for agent IDs and approval IDs. Resolve the
approval and agent change in one transaction. Expose policy
reconciliation on the command object.
- Save configuration and verification invalidation through one module
operation. Ordinary commands own their transaction. Restrict the shared
transaction integration to its listed callers. Reject identity, company,
accounting, timestamp, and unknown fields.
- Require completed termination before deleting agent records. The
company deletion service calls the module through a separate deletion
interface.
- Save an approved secret before applying its agent binding. A failed
binding leaves the secret approved. A retry reuses it.
- Remove old permission and metadata export files. Move hire
configuration rules out of lifecycle persistence.
- Report failed worker steps and scans with operation context and known
error codes. Omit provider messages, stacks, and configuration values.
Document ordinary application use and extension rules.
- Recover expired native owners only after execution stops. Retry
cleanup of failed retained leases. Wait for setup in the live smoke
script.
- Keep manual, budget, and company pause reasons separate. Guard
execution status writes with the ready state.
- Add a migration, boundary checks, regression tests, and a lifecycle
guide. Keep the current UI.
- Limit periodic policy checks to 100 eligible agents per minute.
Continue from the last agent ID. Exclude pending hires and agents in
termination or a final state. Keep explicit company and budget updates
immediate.

## Verification

- Latest head `ccc3c86b0` aligns the shared status update schema with
the PATCH route. All 15 shared tests, 104 server route/OpenAPI tests, 2
CLI tests, and shared/server/CLI TypeScript checks pass. The Greptile
comment is fixed. CI is running; no new Apex review was requested for
this small fix.

- Rebase `75708ac94` rebases onto master at `2eb5187e4`. The backup test
uses the shared 90-second timeout from master; the older timeout commit
was removed. The affected backup test passes.
- Agent budget edits now pass the target agent ID through enforcement.
All 50 affected tests, server TypeScript, and 11 boundary tests pass.
Both configuration update paths have regression coverage. CI is running
on the latest head. No new Apex review was requested for these small
fixes.

- Rebase `9b568666a` preserves the run-control extraction and
delivery-work startup changes from master. All 559 affected tests,
server TypeScript, and 11 boundary tests pass. CI and Apex review are
running on the final head.
- The policy scan fix passes 56 lifecycle and diagnostic tests, 24
policy and caller tests, and 11 boundary tests. A regression test checks
the page limit, interval, excluded states, and cursor wraparound.

- Rebase `fd4698750` passes all 611 tests in eight affected lifecycle,
route, and heartbeat suites. Server TypeScript and all 11 module
boundary tests pass. The company deletion rollback and concurrent hire
tests are included.

- Follow-up `00aac7646` fixes the failed decision deletion test and the
public update schema. All 126 affected server tests, 8 shared tests, 2
CLI tests, and 11 boundary tests pass. Shared, server, CLI, and UI
TypeScript checks pass.

- Server TypeScript passes: `pnpm --filter @paperclipai/server exec tsc
--noEmit`.
- All 11 boundary tests pass: `node --test
scripts/check-module-boundaries.test.mjs`. The repository boundary scan
also passes.
- All 68 lifecycle, failure diagnostic, and harness verification tests
pass. Tests reject forbidden fields through the root command, combined
transition command, and restricted transaction integration. They also
check invalid fields returned by configuration preparation.
- Failure tests cover host work, plugin work, claims, renewals,
deferrals, and scans. Log tests check known error codes and exclusion of
private error contents.
- All 355 tests in 10 existing caller suites pass. These tests cover
secret approval, pairing, pending hires, credentials, company import,
built-in agents, managed agents, instructions, and skills.
- The branch is rebased onto `origin/master` at `2eb5187e4`. The queue
guards and cancellation lock fix use the extracted heartbeat queue.
Agent cancellation uses the extracted run-control file. The source diff
scan found no secrets or private URLs.
- Earlier checks cover approval retries, rollback, late verification
results, policy holds, deletion rollback, concurrent company deletion,
and queue admission during setup.
- Before the rebase, CI passed on `00aac7646` and Greptile Apex rated it
5/5 with no open comments. CI and a new Apex review are pending for the
rebased commit. The branch has no merge conflicts.
- The full local build and native Dot suite were not run. The host has
no Cargo. No live provider test was run.

## Risks

- Background policy recovery takes more than one minute when more than
100 agents are eligible. Explicit company and budget updates still
reconcile immediately.

- The restricted configuration integration keeps an agent reference and
its dependent records in one transaction. It cannot change a budget or
invoke a lifecycle transition. A periodic scan starts pending setup
after that transaction commits.
- Secret approval and binding use separate transactions. A failed
binding can leave an approved secret with no agent binding.
- New approved hires remain unavailable until setup and verification
complete. Existing clients see `paused` during these steps. This PR does
not change the UI.
- Migration `0320_daily_onslaught.sql` maps existing statuses and pause
reasons. It does not create external resources or claim that an existing
harness passed verification.
- Use a coordinated server update for this migration. Older server
versions can write legacy status without the new lifecycle checks.
- A failed onboarding or invitation request can leave a committed hire.
A retry reuses that hire and completes the remaining records.
- Deletion requires completed termination. Company deletion can require
another request while cleanup is pending.
- Required plugins must make repeated calls safe and reject older
revisions. A disabled or removed required plugin blocks completion until
it is restored.
- A new hire can run a second harness test after the existing UI test.
Some adapters can charge for this test.
- No live provider test was run. Provider implementation and resource
backfill remain separate work.

DottaandPaperclip 7a52dcdc74 fix: repair MCP validation and cancelled execution recovery (#14951)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - The tool gateway gives agents access to connected services. Recovery
controls what happens when a run stops.
> - Generated tool names can exceed the provider limit after the MCP
client adds its prefix.
> - The same invalid definition can fail each automatic retry. A
cancelled run can also hold saved messages without showing its cause.
> - This pull request bounds tool names, stops configuration retries,
and retains cancellation evidence.
> - It shows the stopped run and admits saved input only after the
existing safety checks pass.
> - The benefit is a clear recovery path that preserves operator Stop
and prevents duplicate message delivery.

## Linked Issues or Issue Description

**What happened?**

A long connected MCP tool name makes the provider reject the entire
request. Automatic recovery repeats the invalid request. Separately,
unexpected legacy cancellations can leave saved input behind a recovery
hold. The notice does not identify the stopped run or its cause.

**Expected behavior**

Complete MCP names fit the provider limit. Tool-definition errors
require configuration repair. Cancelled runs retain their source and
reason. The recovery notice shows the cause and saved-message count.
Verified unexpected cancellations can start a fresh turn through the
existing admission checks.

**Steps to reproduce**

1. Assign an App gallery connection with a long application key and tool
name to a Claude agent.
2. Start a run. The provider rejects a name over 128 characters,
including its MCP prefix.
3. For cancellation recovery, stop a legacy provider turn without an
operator Stop request and send a user message while the recovery hold is
active.
4. Inspect the recovery notice and the deferred message queue.

**Paperclip version or commit**

Rebased onto master at `cf8ad63c806685bfd7c48e3ed4a919d61a7c55f1`.

**Deployment mode**

Hosted or self-hosted server with legacy Claude or Codex execution.

Related public work:

- Refs #14017. That PR caps name segments. This PR preserves existing
short names and uses stable hash aliases for long complete names. It
also covers classification and recovery.
- Refs #4510. That PR adds a cancellation-source column. This PR records
bounded evidence in the existing run result, without a migration.
- Refs #12552 and #4506. Those PRs suppress recovery after operator
cancellation. This PR preserves operator intent and uses the existing
continuation gates.

## What Changed

- Bound gateway names with the full provider prefix in the 128-character
budget. Retain the original upstream tool name for dispatch and
permissions.
- Classify invalid tool definitions as configuration failures before
diagnostic redaction. Stop automatic retries and continuation attempts
for that error code.
- Persist cancellation source, expectedness, initiator, reason, and
time. Preserve recorded Stop intent when adapter results arrive. Report
unexpected started cancellations with closed diagnostic labels.
- Show the run cause, saved-message count, and Inspect run link. Offer
Continue for eligible unexpected cancellations. Require verified
provider stop, empty tool inventory, ownership, and the existing pause,
budget, approval, and dependency gates. Use the existing queue for
single delivery.
- Add regression coverage and update the execution, MCP gateway, and
run-log documentation.

## Verification

- `pnpm -r typecheck` and `pnpm build` passed.
- `pnpm check:token-gates` passed.
- Ran `pnpm test:run` and completed its workspace and serialized groups.
Initial resource and timing failures passed on isolated reruns. All 149
serialized route suites passed.
- Reran the changed server, adapter, and UI suites after the rebase.
Coverage includes long-name upstream dispatch, configuration retry
suppression, cancellation evidence retention, privacy labels, oversized
run projection, and concurrent saved-message delivery.
- `pnpm test:e2e tests/e2e/legacy-failure-continuation.spec.ts` passed
all six browser scenarios. The recovery notice shows the run cause and
inspection link, and each recovery entry point reaches one new response.
- Added database-backed checks for active, removed, paused, unavailable,
and disabled chat connections. The final continuation and
recovery-notice suites passed 167 tests. Externally bound chats hide
board Continue and show a usable next action.
- All 55 GitHub checks passed on
`42afbf1371dcaeb72646e3d8f65c19ff7cddf8de`. Two unrelated Storybook jobs
were skipped by their normal conditions. Greptile reviewed that commit
at 5/5 with no findings and no open review threads.

## Risks

- Long tool names change to aliases. Existing short names stay
compatible. The original connection and upstream name remain the
dispatch authority.
- Invalid tool definitions no longer get automatic retries. An operator
must repair the configuration before a new attempt.
- Continuation changes apply only to positively identified unexpected
legacy cancellations with complete empty tool inventory. Operator Stop,
unknown historical cancellations, outstanding tools, and unverified
provider termination keep their holds.
- No database migration. The added projection fields are optional.
Cancellation reason and initiator IDs remain local run evidence; Sentry
receives only closed source and initiator-type labels and expectedness.

## Model Used

- OpenAI GPT-6 through Codex, with reasoning, repository editing, shell
execution, and GitHub tool use. The runtime does not expose the exact
model variant or context-window size.

DottaandPaperclip 422287eecd fix: preserve runner recovery, warm sessions, and task outcomes (#13338)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - The native runner connects task messages, provider execution, and
task outcomes.
> - First-time user tests exposed gaps in recovery, completion
permissions, message delivery, and Stop behavior.
> - These gaps left usable output hidden, completed work waiting for
bookkeeping, or safe work unable to continue.
> - This pull request fixes the shared lifecycle and receipt paths while
preserving process ownership and action checks.
> - Users can continue work with accurate task state and durable
messages.

## Linked Issues or Issue Description

**What happened?**

A stopped local Codex execution could remain blocked even after its
processes had stopped and its complete transcript proved that no
external action needed replay. Claude under Conservative permissions
could fail to call task completion tools. Recovery could reuse an
assistant item ID and overwrite prior output. A delivered comment could
remain marked uncertain after navigation. Stop could look like Pause or
a new recovery incident. Workspace contention could look like
cancellation. A direct reply reopening Done could enter a clarification
loop.

**Expected behavior**

Recover automatically only with verified termination and complete action
receipts. Preserve answers and messages. Keep task completion available
under Conservative permissions without broad tool access. Show crashes
as Blocked, actual human decisions as In Review, and ordinary workspace
contention as waiting. Stop the current response and allow a new
direction.

**Steps to reproduce**

1. Create ordinary response tasks with local Codex and Claude Code, then
send follow-up messages through the task composer.
2. Interrupt a disposable local Codex runner during text-only work.
Verify automatic continuation and retained output.
3. Stop a response, send a new request, answer a clarification, and
reopen completed work with another message.
4. Navigate or reload while a comment submission is pending. Confirm the
exact persisted request receipt settles it without removing newer draft
text.
5. Run two tasks in a shared Daytona workspace. Confirm waiting does not
appear as failure.

**Paperclip version or commit**

Initial acceptance baseline: `c9021c6721f91e2c74bd9fee9d3fd41c999d17b7`.
Current integration base: `6cef9743c`. Both operator-interruption and
workspace-waiting guards are preserved; native restart and legacy
permission rules remain documented.

**Deployment mode**

An isolated source-built test-drive instance, with real local Codex and
Claude Code providers and disposable Daytona environments.

Related work: #13314, #13316, #13327, #13344, #13239, #13254, #13163.
This PR addresses additional failures from ordinary task journeys,
including controller restart handoff and repeated warm sandbox setup.
Historical task status reconciliation is excluded.

## What Changed

- Persist runner ownership immediately at spawn and resume an explicitly
adopted runner even when the controller crashed before the first driver
checkpoint. Detach the controller safely across graceful restarts,
including session startup. Prevent an old finalizer from suspending or
signaling an adopted runner. Checkpoint idle warm sessions before
shutdown. Preserve the same run and queued follow-up messages.
- Scope saved legacy queue successor checks to the queue owner while
preserving ordinary task locks, operator identity, assignment gates, and
exactly-once delivery.
- Preserve managed Codex credential files when an old session is
detached for restart; normal owned cleanup still copies refreshed auth
back and removes the scoped copy.
- Reuse the bound warm shared sandbox and fully verify an existing
staged provider pack before using it. This avoids repeated uploads when
the pack is already valid.
- Add a narrow local Codex replacement path with stopped-process proof,
a closed transcript inventory, exact completion receipts, and
fresh-session lineage. Preserve no-replay holds when evidence is
incomplete. Recovery may clear only the same run's recorded Blocked
status version; manual re-blocking and dependency changes invalidate
that receipt, while queued comments do not. Later blocks stop scheduled,
queued, and final dispatch; queued/final checks re-read dependencies
even when the task status stays In Progress.
- Permit only task delivery and human-input tools through the isolated
Claude runner's exact task bridge.
- Scope assistant item identity to the provider turn and ignore only
authority-free Codex skill-change notifications during startup.
- Reconcile composer submissions by client request ID across response
loss, navigation, and reload. Retain text typed during delivery.
- Keep acknowledged run-only Stop neutral and show workspace contention
as waiting. Project exhausted native failures as Blocked.
- Restore the guarded task-page retry action for failed legacy runs,
including the server-supported explicit new-attempt path for stopped
conversation adapters. Preserve native/process recovery holds and avoid
promising Retry while a decision or execution gate hides it.
- Refresh delivered artifacts and handle direct user replies that reopen
completed work without a clarification loop.
- Check the embedded PostgreSQL PID, data directory, and actual port
before connecting or migrating.
- Document accepted behavior and add focused regressions at lifecycle,
route, transcript, and UI boundaries.

## Verification

- Final head `fece606ac2` passes the complete GitHub CI matrix: **34
green checks, two expected Storybook skips, no failures or pending
checks**, including `ci / verify`, `ci / e2e`, full runner verification,
typecheck, build, every server/workspace shard, and all browser shards.
[CI
run](https://github.com/paperclipai/paperclip/actions/runs/34727183287).
Greptile is **5/5 with no open findings**. The final two commits only
refine test fixtures; both affected suites pass 24/24 locally and in CI,
with server typecheck green.
- Complete local Vitest coverage uses the canonical groups/shards: all
635 general server suites, all 145 serialized suites, and all workspace
packages. The aggregate began on `0a8001c18` while the final queue fix
arrived: 23,903 passed, five failed, 87 skipped. The five
port/socket/timing failures passed unchanged in follow-ups (60 tests in
the exposure/file suites and 412 tests covering the serialized failures
and unrun tails). The final queue/operator-identity suites separately
passed 52/52. This is aggregate coverage plus explicit reruns, not a
pristine single-command final-head run.
- After integration with current master,
queue/operator-identity/continuation suites passed 162/162 and affected
UI suites passed 140/140. ACP Stop/continuation and legacy
task/Inbox/message browser suites passed 9/9, including both task
recovery Retry and thread Try again, automatic saved-message delivery,
exactly one new run, Done, and retained output after reload. The default
process Stop/Pause/Resume browser case passed (the native-provider case
is opt-in and skipped by default). The complete Board attachment/receipt
browser suite passed 11/11 on a disposable instance, covering both
composers, exact receipts after lost responses, no replay, bound
attachments, and newer drafts after reload.
- Blocking-intent regressions cover pre-existing Blocked, a mismatched
run/cause, an explicit manual re-block, changed dependencies, a queued
comment after failure, and a block arriving between scheduling and
provider dispatch. The negative cases reproduced before the fix. All 478
affected executor/recovery/dispatch tests passed; both database suites
ran separately after availability-probe skips in the first combined
command. The final late-dependency check passed all 143 affected
recovery/dispatch tests (zero skips) after two new negative cases
reproduced the bug.
- Focused runtime regressions cover awaited runner ownership
publication, authenticated adoption before the first checkpoint,
old-finalizer detachment, idle and busy warm-session shutdown, rejected
checkpoint propagation, provider-pack verification, and managed-Codex
credential preservation. Four managed credential detachment cases
reproduced the bug before the fix; normal owned cleanup still succeeds
exactly once.
- Live local Claude: SIGKILL 2.6 seconds into startup recovered the same
run automatically in 53 seconds, then a normal follow-up completed in 24
seconds. SIGTERM 2.5 seconds into startup preserved the same run (54
seconds) and its queued follow-up (21 seconds). Answers remained visible
and the task reached Done.
- Live Claude Daytona: a warm follow-up retained its sandbox and fell
from 121 seconds to 44 seconds. A separate cold turn took 127 seconds;
after controller shutdown and checkpointing, its follow-up completed in
33 seconds with the same sandbox, workspace, native session, and runner.
Both answers remained visible and the task was Done.
- Other live journeys covered task completion and follow-up with local
and Daytona Codex, local Codex crash recovery, Stop then new direction,
clarification response, live artifact refresh, and shared-workspace
waiting.
- Validation limits: the opt-in native composer Stop/Pause→subtree
Resume fixture exposes terminal/result ordering and subtree-cancellation
attribution bugs that can leave a child task blocked; that new finding
is assigned to a separate follow-up and is not claimed fixed here.
Default CI skips this optional native-provider fixture. Managed-Codex
credential handoff and the queue-agent integration use automated
regression evidence. Cold custom provider-pack uploads still add startup
latency.

## Risks

- Automatic replacement remains deliberately narrow: local Codex,
verified stopped identities, unchanged retained state, and a complete
text/completion-only turn. Unknown actions, partial history, or changed
ownership remain blocked.
- Claude completion permission handling changes an upstream package
patch. The exact isolated task bridge must remain pinned; unrelated
tools keep their existing permissions.
- New task failure projection changes user-visible status. No historical
status backfill or database migration is included.
- This is a broad lifecycle fix across server and UI. Live proof covers
graceful local Claude restart during startup and idle Claude Daytona
session recovery across controller shutdown. Live abrupt SIGKILL during
local Claude startup also recovered the same run. Unknown ownership or
missing action evidence still blocks reuse. Cold custom provider-pack
uploads still add startup latency; this change avoids unnecessary repeat
uploads.

## Model Used

OpenAI GPT-6 (Codex), with reasoning, code execution, browser
automation, and tool use. The exact hosted model ID and context window
are not exposed in this task.

DottaandPaperclip 6cef9743c0 fix: deliver saved user messages after recovery stops (#13327)
Deliver saved user messages after legacy recovery stops. Validate undelivered comments and queue ownership under the task lock, preserve the operator identity checks from #13315, and prevent duplicate successors.

Add a recovery notice with Retry and inline errors, plus service, route, component, and browser coverage.

DottaandPaperclip 9031516a7e fix: recover legacy Daytona startup failures from task and inbox (#13272)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Legacy conversation adapters can run in Daytona sandboxes.
> - A server restart during provisioning can occur before the invocation
event exists.
> - Recovery then lacks the old adapter identity and leaves a hold that
ordinary user retries cannot clear.
> - A remote launch can also fail when its host relay looks for Node in
the sandbox PATH.
> - This pull request records the adapter at claim time and restores
explicit user continuation after verified cleanup.
> - Users can recover from the task or inbox while the failed run and
uncertain action history remain intact.

## Linked Issues or Issue Description

Refs #13237, #13239, #13254. Those changes cover recorded conversation
runs, native user continuation, and explicit remote Stop. This change
covers legacy failure before `adapter.invoke` and exact task/inbox
Retry.

Refs #9771 for overlapping generated-command quoting. This change also
supplies the absolute host Node executable. Refs #13163 and #13264 for
the separate native restart and retained-workspace work.

**What happened?**
A legacy Daytona run interrupted during provisioning became
`process_lost` without an invocation event. Recovery preserved an
execution hold, and Retry or a new task reply could not resume it.
Cleanup could also run before the Daytona plugin was ready. On a macOS
host, a subsequent ACP relay launch failed with `env: node: No such file
or directory` because the remote launch environment did not contain the
host Node path.

**Expected behavior**
An interrupted conversation can continue after its previous execution
stops. Explicit Retry and new user replies should start a fresh turn
with the task history. Cleanup failures must remain visible and
recoverable. The host relay must use the host Node executable.

**Steps to reproduce**
1. Use a legacy Claude adapter with a Daytona environment.
2. Interrupt the server after it acquires the sandbox lease and before
it records `adapter.invoke`.
3. Restart and inspect the task hold.
4. Retry from the task or inbox, or send a new task reply.
5. Confirm the old sandbox has stopped and one new response arrives.

**Paperclip version or commit**
Reproduced from master at `3bafac12f796fbea02e609e1074a9639f872e9c4`.
The branch is rebased on `51b0e01ea`, including #13261 and #13270.

**Deployment mode**
Built from source on macOS with a real Daytona sandbox and the legacy
Claude ACP adapter.

## What Changed

- Count new browser specs with the scheduler's median duration in the
shard-balance check. This fixes a false policy failure after new specs
arrive from both branches. The balance threshold is unchanged.

- Persist server-owned adapter identity in the queued-to-running claim
before provisioning starts.
- Wait for provider plugin startup before restart cleanup. Keep failed
cleanup leases as active ownership blockers.
- Admit exact board retries and new user comments after verified
termination. Retain the old run, task history, approvals, and unknown
action outcomes.
- Adopt repeated Retry requests. Permit one scoped cleanup attempt per
explicit user Retry after the automatic limit, with an activity record.
A later user Retry can recover after a transient provider failure;
automatic attempts remain capped.
- Resume replies deferred during cleanup, including historical legacy
startup failures.
- Launch the host ACP relay through the absolute host Node executable.
- Add a task-level Retry button and return actionable blockers when
retry admission is refused.
- Add database regressions and three browser recovery journeys. Exclude
installed third-party dependency skills from the shipped-skill audit.

## Verification

- Current head: `d23c84181`, rebased on `51b0e01ea`. Conflict resolution
retains the saved-message recovery, local stop receipts, and wait
reasons from #13270 alongside exact legacy Retry support.
- Real Daytona: interrupted the server after lease acquisition and
before adapter invocation. Restart cleanup confirmed provider
termination. Task Retry cleared a seeded historical hold and a real
Claude agent returned `Recovery verified.` in the task. Removed the
disposable sandbox and environment after testing.
- All three browser recovery journeys passed again after the final
rebase. Task Retry, Inbox Retry, and a new reply each produced one fresh
successor, completed the task, preserved the failed run, and retained
the answer after reload.
- All 29 e2e/server shard-partition tests passed. The balance check now
uses the scheduler's median fallback for unmeasured specs, with the same
balance threshold.
- Server typecheck passed after rebuilding the generated runner
dependencies. The combined recovery/route run passed 136 of 137 tests.
Its remaining route test timed out during the first cold module import
at its explicit 10-second limit; an isolated rerun reproduced that
timeout and passed the other 51 route cases. The complete CI suite
passed on this head. The same route file passed all 52 cases in CI,
including the first cold import in 7.5 seconds.
- Before the final rebase, recursive typecheck, full build, UI token
gates, 132 targeted server tests, and the complete [CI
workflow](https://github.com/paperclipai/paperclip/actions/runs/34650004085)
passed. The subsequent CI failure was the shard-balance accounting
mismatch fixed here.
- Greptile reviewed `d23c84181` at 5/5 with no outstanding actionable
findings. The complete [current CI
workflow](https://github.com/paperclipai/paperclip/actions/runs/34653327949)
passed on attempt 2. All test, typecheck, build, and canary jobs passed
on the first attempt. Docker setup timed out fetching BuildKit from
Docker Hub; retrying that job and its dependent aggregate succeeded.

## Risks

- Recovery admission changes executable authority. Company, task, agent,
user, approvals, process ownership, and provider termination checks
remain required.
- Explicit continuation starts a fresh conversation with history. It
does not certify unknown external action outcomes or rerun
non-conversation adapters automatically.
- Changing task status alone does not clear an execution hold. The task
now offers an explicit Retry action.
- Historical adapter claims and invocation events take precedence over
current agent settings. Known process or webhook runs retain their hold.
Pre-upgrade rows with no adapter evidence may receive only a new
explicit user turn after termination proof; they do not become eligible
for automatic replay.
- No schema migration or sandbox-image change is required. This branch
has not been deployed to production.

## Model Used

OpenAI GPT-6 through Codex, with repository inspection, code execution,
browser automation, and test execution. The exact deployment model ID
and context window are not exposed in this session.
