79ce86630a fix(server): schedule governed review response recovery (#17702)
## Thinking Path

> - Paperclip manages agents, tasks and their external chat responses.
> - A completed chat turn can owe a response while an earlier review
gate remains pending.
> - Periodic recovery revisits that response history even when no work
can advance.
> - This change uses the existing delivery coordinator and saved retry
deadlines.
> - Approval, interaction and authority changes wake the affected issue
or run after commit.
> - The existing presentation owner verifies the durable result and
current authority.
> - Settled and dormant history becomes quiet without losing future
eligible responses.

## Linked Issues or Issue Description

Refs #17696 (merged) and #15625. This is slice 49, now based on master.
It owns governed-review response presentation. Accepted responses and
pending decision effects keep their existing owners.

**What happened?**

Native finalization recovery repeatedly inspects committed review
responses. Removing the scan without covering approval, interaction,
task-policy and authority changes would strand responses that become
eligible later.

**Expected behavior**

Recover an owed response once from existing durable facts. Enforce
authority at publication, wake after relevant outer transactions commit,
retain real retry deadlines and restart/drain recovery, and stop
recurring SQL when no response can advance.

**Steps to reproduce**

Complete an external chat turn while an earlier completion review
remains pending. Block its response behind an approval or interaction,
then resolve that blocker. The response must appear once without a
periodic scan or resolving the original review gate.

## What Changed

- Add a scoped governed-review worker using the existing coordinator,
finalizations, decisions and comment receipts. No queue table or
transaction wrapper is added.
- Keep recovery and publication policy in the native finalizer. Preserve
binding, source/session authority, ownership, deadline and
deleted-comment fences; match coordinator-before-issue lock ordering.
- Notify through actual approval, interaction, OAuth, task-policy and
lifecycle producers before their owned writes. Serialize approval
resolution with task linking so a concurrent link retains its wake.
Nested transactions still settle at outer commit.
- Scope ordinary milestones, finalization and coalesced input to their
issue/run. Use the explicit company authority boundary for genuine
global restoration, including membership changes. Both response workers
share this boundary.
- Register under maintenance and drain gates, disable the two periodic
repair entrances, and retain fresh finalization and explicit recovery.
Add the concurrent 0413 comment-receipt index and migration
registration; retain five snapshots.
- Preserve the producer inventory, source qualifications and earlier
failures in the owner audit. Retain a real Photon regression proving
routine ownership assertions stay quiet and concurrent authority
restoration wakes each worker once.

## Verification

- Scoped repair source `2341714720b5c273f0ce011b48345af0ef39227f` passes
**2053 distinct cases**: 1703 in 15 complete producer/authority/recovery
files, including all 273 governed-response cases and 95 wake-queue
adapter cases, plus 350 selected chat cases. Another 1463 chat cases are
outside that selection. Thirteen fresh PostgreSQL 17 fixtures migrate
through unchanged 0413; the one temporary queue-fixture import restores
exactly. All sixteen captured hashes match.
- Two actual repeated coarse-hint SQL regressions fail before repair and
pass afterward. Actual membership restoration passes before and after.
The same assertions and time limits prove dormant work stays quiet and
real authority restoration still publishes once. Real coalesced input
covers both queues after commit.
- Combined source `586f5674cd9798b33cd22437255e0480b5726d20`, including
independent master #18129, passes **1297 distinct cases**: 1093 in five
complete server files, 185 in five runner files, 14 Node protocol cases
and five UI registry cases. This includes the complete 22-case Photon
file for both response workers. The external Photon transport is mocked;
Paperclip callbacks, transaction fences and observed SQL are real. The
earlier and combined case counts are separate, with no cumulative
double-counting.
- A normal merge retains both complete owned and independent master
binary patches. All twelve new-context hashes and sixteen earlier repair
hashes match. Three dedicated fresh 0413 fixtures plus the live-event
helper's own fresh database use PostgreSQL 17 without source
adaptations. Database build, server types/build, boundaries, full-SHA
migration order and generated TS/Rust profiles pass. The new inherited
Runner vendor inputs rebuild successfully.
- Database bytes remain identical to the previously qualified 0413
source. The recorded populated 0412-to0413 upgrade retains all 411 prior
receipts and 38 companies, appends one receipt and none on replay; the
index is valid and ready. The earlier 2828-case acceptance, CLI journey,
setup failures and their repairs retain their recorded source scope in
`doc/plans/2026-10-26-governed-review-response-sleep-audit.md`.
- Parent #17696 merged with 58 clean checks, Greptile 5/5 and Ponytail
5/5. Its Photon warning was disproved by real callback/SQL evidence and
the security check became successful. This PR still requires its own
fresh exact-head CI and review gates. No deployed sleep, external alarm
or fleet savings claim is made.

## Risks

- Notifications use the existing process-local writer contract.
Independent SQL writers and uncoordinated replicas are outside its
coverage.
- Genuine startup and company authority reconstruction remain
proportional to the owed backlog. Queries page 100 rows, but discovery
accumulates the candidates; this is not a constant-cost global
operation.
- Current publication authorization, durable binding, ownership and lock
ordering remain mandatory. The worker does not resolve the original
review gate.
- The concurrent index uses existing migration replay handling. Other
recovery duties and Cloud sleep eligibility remain separate.

## Model Used

OpenAI Codex, GPT-6 family, with reasoning, repository tools, shell
execution and PostgreSQL tests. The runtime does not expose a more
specific serving snapshot or context-window size.

DottaandPaperclip b14e1ecaf8 feat: add a complete agent YAML configuration editor (#18036)
<!-- entire-trail-link-start -->
https://entire.com/gh/paperclipai/-/trails/175/changes/paperclip/175
<!-- entire-trail-link-end -->

## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Operators configure each agent through several settings tabs.
> - Operators also need one complete, editable configuration document.
> - The document must retain each resource service's access and
validation rules.
> - This pull request defines the `paperclip/v1` Agent YAML format and
adds an Advanced editor.
> - The editor previews changes, rejects stale core writes, and explains
partial saves.
> - Operators can edit agent settings in one place and verify the saved
state after reload.

## Linked Issues or Issue Description

Refs #2219. Related settings work: #10974.

**Problem or motivation**

Agent configuration spans profile, runtime, instructions, skills, tools,
channels, permissions, secrets, keys, and revision history. Operators
cannot inspect and edit those settings as one versioned YAML document.

**Proposed solution**

Add an Advanced agent tab with a YAML editor. Define a shared document
schema. Read and write resources through their existing APIs. Separate
editable settings, explicit commands, and read-only observed state.

**Alternatives considered**

A raw agent-row editor would omit related resources. A generic database
update would bypass resource ownership. The editor therefore uses the
existing resource contracts and records partial results when a later
request fails.

**Roadmap alignment**

This extends the shipped agent configuration capability. It relates to
easy AGENTS.md configuration, multi-harness teams, governed tool access,
and secrets management. Stored credentials remain redacted. This format
is an editable settings view; it does not add company portability or
provider consent.

## What Changed

- Add the `paperclip/v1` Agent document schema and format documentation.
- Add the Advanced route, sidebar entry, YAML editor, validation, change
summary, save progress, and reload recovery.
- Cover core settings, instruction files, skill pins, tool
installs/profiles/policies, channels, permissions, secret operations,
API keys, and revision restore.
- Add private-keyed configuration revisions checked under the agent
write lock. Read the current key through its owner so file-based
rotation does not require restarting the server. Preserve unchanged
secret markers at their original paths.
- Retain drafts and returned credentials after partial saves. Require
reload before retrying commands. Prevent duplicate submissions. Keep new
key tokens visible through rename and disable saves with an empty change
plan.
- Send only changed channel settings. Require explicit intent for shared
profile edits. Load destination instruction files when changing their
location.
- Update Storybook fixtures, verification maps, and regression tests.
Add browser acceptance through real APIs and PostgreSQL.

## Verification

- `pnpm -r typecheck` passes.
- `pnpm build` passes.
- The focused UI suite passes all 50 tests. The API/revision helper
suites pass all 93 tests.
- All 65 lifecycle tests pass with a dedicated PostgreSQL database. They
exercise real row locks, redaction, revision history, audit, and company
access.
- All six browser acceptance cases pass. They check saved settings and
clears, secret redaction, key creation/revocation, stale drafts,
lost-response recovery, instruction relocation, revision restore, and
key-token retention through rename. The tests use real resource APIs and
storage. The MCP target uses the existing local provider fixture. No
model run or provider login is claimed.
- Signing-key regressions cover both environment changes and real
in-place key-file rotation. The protected database save/fencing cases
pass after removing the cache. A local measurement of 1,000 file-backed
revision calculations took 48 ms.
- `pnpm check:token-gates`, `node scripts/check-maps.mjs`, and Storybook
catalog checks pass.
- The full [PR CI
run](https://github.com/paperclipai/paperclip/actions/runs/40725425470)
passes on `34ed97bbf9`: all 48 jobs, including all eight browser shards.
Greptile is 5/5 on this commit, with no unresolved review threads. This
final commit refreshes CI after a runner-routing fallback; its source
tree matches `fb4cf488cf`.
- Paperclip Review and Superagent Security Scan ended neutral because
their external agents did not complete a valid assessment/receipt.
Superagent passed on `fb4cf488cf`, which has the identical source tree.
One request to retry the final scan returned HTTP 404. Neither check
reports a code finding.
- The local full-suite attempt was blocked by host System V semaphore
exhaustion. Acceptance and lifecycle verification used an isolated
PostgreSQL 18 container. Earlier CI runners were interrupted. A local
combined run hit hook/test timeouts; the full 88-test route file passed
after the builds finished. The final full CI run passes. Earlier failure
logs and browser traces remain retained outside the repository.

To run the browser regression:

```sh
pnpm exec playwright test --config tests/e2e/playwright.config.ts tests/e2e/agent-configuration.spec.ts
```

## Risks

- Multi-resource saves are sequential. Earlier operations can remain
saved if a later operation fails. The editor reports the completed steps
and requires reload.
- Core settings and managed instruction files have atomic revision/hash
checks. Other unversioned resource writes and external supporting files
retain a race window after preflight.
- Shared profile definitions affect other bindings. The editor requires
`updateSharedDefinition: true` for those edits.
- The editor follows native service normalization and experimental
settings. Canonical reload can add generated access profiles or mask
environment values.
- New editor dependencies require the repository's normal automated
lockfile update. This pull request does not edit the lockfile or add a
database migration.

## Model Used

OpenAI Codex, GPT-6 family. The exact current model version was not
exposed. The work used reasoning, repository tools, code execution, and
browser automation. Earlier work in this chat used the configured Codex
model before a model switch; its exact model ID and context window were
not exposed. The current context window size is also not exposed.

DottaandPaperclip 4e27a82a42 fix(server): wake approval queue seeding from committed changes (#17510)
## Thinking Path

> - Paperclip manages work and human decisions for AI-agent companies.
> - Automatic decision queues store membership for pending attention
sources.
> - The old attention maintenance scan created that membership as a side
effect.
> - Moving approval retention out of the scan also removed approval
queue discovery.
> - Queue API consumers do not build the attention feed first, so they
can miss pending PR approvals.
> - This pull request gives approval queue discovery its own worker and
commit signals.
> - The worker becomes quiet after it saves every missing membership.

## Linked Issues or Issue Description

Refs #17497. Stacked on #17508, following #17505.

This is a correctness dependency for the approval retention migration in
#17497. Do not treat that migration as complete without this
queue-population path. A real PostgreSQL regression found no PR queue
when a pending approval had a linked PR work product and the maintenance
feed excluded approvals.

Related: #13364 proposes pruning stale seeded memberships. This PR
preserves existing membership retention and does not implement pruning.
#15149 documents the decision-queue APIs.

## What Changed

- Add an approval queue worker that selects missing PR-queue memberships
from existing durable records. Bound each pass to 100 rows. Continue
past failed rows and revisit them on retry.
- Reuse the queue service's seed writer through a normalized source
contract. Keep queue rules, uniqueness, and audit writes in that
service. Keep feed readers compatible.
- Wake from committed approval and link changes, issue/company
visibility, PR work-product changes and imports, and queue-rule changes.
Reuse the existing transaction boundaries.
- Recheck approval status under a row lock. Serialize queue-rule changes
with the seed writer. Preserve required recovery during drain and
shutdown.
- Register startup reconstruction under the existing maintenance switch.
Leave other attention-source seeding with its current owner until its
own migration.
- Document the complete producer audit and stack dependency. Add a
validated external PostgreSQL option to the existing queue route suite
so it runs on hosts without embedded database support.

## Verification

Current tested revision: `7948136ef8`. This PR targets master after
#17508 merged as `8406047b4e`. Normal ancestry reconciliation preserved
the complete tested integration tree, before the review repairs below.
Only the original 25 seeding paths differ from master, and database
schema/migration metadata is identical.

- **56 cases pass in the three complete affected suites:** 28 approval
seeding, 6 queue routes and 22 task-watchdog cases. Server typecheck and
production build pass.
- Three real PostgreSQL retry regressions failed then passed: new
matching arrivals cannot postpone a recovered failure indefinitely;
successful partial progress resets old failure penalties; an empty
cursor rollover does not reset outage backoff. Exact one- and two-second
retry assertions, eventual complete recovery and existing
capped-backoff/company isolation tests pass.
- Three genuinely empty external databases failed before setup repair
because tables were missing. Each suite now applies migrations itself.
All 55 cases passed on a second set of fresh databases; adding the final
rollover guard then passed all 56 cases using those now-migrated
fixtures. No production migration or table is added.
- The initial expanded fresh run passed 54 cases but failed a cleanup
expectation that omitted a required cursor rollover pass. The fixture
now permits that explicit continuation while preserving exact deadline
and complete-membership assertions. All failure records remain in
`doc/idle-approval-queue-seeding.md`.
- The prior integrated revision `4e725e2055` passed **522 cases in
fifteen complete producer/consumer suites**, plus types/build. Its
unchanged consumer code and historical deadlock, cross-company
amplification, backoff, gateway integration and setup evidence remain
documented; those counts are not relabeled as current acceptance.

Commands: server Vitest for approval queue seeding, queue routes and
task watchdogs; server typecheck/build. Fresh exact-head
Greptile/Ponytail reviews and all CI checks remain required. No full
local repository-suite, deployed suspension or cost claim is made.

## Risks

- Signals cover the supported in-process service writers. Independent
SQL writers and multiple server replicas need a separate notification
contract.
- The source predicate must stay aligned with unscoped attention
maintenance. The first linked issue, company scope, pending status, and
execution visibility are tested. A pending approval itself keeps its
linked external conversation active.
- Existing enabled seed rules can re-add manually removed matching
memberships. This preserves the old scan's behavior. Completed or
archived membership is not pruned.
- Queue seeding and retention are separate owners. Other attention
sources still need their own event-driven discovery before the aggregate
scan can be retired.
- This PR does not extend Cloud wake coverage, relax sleep admission, or
claim measured host/database suspension.

## Model Used

OpenAI Codex, GPT-6 family, with reasoning and code execution. The exact
serving model identifier and context window were not exposed.

DottaandPaperclip 553a004926 fix(server): schedule approval retention from durable deadlines (#17497)
## Thinking Path

> - Paperclip manages agent work and should become idle when it owes no
background work.
> - The attention feed archives old pending approvals under the existing
90-day policy.
> - The server rebuilds the feed on each maintenance tick, even when
approval state has not changed.
> - Some approvals have never been read, so retention rows alone cannot
define the work.
> - Approval records and committed eligibility changes define the real
deadline and wake triggers.
> - This PR gives approval retention its own deadline worker and reuses
the existing retention and archive-delivery contracts.
> - The approval lane stops idle scans while other attention sources
remain separate migration work.

## Linked Issues or Issue Description

Stacked on #17490. This PR owns approval retention only. Archive
notification delivery is already separated in #17475. Other
attention-source retention and remaining recurring server duties remain
in the stack inventory. The related-PR search found no overlapping
approval-retention worker migration.

**What existing behavior does this improve?**
Automatic archival of old pending approvals from attention.

**Current behavior**
The shared maintenance tick builds the attention feed for each active
company. That query discovers pending approvals and materializes
retention records before checking their age. An unchanged or empty
approval source is read repeatedly.

**Proposed behavior**
Reconstruct eligible approvals at startup. Schedule the earliest
approval activity timestamp plus 90 days. Wake after committed changes
to approvals, links, issue visibility, company activity, or retention
controls. Stop scheduling when no eligible approval remains. Archive
records through the existing version checks, audit writes, and
notification outbox.

**Reason and benefit**
Idle approval retention should generate no recurring SQL. Unseen
approvals still need eventual archival. A stale candidate must not
archive an approval that was resubmitted while the worker waited for its
source lock.

**Breaking changes**
None intended. The archive age, eligibility, keep controls, audit, and
notification policy remain. Normal attention reads still include
approvals. No Cloud sleep blocker is relaxed.

## What Changed

- Restrict tool-review signals to their existing terminal transition
owners (approve/reject/cancel). Creation/link/revision/resubmit signal
retention only.
- Coalesce retention notifications under one thirty-second aggregate
read budget. Known deadlines still preempt it; no notification or
durable deadline means no scan.
- Add a bounded approval-retention worker. It uses the shared deadline
coordinator and reads pending approvals directly.
- Share first-linked-issue ordering and execution visibility through
`approval-visibility.ts`, used by both the feed and worker. Preserve
actor-specific read filtering and locked archival rechecks.
- Materialize unseen approvals on demand inside manual
keep/archive/revive transactions. Use the saved activity timestamp and
existing versioned state; retain company scoping and rollback on
failure.
- Add awaited pre-write signals to approval, budget, tool, link,
visibility, company, and retention mutation owners. Preserve outer
transaction ownership; keep provider calls outside SQL wrappers.
- Narrow retention inputs to the source fields the existing service
uses. Signal materialization only for inserted or changed state so
unchanged reads converge.
- Exclude approval collection from the maintenance feed before its
query. Other source kinds remain in that sweep.
- Add the pending-only approval deadline index with concurrent,
idempotent creation. Keep the five newest migration snapshots.
- Add real-Postgres lifecycle tests and startup wiring tests. Enable
validated external test databases for existing attention and budget
suites.
- Document producer coverage, quiet conditions, recovery and remaining
scope in `doc/idle-approval-retention.md` and the stack ledger.

## Verification

Current test revision: `dd844f4e8cbbf1db681a774dc31cd552301bef48`,
followed by docs-only `3efda33812`, which received both 5/5 reviews and
passing CI. This candidate normally integrates landed Grok `2adcde162e`.
All runtime and acceptance-test sources are byte-identical to that
tested/reviewed tree; only four independent Runner E2E accounting files
and the ledger differ. Migration 0392 and the parent journal prefix are
unchanged. Local checks are reused for their unchanged source; fresh
final-head CI and reviews are required against master. This remains the
approval-retention duty.

- **315 cases pass** across eight complete suites: approval
retention/service, attention, decision retention, budgets, tool gateway,
server startup and scoped deadlines. All 30 real-PostgreSQL lifecycle
cases pass. Server production typecheck and build pass.
- **Four regressions failed before repair**: unseen young
keep/archive/revive returned 404; the manual keep rollback case could
not reach its update. They now pass. A failed keep update leaves no
retention row or audit; retry succeeds with one audit.
- Both linked-issue orders are checked against the feed: hidden
first/visible later is excluded; visible first/hidden later remains
eligible. The feed and worker use the same owner query, with actor read
filtering preserved.
- Generated concurrent/idempotent migration
`0392_small_frightful_four.sql` adds only the pending approval index.
Every historical parent SQL migration and all 390 parent journal entries
are unchanged. Five generated snapshots remain (0388–0392). Schema
drift, migration safety/numbering and actual fresh apply/replay on five
isolated PostgreSQL databases pass.
- Existing precision, transaction, drain/resume, failure fairness and
shutdown tests remain. Earlier transaction-stub, budget-import,
diagnostic-client and CI failures are recorded in the owning audit and
stack ledger; later passes do not erase them.

- Three additional real regressions failed before the wake repair:
ordinary mutations emitted tool-review signals in both resolution cases,
and sequential tenant writes caused repeated global reads. Tests now
prove terminal approve/reject/cancel signals and one aggregate
reconciliation per thirty-second window with a different company's
pending approval retained. The aggregate budget is stricter than a
separate per-company budget and matches the old default cadence; known
deadlines preempt it.
- The initial coalescing fixture expected the concrete pending-work hold
to clear before reconciliation; it now preserves that hold and measures
SQL. The first broad repair passed 313 cases but failed the existing
parked-review cancel case. The original cancel producer is restored; all
315 now pass without weakening assertions. At old head `5991f257bb`,
security was action-required and Greptile returned service error
`8fb1835c-e564-476b-b802-d5be164ea915`. Fresh repaired-head results are
required, not assumed.

Commands: server Vitest for `approval-retention-work`,
`approvals-service`, `attention-service`, `decision-retention-service`,
`budgets-service`, `tool-gateway-service`,
`server-startup-feedback-export`, `scoped-deadline-work`; server
typecheck/build; DB snapshot drift, migration checks and fresh
apply/replay. Fresh exact-head Greptile/Ponytail and CI remain merge
gates. No full local repository suite or deployed host suspension/cost
claim is made.

Earlier stack verification below is historical evidence at the
explicitly named revisions, not this head's acceptance:

Approved delegated Test-tab calls now evaluate caller and target
policies in the existing claim transaction. Real HTTP regressions first
showed a missing caller charge and shared-limit self-rejection. All 78
connection-management cases and 92 gateway cases now pass on the owning
branch, along with server production typechecking. All 78
connection-management cases also pass at combined tip
`d25f830db89c1623bff3547bfc68015c1c66e0cf`. The three new cases cover
shared charges, separate charges, rollback on denial, duplicate
approval, and rejection of a later authorized call. Intermediate
counter/identity fixture failures and a reused-database Slack collision
are recorded in [the owning
audit](https://github.com/paperclipai/paperclip/blob/20557d2c3dd0949c734eaa8095fd78b9b07d7fd5/doc/idle-approved-tool-recovery.md#delegated-test-tab-admission-after-policy-changes).
The earlier combined checks below remain evidence for their stated
revisions. Current-head CI and review remain required.

Earlier published head: `20557d2c3dd0949c734eaa8095fd78b9b07d7fd5`. This
remains a single-duty PR stacked on #17490.

- Integrated pinned master `8548173fc9efdc0fa07644fb92dcc0b6851a95e5`
with normal merges. Every branch passed generated-schema checks and its
target-branch migration-order gate. Existing SQL is unchanged; the 25
stack migrations follow master’s new migration.
- Duty-specific producer coverage, lifecycle tests, and earlier failure
records:
[2026-10-24-recurring-server-sleep-stack.md](https://github.com/paperclipai/paperclip/blob/76a49810c7ff403ae7720bef0d453747bd844731/doc/plans/2026-10-24-recurring-server-sleep-stack.md).
- Combined tip `51e6cbe67979b55b340ea9f0f434c5b5c682ccd6`: **355
startup/cancellation/recovery/dispatch/preservation tests**, **75
connection-management tests**, and production server typechecking pass.
These combined checks do not mean every intermediate PR independently
ran the full repository suite.
- Real master-to-stack database upgrade passes: **379 unchanged prior
migration receipts plus 25 new migrations**, with the gateway
responsibility fixture preserved.
- Owning-branch checks: 84 runner installation/settlement cases plus
runner typechecking and generated TypeScript/Rust profile agreement; 253
GitHub review workflow cases. Stop-attribution and held-worker readiness
regressions failed before their fixes.
- Failure accounting: the initial combined run passed 349 cases and
failed six idle assertions in a reused preservation database containing
old cancelled runs. Repeating the combined command with a fresh
preservation database passed. Earlier invalid project selection, merge
import/type errors, and the interrupted full local suite are not counted
as passing checks.
- **Latest-head CI and review remain required.** Prior CI findings
include GitHub idle observation, route teardown, attention cleanup,
touch-picker E2E, workspace health, and voice-cleanup timing; local
passes do not erase those failures. No full local repository-suite,
deployed sleep, fleet cost, or live Copilot qualification claim is made.

## Risks

- Signals cover the application database owner and its child pools.
Independent SQL writers and other active processes need an explicit wake
integration. Startup reconstructs committed durable state.
- The source row lock prevents stale approval-activity archival. It does
not add cross-table serializability for concurrent link, visibility, or
company-policy changes.
- Newly changed eligibility can wait up to the thirty-second
notification window, matching the former default maintenance cadence.
Saved deadlines still preempt; no new idle polling is added.
- Failed records retain recovery debt. Bounded cursor progress lets
later rows proceed without dropping failures.
- The shared retention transaction now groups materialization and
activity-version updates. Existing attention and retention consumers are
tested against PostgreSQL.
- Kept, archived, resolved or inactive-company approvals can become
quiet, but their supported mutations must retain their wake
notifications.
- The local scheduler cannot wake stopped compute. This PR neither adds
approval deadlines to Cloud's external alarm contract nor claims
measured host or database suspension.

Nicky LeachandPaperclip 79b2bf78dd Create agent records when onboarding names are submitted (#17005)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Core onboarding selects an agent and connects its provider account.
> - The current flow creates the agent record after authentication.
> - Setup needs a stable agent identity as soon as the user submits its
name.
> - This PR saves a paused setup record on name submission and finishes
the hire on that same record.
> - The existing sandbox authentication and approval paths continue to
apply.

## Linked Issues or Issue Description

**Subsystem affected**

Core agent lifecycle, shared validators, and onboarding UI.

**Current behavior**

Onboarding creates the database record at the final hire step. Earlier
authentication and saved setup have no agent record.

**Proposed behavior**

Create a paused agent with a setup hold when the user submits its name.
Do this before adapter selection or authentication. Update that same
record after adapter selection. Reuse its ID on retries and saved setup.
Finish that record through the existing hire path.

**Reason and benefit**

Give setup a stable identity without coupling this refactor to computer
provisioning or a new execution backend.

**Breaking changes**

Incomplete setup now leaves an inactive agent record. Existing clients
can still create and hire agents without the new optional fields. The
sandbox authentication flow and API key support remain in place.

**Additional context**

Requested Core-only refactor. Related work: #15422 changes runner
defaults. This PR preserves those choices. No duplicate early-record
refactor was found in the issue and PR search.

## What Changed

- Save the agent when the user submits its name in both onboarding entry
points. Use a provisional adapter until the user chooses one. Reopen
name-only drafts at adapter selection.
- Add retry-safe setup creation and same-record hire completion. Keep
approval checks at final hire.
- Save and restore setup configuration in both onboarding entry points.
Show Finish setup for paused setup agents.
- Restrict setup changes to authorized board users. Block generic resume
until setup finishes.
- Add lifecycle, authorization, connection, and UI coverage. Update the
specification and feature maps.

## Verification

- Latest focused server run: 126 lifecycle and AI-connection tests
passed after the Apex fixes. An earlier broader run passed 301
lifecycle, instruction, adapter-validation, and AI-connection tests.
Earlier authentication, retry, cross-company, and direct-create
regressions also passed.
- Name-save verification: 336 tests across the wizard, naming dialog,
New agent, action-button, and wizard-step suites passed. Coverage
includes retry identity, repeated submission, closing during a save,
saved-model restoration, and a disabled process adapter.
- Shared, server, and UI TypeScript checks passed. The latest UI package
check and direct server TypeScript check passed. The server package
preparation command could not finish locally because Cargo is
unavailable; full build preparation runs in CI. Map checks, token gates,
and `git diff --check` passed; each pushed diff was scanned for
credentials and PII.
- All six onboarding browser tests passed together with installed Chrome
against a fresh throwaway instance after the Apex fixes. They include
numbered duplicate names, first-time Copilot without a connected
account, and Grok’s saved model. They verify early persistence in both
entry points, closing and reopening a name-only draft, same-record
adapter selection, sandbox sign-in, login reload, and failed-connection
gating. Provider responses are simulated; Core persistence is real.
- Regression coverage includes incomplete Copilot creation versus final
validation, Kimi settings through redacted responses, stale local
drafts, company switches during creation, saved environments, and
provider changes with personal/shared old bindings.
- Apex found five name-only setup gaps: numbered names, early Copilot
account validation, the Grok model default, same-name draft ownership,
and provisional runner restoration. The fixes preserve server names,
defer implicit account binding until hire, set Grok’s model, avoid
name-based reuse of unfinished records, and restore Auto for pending
adapter choices. UI, database, and browser regressions cover these
cases.
- Standard Greptile found a Grok provider mismatch in the new adapter
save. The save now uses ACPX with the Grok profile. Both the 129-test
dialog/setup run and a real-API browser regression passed after the fix.
The first browser attempt stopped on an ambiguous empty-roster button
selector; it now scopes the click to the toolbar.
- Moving the save point exposed a saved-model reset and timing
assumptions in the same-name and portal fixtures. The source choice now
preserves the restored model, and tests cover same-name recovery without
a second hire.
- Initial CI exposed direct-create compatibility, concurrent setup
effects, fixture assumptions, and a missing-fields assumption in the
provider-validation guard. These were fixed and verified locally. Review
findings were addressed with authorization, recovery,
credential-persistence, and restored-configuration regressions.
- The final desktop capture passed after host resource cleanup. A repeat
first failed before Core started because macOS had exhausted
shared-memory IDs. Standalone PostgreSQL initialization reproduced the
same error. Releasing only unattached control segments from this task’s
stopped test databases allowed the unchanged revision to pass. No Core
or system setting changed.
- Local database tests required port access outside the sandbox. Missing
declared development dependencies were supplied outside the branch. No
live provider sign-in was tested locally.
- [Full monorepo
CI](https://github.com/paperclipai/paperclip/actions/runs/39522434236)
is green on `82b82fec189a172a885d4dd565ae2f4a18c73efc`. Standard
Greptile scored 5/5 with zero new comments (check `118969424903`). The
subsequent Apex review also scored 5/5 with zero new comments (check
`118972866331`) on that same commit. All review threads are resolved.
- The separate Superagent Security scan returned neutral/inconclusive
because its pinned merge-base checkout failed or timed out. GitHub
returned 404 for check and suite rerun requests. This is not a
successful security scan; the CI workflow and other security checks
passed.

## Risks

- Users can leave inactive setup records behind, including after name
submission alone. These records cannot run until hire completes.
- Setup completion touches hiring and approval boundaries. Tests cover
retries, company isolation, creator permissions, and approval snapshots.
- An uncertain credential-save result retains its new secret reference
for retry. A confirmed client rejection removes only credentials staged
by that attempt.
- No schema migration or VM provisioning change is required.
Agent-computer integration is out of scope.

## Model Used

OpenAI Codex, based on GPT-6, with repository search, code editing, and
tool execution. The exact deployment ID and context window are not
exposed in this session.

DottaandPaperclip cc96c9f832 fix(server): probe libc asynchronously before serving requests (#16335)
Linux runner qualification generated a full Node diagnostic report to read one libc version field. Reverse DNS on open sockets could stop the event loop for minutes.

Replace report generation with a one-time asynchronous getconf probe during module initialization. Bound it to one second and 1 KiB of output, cache the measured facts, and preserve unknown-libc failure behavior.

Verification: 149 focused tests, repository typecheck and build, full CI, and fresh Greptile 5/5 with no comments. A timed-out local heartbeat fixture passed when rerun alone.

Devin FoleyandPaperclip 8ffc5748a2 fix(server): stop libc detection from reverse-resolving every socket (#16333)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work
> - #15683 made new Codex agents default to Paperclip Runner on Linux,
which needs the host's glibc version
> - `localAgentRunnerTarget()` reads that version from
`process.report.getReport()`
> - Unless `excludeNetwork` is set, Node's diagnostic report resolves
the endpoints of every open socket with synchronous reverse DNS on the
main thread
> - `GET /adapters` calls it once per adapter on every request, so on a
host whose proxy address has no fast PTR answer, the whole server stops
responding
> - This pull request excludes network data from the report and detects
libc once per process
> - The benefit is that runner selection costs a few milliseconds once,
instead of blocking the server on every request

## Linked Issues or Issue Description

Regression from #15683.

**What happened?**
After #15683, a server running behind a reverse proxy stopped
responding. Requests, database replies, log output and health checks all
stalled for minutes. The Node main thread was idle on CPU but blocked in
a synchronous DNS read: reverse (PTR) lookups of the proxy's address,
each waiting for the resolver timeout.

**Expected behavior**
Detecting the C library must not block the event loop on DNS, and must
not repeat on every request.

**Steps to reproduce**
1. Run the server on Linux where the reverse DNS lookup of a connected
peer's address is slow (for example, a proxy address whose PTR lookup
times out).
2. Open the board, which polls `GET /adapters`.
3. The server stops answering. Each `getReport()` call reverse-resolves
every open socket synchronously.

Measured on Node 24.21 with one open TCP connection to such a peer:
`process.report.getReport()` took **9,774 ms** with network data and **4
ms** with `excludeNetwork`. A busy server has hundreds of open sockets.

**Paperclip version/commit**
`master` after #15683.

## What Changed

- `server/src/lib/agent-runner-platform.ts`:
- Generates the report with `process.report.excludeNetwork = true` and
restores the previous value in `finally`.
- Detects libc once per process and caches the result, because the C
library cannot change while the process runs. A failed report also
caches "unknown", so it is never retried per request.
  - Adds `resetLocalAgentRunnerTargetCacheForTests()`.
- Reads `excludeNetwork` through a narrow typed view, because the
installed Node type definitions do not declare it yet.
- New `server/src/__tests__/agent-runner-platform.test.ts`.
- `agent-runner-selection.test.ts` and
`dev-runner-native-binary.test.ts` reset the cache between cases,
because they change the mocked glibc version per case.

## Verification

- `pnpm exec vitest run src/__tests__/agent-runner-platform.test.ts
src/services/agent-runner-selection.test.ts
src/__tests__/dev-runner-native-binary.test.ts
src/services/local-ai-browser-login.test.ts` (in `server/`): 165 passed.
New tests:
- The report is generated with `excludeNetwork` set, and the previous
setting is restored.
  - Five calls generate one report.
- A failing report gives `libc: "unknown"` once and restores the
setting.
  - No report is generated off Linux.
- `pnpm typecheck` in `server/`: passed.
- Measured on Node 24.21 (above): 9,774 ms → 4 ms for one report with
one slow-PTR socket open.

## Risks

- Low risk. The detected glibc version is the same; only the network
section of the report is skipped, and nothing reads it.
- The cached result does not change until restart. The C library cannot
change while the process runs.

## Model Used

- Claude Opus 5.5 (`claude-opus-5-5`), 1M context, through Claude Code
with tool use (shell, file edits, test runs, live diagnosis of the
stalled process).

DottaandPaperclip adb69e8f09 feat: default new Codex agents to Paperclip Runner on Linux and macOS (#15683)
## Thinking Path

> - Paperclip is the open source control plane for agent companies.
> - People choose a harness when creating an agent.
> - The server should resolve that harness to a qualified runner.
> - Codex is the first harness in the staged default rollout.
> - Saved agents must retain their execution choices.
> - New Codex agents therefore use Paperclip Runner on qualified
macOS/Linux targets, with an explicit Legacy override.

## Linked Issues or Issue Description

Refs #16028, #15555, #15673, #15422, #15631, #15926, #15932, #16149.

The runtime/install prerequisite and release repair are merged. This
slice owns Codex creation, setup, configuration and preservation. Other
harness defaults remain unchanged. It adds no database migration;
incoming master migrations, required maps and regression coverage are
retained.

## What Changed

- Resolve `runner: auto | paperclip | legacy` at the common creation
boundary across onboarding, API/CLI, hires, built-ins, plugins and
imports. Freeze pending hires and export resolved execution choices.
- Preserve existing runners on upgrades, unrelated edits and import
updates, including compatible accounts, workspace settings, Codex effort
and intentional clears.
- Use the underlying harness picker and branding; put the runner
override in Advanced. Use the shared dropdown and update
production-component stories and creation contracts.
- Support qualified Apple Silicon, Intel Mac, Linux x64 and Linux ARM64.
Linux requires GNU libc 2.39+. Disabled, external, unsupported and
unqualified combinations retain their applicable Legacy execution.
- Test the selected runtime/account/model/environment. Ordinary Connect
probes eligible immutable managed targets and refreshes scoped
discovery. Creation/discovery do not provision. Auth, dependency,
incompatible-setting and cleanup failures remain actionable without
changing runners.
- Keep setup probes read-only, preserve local/SSH isolation and resolve
packaged assets before staging credentials. Bound cleanup reads by the
remaining absolute deadline; physically close every owned ingress socket
before endpoint release. Unconfirmed cleanup retains recovery state.
- Preserve validated completed-onboarding callbacks through startup
recovery. First file-producing work belongs to the child task and uses
the existing dependency/block handoff and task-scoped registration.
- Reuse existing E2E fixtures, artifact checks, reporting and controls.
Keep Docker/Cargo/native compilation remote. Paperclip tarballs contain
no Codex payloads; official optional npm platform dependencies remain.

## Verification

Current tested head: `1c03a8815672d82c264a9b08d4ea1984e8fae107`.
**Merge-ready: required CI, fresh review, package qualification, Mac
core acceptance and Test task/follow-up acceptance pass.** The existing
attachment and native-continuity oracles independently verify both Test
downloads, run attribution and unchanged native/provider/workspace
identities. The maintainer authorized a code-owner-review exception for
this PR only. The first Test setup failure and normal Board
completion-review friction are retained below. Actual post-merge
packages and private Cloud distribution remain separate qualification
gates.

- [Current
CI](https://github.com/paperclipai/paperclip/actions/runs/38564241625):
all 48 ordinary jobs and required verify/E2E pass. Attempt 1 retained
six simultaneous host-shutdown cancellations. One failed-only retry
passes those six lanes and two aggregates, preserving 40 successes.
Actual integration checkout `53104c1` combines this head with master
`c80`; it is not relabeled as the branch tree. Fresh exact-head Greptile
5/5, 30 resolved threads, no actionable findings.
- [Exact-source package
qualification](https://github.com/paperclipai/paperclip/actions/runs/38565287239):
nine applicable jobs pass, nine conditional jobs skip. Four remote
native producers; Linux x64 clean install; Intel Mac, Apple Silicon and
Linux ARM64 native startup/physical cleanup controls. All 18
tarballs/four daemon source/hash/manifest bindings and 343 migrations
through 0344 verify. No Codex payload is packed. These hosted tarballs
are not a published npm canary.
- Normal Apple Silicon installation and credential-free native
startup/cleanup pass using the packaged daemon, with no source build or
provider turn. Fresh installed DB initialization passes. Upgrade
651a→1c03 preserves all three complete execution configs, accounts, ten
recorded run profiles, paused budgets, active incidents, cost summaries
and two historical unpriced events. Historical recovery state is
retained.
- Cleanup regressions: four delayed-read baseline failures become 15
passing deadline/authority controls. No-acknowledgement and prior-error
socket ownership regressions become 12 passing controls. Direct
runtime/server types, maps: 35 areas / 176 IDs, diff checks and
independent reviews pass. Permission/auth/reconnect policy and the
10-second setup budget remain intact.
- Existing source coverage includes creation/selection/imports, pending
approvals, recovery, account/lifecycle handling, mounted forms,
dropdowns and E2E support. Full typechecks/tests/build/token and
applicable Storybook checks pass in the recorded current CI; no
duplicate suite or weakened grader.
- Literal 9546 live Mac proof: ordinary Auto Codex onboarding, useful
document, real completed-child callback/reply, seven successful native
runs, manual follow-up and independently downloaded1,101-byte child
file. Existing Legacy title edit/reload preserves execution; two real
Legacy runs produce independently checked15/34-byte files. These remain
historical source records.
- Current literal 1c03 Mac core acceptance passes independent checks:
ordinary Auto native Codex onboarding without runner override; only
timeout 180 seconds/concurrency 1 saved; six successful native runs and
six serialized turns (maximum overlap 1); both same-task continuity
checks and five mechanical callback checks; actual rendered parent reply
and exact child downloads. The useful handoff advances from revision 1
to 2. Both files are 48 bytes, SHA-256
`8b742bd4e8123a7c078f0a8d9f3449fbfa8a320d7d90be641a2818db3c53d92c` and
`f42db9774fa06e804ab7d596fc8b922a2b638921c418b87d0111151d290a6afe`;
exactly one byte changes and old history remains. The agent is paused
with zero active runs/errors. Both artifact work products have
`isPrimary=false`, and descriptive titles fail a stricter title-helper
assumption; these retained results are scoped editorial follow-up, not
new merge gates. Six source-bound screenshots are uploaded and verified
in the private primary QA report.
- [Literal 9546 managed
proof](https://d1p6rlowie26tp.cloudfront.net/runner-e2e/campaigns/gha-38485488621-1/index.html):
original 19 assertions and completed judge pass, four native turns,
child-bound240-byte file and session/workspace continuity. Checkout
reports a verified generated target-lock mutation; full final porcelain
was not retained. Signed execution-image identity remains separate from
Core source.
- Actual 651a Test deployment succeeds on one owned/pinned target. One
ordinary Auto API-key Connect then fails native cleanup before hire.
Controller turn/drain/suspend completion and service-recorded
environment cleanup are observed; they do not certify physical deletion
or native authority retirement. Failure/recovery/unknown holds are
retained. No Legacy switch or unchanged paid retry. The two source
repairs above require actual treatment.
- [Current immutable Core
preview](https://github.com/paperclipai/paperclip/actions/runs/38570365725)
and matching DB/shared artifacts pass. One owned, pinned Test target
serves exact `1c03`; identical migration history correctly skips the
migrator. Ordinary Auto Codex Connect succeeds and hires native; saved
native selection and QA bounds survive reload. The initial child
preparation attempt fails before provider work, with actual stdout
identifying `renewal_deadline`; its trigger remains unknown and the
failure is retained. Normal Board **Try again** and **Resume** recover
the child to Done, producing a browser-verified 44-byte file, SHA-256
`0a8458b1efcc2d7a0d815689458c4caab039a46c7c7a3e5805de6bc8b1a96ecc`. The
actual parent callback reports the completed child document/file. The
existing validator requires a parent-owned attachment, so recovery
enters normal Review; after byte checking, normal **Approve completion**
reaches parent Done. No forced status or duplicate parent file is used.
The validator is unchanged from integrated and current master.
Autonomous initial callback-to-Done is not proven. The same-child
follow-up completes document revision 2 and publishes a second
independently downloaded 44-byte file, SHA-256
`22022257300fae762784632d08d3bbe0bab61e2ce81f91864a14dcd7aa464591`; only
the revision digit changes. The unchanged existing native-continuity and
attachment oracles pass. All ten recorded runs are terminal: nine native
successes and the retained pre-provider setup failure. The QA agent is
paused with zero active runs. The pre-existing parent evidence scope
mismatch is a separate follow-up, with ordinary completion review
exercised rather than a forced status or relaxed validation. This
unsigned Core preview remains distinct from post-merge private Cloud
composition.

## Risks

- Saved agents are not migrated. Explicit runner changes use normal
configuration/session rules; historical runs retain their recorded
execution.
- Native readiness depends on verified assets and server-owned target
qualification. A Legacy probe, HTTP200 or successful account save cannot
prove native readiness.
- Recovery exceptions remain restricted to validated
completed-onboarding reports; process-loss, ownership and terminal-state
safeguards remain active. Unexecuted live journeys stay open.
- Linux ARM paid tasks, Windows, Alpine and 32-bit ARM remain deferred.
Cold Cargo cache misses are recorded; no warm-cache speedup is claimed.
- Historical invoices/physical cleanup remain unconfirmed and held.
After merge, exact published npm canary, signed standard image/migrator,
private composition and serving/live acceptance still require
qualification. Production deployment/default promotion is not
authorized.

## Model Used

OpenAI Codex, GPT-6 family, with reasoning, tools, code execution and
parallel agents. Exact serving snapshot and context-window size are not
exposed.

Nicky LeachandPaperclip 2dd9811e8b Own agent hiring and termination in a lifecycle module (#15631)
## Thinking Path

> - Paperclip manages AI agents and their work.
> - Agent creation, approval, pause, and termination change whether an
agent can work.
> - These changes currently use separate service and database paths.
> - Some agents need setup after their record exists and before their
harness can run.
> - This PR adds one module that owns those state changes and their
transactions.
> - The module records progress and retries incomplete work after a
server restart.

## Linked Issues or Issue Description

**Problem or motivation**

A hire can become active before its execution environment is ready.
Direct agent state writes also bypass one common lifecycle boundary.
Termination does not have a durable step for resource cleanup.

**Proposed solution**

Use agent lifecycle commands for hiring, approval, pause, resume, and
termination. Keep proposed hires pending until approval. Prepare each
approved hire, test its saved harness configuration, and then permit
work. Wait for required plugin work before completing a step.

**Alternatives considered**

A general database commit hook would expose transaction details to
callers. A timer alone would delay each new hire. This module owns each
command transaction and starts its worker after commit. A periodic scan
recovers work after a process stops.

**Roadmap alignment**

This change adds the lifecycle boundary for the existing agent and
plugin systems. It does not add a user interface or a provider
implementation. It replaces the lifecycle delivery approach in #15391.
Related proposals include #11475 and #13099; this change does not add
their external executor protocols.

## What Changed

- Add durable approval, preparation, verification, pause, resume, stop,
and cleanup states.
- Keep agent creation, lifecycle state, configuration writes, and agent
record deletion behind `modules/agent-lifecycle`. Keep validation,
credential rules, reads, onboarding, invitations, general approvals, and
company data deletion in their services.
- Commit a hire before the remaining onboarding or invitation work.
Serialize requests with the existing dedicated database connection
helper. Reuse the hire after a failed attempt.
- Keep accepted work in the run queue during setup. Require execution
and cleanup evidence before completing pause or termination.
- Retain the original credential owner when approving migrated hires.
- Start lifecycle work after each command commits. Use database leases
and revision checks for retries and late results.
- Add a generic required plugin method and capability. Store the
required plugin IDs for each agent. Track host and plugin completion
separately.
- Keep harness verification and credential rules in services. Supply
their results to lifecycle through explicit ports. Reject failed logins
and incomplete probes.
- Remove reverse imports from lifecycle adapters into services. Share
record queries and validation below both layers.
- Use one hire decision path for agent IDs and approval IDs. Resolve the
approval and agent change in one transaction. Expose policy
reconciliation on the command object.
- Save configuration and verification invalidation through one module
operation. Ordinary commands own their transaction. Restrict the shared
transaction integration to its listed callers. Reject identity, company,
accounting, timestamp, and unknown fields.
- Require completed termination before deleting agent records. The
company deletion service calls the module through a separate deletion
interface.
- Save an approved secret before applying its agent binding. A failed
binding leaves the secret approved. A retry reuses it.
- Remove old permission and metadata export files. Move hire
configuration rules out of lifecycle persistence.
- Report failed worker steps and scans with operation context and known
error codes. Omit provider messages, stacks, and configuration values.
Document ordinary application use and extension rules.
- Recover expired native owners only after execution stops. Retry
cleanup of failed retained leases. Wait for setup in the live smoke
script.
- Keep manual, budget, and company pause reasons separate. Guard
execution status writes with the ready state.
- Add a migration, boundary checks, regression tests, and a lifecycle
guide. Keep the current UI.
- Limit periodic policy checks to 100 eligible agents per minute.
Continue from the last agent ID. Exclude pending hires and agents in
termination or a final state. Keep explicit company and budget updates
immediate.

## Verification

- Latest head `ccc3c86b0` aligns the shared status update schema with
the PATCH route. All 15 shared tests, 104 server route/OpenAPI tests, 2
CLI tests, and shared/server/CLI TypeScript checks pass. The Greptile
comment is fixed. CI is running; no new Apex review was requested for
this small fix.

- Rebase `75708ac94` rebases onto master at `2eb5187e4`. The backup test
uses the shared 90-second timeout from master; the older timeout commit
was removed. The affected backup test passes.
- Agent budget edits now pass the target agent ID through enforcement.
All 50 affected tests, server TypeScript, and 11 boundary tests pass.
Both configuration update paths have regression coverage. CI is running
on the latest head. No new Apex review was requested for these small
fixes.

- Rebase `9b568666a` preserves the run-control extraction and
delivery-work startup changes from master. All 559 affected tests,
server TypeScript, and 11 boundary tests pass. CI and Apex review are
running on the final head.
- The policy scan fix passes 56 lifecycle and diagnostic tests, 24
policy and caller tests, and 11 boundary tests. A regression test checks
the page limit, interval, excluded states, and cursor wraparound.

- Rebase `fd4698750` passes all 611 tests in eight affected lifecycle,
route, and heartbeat suites. Server TypeScript and all 11 module
boundary tests pass. The company deletion rollback and concurrent hire
tests are included.

- Follow-up `00aac7646` fixes the failed decision deletion test and the
public update schema. All 126 affected server tests, 8 shared tests, 2
CLI tests, and 11 boundary tests pass. Shared, server, CLI, and UI
TypeScript checks pass.

- Server TypeScript passes: `pnpm --filter @paperclipai/server exec tsc
--noEmit`.
- All 11 boundary tests pass: `node --test
scripts/check-module-boundaries.test.mjs`. The repository boundary scan
also passes.
- All 68 lifecycle, failure diagnostic, and harness verification tests
pass. Tests reject forbidden fields through the root command, combined
transition command, and restricted transaction integration. They also
check invalid fields returned by configuration preparation.
- Failure tests cover host work, plugin work, claims, renewals,
deferrals, and scans. Log tests check known error codes and exclusion of
private error contents.
- All 355 tests in 10 existing caller suites pass. These tests cover
secret approval, pairing, pending hires, credentials, company import,
built-in agents, managed agents, instructions, and skills.
- The branch is rebased onto `origin/master` at `2eb5187e4`. The queue
guards and cancellation lock fix use the extracted heartbeat queue.
Agent cancellation uses the extracted run-control file. The source diff
scan found no secrets or private URLs.
- Earlier checks cover approval retries, rollback, late verification
results, policy holds, deletion rollback, concurrent company deletion,
and queue admission during setup.
- Before the rebase, CI passed on `00aac7646` and Greptile Apex rated it
5/5 with no open comments. CI and a new Apex review are pending for the
rebased commit. The branch has no merge conflicts.
- The full local build and native Dot suite were not run. The host has
no Cargo. No live provider test was run.

## Risks

- Background policy recovery takes more than one minute when more than
100 agents are eligible. Explicit company and budget updates still
reconcile immediately.

- The restricted configuration integration keeps an agent reference and
its dependent records in one transaction. It cannot change a budget or
invoke a lifecycle transition. A periodic scan starts pending setup
after that transaction commits.
- Secret approval and binding use separate transactions. A failed
binding can leave an approved secret with no agent binding.
- New approved hires remain unavailable until setup and verification
complete. Existing clients see `paused` during these steps. This PR does
not change the UI.
- Migration `0320_daily_onslaught.sql` maps existing statuses and pause
reasons. It does not create external resources or claim that an existing
harness passed verification.
- Use a coordinated server update for this migration. Older server
versions can write legacy status without the new lifecycle checks.
- A failed onboarding or invitation request can leave a committed hire.
A retry reuses that hire and completes the remaining records.
- Deletion requires completed termination. Company deletion can require
another request while cleanup is pending.
- Required plugins must make repeated calls safe and reject older
revisions. A disabled or removed required plugin blocks completion until
it is restored.
- A new hire can run a second harness test after the existing UI test.
Some adapters can charge for this test.
- No live provider test was run. Provider implementation and resource
backfill remain separate work.

DottaandPaperclip d66acb7ac1 feat: automate Slack bot app setup and installation (#15413)
## Thinking Path

> - Paperclip is the open source app people use to manage AI agents for
work.
> - Chat connectors give each agent a customer-owned bot and task-backed
conversations.
> - Manual Slack setup requires app creation and copying durable
credentials.
> - Operators need a shorter setup that an assisting agent can use
safely.
> - This pull request creates the app through Slack's Manifest API and
installs it through OAuth.
> - Durable registration state supports recovery without creating
another app.
> - A four-screen wizard, automatic avatar upload, and OAuth account
linking reduce setup work.
> - Connector settings and per-turn tool guidance support daily use
after installation.

## Linked Issues or Issue Description

**Subsystem affected**

Native Slack bot setup, company secret storage, chat connector
management, and agent tool guidance.

**Problem or motivation**

New Slack bots require manual app creation and copying a signing secret
and bot token. Interrupted setup can create duplicate apps. The setup
and management screens contain unnecessary controls. Agents also need
guidance for native questions, files, thread replies, and governed Slack
actions.

**Proposed solution**

Use a temporary app-configuration access token to create a
customer-owned app. Save durable secrets in the vault. Bind OAuth to the
initiating actor, company, endpoint, registration revision, scopes, and
configured origins. Preserve manual and existing-app recovery. Link the
installing user's account, send a welcome DM, and advance from saved
server evidence. Keep request-URL recovery instructions available if
automatic connection detection waits.

**Alternatives considered**

The Slack CLI adds installation requirements. Socket Mode changes
transport. A shared Paperclip-owned app changes app ownership. These
alternatives are outside this change.

**Roadmap alignment**

This extends existing chat connectors and secrets capabilities. Related
public work: #14037 and #13954 cover Slack MCP prerequisites and user
OAuth. No duplicate bot-registration PR was found.

## What Changed

- Share one reviewed manifest builder between automatic registration and
manual setup.
- Add replay-safe migration 0318 and company-bound registration state
with vault references and uncertain-creation recovery.
- Add registration, installation, callback, and resume APIs with
short-lived, single-use OAuth state.
- Save installation credentials before downstream checks and preserve
bot identity constraints.
- Reduce automatic setup to four screens. Keep advanced app details,
manual recovery, and existing-app setup.
- Upload the agent avatar with the Paperclip dark background. Link the
OAuth installer's account and send setup DMs.
- Show agent and connector-owner avatars. Simplify settings, access, and
conversation screens.
- Discover joined Slack channels and enable them by default. Start a
task from a bare mention and admit same-thread follow-ups.
- Refresh Slack tool guidance each turn. Add native-form, file,
approval, and delivery regressions plus manual model probe definitions
and sanitized acceptance records.
- Update deployment/database docs, OpenAPI, redaction, removal cleanup,
production Storybook stories, and provider browser tests.
- Merge current master and move the registration migration after its
latest migration without rewriting published commits.

The completed Slack success view intentionally has a single centered
**Done** action and no **Save & exit**, as explicitly requested by the
product owner. `DESIGN.md` records this exception; unfinished setup
steps retain the aligned wizard footer.

## Verification

- Passed after the master merge: repository typecheck, full build,
Storybook build, design-token gates, module-boundary gates, and
migration generation.
- Passed: all 352 focused Slack deterministic tests and all 14 affected
provider browser tests. Browser tests use controlled provider fixtures
and a separate throwaway instance.
- Passed on current head `c5d01e0e2`: the complete GitHub test matrix
(general server, chat, all workspaces, serialized server, and Runner),
all eight browser shards, typecheck/release registry, build, canary dry
run, security checks, and policy gates. There are 52 passing checks and
no pending or failing checks.
- Greptile completed on the exact current head with 5/5 and no
actionable findings or open review threads.
- Local repair verification passed 93 focused tests, including same-app
reinstall after revocation and rejection of consent started before
revocation, the AgentMail browser journey, and repository typecheck.
Local build and Storybook build also passed. The redundant local
full-suite rerun was stopped after the complete current-head CI matrix
passed.
- Real Slack setup and agent replies were exercised in the authorized
isolated test drive during the setup iteration.
- The ten additional model probes were attempted with legacy
`codex_local`, `gpt-5.6-sol`: five passed, two failed, and three were
partly verified. Native runtime is not qualified. See
`server/src/services/connectors/slack/evals/2026-10-08-acceptance.md`
for evidence and limits.
- Passing model probes cover native forms, downloaded file bytes, bare
mentions with thread replies, explicit posts/reactions, and saved
approval denial.
- The controlled uncertain-write probe found wrong delivery-check IDs.
The canvas fallback attempt used an invented tool name. Search
pagination/native search, a private-source denied-tool receipt, and
distinct board/webhook origins remain unqualified.

Reviewer path: enable Chat connectors, start Slack chat setup, select an
agent, enter an app-configuration access token, and approve Slack
installation. Send a message to the bot and confirm that setup advances
to success. Inspect settings and allowed channels. See
`doc/connections/SLACK-AUTOMATIC-SETUP.md` for deployment and recovery.

## Risks

- Slack app creation has no provider idempotency guarantee. A timeout
after dispatch stays uncertain until the operator checks Slack.
- OAuth needs a stable public HTTPS board origin. Webhook ingress may
use a separate configured HTTPS origin. Workspace policy can delay
installation.
- Migration 0318 can replay safely on instances that applied the earlier
development migration.
- OAuth installation now links the installer to the initiating Paperclip
user. Identity checks and company access rules still apply.
- Joined channels now enable bot responses by default. Linked-user
authorization and per-action approval rules still apply.
- Model behavior has the documented delivery-check and canvas fallback
failures. A passing CI run does not establish that every model probe
passed.
- Removing the connection does not delete the customer's Slack app. No
new first-party telemetry is added.

## Model Used

OpenAI Codex, GPT-6 family, with reasoning, repository tools, code
execution, and browser verification. The runtime does not expose a more
specific authoring model ID or context-window size. The live bot probes
used OpenAI `gpt-5.6-sol` through `codex_local` in legacy mode.

b847e8b6f6 perf(server): reduce issue detail request overhead (#10414)
## Thinking Path

> - Paperclip is the open source control plane people use to coordinate
AI-agent work
> - Opening an issue fans out into several authenticated issue-detail
reads, so repeated work on that path directly affects perceived latency
> - Those reads repeated issue and authorization lookups, returned full
private JSON even when unchanged, and performed non-critical bookkeeping
writes on the request path
> - Interaction reads also performed lifecycle writes even though `GET`
must be read-only
> - This pull request adds request-scoped reuse, private conditional
responses, read-only interaction access, and bounded write debouncing
without crossing actor, request, or company boundaries
> - The result is less database, serialization, logging, and
response-body work while preserving authorization and interaction
lifecycle invariants

## Linked Issues or Issue Description

This is the server-only latency phase. Related work is tracked
separately in #10415 (aggregate view), #10416 (warm navigation, merged
into the base), and #10463 (bundle split). This pull request
intentionally excludes those scopes.

**What happened?**

Opening an issue detail view caused avoidable server costs: repeated
issue and authorization reads within one request, full private JSON
responses when a representation was unchanged, writes during
interaction-list reads, production debug transport setup, and immediate
bookkeeping writes for cloud tenant activity and board-key usage.

**Expected behavior**

All successful JSON `GET /api/issues/:id/*` responses should support
strong private ETags and `304 Not Modified`. Repeated work may be reused
only within the current request. `GET /interactions` must not modify
stored interactions. Non-critical activity timestamps may be debounced
without weakening authentication or stale instance-admin cleanup.

**Steps to reproduce**

1. Start Paperclip in local development or self-hosted server mode.
2. Open one issue and request its detail subresources with the same
authenticated actor.
3. Repeat a successful JSON request with its `ETag` in `If-None-Match`.
4. Observe `304 Not Modified`, no interaction writes from `GET
/interactions`, and unchanged authorization boundaries.

**Deployment mode / installation**

- Local development or self-hosted server
- Built from source
- Core server behavior; not adapter-specific

## What Changed

- Added strong ETags and `Cache-Control: private, must-revalidate` to
successful JSON reads under `/api/issues/:id/*`, including
standards-compliant `If-None-Match` handling.
- Added request-scoped promise memoization for issue and authorization
lookups; no authorization result survives the request.
- Made `GET /interactions` read-only, moved supersession and
terminal-state handling to mutation paths, and prevented plugin callers
from accepting or rejecting interactions after an issue closes.
- Removed the production debug-file logger transport while preserving
development formatting.
- Debounced cloud-tenant activity and board-key `lastUsedAt`
persistence, while keeping stale instance-admin deletion unconditional
and authentication checks per request.
- Added focused tests for ETags, request isolation, authorization
lifecycle behavior, interaction invariants, logger configuration, and
retry-safe debounce behavior.

## Verification

- `pnpm exec vitest run server/src/__tests__/private-json-etag.test.ts
server/src/__tests__/issue-thread-interaction-routes.test.ts` — 2 files,
23 tests passed.
- Focused Vitest run covering request memoization, authorization,
interactions, plugin orchestration, logger, cloud tenant, board auth,
and issue services — 9 files, 264 tests passed.
- `pnpm --filter @paperclipai/server typecheck` — passed.
- `git diff --check origin/master...HEAD` — passed.
- Scope guardrails: 21 changed files under `server/src`; no lockfile,
workflow, migration, UI, aggregate-view, or bundle-split changes.

## Risks

- Strong ETags hash each successful serialized JSON response. This adds
a small CPU cost but avoids transferring unchanged bodies.
- Debounced bookkeeping timestamps can lag by the bounded debounce
interval. They are non-critical usage metadata; authentication still
runs per request, and stale instance-admin deletion remains
unconditional.
- Legacy pending interactions on terminal issues are projected as
expired by reads and are finalized only by mutation paths. The stored
record remains unchanged on `GET` by design.
- No database schema or migration changes are included.

> This is a focused performance correction and does not duplicate a
planned core feature in `ROADMAP.md`.

## Model Used

OpenAI Codex using `gpt-5.3-codex` for the initial implementation and
`gpt-5.6-sol` for isolation, verification, and PR preparation, with
reasoning, repository tool use, code execution, and GitHub CLI access.
The runtimes did not expose authoritative context-window sizes.

DottaandPaperclip dbebf30c89 Add low-trust review containment (#7530)
## Thinking Path

> - Paperclip is a control plane for AI-agent companies, so execution
policy and trust boundaries are part of the product's safety contract.
> - Low-trust review work needs narrower authority than normal
same-company agents because hostile PRs, comments, attachments, and
generated output can carry prompt-injection payloads.
> - The current V1 shape gives trusted workers broad company context,
which is useful for normal execution but too permissive for a reviewer
assigned to hostile content.
> - This branch adds a `low_trust_review` preset, source-trust tagging,
route-level containment, and quarantine handling so low-trust output
does not automatically flow into higher-trust wake context.
> - The branch has been rebased onto current `origin/master`, and the
low-trust migration was renumbered to `0097_low_trust_source_trust.sql`
to avoid collisions with existing `0091` through `0096` migrations.
> - Greptile feedback was addressed by tightening low-trust detection,
preserving project-level trust policy checks, fixing issue-kind
promotion lookup, removing duplicate post-lease isolation assertion,
documenting fail-closed source-trust behavior, bounding ancestry checks,
enforcing runtime issue context for CEOs, awaiting accepted-plan monitor
authorization, and making low-trust issue source-trust tagging atomic.
> - The benefit is a first production slice of deny-by-default review
containment with regression coverage for the main control-plane pivot
surfaces.

Fixes #7531.

## What Changed

- Added shared trust-policy types and validators, plus
database/source-trust fields for issues, comments, documents, and work
products.
- Implemented server enforcement for low-trust issue scope, agent
self-view redaction, secret/plugin/runtime denial paths, promotion
checks, and quarantined continuation/wake context.
- Added focused low-trust regression tests for resolver behavior, source
trust, route authorization, heartbeat preflight ordering, runtime
containment, and quarantine redaction.
- Added board UI affordances for selecting/reviewing the low-trust
preset and surfacing source-trust badges in relevant issue views.
- Added `doc/LOW-TRUST-PRESETS.md`, updated
`doc/SPEC-implementation.md`, and committed the low-trust review
contract plan under `doc/plans/`.
- Rebasing note: the original `0097_low_trust_source_trust.sql`
migration was renamed to `0097_low_trust_source_trust.sql`; the SQL uses
`ADD COLUMN IF NOT EXISTS` so users who already applied the old-numbered
migration are not broken by the renumbered migration.

## Verification

- Rebased branch onto current `origin/master` and force-pushed with
lease to `origin/PAP-10211-low-trust-agent` at head `2719f31e3`.
- Confirmed the PR diff does not include `pnpm-lock.yaml` or
`.github/workflows` changes.
- Resolved upstream UI/comment conflicts by preserving deleted-comment
tombstone behavior and low-trust source-trust badges/metadata.
- Renumbered the low-trust source-trust migration to
`0097_low_trust_source_trust.sql`; the SQL uses `ADD COLUMN IF NOT
EXISTS` so users who already applied an old-numbered copy are not
broken.
- `pnpm exec vitest run ui/src/lib/issue-chat-messages.test.ts
server/src/__tests__/heartbeat-workspace-session.test.ts`
- `pnpm exec vitest run server/src/__tests__/source-trust.test.ts
server/src/__tests__/workspace-runtime-service-authz.test.ts
ui/src/lib/trust-policy-ui.test.ts
ui/src/components/TrustPresetSection.test.tsx`
- `pnpm run typecheck:build-gaps`
- `git diff --check`
- GitHub checks pass on head `2719f31e3`: build, typecheck/release
registry, general tests, serialized server suites, e2e, canary, verify,
policy/review, Socket, and Snyk.
- Greptile Review passes with Confidence Score 5/5 and zero unresolved
Greptile review threads.
- No design screenshots/images were added because the task explicitly
says not to add them unless they are specifically part of the work.

## Risks

- Medium risk: this touches shared trust-policy contracts, server
authorization paths, heartbeat context generation, migration metadata,
and UI preset controls.
- Low-trust containment is intentionally deny-by-default; legitimate
future review workflows may need explicit allowlisted exceptions.
- Plugin/runtime/security surfaces are broad, so regression tests cover
the current known routes but future integrations must route through the
same containment layer.
- The PR is ready for review; GitHub checks are green and Greptile is
5/5.

> For core feature work, check [`ROADMAP.md`](ROADMAP.md) first and
discuss it in `#dev` before opening the PR. Feature PRs that overlap
with planned core work may need to be redirected — check the roadmap
first. See `CONTRIBUTING.md`.

workflow.

flows, profile surfaces, and company-scoped access controls.
> - Agents and operators also need the existing issue, inbox, workspace,
approval, and plugin flows to keep working under those authenticated
boundaries.
> - This branch accumulated the multi-user implementation, follow-up QA
fixes, workspace/runtime refinements, invite UX improvements,
release-branch conflict resolution, and review hardening.
> - This pull request consolidates that branch onto the current `master`
branch as a single reviewable PR.
> - The benefit is a complete multi-user implementation path with tests
and docs carried forward without dropping existing branch work.

## What Changed

- Added authenticated human-user access surfaces: auth/session routes,
company user directory, profile settings, company access/member
management, join requests, and invite management.
- Added invite creation, invite landing, onboarding, logo/branding,
invite grants, deduped join requests, and authenticated multi-user E2E
coverage.
- Tightened company-scoped and instance-admin authorization across
board, plugin, adapter, access, issue, and workspace routes.
- Added profile-image URL validation hardening, avatar preservation on
name-only profile updates, and join-request uniqueness migration cleanup
for pending human requests.
- Added an atomic member role/status/grants update path so Company
Access saves no longer leave partially updated permissions.
- Improved issue chat, inbox, assignee identity rendering,
sidebar/account/company navigation, workspace routing, and execution
workspace reuse behavior for multi-user operation.
- Added and updated server/UI tests covering auth, invites, membership,
issue workspace inheritance, plugin authz, inbox/chat behavior, and
multi-user flows.
- Merged current `public-gh/master` into this branch, resolved all
conflicts, and verified no `pnpm-lock.yaml` change is included in this
PR diff.

## Risks

- High review surface area: this PR contains the accumulated multi-user
branch plus follow-up fixes, so reviewers should focus especially on
company-boundary enforcement and authenticated-vs-local deployment
behavior.
- UI behavior changed across invites, inbox, issue chat, access
settings, and sidebar navigation; no browser screenshots are included in
this branch-consolidation PR.
- Plugin install, upgrade, and lifecycle/config mutations now require
instance-admin access, which is intentional but may change expectations
for non-admin board users.
- A join-request dedupe migration rejects duplicate pending human
requests before creating unique indexes; deployments with unusual
historical duplicates should review the migration behavior.
- Company member role/status/grant saves now use a new combined
endpoint; older separate endpoints remain for compatibility.
- Full production build was not run locally in this heartbeat; CI should
cover the full matrix.

## Model Used

- OpenAI Codex coding agent, GPT-5-based model, CLI/tool-use
environment. Exact deployed model identifier and context window were not
exposed by the runtime.
