Preserve rejected native completion reports as failures (#18227)
Keep rejected declared completion reports on the existing permanent native failure path. Preserve rejection across restoration and order permanent failure ahead of terminal goal facts, goal results, and continuation while retaining the original provider terminal. Validated runner diagnostics retain this scalar metadata even when the operation

If a declared native completion tool is rejected and the turn ends without an

accepted replacement report, the runner emits `session.failed` with code

original turn terminal. It emits no successful result proposal or human review

a missing-result recovery turn. This failure also precedes terminal goal updates;

Core records the original turn terminal before rejecting goal success or further

The rejection survives same-turn restoration;

their existing fallback.

Before interrupting a Codex turn, the runner durably records a

`harness.diagnostic` event with code `provider_interrupt_requested`, the stop

reason, provider turn ID, and pending tool call IDs. A harmless identical result
